Abstract
Un stent (10, 22, 40, 80) de forma generalmente cilíndrica que comprende una pluralidad de celdas interconectadas (24, 42, 62, 82) que tienen unas partes de extremo relativamente anchas (26, 44, 64, 84) unidas por una parte de centro estrecha (28, 46, 66, 86), las celdas (24, 42, 62, 82) dispuestas extremo con extremo, longitudinales en filas longitudinales con respecto al eje longitudinal del stent, caracterizado porque las celdas longitudinalmente adyacentes (24, 42, 62, 82) se acoplan por pares de órganos de soporte longitudinales (30, 50, 68, 88) que tienen partes de extremo curvas (32, 52, 70, 90).
Description
Stenst de configuración mejorada.
Esta invención se refiere a stents de
configuración mejorada.
Los stents son endoprótesis expansibles
radialmente que son normalmente implantes intravasculares capaces
de ser implantados transluminalmente y agrandados radialmente
después de ser introducidos percutáneamente. Se han implantado
también en tractos urinarios y conductos biliares. Se usan para
reforzar los vasos del cuerpo y para prevenir reestenosis después
de angioplastia en el sistema vascular. Pueden ser de autoexpansión
o expandirse por una fuerza radial interna, como cuando se montan
en un balón.
En el pasado, los stents han sido generalmente
tubulares pero han estado compuestos por muchas configuraciones y
se han hecho de muchos materiales, entre ellos metales y plástico.
Se han usado metales corrientes como acero inoxidable de igual modo
que metales de memoria de forma como Nitinol y similares. Los stents
también se han hecho de materiales plásticos biodegradables. Dichos
stents se han formado de alambre, tubo semiacabado, etc.
El documento WO-97/33.734 en el
que se basa el preámbulo de la reivindicación 1, desvela marcadores
radioopacos descritos en forma de remaches hechos de un material
que es más radioopaco que la sustancia del stent de manera que
puede identificarse la posición del stent. El stent tiene una
pluralidad de bandas en serpentín conectadas por conectores que se
extienden desde posiciones a medio camino entre picos y valles de
las bandas en serpentín.
El documento EP-0.875.215
desvela un stent que tiene una pluralidad de segmentos sucesivos que
tienen una forma de serpentín con partes de bucle que tienen
concavidad secuencialmente opuesta. Las partes de bucle están
conectadas por partes de conector y puentes.
El documento EP-0.893.107
desvela un stent que tiene una pluralidad de arcos circunferenciales
y una pluralidad de montantes diagonales que forman una pluralidad
de celdas de perímetro cerradas.
Esta invención proporciona nuevas
configuraciones de las celdas que forman los stents que pueden
adaptarse a todos los diversos tipos de los stents de la técnica
anterior descritos anteriormente y/o conocidos previamente en la
técnica. Existen numerosas ventajas en las nuevas configuraciones.
Las configuraciones de la invención limitan el retroceso y añaden
resistencia a la compresión para un stent expandido, entre otras
cosas. Se contemplan configuraciones distintas a las cilíndricas,
por ejemplo, cuadradas, triangulares, octagonales, etc. Los stents
de esta invención son longitudinalmente flexibles y expansibles.

En las fig. 1 a 4 se ilustra un ejemplo de un
stent generalmente cilíndrico 10 no según la invención. Comprende
un tubo metálico según se muestra en las fig. 2 y 4, como nitinol o
acero inoxidable preferentemente, que se ha grabado o
preferentemente cortado con láser en la configuración mostrada en la
vista en planta plana de la fig. 1. En la fig. 3 se muestra un
detalle aumentado de la fig. 1. La configuración está formada por
una serie de elementos de celdas de expansión curvilíneas indicados
generalmente en 12 (véase ejemplo sombreado en la fig. 3 para mayor
claridad) que tienen partes de extremo relativamente anchas 14
unidas por partes de centro relativamente estrechas 16. Las celdas
12 están dispuestas longitudinalmente según se muestra en la fig. 1
de extremo con extremo con respecto al eje longitudinal del stent 10
y en filas sustancialmente paralelas como también se muestra en la
fig. 1. Se incluye una pluralidad de órganos de soporte alargados 18
que se extienden longitudinalmente, cada uno de los cuales están
dispuestos entre filas de celdas adyacentes 12. Asimismo, se coloca
también una pluralidad de órganos de soporte que se extienden
circunferencialmente 19, preferentemente sustancialmente normales a
órganos de soporte 18 entre las filas de celdas 12 para intersecar
partes de los órganos de soporte 18 y para interconectarlos con las
partes de centro estrechas 16 de las celdas 12. Como puede verse en
la fig. 1a, las celdas 12 también pueden disponerse en una
disposición escalonada. Las fig. 1b y 1c muestran diferentes
disposiciones e interconexiones para celdas 12.
