Description
Un stent para angioplastia y procedimiento asociado de producción
La presente invención se refiere a un stent para angioplastia y a un procedimiento asociado de producción delmismo.
Con el término “stent para angioplastia” se pretende indicar dispositivos generales concebidos para aplicación endoluminal (por ejemplo, en el interior de un vaso sanguíneo) en asociación con la técnica de la angioplastiacoronaria transluminal percutánea o PTCA, que habitualmente se efectúa por cateterización de un sitio estenótico.
La expansión del stent en el sitio provoca que el lumen se expanda dando origen a la consecuente eliminación de laestenosis y el soporte local del lumen por el stent, que se deja en posición expandida, evita la restenosis del sitiotratado debido a la relajación posterior de la pared del vaso sanguíneo. El uso de una estructura sustancialmente similar para desplegar los injertos vasculares y fijarlos en el lugar ya ha sido propuesto en la técnica: naturalmente,esta posible extensión del campo de aplicación deber verse como incluida en el ámbito de la presente invención.
Para una revisión general de stents vasculares, es útil hacer referencia al trabajo “Textbook of InterventionalCardiology” editado por Eric J. Topol, W.B. Saunders Company, 1994 y, en particular, a la sección IV del volumen II,titulado “Coronary Stenting”.
Muchos documentos de patente han tratado dicho problema, por ejemplo, los documentos US-A-4 776 337, US-A-4800 882, US-A-4 907 336, US-A-4 886 062, US-A-4 830 003, US-A-4856 516, US-A-4 768 507 y US-A-4 503 569.
La figura 7 del documento EP-A-747 069 revela un stent, donde un material bioactivo, por ejemplo fármacos omedicamentos, se encuentra sobre la parte exterior de la base de metal de los stents, que ha procesado toda susuperficie. La superficie externa del stent es una capa porosa para controlar la liberación de fármacos.
La implantación de estos dispositivos, que es un factor en el tratamiento de diversas enfermedades cardiacas, puederequerir, o al menos obtener beneficios particulares de la posibilidad de ser capaces de administrar en el sitio deimplantación del stent agentes o principios activos (los dos términos utilizados a continuación en un sentido equivalente) que tienen diversos fines: pueden ser, por ejemplo, agentes antitrombogénicos o, más generalmente,agentes para resistir directamente la restenosis del sitio tratado debido a la formación de depósitos, proliferación de tejidos, etc. Relacionado con esto, es útil hacer referencia a los siguientes trabajos:

