Abstract
Un injerto óseo, que comprende: un bloque de sustrato cerámico que comprende una primera región de menor porosidad (40) y una segundaregión de mayor porosidad (44), en donde dicha segunda región de mayor porosidad está configurado parafacilitar la conexión por crecimiento y fusión óseos con hueso adyacente del paciente caracterizado porque dicho bloque de sustrato cerámico (410) está formado por un material con una resistencia ala flexión de más de 500 megaPascal y una tenacidad a la fractura de más de aproximadamente 5 megaPascal raízde metro.

ANTECEDENTES DE LA INVENCIÓN
Esta invención está relacionada en general con mejoras en injertos óseos, tales como jaulas de fusión espinal del tipo diseñado para implante humano entre vértebras espinales adyacentes, para mantener las vértebras con una relación espaciada sustancialmente fija al tiempo que se promueve el crecimiento y la fusión óseos entre cuerpos vertebrales entremedio. Más particularmente, esta invención está relacionada con un injerto óseo, tal como una jaula de fusión espinal que tiene una mejor combinación de mayor resistencia mecánica junto con propiedades osteoinductivas y osteoconductivas, en un dispositivo a base de cerámica que proporciona además y de forma beneficiosa radiolucencia para la monitorización postoperatoria.
El documento US-A-6.149.688 representa la técnica anterior más cercana. Los injertos óseos implantables entre cuerpos vertebrales, tales como los dispositivos de fusión espinal son conocidos en la técnica y son utilizados de manera rutinaria por los cirujanos de espina dorsal para mantener vértebras adyacentes con una relación espaciada deseada mientras tiene lugar la fusión y el crecimiento óseos entre cuerpos vertebrales. Tales dispositivos de fusión espinal también se utilizan para proporcionar un soporte de peso entre cuerpos vertebrales adyacentes y de este modo corregir problemas clínicos. Los dispositivos de fusión espinal están indicados para el tratamiento médico de la enfermedad degenerativa del disco, dolor discogénico en la parte baja de la espalda y espondilolistesis. Estas condiciones han sido tratadas utilizando construcciones, típicamente hechas de metales, tales como el titanio o aleaciones de cromo-cobalto, tal como se utiliza en los implantes ortopédicos, y hueso de aloinjerto (donante) o de autoinjerto (paciente) para promover el crecimiento y la fusión óseos.
Los dispositivos típicos de fusión espinal entre cuerpos vertebrales, tal como por ejemplo los tapones, tienen espacios huecos o abiertos que suelen estar rellenos de material de injerto óseo, ya sea material de hueso autógeno proporcionado por el paciente o material de hueso alogénico proporcionado por un tercer donante. Estos dispositivos también tienen ranuras o aberturas laterales que se utilizan principalmente para promover el crecimiento del suministro de sangre y hacer crecer hueso vivo y activo. Estos implantes también pueden tener una superficie exterior con patrón, tal como una superficie ribeteada o serrada o una rosca helicoidal para lograr un mayor trabado mutuo mecánico entre vértebras adyacentes, con un riesgo mínimo de que el implante sea desalojado del sitio. Véase, por ejemplo, las patentes de EE.UU. 5.785.710 y 5.702.453. Los materiales típicos de construcción para tales dispositivos de fusión espinal entre cuerpos vertebrales incluyen los polímeros biocompatibles reforzados con fibra de carbono, aleaciones de cromo-cobalto y acero inoxidable o aleaciones de titanio. La mayoría de los implantes de fusión espinal en el estado actual de la técnica se hacen de aleación de titanio y hueso de aloinjerto (de donante) y han disfrutado de éxito clínico, así como un rápido y generalizado uso debido a los mejores resultados en pacientes. Sin embargo, los dispositivos de implantes a base de titanio exhiben pobres características de radiolucencia, presentando dificultades en la monitorización y evaluación postoperatorias del proceso de fusión debido a la radio-sombra producida por el metal no transparente. También hay evidencia clínica de hundimiento y aplastamiento del hueso que se cree que es atribuible a la incompatibilidad mecánica entre el hueso natural y el material de implante metálico. Por otra parte, los dispositivos de implantes a base de titanio principalmente soportan carga pero no son osteoconductivos, es decir, no son propicios para una conexión mecánica fuerte y directa al tejido óseo del paciente, lo que lleva a una potencial necrosis, pobre fusión y estabilidad del injerto. Por el contrario, los implantes óseos de aloinjerto exhiben buenas propiedades osteoconductivas, pero pueden hundirse con el transcurso del tiempo ya que se asimilan en el hueso natural. Además, sufren de mala resistencia a la tracción, lo que tiene como resultado una escasa estabilidad, debido principalmente a las limitadas opciones en el mecanizado de las superficies de contacto. Los implantes óseos de aloinjerto también tienen propiedades materiales variables y, quizás lo más importante de todo, tienen un suministro muy limitado. También es un factor un pequeño pero finito riesgo de transmisión de enfermedades con aloinjerto óseo. En respuesta a estos problemas, algunos desarrolladores están tratando de utilizar construcciones metálicas porosas a base de tantalio, pero han tenido un éxito limitado debido al escaso módulo de elasticidad de los metales porosos.
