Definitions
- the invention relates to a device for needle biopsy with a syringe cylinder, a plunger which can be displaced therein and with a needle device.
- the aspiration is done either by hand by pulling the plunger out of the interior of the syringe barrel or with auxiliary mechanisms, e.g. B. performed a spring-actuated, triggerable movement of the stamp.
- syringe for short, which can usually be attached to the cone of the syringe barrel with an attachment.
- the skin is turned on at the beginning of the procedure the puncture site is cleaned and disinfected and then, with or without additional anesthesia, the needle is inserted into the thyroid tissue and by suction, which is done by tightening the plunger in the syringe barrel, cells are brought into the needle channel or possibly into the interior of the syringe barrel
- suction which is done by tightening the plunger in the syringe barrel
- Such puncturing can be carried out blindly or under ultrasound control and the cells obtained are then transferred into a vial for further diagnostic purposes, for example microscopic or chemical examinations.
- a fine needle biopsy of the thyroid is often preferred because it is almost painless and only a small one Complication rate. '
- a disadvantage of the known fine needle biopsy is that only a single needle is used and therefore material is only supplied from a certain point. However, there may also be a tumor in the immediate vicinity of healthy material, which may be missed.
- a fine-needle biopsy which is not meaningful, can subsequently lead to patients with suspected carcinoma (e.g. cold thyroid nodules of the Thyroid), although the likelihood of carcinoma of the thyroid in cold thyroid nodes is between 5 - 15%.
- a generous surgical strategy, i.e. surgery on all patients who are suspected to have carcinoma, without selection using fine needle biopsy, must be described as unethical because of the unnecessary loss of the organ, high costs, etc.
- WO 01/52742 AI discloses a device with several, in one embodiment three biopsy needles. However, there is no aspiration here, but the needles are moved in channels, cut samples from the tissue and can then be withdrawn.
- devices with a single needle device are also known, with which several samples can be taken one after the other. Such devices are shown for example in US 6083150 or in EP 1250890 A2. With this device, which is also very complex, samples can be collected in succession in one device, but this does not eliminate the problem that multiple punctures and a corresponding burden on the patient are required.
- These known devices are mainly used in biopsies in hollow organs, eg. B. stomach, intestine, in use.
- hypodermic syringe-like devices with multiple needles have also become known.
- DE 1 907296 A shows an adapter for injection syringes, which is used to connect a larger number of injection needles, 7 in the example shown, to an injection device.
- syringe is used and should be used in particular for subcutaneous injections, for example of local anesthetics.
- a similar device is described in DE 30 35 009 AI, in which case the adapter can be screwed onto the outlet of a hypodermic syringe, whereas it is plugged on according to the aforementioned document.
- This device too, is intended to enable a medicinal product to penetrate locally, for example intermuscularly or subcutaneously.
- a suction device for snake bites and insect bites according to DE 202 12 639 Ul likewise has a plurality of needles which are seated in a movable piston which is first pushed down when the device is used, the needles penetrating the skin. Then a second piston is pushed up and sucks in liquid in the sense of removing snake venom. Both pistons are automatically moved by spring force after they have been triggered.
- Another problem with needle biopsy is that when a needle is pulled out after the actual puncturing process, unwanted tissue fluids and / or blood can also be sucked in. This problem is due to the vacuum that is still present in the syringe barrel after the puncturing. If you pull a needle out of the patient's body after aspiration of a target tissue, the needle tip will e.g. pulled through vessels or other tissue parts and liquid or tissue is sucked in, the assessment of which is not desired or required.
- An object of the invention is to provide a simple device with which the above-mentioned disadvantages of fine needle biopsy can be countered.
- the devices should have a simple structure and be correspondingly inexpensive.
- the needle device has a plurality of puncture needles, the channels of which open into the interior of the cylinder and a venting means is provided for the interior of the syringe cylinder, with the aid of which the volume between the base and the plunger is temporary is connectable to the environment.
- a number of tissue samples can be taken simultaneously with a single application, the burden on the patient being significantly lower than if the same number of samples were taken sequentially and the risk of sucking in undesirable substances, which is multiplied with multiple needles reduced.
- a very advantageous embodiment of the device according to the invention is characterized in that it has a stop means which determines the penetration depth of the needles in defines the body limits.
- at least one spacer can be provided as the stop means, which has bores assigned to the needles and can be pushed onto the needles in order to limit their depth of penetration into the body. In this way, the desired depth of penetration can be determined, which is done, for example, after a previous ultrasound examination.
- sample container is assigned to each needle and the sample containers are combined to form a unit which can be temporarily connected to the plurality of needles for emptying the collected samples into the containers. In this way, all samples can be transferred into the sample container by a single movement of the stamp. It is expedient if the sample container unit can be plugged onto the syringe cylinder, wherein a groove-and-bar arrangement on the sample container and cylinder enables an unmistakable assignment of the individual needles to the sample containers. When using such a variant of the device, an assignment of the samples taken to individual areas of the examined organ is ensured.