Muchos de los problemas aplicacionales surgen de este modo de operación, relacionados en su mayor parte con lassoluciones específicas adoptadas. Por ejemplo, existe el problema de evitar el agente o agentes concebidos para administración en la zona donde se suministra o transporta el stent a diferentes áreas en las que pueden tenerefectos negativos o perjudiciales. Pueden surgir otros problemas, por ejemplo, a la hora de asegurar la permanenciay la liberación gradual en el tiempo de sustancias activas que pueden ser, como si dijéramos, lavadas por la sangreque pasa a través del stent.
Estos problemas no se pueden evitar o resolver por sí mismos recurriendo a otras soluciones como stents radiactivos o denominados también stents biodegradables, como se ilustra, por ejemplo, en el trabajo “Biodegradable Stents: The Future of Interventional Cardiology” de M. Labinaz y col.; Journal of InternationalCardiology, Vol. 8, n. 4, 1995, pág. 395-405. Los stents radiactivos propuestos públicamente hasta la fecha dan origen a otros problemas relacionados esencialmente con el hecho de que, en la mayoría de los casos, su usoasume las características típicas de la radioterapia y/o la medicina nuclear. La principal desventaja de los stents biodegradables es que, al menos a largo plazo, cuando el stent se ha degradado completa o sustancialmente, hayuna reducción en el soporte mecánico de la pared del vaso sanguíneo contra el riesgo de colapso.
Como otra solución para administrar diversas clases de principio activo en el sitio de implantación del stent,recientemente se ha propuesto una solución en la que al menos una parte de la superficie del cuerpo del stent (odispositivo de implantación en general) se reviste con un receptor capaz de unirse a un ligando formado por combinación de un principio activo con una sustancia capaz de unirse al receptor.
Para que esa nueva solución sea completamente provechosa, es decir, para que también se pueda utilizar contécnicas más convencionales para la administración tópica eficaz de los principios activos, parece importanteobtener una buena adhesión y/o retención sobre el stent de la sustancia o sustancias a las que están asociadosestos principios activos y/o se pretende que estén asociados.
En relación con esto, es necesario, por tanto, tener en cuenta diversos factores concomitantes que a menudo seoponen entre sí.
En un número significativo de aplicaciones es importante que los principios activos estén presentes, principalmente,aunque no exclusivamente, sobre la superficies exterior del stent. A la inversa, normalmente es deseable que lasuperficie interna del propio stent sea tan inerte como sea posible, es decir, tanto del punto de vista químico comodesde el punto de vista del posible anclaje mecánico de los posibles depósitos.
Esta es la razón por la que los stents vasculares normalmente disponibles están sometidos a un proceso de pulido, cuyo objetivo es hacer más suave la superficie del stent (tanto la del interior como la del exterior). Relacionado conesto, también es posible revestir el stent con una capa de material biocompatible, como por ejemplo un material decarbono biocompatible (depositado, por ejemplo, utilizando técnicas de pulverización catódica), para conferir un altogrado de homocompatibilidad sobre todo el stent. Adoptando esta técnica para la deposición de dicha capa, dadaslas minúsculas dimensiones de un stent para angioplastia, significa que es prácticamente imposible limitar ladeposición solo a la superficie interior del stent. Por tanto y en consecuencia, se reviste la superficie completa delstent con una capa que, por su naturaleza, hace que la deposición de sustancias sobre el propio stent sea, dehecho, imposible.
Hay otro aspecto que no debe olvidarse: un stent para angioplastia es, por su naturaleza, una estructura muyabierta, habitualmente una estructura de tipo malla en la que, especialmente en la posición radialmente extendida, la superficie eficaz concebida para entrar en contacto con la pared del vaso sanguíneo es una pequeña fracción delárea superficial teórica del tubo definida por el exterior del propio stent. En otras palabras: incluso teniendo en menorconsideración los otros problemas descritos, hay muy poca superficie disponible sobre el stent para llevar losprincipios activos deseados para administración local.
El objeto de la presente invención es resolver las ventajas descritas anteriormente.
En particular, la solución según la invención, que tiene las características expuestas en las reivindicaciones siguientes, permite la aplicación selectiva, específicamente a la superficie externa sólo del stent, de una cantidadcompletamente eficaz de principio activo (tanto directamente como en forma de un receptor capaz de unirse a unligando que lleva el principio activo) sin perder, por ello, la posibilidad de tener una superficie muy suave, al menosen el interior del stent, incluso si está revestido con revestimientos como por ejemplo revestimientos de carbono homocompatibles.
La invención, que se refiere a un stent, así como al procedimiento asociado para la fabricación del mismo, sedescribe ahora haciendo referencia a los dibujos adjuntos, que comprenden las Figuras 1 a 6, que ilustran diferentesformas posibles de realización de la invención.
En todas las Figuras adjuntas, la referencia numérica 1 indica una parte de pared de un stent para angioplastia.Como ejemplo, se pueden considerar las Figuras 1 a 6 como vistas parciales en una escala aumentada de unsegmento de un stent en la sección transversal. Dicha sección, normalmente, tiene forma circular indiferentementede que el stent esté radialmente contraído o radialmente expandido.
Los detalles específicos de construcción del stent y, en particular, su geometría, son factores que están claros en símismos en el contexto de la invención y que se aplican indistintamente de la estructura particular del stent. Esto es también sustancialmente cierto por lo que se refiere a la técnica básica de fabricación (por ejemplo, que parte de uncable o microtubo que después se somete a una operación para cortar las aberturas, por ejemplo, utilizando láser)y/o al material (normalmente metal) del que suele estar hecho el stent. Todos estos factores están relacionados conun volumen bastante grande de bibliografía y no requiere una descripción detallada en la presente memoria.
Fundamentalmente, la invención proporciona la formación del esculpido de la superficie del stent 1, al menos – ypreferiblemente – sobre una parte o sobre toda la superficie externa, indicada como 2, y que cuyo objetivo es,sustancialmente:

