Den foreliggende opfindelse angår et hidtil ukendt knoglesøgende kompleks dannet af et technetium-99m atom og en bis(ami-nomethyl)bicyklo[2,2,l]heptan og et middel indeholdende komplekset .
5 Norbornan er en cyklisk forbindelse, der findes i eller kan opnås ud fra de tilbageværende tunge hydrocarboner i bunden af destillationstårne i anlæg til fremstilling af lette hydrocarboner. Forskellige derivater af norbornan har været fremstillet og blandt disse bi s(methyl ami η) . Det er denne forbindelse, 10 der er blevet phosphonomethyleret og/eller carboxylalkyleret, som sammen med technetium-99m atomet danner komplekset ifølge den foreliggende opfindelse.
Det er velkendt, at aminer, såsom ethylendiamin og diethylen-triamin, kan omsættes med formaldehyd og phosphorsyr1 ing til •*-5 opnåelse af methylenphosphonatderivater af aminen, hvori methyl enphosphonatgruppen

er indført i stedet for aminhydrogenatomerne (US-patentskrift nr, 3.288.846).
Anvendelsen af methylenphosphonsyresubstituerede alkylen-20 polyaminer til metalionkontrol i mindre end støkiometrikse mængder blev foreslået i et patent (US-patentskrift nr. 2.609.390), der blev udstedt i 1952. Senere blev et vand-dispergerbart polymert amin-chelatdannende middel, som indeholdt alkylenphosphonatderivater, omtalt som havende "tærskel"-25 virkninger ved anvendelse til inhibering af kedelsten (se US-patentskrift nr. 3.331.773), idet dette udtryk benyttes til beskrivelse af anvendelsen af midlet i mindre end støkiometriske mængder. Det har nu vist sig, at hidtil ukendte kompleksdannende forbindelser kan fremstilles ud fra bis(methy!amin)derivaterne af norbornan. Disse derivater har formlen H2!rø2C-j-^-|-CH2NH2
Derivaterne er også kendt under det mere formelle navn 10 2(3),5(6) -bis (aminomethyl) bicyklo [2,2,1] heptan. De kompleks dannende forbindelser dannes ved erstatning af den primære amins hydrogenatomer med forskellige substituenter. DK 169034 B1 4 og det knoglesøgende kompleks ifølge opfindelsen er ejendommeligt ved, at det er dannet af et technetium-99m atom og en forbindelse med den nævnte formel I, hvori substituenterne A, B, X og Y uafhængigt af hinanden er valgt blandt methylenphos-5 phonsyregruppen, carboxylsyregrupper (med 2-4 carbonatomer) og alkali- eller jordalkalimetal-, ammonium- og aminsaltene af phosphon- eller carboxylsyregruppen.
Den foreliggende opfindelse angår endvidere et middel omfattende det knoglesøgende kompleks og et reduktionsmiddel i en 10 saltopløsning.
Fremgangsmåder til opnåelse af carboxylsyreforbindelser med formlen I er velkendte.
Carboxylsyregruppen kan indføres i stedet for et amin-hydrogen-atom ved omsætning af aminen i vandig opløsning med et passen-15 de nitril, såsom glycolonitril og acrylonitril, i nærværelse af baseoverskud (NaOH) og derpå omdannelse af saltformen af syregruppen til den frie carboxylsyregruppe i overensstemmelse med velkendte procedurer.
Med hensyn til fremstillingen af aminsalte af syregrupperne 20 er omsætningen velkendt. I Richter's 3.udgave (1952) af hans Textbook of Organic Chemistry, side 246, er det anført, at "Amines of all classes, like ammonia, are basic substances and readily react with acids to form ionizable salts".