Cuando el stent está expandido, según se muestra
en la fig. 4, en un balón 20 las celdas 12 adoptan una nueva
configuración según se muestra, con los órganos formando el stent
que se indica por los mismos números según se usa en la fig. 1 y la
fig. 3. De nuevo, se muestra una celda sombreada para mayor
claridad.
En referencia ahora a las fig. 5 a 7, se muestra
una forma de realización de stent indicada generalmente en 22 de la
invención. En esta forma de realización, como se ve en las fig. 5 y
6, las celdas de expansión 24, vistas mejor en el detalle de la
fig. 6 e indicadas por la parte sombreada, tienen partes de extremo
relativamente anchas 26, que se ven mejor en la fig. 6, y partes de
centro estrechas 28 y están dispuestas extremo con extremo en filas
longitudinales según se describe con respecto al primer ejemplo. Las
partes de extremo adyacentes 26 están interconectadas por pares de
órganos de soporte longitudinales en forma de segmentos 30 que
tienen partes de extremo curvas 32. Los segmentos que se extienden
circunferencialmente 34 se extienden entre filas de celdas 24 para
interconectar las partes de centro estrechas 28.
Con la expansión radial del stent, por ejemplo
como un balón 20, su configuración cambia por fuerza de deformación
en las direcciones mostradas por las flechas en la fig. 6 a la
configuración mostrada en la fig. 7. Los elementos indicados en la
fig. 7 se identifican por los mismos números indicados para
elementos similares en las fig. 5 y 6.
Las fig. 20 y 21 muestran una configuración en
cierto modo similar a la de las fig. 5 a 7 pero sin elementos de
interconexión 28.
En referencia ahora a las fig. 8 a 10, se
muestra otra forma de realización de stent de la invención y
generalmente se indica en 40. De nuevo, como se observa en las fig.
8 y 9, las celdas de expansión 42 (ejemplo sombreado para mayor
claridad) tienen partes de extremo relativamente anchas 44 y partes
de centro estrechas 46. Las partes de extremo incluyen partes de
bucle que se extienden hacia dentro 48. Las celdas 42 se disponen
extremo con extremo en filas longitudinales como en la forma de
realización precedente. Las partes de extremo adyacentes 44 están
interconectadas por pares de segmentos de órganos de soporte
longitudinales 50 que tienen partes de extremo curvas 52. Los
segmentos que se extienden circunferencialmente 54 se extienden
entre filas de celdas 42 para interconectar las partes de centro
estrechas 46 de las celdas. La fig. 8a muestra una variación de
forma para las celdas 42.
Con la expansión radial del stent en un balón
20, la configuración cambia a la mostrada en la fig. 10. Las flechas
muestran la dirección de fuerza de deformación con la expansión.
En referencia ahora a las fig. 11 y 12, se
muestra otra forma de realización más de un stent 60. De nuevo,
según se muestra en las fig. 11 y 12, las celdas de expansión 62
(ejemplo sombreado para mayor claridad) tienen partes de extremo
relativamente anchas 64 que tienen un ligero pliegue hacia dentro 65
hacia ellas y partes de centro estrechas 66. Las celdas 62 están
dispuestas extremo con extremo en filas longitudinales como en las
formas de realización precedentes. Las partes de extremo adyacentes
64 se interconectan por pares de segmentos de órganos de soporte
longitudinales 68 que tienen partes de extremo curvas 70. Los
segmentos que se extienden circunferencialmente 72 se extienden
entre filas de celdas 62 para interconectar las partes de centro
estrechas 66 de las celdas.
La referencia a la fig. 13 mostrará la
flexibilidad inherente de los stents de esta invención.
Con la expansión radial del stent en un balón
20, la configuración cambia a la mostrada en la fig. 14.
En referencia ahora a las fig. 15 y 16, se
muestra otra forma de realización más de un stent 80 en una
configuración bastante similar a la de las fig. 11 a 14 pero con un
elemento estructural añadido que se extiende circunferencialmente
81. De nuevo, como se observa mejor en la fig. 16, las celdas de
expansión 82 (ejemplos sombreados para mayor claridad) tienen
partes de extremo relativamente anchas 84 que tienen un ligero
pliegue hacia dentro 85 hacia ellas y partes de centro estrechas
86. Las celdas 82 están dispuestas extremo con extremo en filas
longitudinales como en las formas de realización precedentes. Las
partes de extremo adyacentes 84 se interconectan por pares de
segmentos de órganos de soporte longitudinales 88 que tienen partes
de extremo curvas 90. Los segmentos que se extienden
circunferencialmente 92 se extienden entre filas de celdas 82 para
interconectar las partes de centro estrechas 86 de las celdas. Los
segmentos que se extienden circunferencialmente 81 interconectan
pares de segmentos de órganos de soporte 88.
