Den foreliggende opfindelse angår sårheling.
5 Vækstfaktorer er polypeptidhormoner, som stimulerer en defineret population af målceller. Eksempler på vækstfaktorer omfatter blodpladeafledt vækstfaktor (PDGF), insulinlignende vækstfaktor (IGF-I), transformerende vækstfak-tor-β (TGF-β), transformerende vækstfaktor-α (TGF-a), epidermal vækstfaktor (EGF) og fibroblast vækstfaktor (FGF). PDGF er et kationisk, varmestabilt 10 protein, der findes i komene fra cirkulerende blodplader, der er kendt for at stimulere in vitro proteinsyntese og kollagenproduktion med fibroblaster. Det er også kendt for at virke som et in vitro mitogen og kemotaktisk middel for fibroblaster og glatmuskulatur-celler.
15 Det har været foreslået at anvende PDGF til at fremme in vivo sårheling.
F.eks. beskriver Grotendorst (1984) J. Trauma 24:549-52 tilsætning af PDGF til Hunt-Schilling trådnetkamre imprægneret med en kollagengel og implante-ret i ryggene på rotter; PDGF viste sig at forøge den mængde nyt kollagen, i der blev syntetiseret. Leitsel et al. (1985) J. Dermatol. Surg. Oncol. 11:617-20 22 var imidlertid ude af stand til at accelerere normal sårheling i hamstre un der anvendelse af PDGF alene eller i kombination med FGF og EGF.
I Soft and Hard Tissue Repair (Hunt, T.K. et al., udg), Praeger Publishers,
New York, pp. 380-394 (1984) rapporterer Michaeli, et al., at anvendelse af 25 et delvist renset PDGF-præparat opnået ud fra blodpladerig plasma stimulerede angiogenese, når det blev implanteret i kanincornea. Fordi PDGF ikke er en angiogen vækstfaktor, foreslog forskerne, at en hidtil ukendt faktor i deres delvist rensede PDGF-præparat var ansvarlig for den angiogene virkning.

Schultz, G.S. et al., (1987) Science rapporterede, at lokal anvendelse af TGF-α på delvist fortykkede hudforbrændinger på svin accelererede epidermal regenerering i sammenligning med ubehandlede forbrændinger.
5 EP-A-0105014 beskriver sårhelingspræparater omfattende TGF-β og et akti verende middel valgt blandt TGF-α og EGF. Disse præparater kan yderligere indeholde en sekundær vækstfaktor som for eksempel PDGF.
Opfindelsen angår et sårhelingspræparat omfattende renset blodpladeafledt 10 vækstfaktor og renset transformerende vækstfaktor-α, uden indhold af transformerende vækstfaktor-β. TGF-α er foretrukkent human TGF-α, men kan også være fra en anden pattedyreart, f.eks. rotte. TGF-α kan isoleres fra naturlige kilder eller, mere foretrukkent, fremstilles af rekombinante celler eller ved fastfasepeptidsyntese. Præparatet ifølge opfindelsen hjælper ved sårhe-15 ling, i det mindste delvist, ved at fremme væksten af epithelialvæv og bindevæv og syntesen af totalpnotein og kollagen. Sårheling under anvendelse af præparatet ifølge opfindelsen er mere effektiv end den, der opnås uden behandling (dvs. uden påføring af exogene midler) eller ved behandling med renset PDGF alene eller renset TGF-α alene. Opfindelsen angår også et 20 præparat omfattende renset blodpladeafledt vækstfaktor og renset transformerende vækstfaktor-α til anvendelse ved heling af sår, uden indhold af transformerende vækstfaktor-β. '
Ved foretrukne udførelsesformer for opfindelsen fremstilles præparatet ved i 25 et farmaceutisk acceptabelt bæremedium, f.eks. i handelen tilgængelige inerte geler eller væsker (f.eks. saltvand suppleret med albumin eller methylcellu-lose) at kombinere renset PDGF og TGF-α (der begge er tilgængelige i handelen). Mest foretrukkent kombineres renset PDGF og TGF-α i et vægtforhold på mellem 1:4 og 25:1, foretrukkent mellem 1:2 og 10:1, og mere fore-30 trukkent 1:1 eller 2:1. Den rensede PDGF kan være opnået fra humane blodplader eller ved rekombinant DNA-teknik. Udtrykket "PDGF" omfatter således 3 DK 175947 B1 såvel blodpladeafledte som rekombinante materialer fra pattedyr, foretruk-kent primater, særlig foretrukkent mennesket, men kan også være fra en chimpanse eller en anden primat. Rekombinant PDGF kan være rekombinant heterodimer fremstillet ved indsætning i dyrkede prokaryote eller eukaryote 5 celler af DNA-sekvenser kodende for begge underenheder, og derpå tillade, at translaterede underenheder behandles af cellerne til dannelse af heterodimer, eller DNA kodende netop for en af underenhedeme (foretrukkent β-eller "2"-kæden) kan indsættes i celler, som derpå dyrkes til fremstilling af homodimer PDGF (PDGF-1 eller PDGF-2 homodimer).