De knoglesøgende komplekser dannet af bis(aminomethyl)bicyklo-[2,2,ljheptaner med formlen I og technetium-99m atomet kan anvendes til afbildning af dyrs skeletsystem. Det er vigtigt, at sådanne komplekser kan administreres intravenøst til opnå-5 else af knoglescanning, og at metallet forbliver bundet til chelatoren med henblik på dette formål. Komplekserne ifølge opfindelsen udviser højere knogleoptagelse med hurtigere blod-clearance end kendte komplekser af Tc-99m med ethylendiaminte-tra (methylenphosphonsyre) (EDTMP) og med diethylentriaminpenta-10 (methylenphosphonsyre) (DTPMP), således som vist i det efterfølgende eksempel 4. Det bemærkes i øvrigt, at komplekserne ifølge opfindelsen er relativt stabile, og let passerer gennem nyren, medens en stor mængde optages i skeletsystemet til den ønskede billeddannelse, og meget lidt af komplekset bliver 15 tilbage i blodet eller i blødt væv i løbet af et rimeligt tidsrum. Eftersom forholdet mellem optagelsen i knogler og optagelsen i omgivende blødt væv er højt, bliver billederne skarpe og klare. Herved adskiller komplekset ifølge opfindelsen sig væsentligt fra lignende, tidligere kendte komplekser.
20 Den første radioatomkerne, der i vidt omfang blev benyttet til knoglescanning, var Sr-85. Strontium-85 akkumuleres i knogler efter intravenøs administration, og afbildninger af skeletsystemet er mulige. Sr-85 har imidlertid en lang fysisk halveringstid (65 dage) og en lang biologisk halverings-25 tid (ca. 800 dage), hvilket begrænser de mængder, som kan administreres. Også den emitterede gammaphotons høje energi (514 kev) er vanskelig at kollimere.
Fluor-18 har også været benyttet til afbildning af skeletsystemet. Den er en positron-emitter med en halveringstid på 1,85 time. Selv om F-18 har gode fysiske egenskaber til afbildning, har den nogle alvorlige ulemper. Fluor-18 produ- DK 169034 B1 6 ceres i cyklotron og er derfor kostbar. Dens distribution er også begrænset som følge af dens korte halveringstid.
Mange midler til organscanning, herunder midlerne til skeletsystemet, er nu blevet erstattet med komplekser af technetium-5 99m. Denne atomkerne har ideelle fysiske egenskaber (T-jy2 = 6 timer, gammaphoton på 141 kev) til afbildning. Endvidere er den relativt let tilgængelig som følge af Mo-99/Tc-99m-generatorerne. Størstedelen af afbildningen foretages således nu ved anvendelse af Tc-99m.
10 Technetium-99m opnås fra generatorer i +7-oxidationstrinet som pertechnetationen (Tc04_). For at danne et kompleks skal Tc være i et lavere oxidationstrin, d.v.s. +3, +4 eller +5. Selv om andre reduktionsmidler kan anvendes, har Sn som oftest været benyttet, Tc-99m-komplekser kan således 15 dannes ved reduktion af TcO^ under anvendelse af Sn^+ i nærværelse af et kompleksdannende middel. Dette foretages sædvanligvis i en vandig saltopløsning, som er egnet til intravenøs injektion.
Kommercielle kompleksdannende midler sælges som "radiofarma-20 ceutiske sæt". Et "sæt" består af et evakueret lille glas indeholdende det kompleksdannende middel, et reduktionsmiddel og eventuelt en puffer og stabilisatorer. Til fremstilling af Tc-99m-komplekserne indsprøjtes nogle få ml natrium-pertechnitatopløsning i saltvand i glasset. En passende mæng-25 de af den resulterende opløsning anvendes til afbildning.
Subramanion et al. (Radiology, bind 99, side 192-196, 1971) omtalte anvendelsen af et kompleks af Tc-99m og et uorganisk DK 169034 B1 7 polyphosphat til skeletafbildning. Flere andre har omtalt uorganiske polyphosphater som anvendelige til dette formål (se US-patentskrifterne nr. 3.852.414, nr. 4.016.249 og nr. 4.082.840). Anvendelsen af pyrophosphat (PYP) til knogleafbildning har også 5 været omhandlet (US-patentskrifterne nr. 3.851.044, nr. 3.931.396 og nr. 4.075.314). Tc-phosphaterne havde god succes, men har været erstattet af Tc-phosphonater.
Komplekser af Tc-99m med phosphonsyrer viser højere knogleoptagelse med hurtigere blod-clearance end Tc-99m/phosphat-10 komplekser. Phosphonsyrer, der anses for de bedste midler til knoglescanning, når de danner kompleks med Tc-99m, indbefatter hydroxyethandiphosphonat (EHDP), methylendiphosphonat (MDP) og hydroxymethylendiphosphonat (se US-patentskrifterne nr. 3.983.227, nr. 3.989.730 og nr. 4.032.625 samt J.Nucl.