Un típico dispositivo de aleación de titanio para fusión espinal se construye a partir de una construcción cilindra hueca similar a una jaula metálica roscada externamente con unas fenestraciones que permiten la comunicación del tejido huésped esponjoso con el núcleo hueco, que está lleno de material de injerto óseo troceado. Este diseño, constreñido por las propiedades materiales de las aleaciones de titanio, se basa en el crecimiento óseo adentro de las fenestraciones inducido por el material de injerto óseo. Sin embargo, la estructura a base de titanio puede formar una capa fibrosa delgada en la interfaz hueso/metal, que degrada la conexión del hueso al metal. Además, el núcleo hueco en el que se rellena el material de injerto puede tener una vascularización y transmisión de tensión subóptimas, de este modo lleva finalmente a un fallo de la incorporación del injerto. Para estimular el crecimiento de los brotes vasculares y la proliferación de las células mesenquimales desde el tejido huésped esponjoso al material de injerto se necesita estabilidad mecánica, transmisión de estrés de fluidos y la presencia de agentes osteoinductivos. Sin embargo, la mayoría de dispositivos de fusión espinal basados en titanio que se usan hoy en día tienen unas tapas extremas o paredes sólidas laterales para evitar la salida del injerto hacia fuera desde el núcleo y la entrada de tejido discal remanente y fibroblastos en el núcleo.
En el pasado se ha utilizado la fusión ósea autóloga (de paciente) y en teoría tiene una mezcla ideal de propiedades osteoinductivas y osteoconductivas. Sin embargo, el suministro de material óseo autólogo es limitado y se sabe que se producen complicaciones significativas con la extracción de huesos. Por otra parte, los costes asociados con la extracción de material óseo de autoinjerto son altos, se requieren dos incisiones independientes, por lo que el paciente tiene que soportar más dolor y recuperación debido a los procesos de extracción e implantación. Además, el material óseo esponjoso autólogo tiene una resistencia mecánica inadecuada para soportar las fuerzas intervertebrales por sí mismo, por lo que el material óseo normalmente se incorpora con una construcción de base metálica.
Los materiales cerámicos proporcionan posibles estructuras alternativas para el uso en prótesis y dispositivos de implantes de fusión espinal, véase por ejemplo la patente de EE.UU. 5.871.517 que describe una prótesis de articulación de cadera que tiene una cabeza de zirconio y una copa cerámica y las patentes de EE.UU. 4.000.525 que describe una prótesis cerámica de óxido de aluminio de alta densidad. En este sentido, se han propuesto construcciones cerámicas monolíticas, formadas a partir de materiales convencionales, tales como hidroxiapatita (HAP) y/o fosfato tricálcico (TCP). Véase, por ejemplo, la patente de EE.UU. 6.037.519. Sin embargo, si bien estos materiales cerámicos puede proporcionar propiedades osteoinductivas y osteoconductivas satisfactorias, no han proporcionado la resistencia mecánica necesaria para el implante.
De este modo, existe una importante necesidad de mejoraras adicionales en el diseño de injertos óseos, tales como dispositivos de implantes de fusión espinal, particularmente con el fin de proporcionar un implante de alta resistencia que tiene características de gran crecimiento y fusión óseos, junto con una sustancial radiolucencia para facilitar una efectiva monitorización posoperatoria.
Por lo tanto, un objetivo de la presente invención es proporcionar un injerto óseo mejorado, tal como una jaula o implante de fusión espinal entre cuerpos vertebrales hechos de estructura biocompatible de poro abierto, que tiene una radiolucencia similar a la del hueso circundante. También un objetivo de la presente invención es proporcionar un sustrato de alta resistencia biomecánica para llevar agentes biológicos que promuevan el crecimiento óseo intervertebral, la curación y la fusión. Un objetivo adicional de la presente invención es proporcionar un dispositivo de fusión entre cuerpos vertebrales que tenga propiedades mecánicas que coincidan sustancialmente con las del hueso natural, mediante el uso de materiales cerámicos de construcción en lugar de metal.
COMPENDIO DE LA INVENCIÓN
Las características de la invención se definen en la reivindicación 1. Según las reivindicaciones, por lo tanto, se proporciona un injerto óseo que comprende un bloque de sustrato cerámico formado de un material con una resistencia a la flexión de más de aproximadamente 500 mega-Pascales y una tenacidad a la fractura de más de cinco aproximadamente megaPascales por raíz de metro.
Dicho injerto óseo puede comprender una jaula de fusión espinal. El injerto óseo, tal como se plasma en forma de una jaula de fusión vertebral de este tipo, puede comprender un bloque de sustrato poroso o de célula abierta formado de composición cerámica que tiene una relativamente alta resistencia biomecánica y capacidad de carga.
Convenientemente, dicho bloque de sustrato cerámico puede ser poroso y tener una relativamente alta resistencia correspondiente sustancialmente con el hueso esponjoso y cortical natural. La porosidad del bloque de sustrato puede ser de aproximadamente el 10% a aproximadamente el 50% en volumen con los poros abiertos distribuidos por todo y un tamaño de poro de aproximadamente 5 a aproximadamente 500 1. Dicho bloque de sustrato cerámico puede tener una porosidad que va desde el 2% a aproximadamente el 80% en volumen y el tamaño de poro puede variar de aproximadamente 5 1 a aproximadamente 1500 1. Convenientemente, dicha porosidad puede variar de aproximadamente el 10% a aproximadamente el 50% en volumen, y los tamaños de poro puede variar de aproximadamente de 100 1 a 500 1. Preferiblemente, los poros formados dentro del bloque de sustrato están en comunicación de fluidos substancialmente abierta suficiente para transmitir presión de fluido entremedio.