- aumentar el área superficial teórica del stent para fomentar la aplicación de los revestimientos, como losconcebidos para llevar o unirse a los principios activos,
- -
- crear en cualquier caso oquedades y asperezas para formar sitios de anclaje para las sustancias, sin requerir sitios específicos de adhesión superficial y, como ventaja complementaria,
- -
- mejorar la unión del stent a la pared del vaso sanguíneo que esté ya en fase aguda, específicamentepreviniendo los movimientos relativos que pueden dar lugar a microlesiones.
Como aclaración, el término “esculpido” se utiliza para distinguir claramente la conformación superficial atribuida alstent según la invención a partir del grado de la aspereza de la superficie (residual) que las superficies del stenttienen en cualquier caso, incluso cuando se han sometido previamente, según convenio, a un procedimiento deacabado o pulido.
A modo de ejemplo, un cierto tratamiento confiere un grado tal de rugosidad residual sobre las superficies del stentque las distancias entre el pico y el seno reconocibles en una sección teórica de la superficie en cuestión en ángulorecto respecto a la propia superficie no son, en ningún caso, mayores de aproximadamente 2-3 micrómetros.
El grado de irregularidad superficial, o de esculpido, característico de la invención es, en cambio, tal que lasdistancias entre el pico y el seno encontradas en condiciones similares son, típicamente, de aproximadamente 10-20micrómetros, es decir, con la posibilidad de alcanzar valores de un orden de magnitud incluso mayor que los delacabado superficial normal de un stent.
Las Figuras 1 a 6 ilustran, en orden de preferencia actual, diferentes técnicas que se pueden utilizar para conferir elgrado deseado de esculpido sobre la superficie 2.
En particular, la Figura 1 se refiere a la aplicación de microesferas 3 formadas a partir del mismo material(normalmente metal) que el stent o de diferentes materiales con el posterior anclado de las microesferas (cuyodiámetro medio es de aproximadamente 10-20 micrómetros) utilizando el procedimiento conocido como “fusión parcial en caliente”. Este es un procedimiento conocido en la técnica y se utiliza, por ejemplo, para conferir a unasuperficie una apariencia aproximadamente similar a la apariencia superficial característica de una pieza a máquinaobtenida por sinterización de las superficies de piezas a máquina mecánicas concebidas para diversos propósitos. Apartir de esto, se entiende que tal configuración también se puede practicar junto con un stent efectuado, como untodo, o al menos en las partes correspondientes al esculpido de la superficie, mediante sinterización.
La Figura 2 se refiere a una variante de la configuración mostrada en la Figura 1, en la que, mientras se retiene lamisma irregularidad superficial típica, se usan gránulos 4 con forma irregular en lugar de las microesferas 3. En estecaso también se aplican las mismas observaciones hechas anteriormente con respecto a la posibilidad de fabricar elstent, al menos en parte, mediante sinterización.
La Figura 3 muestra otra configuración basada en la deposición de un material receptor utilizando, por ejemplo, técnicas de pulverización catódica o pulverización de plasma para formar una acumulación con forma irregular, porejemplo, que tenga estructura pseudocolumnar.
Desde este punto de vista, la solución según la Figura 1 (la aplicación de microesferas) parece ser la preferidacuando se desea crear oquedades y rugosidades sobre la superficie 2, que tiene una función de anclaje mecánica ycaracterísticas geométricas definidas con precisión que se identifican por el tamaño de grano (que se pueden determinar con precisión) de las microesferas 3.
Por el contrario, la configuración mostrada en la Figura 3 parece preferible cuando el objetivo es maximizar el efectodel aumento del área superficial teórica que se expone eficazmente. Esta última solución es preferible, por tanto,cuando se desea, por ejemplo, aplicar un revestimiento a la superficie del stent 1; dicho revestimiento transporta y/oestá concebido para transportar principios activos y está esencialmente en forma de capa monomolecular.
La configuración según la Figura 2 constituye, hasta un cierto alcance, un tipo de intermedio entre las configuraciones de las Figuras 1 y 3.
Las configuraciones a las que se refieren las Figuras 1 a 3, que comprenden las posibles configuracionesequivalentes, se caracterizan esencialmente por el hecho de que el esculturado superficial se forma mediante laaplicación a la superficie 2 de un material idéntico o diferente al del stent 1. Estas configuraciones son generalmente preferidas cuando se desean zonas con mayores o menores oquedades sobre la superficie 2.
En cualquier caso, objetos sustancialmente similares a los descritos al comienzo de esta descripción detallada sepueden lograr tratando la superficie 2 de las maneras deseadas para conferirle una apariencia general esculpida.
Relacionado con esto, la Figura 4 muestra los resultados obtenidos sometiendo la superficie externa 2 a chorreadocon arena o granallado (término reservado para un tratamiento que – en lugar de utilizar arena como en el chorreado con arena – se efectúa utilizando microesferas (“bolas”) como agentes balísticos, para impactar con la superficietratada).
La Figura 5 ilustra los resultados de una operación de erosión mecánica (incisión o moleteado) que se lleva a cabosobre la superficie externa 2 del stent.
Finalmente, la Figura 6 muestra los resultados obtenidos de un ataque químico localizado (grabado) de la superficie
2. Este procedimiento se lleva a cabo utilizando un material resistente (por ejemplo fotorresistente) que, tras laexposición a través de máscaras o polimerización utilizando rayos de láser, se elimina selectivamente de algunaszonas de la superficie 2 para permitir su ataque. Los rastros resistentes que quedan en la superficie 2 se eliminanmediante lavado.
Esta tecnología es bien conocida en la técnica (por ejemplo, en la fabricación de circuitos integrados) y no necesita explicarse con detalle en este contexto.
También se pueden obtener unos resultados sustancialmente similares a los mostrados en la Figura 5 y, enparticular en la Figura 6, por incisión utilizando rayos de láser, por ejemplo, antes o después de la operación decortado de la estructura del stent a partir de una pieza en bruto constituida por un microtubo de metal.
Como regla general, todas las soluciones mostradas en las Figuras 1 a 6, y cualquier equivalente, se pueden aplicar a stents obtenidos a partir de un microtubo metálico; posiblemente fabricado en su totalidad o en parte mediantesinterización, así como a stents obtenidos a partir de un cable, llevándose a cabo los procedimientos mostradosantes o después de cortar el tubo y/o enrollar el cable. Para simplificar la fabricación, el solicitante ha establecido entodo caso que es preferible formar el esculpido de la superficie antes de cortar el tubo o enrollar el cable.