De følgende eksempler illustrerer fremstillingen af de kompleksdannende forbindelser med formlen I, samt fremstillingen og anvendelsen af de omhandlede komplekser. Identifikation af 20 de kompleksdannende forbindelser sker ved henvisning til den ovenfor nævnte formel I.

Destilleret vand (10 g) og H^PO^ (9,9 g) blev afvejet i en 50 ml rundbundet kolbe, der var udstyret med en vandkølet svaler, et termometer, en omrørestang og en tilsætningstragt. Koncentreret HCl (11,8 g) blev derpå tilsat, og 5 3,9 g (2(3),5(6)-bis(aminomethyl)bicyklo[2,2,l]heptan blev langsomt tilsat under omrøring. Opløsningen blev opvarmet under tilbagesvaling i ca. en time, og 37% vandig formaldehydopløsning (8,51 g) blev tilsat over et tidsrum på 2 1/2 time. Opløsningen blev opvarmet i yderligere 3 timer under 10 tilbagesvaling. Den dannede forbindelse var det fuldstændigt phosphonornethylerede norbornanderivat, hvori hvert af symbolerne A, B, X og Y stod for methylenphosphonsyregrupper, -CH2P03H2· EKSEMPEL 2.
15 Proceduren ifølge eksempel 1 blev fulgt med tilsætning af 3 molækvivalenter formaldehyd (24,3 g af en 37% opløsning) og phorphorsyrling (25,1 g) til 1 molækvivalent 2(3),5(6)-bis(aminomethyl)bicyklo[2,2,l]heptan (15,2 g). Den resulterende opløsning blev neutraliseret med NaOH (vandig), og der 20 tilsattes 1 molækvivalent glycolonitril (14,7 g af en 38% opløsning) og NaOH (13,8 g af en 33,3% opløsning). Den dannede forbindelse var natriumsaltet af bicykloheptanderivatet, som havde et gennemsnit på 3 methylenphosphonsyregrupper og 1 carboxymethylgruppe pr. molekyle som A-, B-, X- og Y-25 substituenter.

En mængde (15,2 g, 0,1 mol) 1(2),4(5)-bis(aminomethyl)bicyklo-5 [2,2,l]heptan blev anbragt i et rustfrit stålbægerglas, og 50 ml destilleret vand blev tilsat, efterfulgt af tilsætning af 37 ml 50% NaOH. Denne opløsning blev opvarmet til ca.
70°C under omrøring, og carboxymethylering blev foretaget ved dråbevis tilsætning af 58,9 g 38% glycolonitril under 10 vedvarende opvarmning i løbet af et tidsrum på en time.
Efter ca. 15 minutter af tilsætningen var temperaturen på tilbagesvalingstemperatur, og NH^ blev frigjort. Destilleret vand blev periodisk tilsat til erstatning af fordampet vand. Efter afslutning af tilsætningen af glycolonitril 15 opvarmedes opløsningen i yderligere en time under omrøring og rensning med N2· Den dannede forbindelse var tetracarb-oxylmethyIderivatet, hvori A, B, X og Y var CH^-COONa.
EKSEMPEL 4.
2o ----------- 9 100 All af opløsningen fremstillet ifølge eksempel 1 blev anbragt i et lille glas, og 1 ml 0,9% saltvand (vandig) blev tilsat. Opløsningens pH-værdi blev indstillet til 3,5 under 25 anvendelse af fortyndet NaOH og HC1, og 0,1 ml af en frisk elueret Tc04”-opløsning blev tilsat. Dette blev efterfulgt af tilsætning af 100 /ul af en mættet stannotartratopløsning. Eluering af papirkromatografistrimler med saltvand og acetone viste mindre end 5% af aktiviteten som Tc04 eller reduceret 30 ikke-kompleksdannet Tc.
Det ovennævnte Tc-kompleks (50 μΐ, ca. 1 mCi) blev injiceret i rotters halevene. Scinti11 at ionsscanni nger af de bedøvede rotter blev foretaget med flere intervaller efter injektion.
35 Knoglescanningerne var af høj kvalitet, hvilket antyder anvendeligheden deraf som et redskab til menneskepatienter.