Dicho bloque de sustrato poroso cerámico puede comprender una forma de estructura cerámica de alúmina, circonio
o composición de los mismos. Una composición preferida para el bloque de sustrato cerámico comprende una composición de dos fases de alúmina-zirconio que tiene un tamaño adecuado y una forma para la implantación asentada en el espacio intervertebral, con unas caras generalmente planas o con la forma apropiada asentada respectivamente en las placas extremas de vértebras adyacentes. En una forma preferida, el bloque de sustrato puede comprender una composición de alúmina-zirconio con la proporción de alúmina a zirconio comprendida entre aproximadamente el 100% (es decir, 2:1) y aproximadamente el 25% (es decir, 1:4). Más preferiblemente, la composición del bloque de sustrato puede comprender de aproximadamente el 10% a aproximadamente el 20% o 25% en volumen de circonio en alúmina. Para la fase de zirconio se utiliza preferiblemente zirconio estabilizado con itrio (aproximadamente del 2,5 al 5% de mol de itrio en zirconio) o zirconio estabilizado con cerio (aproximadamente del 2,5 al 15% de mol de cerio en zirconio).
Según la invención, el bloque cerámico tiene un gradiente de porosidad variable que imita sustancialmente el hueso esponjoso y cortical natural. De este modo dicho bloque de sustrato cerámico puede tener una primera región de relativamente baja porosidad que imita substancialmente al hueso cortical natural y una segunda región de relativamente alta porosidad que imita substancialmente al hueso esponjoso del paciente.
Dicho bloque de sustrato cerámico puede tener una primera región de menor porosidad y una segunda región de mayor porosidad, en donde dicha segunda región de mayor porosidad está configurada para facilitar la conexión por crecimiento y fusión ósea con hueso adyacente del paciente. En una forma preferida, la porosidad del bloque de sustrato puede ser gradual desde una primera región de porosidad relativamente baja que emula o imita la porosidad del hueso cortical a una segunda región de porosidad relativamente mayor que emula o imita la porosidad del hueso esponjoso.
Dicha primera región puede tener una porosidad de menos de aproximadamente el 5%. Dicha segunda región puede tener una porosidad que va desde aproximadamente el 30% a aproximadamente el 80%. Dicha segunda región puede tener una porosidad superior a aproximadamente el 40%, preferiblemente superior al 45% y más preferiblemente superior a aproximadamente el 50%.
En algunas realizaciones, dicho injerto óseo puede comprender además un revestimiento superficial aplicado al bloque de sustrato, dicho revestimiento superficial tiene propiedades osteoconductivas y osteoinductivas para promover la conexión por crecimiento y fusión óseos entre cuerpos vertebrales con hueso adyacente del paciente. Dicho revestimiento superficial puede aplicarse interna y externamente al bloque de sustrato cerámico poroso. Típicamente, dicho revestimiento superficial puede comprender un revestimiento superficial bioactivo y resorbible. El bloque de sustrato poroso puede revestirse por consiguiente interna y externamente con un material bioactivo de revestimiento superficial seleccionado para tener relativamente altas propiedades osteoconductivas y osteoinductivas, tal como la hidroxiapatita o material de fosfato de calcio.
En algunas realizaciones, dicho revestimiento superficial puede comprender un material osteoinductivo parcial o totalmente amorfo, incluyendo un compuesto de calcio osteoinductivo y vidrio. En algunas realizaciones, dicho revestimiento superficial puede comprender un material orgánico de revestimiento. Dicho material orgánico de revestimiento pueden seleccionarse de aspirados de médula ósea autóloga, proteínas morfogénicas óseas, factores de crecimiento y células progenitoras y mezclas de los mismos. Dichas células progenitoras pueden incluir células madre mesenquimales, células hematopoyéticas y células madre embrionarias. Convenientemente, dichas células progenitoras comprenden células no embrionarias o células obtenidas por métodos que no implican la destrucción de embriones.
Convenientemente, dicho injerto óseo puede incluir uno o más agentes terapéuticos que son llevaos por dicho bloque de sustrato. Dicho uno o más agentes terapéuticos pueden adaptarse para lograr mayor fusión y crecimiento óseos. Estos agentes terapéuticos pueden incluir agentes terapéuticos naturales o sintéticos, tales como proteínas óseas morfogénicas (en inglés BMP, bone morphogenic proteins), factores de crecimiento, aspirados de médula ósea, células madre, células progenitoras, antibióticos u otro material osteoconductivo, osteoinductivo, osteogénico que mejore la fusión o un agente terapéutico beneficioso. En algunas realizaciones, dicho agente terapéutico puede comprender un agente natural o sintético osteoconductivo u osteoinductivo.
Por lo tanto el método puede incluir además las etapas de extracción de dicho agente terapéutico de un paciente y cultivar el agente terapéutico in vitro utilizando el implante de injerto óseo como medio de cultivo y sustrato antes de dicha etapa de implantación. Adecuadamente, dicho método puede comprender las etapas de:
formar un implante cerámico poroso de injerto óseo; extraer de un paciente un agente terapéutico seleccionado de entre aspirados de médula ósea autóloga, proteínas óseas morfogénicas, factores de crecimiento y células progenitoras y mezclas de los mismos; el cultivo del agente terapéutico in vitro utilizando el implante de injerto óseo como medio de cultivo y el sustrato antes de dicha etapa de implantación; e implantar quirúrgicamente el implante de injerto óseo dentro de un paciente en un lugar seleccionado de regeneración/crecimiento óseo.