Udtrykket "renset" som anvendt heri refererer til PDGF eller TGF-α, som før blanding med det andet stof er 95 vægt-% eller mere PDGF eller TGF-α, dvs. er i alt væsentligt frit for proteiner, lipider og kulhydrater, med hvilke de er naturligt forekommende.

Et renset proteinpræparat vil generelt give et enkelt hovedbånd på en po-lyacrylamidgel for hver PDGF eller TGF-α komponent. Renset PDGF eller TGF-α anvendt i præparatet ifølge opfindelsen er mest foretrukkent rent som bedømt ved amino-terminal aminosyresekvensanalyse.

Opfindelsen angår endvidere anvendelse af blodpladeafledt vækstfaktor og transformerende vækstfaktor-α til fremstillingen af et lægemiddel til heling af sår.
25 Præparatet ifølge opfindelsen tilvejebringer en hurtig, effektiv metode til heling af ydre sår hos pattedyr, f.eks. liggesår, hudødelæggelser og forbrændinger. Præparatet forbedrer bindevævsdannelse sammenlignet med naturlig heling (dvs. uden nogle exogene midler tilsat) eller ren PDGF eller TGF-a alene. Ulig ren PDGF alene fremmer præparatet en signifikant forøgelse i 30 såvel nyt bindevæv som epithelvæv. Det opnåede epithellag er tykkere end det, der dannes ved naturlig heling, eller med TGF-α alene, og indeholder 4 DK 175947 B1 også flere epithelfremspring, der forbinder det med det nye bindevæv; det er således mere fast bundet og beskyttende.
Andre træk og fordele ved opfindelsen fremgår af den efterfølgende beskri-5 velse af særlige foretrukne udførelsesformer derfor, og af kravene.
I det følgende beskrives foretrukne udførelsesformer for opfindelsen.
Ydre sår, f.eks. liggesår og forbrændinger, behandles med PDGF/TGF-a 10 blandinger fremstillet ved at kombinere ren PDGF og TGF-α. Kemisk syntetiseret human og rotte TGF-α er kommercielt tilgængelig fra Peninsula Laboratories (Belmont, CA). Renset rekombinant PDGF og renset PDGF afledt fra humane blodplader er kommercielt tilgængelige fra PDGF, Inc. (Boston, MA), Collaborative Research (Waltham, MA) og Amgen Corp. (Thousand Oaks, 15 CA). Renset PDGF kan ogsåfremstilles som følger.
500 til 1000 enheder vaskede humane blod pladepellets suspenderes i 1 M NaCI (2 ml pr. blodpladeenhed) og opvarmes ved 100 °C i 15 minutter. Den overliggende væske fraskilles derpå ved centrifugering, og bundfaldet extra-20 heres to gange med 1 M NaCI.
Extrakterne kombineres og dialyseres mod 0,08 M NaCI, 0,01 M na-triumphosphatpuffer (pH 7,4) og blandes natten over ved 4 °C med "CM-Sephadex® C-50" bragt i ligevægt med pufferen. Blandingen udhældes derpå 25 i en søjle (5 x 100 cm), vaskes extensivt med 0,08 M NaCI, 0,01 M na-triumphosphatpuffer (pH 7,4) og elueres med 1 M NaCI, mens der opsamles fraktioner på 10 ml.
Aktive fraktioner samles og dialyseres mod 0,3 M NaCI, 0,01 M na-30 triumphosphatpuffer (pH 7,4), centrifugeres og passeres ved 4 °C gennem en 2,5 x 25 cm søjle af "Blue Sepharose" (Pharmacia) bragt i ligevægt med 0,3 5 DK 175947 B1 M NaCI, 0,01 M natriumphosphatpuffer (pH 7,4). Søjlen vaskes derpå med pufferen og delvist renset PDGF elueres med en 1:1 opløsning af 1 M NaCI og ethylenglycol.
5 De delvist rensede PDGF-fraktioner fortyndes (1:1) med 1 M NaCI, dialyseres mod 1 M eddikesyre og lyofiliseres. De lyofiliserede prøver opløses i 0,8 M NaCI, 0,01 M natriumphosphatpuffer (pH 7,4) og passeres gennem en 1,2 x 40 cm søjle af "CM-Sephadex C-50" bragt i ligevægt med pufferen. PDGF elueres derpå med en NaCI-gradient (0,08 til 1 M).