En artikel i Seminars in Nucl.Med. 6(1), 10 - 31 (Januar 1976) giver oplysning om retentionen i kaniners blod, lever og muskler 2 timer efter injektion af komplekser af Tc-99m med ethy-1endiamintetra(methylenphosphonsyre) (EDTMP) og med diethylen-5 triaminpenta(methylenphosphonsyre) (DTPMP).
Tilsvarende data er blevet opnået for det ifølge nærværende eksempel 4 fremstillede kompleks. Dataene er som følger: 10 ____

Den væsentligt lavere retentionsgrad af komplekset ifølge eksempel 4 viser tydeligt, at komplekserne ifølge den foreliggende opfindelse udviser en væsentligt lavere retentionsgrad i det væv, som ikke er target-vævsstedet.
25 EKSEMPEL 5.
Det ifølge eksempel 4 fremstillede Tc-kompleks (50 Ail, ca.
1 mCi) blev injiceret i halevenen på mus. Musene blev 30 dræbt på forskellige tidspunkter efter injektion, organerne og vævet blev fjernet, og strålingen måltes med en NaI-tæller. Aktiviteten fandtes primært i knoglerne og i blæren,idet ingen andre organer viste en høj affinitet for komplekset.
120 minutter efter injektion havde knoglerne, musklerne 35 og leveren henholdsvis 5,1, <0,01 og 0,24% dosis/g af aktiviteten .

Tc-komplekset ifølge eksempel 4 (50 jal, ca. 1 mci) blev inji-5 ceret i en række rotters halevene. Dyrene blev dræbt efter 2 timer, og stråling fra flere væv blev kvantificeret under anvendelse af en Nal-sc inti 11 at ionstæl1 er. Tabel I nedenfor viser resultaterne sammenlignet med Tc-MDP (methylen-diphosphonat), kommercielt Tc-knoglemiddel, bedømt på en 10 identisk måde.

Komplekset ifølge eksempel 4 blev injiceret i en række kaniners ørevene. Kaninerne blev dræbt 3 timer efter injektion. Flere vævs stråling blev kvantificeret ved tælling under anvendelse af en NaI-sci nti nationstæller. Tabel II nedenfor viser re-35 sultaterne sammenlignet med resultaterne af et kommercielt Tc-knoglemiddel, MDP.

1. Knoglesøgende kompleks dannet af et technetium-99m atom og en forbindelse med strukturformlen A\ /x NCH2--\_|~CH2N (I) w 'Nr hvor substituenterne A, B, X og Y hver uafhængigt af hinanden er valgt blandt me thylenphosphonsyregruppen, alkylcarboxylsyregrupper (med 2-4 carbonatomer) og alkali- eller jordalka- DK 169034 B1 limetal-, ammonium- og aminsaltene af phosphon- eller carboxyl syregruppen.
2. Kompleks ifølge krav 1, kendetegnet ved, at A, B, X og Y hver er methylenphosphonsyregrupper eller alkalime- 5 tal-, jordalkalimetal-, ammonium- eller aminsalte deraf.
3. Middel omfattende komplekset ifølge krav 1 eller 2 og et reduktionsmiddel i en saltopløsning.
4. Middel ifølge krav 3, kendetegnet ved, at reduktionsmidlet er Sn^+. 10