El injerto óseo de la invención puede hacerse con una variedad de formas y tamaños para adaptarse a las diferentes necesidades específicas de implantación. Las formas preferidas incluyen las cilíndricas o parcialmente cilíndricas, con extremos opuestos substancialmente planos y una sección transversal estrechada o con lordosis para adaptarse a la curvatura necesaria del espacio intervertebral, en el caso de un dispositivo de fusión espinal. La superficie exterior del cuerpo cilíndrico puede incluir roscas o ribetes para facilitar y asegurar la colocación roscada, por ejemplo, entre vértebras adyacentes. Formas preferidas alternativas incluyen un bloque generalmente rectangular que también puede incluir dientes de sierra o similares en una o más caras exteriores del mismo, y/o puede tener una sección transversal estrechada o con lordosis para mejorar el ajuste en el espacio intervertebral. Dicho bloque de sustrato puede tener una superficie exterior áspera. Convenientemente, dicho bloque de sustrato puede tener una superficie exterior ribeteada.
Típicamente, dicho bloque de sustrato puede incluir además unos medios para facilitar el agarre y la manipulación con un instrumento quirúrgico para la implantación. El injerto óseo puede incluir de manera deseable un extremo posterior que tiene unos medios de acoplamiento, tales como una cavidad roscada para un acoplamiento liberable con una herramienta adecuada de inserción. Además, el injerto óseo también puede incluir uno o más rebajes o perforaciones abiertos lateralmente para recibir y soportar material de injerto óseo osteoconductivo, tal como material de aloinjerto (donante) o de autoinjerto (paciente).
Configuraciones alternativas adicionales de injerto óseo pueden incluir gradaciones controladas de porosidad para definir una primera región o regiones de porosidad relativamente baja que emulan sustancialmente el hueso cortical, para definir un puntal o zona de apoyo de carga de alta resistencia para la absorción de cargas de impacto e inserción, en combinación con una o más segundas regiones de porosidad relativamente alta que emulan sustancialmente el hueso esponjoso para el contacto con hueso adyacente del paciente para mayor crecimiento y fusión óseos. En algunas realizaciones, dicha primera región puede comprender por lo menos un puntal de apoyo de carga estructural que se extiende a través del bloque de sustrato. Dicho por lo menos un puntal puede imitar sustancialmente las características estructurales del hueso natural, dicho por lo menos un puntal puede formarse a partir de un material cerámico poroso.
Preferiblemente, dicha primera región de menor porosidad puede disponerse generalmente en las superficies anterior y posterior del bloque de sustrato y puede definir además por lo menos un puntal de apoyo de carga estructural que se extiende a través de dichas superficies anterior y posterior.
Dicha segunda región puede incluir un área superficial expuesta extendida para ponerse en contacto con el hueso adyacente. En algunas realizaciones, dicha segunda región puede ser expuesta substancialmente en las superficies media y lateral del bloque de sustrato.
Dicha primera región puede rodear circunferencialmente y soportar a la segunda región.
En ciertas realizaciones, el cuerpo de substrato del injerto óseo puede tener una perforación formada en el mismo. Adecuadamente, el cuerpo de substrato puede tener una perforación abierta lateralmente formada en el mismo, y un material osteoconductivo puede ser soportado dentro de dicha perforación. Dicho material osteoconductivo puede comprender material troceado de injerto óseo.
El injerto óseo puede exhibir una resistencia mecánica relativamente alta para el soporte de carga, por ejemplo, entre vértebras adyacentes en el caso de una jaula de fusión espinal, mientras que además y de manera deseable proporciona propiedades altamente osteoconductivas y osteoinductivas para lograr mayor crecimiento óseo y fusión entre cuerpos vertebrales. Es importante señalar que estas características deseables pueden alcanzarse en una estructura que es sustancialmente radiolúcida de modo que el implante no interfiere con la monitorización radiológica posoperatoria del proceso de fusión. Como tal, el bloque cerámico puede ser sustancialmente radiolúcido.
Otras características y ventajas de la invención se harán evidentes a partir de la siguiente descripción detallada, tomada conjuntamente con los dibujos acompañantes, que ilustran, a modo de ejemplo, los principios de la invención.

DESCRIPCIÓN DETALLADA DE LAS REALIZACIONES PREFERIDAS
Como se muestra en los ejemplos de dibujos, se proporciona un injerto óseo radiolúcido, tal como una jaula de fusión espinal con la referencia en general en las FIGURAS 1-3 con el número de referencia 10, para la implantación asentada entre un par de huesos adyacentes del paciente, tal como vértebras espinales 12 (FIG. 3), para mantener las vértebras en relación espaciada al tiempo que promueven el crecimiento y fusión óseos entre cuerpos vertebrales. En general, el injerto óseo mejorado 10 comprende un sustrato cerámico biocompatible que tiene una construcción porosa para definir un entramado abierto propicio para el crecimiento y la fusión óseos entre cuerpos vertebrales, al tiempo que proporciona una fuerte estructura de apoyo de carga mecánica análoga a las propiedades de apoyo de carga de hueso cortical y esponjoso. Este sustrato cerámico de celda abierta está revestido interna y externamente con un revestimiento superficial bioactivo seleccionado para tener propiedades osteoinductivas y osteoconductivas relativamente fuertes, por lo que el sustrato cerámico revestido proporciona un esqueleto propicio para la conexión y proliferación celular para promover la conexión por fusión y crecimiento óseo entre cuerpos vertebrales. El sustrato cerámico también puede llevar uno o más agentes terapéuticos seleccionados adecuados para la reparación ósea, el aumento y otros usos ortopédicos.