- a closure means is assigned to the sample containers, wherein this closure means can have a closure for each individual sample container and the closures for the sample containers can be captively connected to them.
- a closure means prevents the samples from becoming dirty or lost immediately after the puncture.
- a filter medium is arranged in the path between the mouth of the channels into the tips of the needles and the interior of the syringe barrel.
- the filter means can advantageously comprise individual filter inserts in the tip-side end region of the needles.
- venting means is formed by at least one overflow channel, which is formed at a distance from the syringe base in the inner wall of the cylinder, the length of the channel in the direction of the cylinder axis allowing the volume between the base and the plunger on the at least one overflow channel can be temporarily connected to the cylinder interior located above the plunger.
- This venting device allows the vacuum to be released and thus the inflow of blood and other undesirable tissue parts when the needles are pulled out.
- the venting means can advantageously be formed by at least one venting bore which passes through the wall of the cylinder and is at a distance from the bottom thereof.
- FIG. 1 Another simple variant provides that a ventilation hole is provided in the cylinder, which in use is closed by means of a closure piece, which, however, can be operated manually in the sense of temporarily releasing the hole.
- a ventilation hole is provided in the stamp, which is closed in use, but can be opened in the sense of a temporary release of the hole.
- At least one indicator projection protruding from the inner wall of the cylinder and which can be overcome by the stamp is provided at a distance from the bottom of the cylinder.
- the puncture needles of the needle device may have at least partially different lengths.

- a device according to the invention for needle biopsy has a syringe cylinder 1 in which a plunger 2 can be displaced.
- the stamp 2 is connected in a known manner via a shaft 3 to a handle 4, so that the stamp in the sense of an aspiration, ie. H. a suction can be drawn from the cylinder 1 or pressed into the cylinder in the sense of ejection.
- FIG. 3 shows the device according to the invention with five attached puncture needles 9, such a needle being shown in FIG. 4.
- Each of the needles 9 has an extension 10 which has an inner cone 11 which mates with the cones 7.
- the approach 10 can be made of plastic and molded onto the steel needle 9.
- the needle 9 itself is advantageously designed as a so-called “atraumatic” needle, ie with a special cut of the tip.
- a filter means can be arranged in the path between the mouth of the needle channel 12 into the needle tip and the interior of the syringe barrel 1 in order to avoid the aspiration of sample material into the interior of the syringe.
- a filter insert 13 is arranged at the inner end of the needle 1 at the point where it opens into the inner cone 11.
- a common cylindrical protective cover 14 is provided for all needles 9, which can be plugged onto the syringe barrel 1 and then covers all needles 9.
- a short, outwardly projecting web 15 which engages in a groove 16 or a slot in the casing when the protective casing 14 is attached. This can improve the fit of the protective cover 14.
- a special meaning of such a web / groove connection is explained in connection with the variant described below.
- FIG. 6 shows a sample container unit 17 belonging to the device according to the invention, which has an annular collar 18 with which it can be placed on the bottom end of the syringe barrel 1.
- a groove 19 in the collar 18 interacts with the web 15 of the syringe barrel 1 when the sample container unit 17 is attached, so that there is a clear association between each of the needles 9 and individual sample containers 20 of the unit 17.
- the web 15 can also serve to determine the position of the device during the biopsy process, for. B. be directed upwards or downwards.
- a preservation liquid can be present in the sample containers 20.
- the entire sample container unit 17 and / or each individual container 20 can be closed with the aid of a closure, not shown here.
- a stop means is also provided within the scope of the invention, which comes into contact with the skin after a penetration of the needles into the body and a further advance of the Inhibits needles.
- Fig. 7 shows schematically a stop ring 21 which surrounds the needles 9 from the outside and, for example, can be screwed onto the outside of the syringe barrel 1, as a result of which the penetration depth denoted by d in FIG. 6 can also be set.
- FIG. 8 Another embodiment of an actuating means is shown in FIG. 8.
- a spacer 22, which is cylindrical and has holes 23 assigned to the needles 9, can be pushed over the needles 9 in the manner shown until it lies against the outside of the bottom of the syringe barrel 1. With a set of spacers of different thicknesses, the penetration depth of the needles 9 can be adjusted as desired.
- Fig. 9 shows a holding piece 24 which is disc-shaped and defines the position of the needles 9 to each other.
- This holding piece 24 can consist of plastic and can be produced in one piece with the lugs 10, which also consist of plastic.
- Such a holding piece can also - alternatively or additionally - be arranged in the area of the metallic needle shafts 9, which is indicated by a holding piece 25 shown in broken lines in FIG. 9.
- each needle can be connected individually to the syringe barrel 1, which is done via individual lugs 10 and cones 7.