De aktive fraktioner kombineres, dialyseres mod 1 M eddikesyre, lyofiliseres og opløses i et lille volumen 1 M eddikesyre. Portioner på 0,5 ml påføres en 1,2 x 100 cm søjle af "Biogel® P-150" (100 til 200 mesh) bragt i ligevægt med 1 M eddikesyre. PDGF elueres derpå med 1 M eddikesyre, mens der samles 15 fraktioner på 2 ml.
Hver aktiv fraktion indeholdende 100 til 200 mg protein lyofiliseres, opløses i 100 ml 0,4 %'s trifluoreddikesyre og underkastes omvendtfaset højtryksvæskekromatografi på en phényl-"Bondapak®"-søjle (Waters). Eluering med en 20 lineær acetonitrilgradient (0 til 60 %) giver ren PDGF.
PDGF fremstillet ved rekombinant DNA-teknologi kan fremstilles som følger:
Blodpladeafledt vækstfaktor (PDGF) afledt fra humane blodplader indeholder 25 to polypeptidsekvenser (PDGF-1 og PDGF-2 polypeptider; Antoniades, H.N. og Hunkapiller, M. (1983) Science 220:963-965). PDGF-1 indkodes af et gen lokaliseret i chromosom 7 (Betsholtz, C. et al., Nature 320:695-6991. og PDGF-2 indkodes af sis-oncogenet (Doolittle, R. et al., (1983) Science 221:275-277) lokaliseret i chromosom 22 (Dalla-Favera, R. (1982) Science 30 218:686-688). Sis-genet koder for det transformerende protein fra Simian
Sarcoma virus (SSV), som er nært beslægtet med PDGF-2 polypeptid. Det 6 DK 175947 B1 humane cellulære c-sis koder også for PDGF-2 kæden (Rao, C.D. et al., (1986) Proc. Natl. Acad. Sci. USA, 83:2392-2396). Fordi de to polypeptidkæ-der af PDGF kodes for af to forskellige gener lokaliseret i separate chromo-somer, eksisterer der den mulighed, at human PDGF består af en disulfid-5 kædedannet heterodimer af PDGF-1 og PDGF-2 eller en blanding af de to homodimere (homodimer af PDGF-1 og homodimer af PDGF-2) eller en blanding af den heterodimere og de to homodimere.
Pattedyrsceller i kultur inficeret med Simian Sarcoma virus, som indeholder 10 genet kodende for PDGF-2 kæden, blev vist at syntetisere PDGF-2 polypep-tidet og at overføre det til en disulfid-bundet homodimer (Robbins, K. et al.
(1983) Nature 305:605-608). Desuden reagerer PDGF-2 homodimeren med antisera dannet mod human PDGF. Endvidere ligner de funktionelle egenskaber for den sekreterede PDGF-2 homodimer egenskaberne for blod-15 pladeafledt PDGF derved, at DNA-syntese i dyrkede fibroblaster stimuleres, der indføres phosphorylering ved tyrosinresten for et 185 kD cellemembranprotein, og den er i stand til at konkurrere med human (125I)-PDGF for binding til specifikke celleovérflade-PDGF-receptorer (Owen, A. et al., (1984) Science 225:54-56). Tilsvarende egenskaber blev vist for sis/PDGF-2 genpro-20 duktet afledt fra dyrkede normale humane celler (f.eks. humane arterieen-dothelialceller) eller fra humane maligne celler udtrykkende sis/PDGF-2 genet (Antoniades, H. et al., (1985) Cancer Cells 3:145-151).
Den rekombinante PDGF-2 homodimere (der heri kaldes rekombinant 25 PDGF) opnås ved indføring af cDNA kloner af c-sis/PDGF-2 gen i museceller, under anvendelse af en expressionsvektor. c-sis/PDGF-2 klonen anvendt til expression blev opnået fra normale humane dyrkede endothelialceller (Collins, T., et al., (1985) Nature 216:748-750).
7 DK 175947 B1 Sårheling
For at bestemme effektiviteten af PDGF/TGF-α blandinger til at fremme sårheling blev følgende eksperimenter foretaget.