Las FIGS. 1-3 ilustran el injerto óseo mejorado en forma de una jaula mejorada de fusión espinal 10 según una realización preferida, en forma de un cuerpo cilíndrico que tiene unos ribetes externos formados en el mismo con forma de una rosca helicoidal 14. Los extremos opuestos de este cuerpo cilíndrico son generalmente planos, con un extremo posterior 16 que se muestra que incluye una hendidura superficial 18 que se extiende diametralmente en combinación con una cavidad roscada 20 abierta hacia atrás y centrada axialmente para el acoplamiento liberable con una herramienta de inserción 22 (FIGS. 2-3), como se describirá con más detalle. El cuerpo cilíndrico se muestra que incluye un rebaje o perforación 24 abiertos lateralmente para recibir y soportar material troceado de injerto óseo 26 (FIG. 2), tal como material de aloinjerto óseo de un tercer donante, o material de autoinjerto óseo del paciente.
La composición preferida de sustrato cerámico comprende un bloque de sustrato cerámico con relativamente alta resistencia. Según una forma preferida de la invención, este bloque de sustrato comprende una composición de dos fases de alúmina-zirconio que tiene una porosidad controlada y que tiene un tamaño y forma adecuados para la implantación asentada, tal como en el espacio intervertebral en el caso de la jaula de fusión espinal 10. En una forma preferida, la composición comprende una composición de alúmina-zirconio con una proporción de alúmina a zirconio comprendida entre aproximadamente el 100% (es decir, 2:1) y aproximadamente el 25% (es decir, 1:4). Más preferiblemente, la composición del sustrato comprende de aproximadamente el 10% a aproximadamente el 20% en volumen de circonio en alúmina. Para la fase de zirconio se utiliza preferiblemente zirconio estabilizado con itrio (aproximadamente del 2,5 al 5% de mol de itrio en zirconio) o zirconio estabilizado con cerio (aproximadamente del 2,5 al 15% de mol de cerio en zirconio).
La composición de alúmina-zirconio se procesa para proporcionar una distribución homogénea de las dos fases, y las temperaturas de sinterización se controlan para proporcionar un tamaño de partícula de aproximadamente 0,5 micrómetros o menos en el estado sinterizado. El bloque de sustrato cerámico resultante tiene una porosidad que va desde aproximadamente el 2% a aproximadamente el 80% en volumen, y, preferiblemente, desde aproximadamente el 10% a aproximadamente el 50% en volumen, con un tamaño de poro que va desde aproximadamente 5 micrómetros a aproximadamente 1.500 micrómetros, y preferiblemente de aproximadamente 100 a aproximadamente 500 micrómetros. En la forma preferida, los poros están dispuestos para una continuidad de fluidos entremedio y con una superficie exterior áspera porosa de área superficial grande o extendida. Por otra parte, en la forma preferida, los poros están dispuestos con un gradiente variable de porosidad para definir una primera región de porosidad relativamente baja o reducida (menos de aproximadamente el 5%) que imita la estructura ósea cortical y una segunda región de porosidad relativamente grande o mayor (que van desde aproximadamente el 30% hasta aproximadamente el 80%), que imita la estructura ósea esponjosa. En una configuración preferida, las superficies exteriores o externas del bloque de sustrato reticulado comprenden la primera región o de baja porosidad para una capacidad de carga mejorada, mientras que las superficies interiores del bloque de sustrato comprenden la segunda región o de alta porosidad que imita el hueso esponjoso para mejorar el crecimiento y la fusión óseos. Este material utilizado para formar el bloque de sustrato exhibe una combinación sustancialmente óptima de resistencia a la flexión (más de aproximadamente 500 MPa [megaPascales]) y tenacidad a la fractura (más de 5 Mpam0,5 [megaPascal raíz de metro]). Estas resistencias se miden en especímenes de barras de curvatura estándar y barras de borde con hendidura individual (en inglés SENB, single edge notched beam), según la norma ASTM E-1304 y los procedimientos C-1162.
Este bloque de sustrato cerámico de alta resistencia está revestido superficialmente interna y externamente con un material bioactivo orgánico o inorgánico de revestimiento superficial seleccionado para propiedades relativamente fuertes osteoinductivas y osteoconductivas para proporcionar un ambiente rico en nutrientes para la actividad celular para promover la conexión por fusión y crecimiento óseo entre cuerpos vertebrales. Materiales preferidos de revestimiento superficial comprenden un material resorbible, tal como la hidroxiapatita o una cerámica de fosfato de calcio. También se pueden utilizar materiales alternativos vítreos (amorfos) que tienen una composición relativamente rica de fosfato y calcio, particularmente en donde dichos materiales incorporan el calcio y el fósforo con una proporción similar al hueso natural o hidroxiapatita. Las composiciones vítreas pueden comprender un material osteoinductivo parcial o totalmente amorfo que comprende un compuesto vítreo y de componente de calcio osteoinductivo, con una composición que varía de aproximadamente el 100% de vidrio al 100% de componente de calcio osteoinductivo. El revestimiento superficial también puede comprender aspirados de médula ósea autóloga.
El injerto óseo resultante 10 comprende de este modo el bloque de sustrato formado a partir del material cerámico de alta resistencia que tiene propiedades biomiméticas y que no es de resorbible o es resorbible lentamente o infinitamente lento cuando se implanta en el paciente, en combinación con el revestimiento superficial bioactivo que es resorbible comparativamente rápido para promover una rápida y vigorosa actividad de crecimiento óseo.
El bloque de sustrato también puede ser ventajosamente revestido o impregnado con uno o más agentes terapéuticos, por ejemplo factores de crecimiento antólogos, sintéticos o derivados de células madre o proteínas y factores de crecimiento tales como proteína morfogénica ósea (BMP) o un precursor para los mismos, que además promueve aún más la curación, la fusión y el crecimiento. Agentes terapéuticos alternativos también pueden incluir un antibiótico o agentes terapéuticos naturales tales como aspirados de médula ósea, y factores de crecimiento o células progenitoras, tales como células madre mesenquimales, células hematopoyéticas o células madre embrionarias, ya sea solas o como una combinación de diferentes agentes beneficiosos.