- several needles or needle shafts can also be connected to a single attachment, which is shown in FIG. 10.
- the tip cylinder 1 has a single cone 7.
- Five puncture needles 9, of which only three can be seen because of the selected side view, are arranged in a fork-like manner and open into a single attachment 10 which can be detachably connected to the cone 7 of the syringe cylinder 1.
- the shafts of the needles 9, as in the previous embodiments expediently run parallel to one another.
- FIG. 10 shows yet another sample container unit 26, in which the individual sample containers 27 each have a closure 28 which can be folded away and is captively connected to the container.
- the variant of the invention shown in FIG. 11 has an essentially cylindrical adapter 29, which is used to connect a syringe barrel 1 to several, for example five, puncture needles 9.
- the syringe cylinder 1 here has only a single cone 30 which can interact with an inner cone 31 which lies in an end face 32 of the adapter 29.
- the bore of the cone 30 is in fluid-conducting connection with the bores of the cones 7 via channels in the interior of the adapter 29, in the present case via a central channel 34 and branch channels 35 leading from this to the five cones 7.
- the adapter 29 is plugged onto the syringe barrel 1, the cone 30 cooperating in a sealing manner with the inner cone 31.
- the end face 32 facing the syringe barrel 1 is designed such that it can rest on the bottom 5 of the syringe barrel 1 when the adapter 29 is attached to the syringe barrel 1. This ensures the required stability when inserting the needles 9.
- an anti-rotation device can be provided between the syringe cylinder 1 and the adapter 29, for example in the form of a web 36 projecting from the syringe cylinder 1, which, when the adapter 29 is attached, engages in a groove 37 formed in the end face 32 thereof - similar to the web-groove connection 15-19 shown in FIG. 6.
- a filter means 13 can prevent puncture material from penetrating into the interior of the cylinder. As shown, this filter means 13 can sit in the individual needles 9 in the form of filter inserts 13. Alternatively, a single filter insert can also sit in the central channel 34 of the adapter 29, which is expediently made of plastic.
- FIG. 11 can also be used with a sample container according to FIG. 9, for which purpose a web, similar to the web 15 in FIG. 6, can be provided on the adapter 29.
- a device for needle biopsy according to the invention can expediently be designed as a disposable device, already sterile packed for use.
- other designs are also possible, especially those in which the aspiration is automatic, e.g. B. by spring force after triggering.
- the device with the needles 9 is placed on the skin and stabbed up to the stop. Now the tissue samples are sucked into the needles 9 either by manually pulling out the plunger 2 or by the aforementioned automatic aspiration.
- a sample container unit 17 according to FIG. 6 is placed on the syringe cylinder and through Depressing the stamp 2, the samples are transferred to the individual sample containers 20, which are then closed with a suitable closure and are available for further analysis of the samples.
- the device according to the invention can also be designed to be reusable, for example like a known multi-pipette system which has a single handle and is used for simultaneously sucking in different samples from pipettes.
- tissue samples on surgically opened body parts e.g. B. during a surgical procedure.
- samples can be taken at different depths, and on the other hand the force required for piercing is reduced since not all needles have to penetrate skin or tissue at the same time.
- the needles can be permanently connected to the syringe barrel, so that no further preparatory actions are required after a corresponding disposable device has been removed from its sterile packaging.
- FIG. 12 Such a design with non-releasably provided needles 9 can be seen in FIG. 12.
- the needles sit very closely together, in the present case five needles 9, which can be covered by a protective cover 14, similar to that according to FIG. 5.
- the embodiment according to FIG. 12, like the embodiments of the following FIGS. 13 and 14, has as a special feature a venting means, the purpose and configuration of which is described below.
- tissue is drawn into the interior of the syringe by pulling on the plunger and finally the needles are pulled out of the patient's body again. It has been shown that the latter process can cause problems insofar as there is usually still a negative pressure inside the cylinder after tissue aspiration. In the course of pulling out the needles, undesired material, especially blood from blood-rich tissue, is also sucked in, which is an undesirable material for the actual biopsy.
- a venting means is provided for the interior of the syringe barrel 1, with the aid of which the volume between the base 5 and the plunger 2 can be temporarily connected to the environment.
- this venting means is formed by a plurality, for example three, overflow channels 37, which are formed at a distance c from the syringe base 5 in the inner wall li of the cylinder 1.
- the length 1 of the channels 37 is dimensioned in the direction of the cylinder axis a such that the volume between the base 5 and the stamp 2 is connected to the cylinder interior lying above the stamp when the densest, here plate-shaped section of the stamp moves over the channels 37 or is in the region thereof
- at least one indicator protrusion 41 can protrude inward from the inner wall of the cylinder, which can be overcome when the stamp 2 is pulled out, but can be felt by the doctor that the aspiration target volume has now been reached and when the stamp 2 is pulled out further the desired ventilation takes place via the channels 37.