Unge hvide Yorkshire svin (Parson's farm, Hadley, MA) med en vægt på mellem 10 og 15 kg blev fastet i mindst 6 timer før kirurgi og derpå bedøvet. Under aseptiske betingelser blev ryg- og thoraxområdeme klippet, barberet og vasket med mild sæbe og vand. Det område, der skulle såres, blev derpå 10 desinficeret med 70 %'s alkohol.
Sår med en størrelse på 1 cm x 2 cm blev påført ved en dybde på 0,5 mm under anvendelse af en modificeret Castroviejo electrokeratom (Storz, St.
Louis, MO, som modificeret af Brownells, Inc). Sårene resulterede i fuld-15 stændig fjernelse af epithelium såvel som en del af den underliggende der mis (sammenlignelig med en andengradsforbrænding). Individuelle sår blev adskilt med mindst 15 mm usåret hud. Sår, der modtog identisk behandling, var anbragt som en gruppe og adskilt fra andre grupper med mindst 3 cm.
Sår, der ikke modtog nogen vækstfaktorbehandling, var adskilt fra sår, der 20 modtog sådan behandling, med mindst 10 cm.
Sårene blev behandlet direkte ved en enkelt påføring af følgende vækstfaktorer suspenderet i en biokompatibel gel: 1) 500 ng ren human PDGF (renset ved højtryksvæskekromatografi) eller rekombinant PDGF alene; 2) 500 ng 25 ren rekombinant PDGF i kombination med hver af følgende: a) 500 ng human TGF-α; b) 500 ng rotte TGF-a; 3) 500 ng human eller rotte TGF-α alene.
Efter såring blev der udtaget biopsiprøver på dag nr. 3 til og med 10. Biopsi-prøver for histologisk bedømmelse blev udtaget som kiler ca. 3 mm dybe og 30 anbragt i 10 %'s formalin. Prøver til biokemisk analyse og autoradiografi blev opnået under anvendelse af en elektrokeratom. De endelige dimensioner af 8 DK 175947 B1 prøverne var 1,5 mm x 10 mm x 1,5 mm. Tre prøver pr. sår blev samlet til biokemisk analyse, mens to prøver pr. sår blev samlet til autoradiografi. Efter samling blev prøverne opbevaret i koldt Eagle’s Modified Essential Medium (EMEM) suppleret med 10 % føtal kalveserum. Biopsiprøverne blev analyse-5 ret som følger.
Histologisk bedømmelse
Histologiske prøver blev fremstillet under anvendelse af standard-10 paraffinimprægnering og -indlejringsteknik. Fire mikrometer snit blev foretaget og farvet under anvendelse af filtreret Harris hemotoxylin og alkoholisk eosin; de blev derpå iagttaget under et mikroskop. Alle prøver blev blindbe-dømt af to personer med jævnt fordelte point gennem snittene. Bredderne af epithelial- og bindevævslagene blev bedømt under anvendelse af et gitter 15 placeret inde i ocularet i mikroskopet; målingen blev derpå overført til millimeter under anvendelse af et mikrometer beskuet under samme betingelser.
DNA- oa proteinbestemmelse 20 DNA-bestemmelse blev foretaget under anvendelse af en modifikation af metoden af Labarca et al., (1980) Anal. Biochem. 120:344-52. En 50 pliter ali-kvot vævsextrakt i koncentreret ammoniumhydroxid blev sat til 400 μ liter af en pufferopløsning indeholdende 1 M natriumphosphat og 2 M natriumchlorid (pH 7,0); pH for den resulterende opløsning blev indstillet til 7,4 under an-25 vendelse af HCI. Bagefter blev slutopløsningsvoluminet bragt til 500 pliter under opretholdelse af pH på 7,4. Opløsningen blev derpå sat til 2,5 ml pufret opløsning (0,05 M natriumphosphat, 2 M natriumchlorid, pH = 7,4) af Hoesht farvestof (1,14 mg/ml). Fluorescens blev indført ved en anslåningsbølge-længde på 352 nm og emission målt ved 454 nm. Kalvethymus DNA fremstil-30 let ved identisk behandling blev anvendt til at udvikle standardkurver.