- REIVINDICACIONES
- 1.
- Un stent para angioplastia que incluye un cuerpo (1) de forma generalmente tubular que tiene una superficieinterna y una superficie externa (2), siendo dicho stent capaz de dilatarse durante su uso desde una posiciónradialmente contraída a una posición radialmente expandida, donde el stent tiene un esculpido superficial (3, 4, 5, 6,7, 8) sobre al menos parte de dicha superficie externa (2), donde dicho esculpido (3, 4, 5, 6, 7, 8) está presente exclusivamente en la superficie externa del stent, con una cantidad eficaz del principio activo aplicado de formaselectiva sobre la superficie externa (2) exclusivamente del stent, siendo la superficie interna del stent (1)sustancialmente suave, estando el stent revestido con material de carbono biocompatible, al menos en dichasuperficie interna.

- El stent de la reivindicación 1, que se caracteriza porque dicho principio activo incluye un agente para resistir directamente la restenosis del sitio tratado.

- Un stent según la reivindicación 11, que se caracteriza porque dichas acreciones (5) son depositadas usando un procedimiento elegido del grupo que comprende pulverización catódica y pulverización de plasma.

- Un stent según cualquiera de las reivindicaciones 1 a 4, que se caracteriza porque dicho esculpido está constituido por las formaciones (6, 7, 8) en la superficie (2) del stent (1).

- El stent de la reivindicación 13, que se caracteriza porque dichas formaciones (6, 7, 8) se obtienen por medio de ataque químico localizado de dicha superficie (2).

- El stent de la reivindicación 13, que se caracteriza porque dichas formaciones (6, 7, 8) se obtienen por incisión por haz láser.

- Un stent según cualquiera de las reivindicaciones 1 a 4, que se caracteriza porque está constituido, en al menos una parte correspondiente a dicho esculpido superficial (3, 4), por un cuerpo sinterizado.
- 17.Un procedimiento para fabricar stents para angioplastia que incluye un cuerpo (1) de forma generalmente tubular que tiene una superficie interna y una superficie externa (2), siendo dicho stent capaz de dilatarse durante su usodesde una posición radialmente contraída a una posición radialmente expandida, que se caracteriza porqueincluye una operación para formar un esculpido superficial (3, 4, 5, 6, 7, 8) sobre al menos parte de dicha superficieexterna (2), donde dicho esculpido (3, 4, 5, 6, 7, 8) está formado solamente en la superficie externa del stent, conuna cantidad eficaz del principio activo aplicado de forma selectiva sobre la superficie externa (2) solamente del stent, estando la superficie interna del stent (1) considerablemente suave, incluyendo el procedimiento la operaciónde formar un revestimiento de material de carbono biocompatible, sobre al menos dicha superficie interna.

- El procedimiento de la reivindicación 17, que se caracteriza porque dicho principio activo incluye un agente para resistir directamente la restenosis del sitio tratado.

- 30. El procedimiento de la reivindicación 29, que se caracteriza porque dichas formaciones (6, 7, 8) se obtienen 20 por medio de ataque químico localizado de dicha superficie (2).