Más particularmente, tal agente o agentes terapéuticos se pueden aplicar al injerto óseo 10 sustancialmente en el momento o en el curso de un procedimiento quirúrgico de implante, tal como empapando el injerto óseo en el agente terapéutico o una solución a base de líquido que contiene el agente terapéutico y luego implantar el injerto óseo en el paciente. En un procedimiento alternativo, unas células progenitoras o similares pueden tomarse de un paciente específico y a continuación ser cultivadas in vitro utilizando el injerto óseo como medio de cultivo y sustrato para producir una concentración alta y de eficacia terapéutica de las celdas seleccionadas que son llevadas dentro del injerto óseo. A partir de ese momento, el injerto óseo que lleva las células cultivadas se puede implantar quirúrgicamente. En una configuración conveniente, el implante de injerto óseo puede formarse con un tamaño y forma de una pequeña pastilla para el llenado adecuado de múltiples implantes en un lugar de regeneración/crecimiento óseo.
El injerto óseo resultante, tal como la jaula ilustrativa de fusión espinal 10 exhibe una resistencia biomecánica relativamente alta similar a las características de soporte de carga del hueso natural. Además, la jaula de fusión vertebral 10 exhibe características osteoinductivas y osteoconductivas relativamente fuertes atribuibles principalmente al revestimiento superficial, también similar al hueso natural. Sobre todo, la jaula de fusión 10 también es substancialmente radiolúcida, de modo que la jaula de fusión no interfiere con los análisis radiológicos posoperatorios de crecimiento y fusión óseos entre cuerpos vertebrales.
La FIG. 2 ilustra la jaula de fusión espinal 10 con el material de injerto óseo 26 que es llevado dentro de una perforación abierta lateralmente 24, para lograr además un mayor crecimiento y fusión óseos cuando el dispositivo se implanta en el espacio intervertebral. La herramienta de inserción 22 incluye una extremidad roscada 28 en un extremo adelantado o de morro para un acoplamiento roscado en la cavidad roscada 20 en el extremo posterior de la jaula de fusión 10. También se proporciona un tope de trabado 30 en el extremo delantero de la herramienta de inserción 22 para el acoplamiento asentado adentro de la hendidura superficial 18 en la jaula de fusión. Ensamblada de este modo, la herramienta de inserción 22 es utilizada por un cirujano como se ve en la FIG. 3 para asentar la jaula de fusión espinal 10 en el espacio que hay entre un par seleccionado de vértebras adyacentes 12, con la jaula de fusión apoyada contra una parte sustancial de las placas extremas de las dos vértebras en lados opuestos del espacio intervertebral. Este proceso de inserción puede ir acompañado de la rotación apropiada de la herramienta de inserción 22 de modo que el tope 30 en la misma impulsa de manera rotatoria la jaula de fusión 10 a la posición implantada asentada deseada y con seguridad. A continuación, el tope 30 se puede retraer desde la jaula de fusión 10 seguido por una rotación hacia atrás de la extremidad roscada 28 para la liberación de la jaula de fusión.
Las FIGS. 4-6 ilustran unas configuraciones alternativas para injertos óseos mejorados, tales como jaulas de fusión espinal, construidas según la presente invención, y se reconoce y entiende que el injerto óseo se puede construir con una amplia gama de diferentes tamaños y formas geométricas. La FIG. 4 muestra una jaula de fusión espinal 110 que tiene en general una forma cilíndrica similar a la jaula de fusión 10 que se muestra y describe en las FIGS. 1-3, pero en donde se omite la rosca helicoidal externa 14. Como se muestra, la jaula de fusión espinal 110 (FIG. 4) tiene una estructura de celdas abiertas definida por bloque de sustrato cerámico poroso de alta resistencia (como se describió anteriormente) revestido con el material bioactivo de revestimiento superficial, pero en donde la superficie cilíndrica exterior se define por la estructura de sustrato relativamente áspero de entramado que tiene suficiente discontinuidad superficial y gran área superficial para un contacto optimizado con el hueso esponjoso para lograr un crecimiento óseo substancialmente optimizado. La FIG. 5 muestra otra jaula alternativa de fusión espinal 210 que comprende un bloque de sustrato cerámico poroso revestido superficialmente de alta resistencia (formado como se ha descrito anteriormente) con una configuración de bloque generalmente rectangular para incluir por lo menos una superficie exterior para incluir una pluralidad de dientes de sierra 214 para el trabado mecánico con seguridad con huesos adyacentes del paciente tales como vértebras adyacentes 12 en lados opuestos del espacio intervertebral. La FIG. 6 representa una jaula de fusión espinal alternativa 310 con forma de bloque rectangular que comprende otro bloque de sustrato cerámico poroso revestido superficialmente de alta resistencia (formado como se describió anteriormente), que tiene una dimensión en sección transversal más estrecha y una superficie exterior definida por el sustrato de entramado abierto con textura relativamente áspera (similar a la FIG. 4). Cada una de las realizaciones representadas en las FIGS. 1-3 y las FIGS. 4-6 tiene una dimensión de altura y puede tener una forma estrechada o con lordosis (como se muestra en la FIG. 6) para un mejor ajuste anatómico, por ejemplo, en el espacio intervertebral o similares.