Con la expansión radial del stent en un balón
20, la configuración cambia a la mostrada en la fig. 17.
En referencia ahora a las fig. 18 y 19, se
muestra otro ejemplo más de una configuración de stent 100. Como
antes, esta forma de realización es similar a la de las fig. 11 y 12
excepto porque los segmentos que se extienden circunferencialmente
101 están dispuestos de forma diferente a la identificada en las
fig. 11 y 12 como 72. En este ejemplo los órganos que se extienden
circunferencialmente 101 se extienden entre los extremos adyacentes
de celdas adyacentes 103 (ejemplos sombreados para mayor claridad)
para interconectar la parte superior de un extremo a la parte
inferior del extremo adyacente y los órganos 101 tienen una ligera
curva o pliegue 105 en su longitud. Los otros órganos están
numerados todos de forma similar a las figuras precedentes.
La fig. 22 muestra otro ejemplo más de un stent
formado por celdas 120 que tienen órganos de interconexión que se
extienden circunferencialmente 122. Las celdas tienen lados u
órganos de extremo comunes 124 y están dispuestas en grupos para
formar bandas 12 6 que se interconectan por celdas unidas 128.
Aunque esta invención puede realizarse de muchas
formas diferentes, en la presente memoria descriptiva se describen
en detalle formas de realización de la invención preferidas
especificas. Esta descripción es una ejemplificación de los
principios de la invención y no pretende limitar la invención a las
formas de realización particulares ilustradas.
Los Ejemplos y la descripción anteriores
pretenden ser ilustrativos y no exhaustivos. Estos ejemplos y

Claims (10)
1. Un stent (10, 22, 40, 80) de forma
generalmente cilíndrica que comprende una pluralidad de celdas
interconectadas (24, 42, 62, 82) que tienen unas partes de extremo
relativamente anchas (26, 44, 64, 84) unidas por una parte de
centro estrecha (28, 46, 66, 86), las celdas (24, 42, 62, 82)
dispuestas extremo con extremo, longitudinales en filas
longitudinales con respecto al eje longitudinal del stent,
caracterizado porque las celdas longitudinalmente adyacentes
(24, 42, 62, 82) se acoplan por pares de órganos de soporte
longitudinales (30, 50, 68, 88) que tienen partes de extremo curvas
(32, 52, 70, 90).
2. El stent según la reivindicación 1, en el que
cada celda (24, 42, 62, 82) que tiene partes de extremo
relativamente anchas (26, 44, 64, 84) unidas por una parte de
centro estrecha (28, 46, 66, 86) está conectada a una de dichas
celdas adyacentes (24, 42, 62, 82) por medio de un conector recto
que se extiende circunferencialmente (34, 54, 72, 92).
3. El stent según la reivindicación 1, en el que
las celdas interconectadas (24, 42, 62, 82) forman bandas de celdas
que se extienden helicoidalmente (24, 42, 62, 82) que tienen partes
de extremo relativamente anchas (26, 44, 64, 84) unidas por una
parte de centro estrecha (28, 46, 66, 86).
4. El stent según la reivindicación 1, en el que
las celdas interconectadas (24, 42, 62, 82) que tienen partes
relativamente anchas (26, 44, 64, 84) unidas por un centro estrecho
(28, 46, 66, 86) incluyen celdas que tienen un eje longitudinal
paralelo al eje longitudinal del stent.
5. El stent según la reivindicación 1, en el que
las celdas interconectadas (24, 42, 62, 82) que tienen partes de
extremo relativamente anchas (26, 44, 64, 84) unidas por un centro
estrecho (28, 46, 66, 86) incluyen celdas que tienen un eje
longitudinal perpendicular al eje longitudinal del stent.
6. El stent según la reivindicación 1, en el que
las celdas interconectadas (24, 42, 62, 82) que tienen partes de
extremo relativamente anchas (26, 44, 64, 84) unidas por un centro
estrecho (28, 46, 66, 86) incluyen celdas que tienen un eje
longitudinal perpendicular al eje longitudinal del stent y celdas
que tienen un eje longitudinal paralelo al eje longitudinal del
stent.
7. El stent según la reivindicación 1, teniendo
las partes de extremo relativamente anchas (44) partes de bucle que
se extienden hacia dentro (48).
8. El stent según la reivindicación 1, teniendo
las partes de extremo relativamente anchas (64, 84) un ligero
pliegue hacia dentro (65, 85).
9. El stent según la reivindicación 2,
comprendiendo además el stent una pluralidad de segmentos que se
extienden circunferencialmente (81) que interconectan pares de
órganos de soporte longitudinales (88).
10. El stent según la reivindicación 2, con el
conector recto que se extiende circunferencialmente (34, 54, 72, 92)
interconectando la parte de centro estrecha (28, 46, 66, 86) de las
celdas (24, 42, 62, 82).