Pnoteinindhold i vævsextrakten i koncentreret ammoniumhydroxid blev målt ved Bradford-metoden (Bradford (1976) Anal. Biochem. 72:248-54) med bo-vinserumalbumin som standard.
‘ 5 Resultaterne fra histologisk bedømmelse indikerede, at sår behandlet med kombinationen af renset human PDGF eller rekombinant PDGF og kemisk syntetiseret human eller rotte TGF-α havde tykkere bindevæv og epitheliallag og mere omfattende epithelialudspring, der forbandt disse lag, end sår, der ikke modtog nogen behandling, human eller rotte TGF-α alene eller ren 10 PDGF alene.
De med PDGF/TGF-α behandlede sår havde større indhold af DNA, protein og kollagen.
15 For at bestemme den tilnærmelsesvise dosis for renset PDGF blev ovennævnte forsøg gentaget med undtagelse af, at sårene blev behandlet med 2,5 ng, 5,0 ng og 10 ng PDGF ækvivalenter renset PDGF pr. mm2 sår dis-pergeret i 30 μ I biokompatibel gel. Resultaterne viste, at optimale virkninger blev opnået, når PDGF-indholdet var 5,0 ng/mm2 eller derover.

For at bestemme den passende dosis ren PDGF plus TGF-α blev kombinationer, i hvilke vægtforholdet mellem PDGF og TGF-α lå i området 1:10 til 25:1, bedømt som beskrevet i det foregående. Optimale resultater blev op-'· nået med et forhold mellem 1:1 og 2:1.

3. Præparat ifølge krav 2, kendetegnet ved, at forholdet er mellem 1:2 og 10:1.
4. Præparat ifølge krav 3, kendetegnet ved, at forholdet er ca. 1:1 15 eller 2:1.
5. Præparat omfattende renset blodpladeafledt vækstfaktor og renset transformerende vækstfaktor-α, til anvendelse ved heling af sår, men ikke indeholdende transformerende vækstfaktor-β. 20
6. Anvendelse af blodpladeafledt vækstfaktor og transformerende vækstfaktor-a til fremstilling af et medikament til heling af sår, hvilket medikament ikke indeholder transformerende vækstfaktor-β.
7. Fremgangsmåde til fremstilling af et præparat til heling af sår, omfattende t blanding af renset blodpladeafledt vækstfaktor og renset transformerende vækstfaktor-α uden tilstedeværelse af transformerende vækstfaktor-β.