Las FIGS. 7-10 describen formas alternativas preferidas de la invención, en donde el gradiente de porosidad dentro del bloque de sustrato cerámico poroso de alta resistencia se varía de manera controlada para proporcionar beneficios de apoyo de carga compatibles con el crecimiento y la fusión óseos. Más particularmente, las FIGS. 7-8 muestran un injerto óseo en general rectangular tal como una jaula de fusión espinal 410 que tiene una dimensión en altura estrechada en la dirección anterior-posterior. El bloque de sustrato está formado con la primera región 40 de porosidad relativamente baja que imita sustancialmente al hueso cortical para extenderse a través de las caras anterior y posterior y además para incluir por lo menos un puntal de apoyo de carga de interconexión 42 que se muestra en los dibujos ilustrativos extendiéndose al centro en una dirección anterior-posterior dentro del cuerpo del bloque de sustrato. El resto del bloque de sustrato comprende la segunda parte 44 de porosidad relativamente alta que imita sustancialmente al hueso esponjoso. La primera región más dura 40, que incluye el puntal central 42, proporciona de manera beneficiosa una estructura dura y fuerte de apoyo de carga capaz de soportar las fuerzas de impacto e inserción en la dirección anterior-posterior sin que se dañe el implante, mientras que la segunda región más blanda 44 presenta un área superficial expuesta y grande para el crecimiento y la fusión entretejida substancialmente optimizada con el hueso adyacente del paciente. En una aplicación de jaula de fusión espinal, las caras medias-laterales del implante están definidas de manera ventajosa por la segunda región más blanda 44, en donde estas regiones se exponen de este modo para la tradicional obtención de imágenes medio-lateral por rayos X para el análisis radiológico posoperatorio de la interfaz hueso-implante. Los expertos en la técnica reconocerán y apreciarán que se pueden utilizar configuraciones alternativas para el puntal o puntales 42 de apoyo de carga, tal como una configuración de puntal con forma de X en dirección cráneo-caudial, en combinación o en lugar de las caras exteriores 40 y/o el puntal central anterior-posterior, como se muestra.
Las FIGS. 9-10 muestran otro injerto óseo en general rectangular tal como una jaula de fusión espinal 510 que tiene la dimensión en altura estrechada en la dirección anterior-posterior. El bloque de sustrato está formado con la primera región 40 de porosidad relativamente baja que imita sustancialmente al hueso cortical para extenderse circunferencialmente sobre el bloque de sustrato en relación circundante con la segunda parte 44 de porosidad relativamente alta que imita substancialmente al hueso esponjoso. La primera región más dura 40 una vez más proporciona de manera beneficiosa una estructura dura y fuerte de apoyo de carga capaz de resistir las fuerzas de impacto e inserción y en la dirección anterior-posterior sin que se dañe el implante, y además proporciona capacidad de llevar carga vertical de alta resistencia. Por el contrario, la segunda región más blanda 44 se expone verticalmente para presentar una gran área superficial para el crecimiento y la fusión entretejidos substancialmente optimizados con el hueso adyacente del paciente. Las esquinas externas del bloque de sustrato pueden incorporar unas ranuras abiertas lateralmente 46 (FIG. 9) para un cómodo acoplamiento y manipulación con una herramienta quirúrgica adecuada (no se muestra).
En las dos realizaciones de las FIGS. 7-10, el bloque de sustrato comprende una cerámica porosa de alta resistencia como se ha descrito previamente, y está revestido con material bioactivo de revestimiento superficial, de nuevo como se ha descrito anteriormente, para mejorar el crecimiento y la fusión óseos. El bloque de sustrato también puede incluir uno o más agentes terapéuticos. Los expertos en la técnica reconocerán y apreciarán que las regiones de porosidad relativamente baja y alta 40 y 44, que se muestran en las FIGS. 7-10, se unirán integralmente mediante una región de gradiente adecuado aunque relativamente estrecha en donde hay una transición de porosidad entremedio.
El injerto óseo mejorado, tal como la jaula ilustrativa de fusión espinal de la presente invención, comprende de este modo una estructura de bloque de sustrato de celda abierta que está revestida con un revestimiento superficial bioactivo, y tiene la resistencia necesaria para la capacidad de apoyo de peso necesaria para un dispositivo de fusión. La capacidad de ser penetrado por el agente apropiado biológico de revestimiento imparte propiedades deseables osteoconductivas y osteoinductivas al dispositivo para mejorar el crecimiento y la fusión óseos entre cuerpos vertebrales, sin menoscabo de las características esenciales de apoyo de carga. Las características radiolúcidas del dispositivo mejorado permiten de manera beneficiosa el examen radiológico posoperatorio para monitorizar el progreso del crecimiento y fusión, substancialmente sin el indeseable radio-sombreado atribuible a la jaula de fusión. Las roscas o dientes de sierra externos de la jaula de fusión pueden tener una profundidad variable para permitir que la base del dispositivo se ponga en contacto con el hueso cortical para una óptima capacidad de apoyo de carga. Además de estos beneficios, la presente invención es fácil de fabricar de una manera con un coste competitivo. La invención proporciona de este modo una mejora sustancial a la hora de abordar problemas clínicos indicados para el tratamiento médico de la enfermedad degenerativa del disco, dolor discogénico en la parte baja de la espalda y espondilolistesis.
El injerto óseo de la presente invención proporciona por lo menos los siguientes beneficios sobre la técnica anterior:
[a] un esqueleto osteoconductivo poroso para una mayor velocidad de fusión;
[b] una superestructura biomimética de apoyo de carga que proporciona una transmisión apropiada de tensiones sin fallo por fatiga;
[c] un tamaño y estructura de poros adecuados para el crecimiento y la vascularización,
[d] la capacidad de absorber y retener un agente osteoinductivo, tal como aspirado de médula ósea autóloga o los BMP;
[e] bioinerte y biocompatible con tejidos adyacentes y seleccionado para facilitar la resorción;
[f] se puede fabricar y mecanizar para tener diversas formas;
[g] esterilizable; y
[h] bajo coste de fabricación. Una variedad de modificaciones y mejoras adicionales en la jaula de fusión espinal de la presente invención serán evidentes para los expertos en la técnica. Por consiguiente, no se pretende una limitación en la invención por medio de la anterior descripción y los dibujos acompañantes, con excepción de lo establecido en las reivindicaciones anexas.
Claims (19)
- REIVINDICACIONES1. Un injerto óseo, que comprende:un bloque de sustrato cerámico que comprende una primera región de menor porosidad (40) y una segunda región de mayor porosidad (44), en donde dicha segunda región de mayor porosidad está configurado para facilitar la conexión por crecimiento y fusión óseos con hueso adyacente del pacientecaracterizado porque dicho bloque de sustrato cerámico (410) está formado por un material con una resistencia a la flexión de más de 500 megaPascal y una tenacidad a la fractura de más de aproximadamente 5 megaPascal raíz de metro.

- El injerto óseo de la reivindicación 1, en donde dicha primera región (40) tiene una porosidad de menos de aproximadamente el 5%.

- 3.
- El injerto óseo de la reivindicación 1 o la reivindicación 2, en donde dicha segunda región (44) tiene una porosidad que va desde aproximadamente el 30% hasta aproximadamente un 80%.

- 4.
- El injerto óseo de cualquiera de las reivindicaciones 1 a 3, en donde dicha segunda región tiene una porosidad de más de aproximadamente un 40%.
-
- 5.
- El injerto óseo de cualquiera de las reivindicaciones 1 a 4, en donde el intervalo de tamaños de poro va de aproximadamente 5 micrómetros a aproximadamente 1.500 micrómetros.
-
- 6.
- El injerto óseo de cualquiera de las reivindicaciones 1 a 5, en donde los poros formados dentro de dicho bloque de sustrato están en una comunicación de fluidos substancialmente abierta suficiente para transmitir presión de fluido entremedio.
-
- 7.
- El injerto óseo de cualquiera de las reivindicaciones 1 a 6, en donde dicha primera región (40) se dispone generalmente en las superficies anterior y posterior de dicho bloque de sustrato (410) y define además por lo menos un puntal de soporte de carga estructural (42) que se extiende a través de dicho bloque de sustrato entre las superficies anterior y posterior.

- El injerto óseo de cualquiera de las reivindicaciones 1 a 7, en donde dicha primera región (40) rodea circularmente y soporta a dicha segunda región (44).

- 9.
- El injerto óseo de cualquiera de las reivindicaciones precedentes que comprende además un revestimiento superficial aplicado a dicho bloque de sustrato, dicho revestimiento superficial tiene propiedades osteoconductivas y osteoinductivas para promover la conexión por crecimiento y fusión óseos con hueso adyacente del paciente.

- El injerto óseo de cualquiera de las reivindicaciones precedentes, en donde dicho bloque de sustrato poroso cerámico es sustancialmente radiolúcido que es visible en obtención de imágenes de rayos X, pero que tiene una densidad que permite el paso de los rayos X a través del material cerámico del cuerpo en obtención de imágenes medio-lateral por rayos X, facilitando de este modo la evaluación posoperatoria del crecimiento del hueso en la región de superior porosidad del bloque de sustrato cerámico.

- 11.
- El injerto óseo de cualquiera de las reivindicaciones precedentes, en donde dicho cuerpo de substrato tiene una perforación (24) formada en el mismo.

- 12.
- El injerto óseo de cualquiera de las reivindicaciones anteriores, en donde dicho injerto óseo comprende una jaula de fusión espinal (10) para la implantación entre vértebras adyacentes y la fusión de las mismas.
-
- 13.
- El injerto óseo de la reivindicación 9, en donde dicho revestimiento superficial se aplica interna y externamente a dicho bloque de sustrato cerámico poroso.
-
- 14.
- El injerto óseo de cualquiera de las reivindicaciones 9 o 13, en donde dicho revestimiento superficial se selecciona de compuestos de calcio e hidroxiapatita.
-
- 15.
- El injerto óseo de cualquiera de las reivindicaciones 9 o 13, en donde dicho revestimiento superficial comprende material osteoinductivo total o parcialmente amorfo que incluye un compuesto de calcio osteoinductivo y vidrio.
-
- 16.
- El injerto óseo de cualquiera de las reivindicaciones 9 o 13, en donde dicho revestimiento superficial comprende un material orgánico de revestimiento.
-
- 17.
- El injerto óseo de la reivindicación 16, en donde dicho material orgánico de revestimiento se selecciona del grupo que consiste en aspirado de médula ósea autóloga, proteínas morfogénicas de hueso, factores de crecimiento y células progenitoras y mezcla de los mismos.
5 18. El injerto óseo de la reivindicación 17, en donde dichas células progenitoras incluyen células madre mesenquimales, células hematopoyéticas y células madre embrionarias. - 19. El injerto óseo de cualquiera de las reivindicaciones precedentes, que incluye además un agente terapéutico que es portado por dicho bloque de sustrato.
- 20. El injerto óseo de la reivindicación 19, en donde dicho agente terapéutico comprende un agente natural o sintético osteoconductivo u osteoinductivo.
