Abstract

La presente invencion se refiere en general a un dispositivo y metodo para tratar la cardiomioparia dilatada de un corazon. La presente invencion se refiere, mas en particular, a un dispositivo y metodo para remodelar el anillo de la valvula mitral.
Antecedentes de la invencion
El corazon humano incluye de forma general cuatro valvulas. De estas valvulas, la mas critica se conoce como la valvula mitral. La valvula mitral esta situada en la abertura ventricular auricular izquierda entre la auricula izquierda y el ventriculo izquierdo. La valvula mitral esta destinada a impedir el reflujo de sangre desde el ventriculo izquierdo a la auricula izquierda cuando el ventriculo izquierdo se contrae. Para impedir el reflujo de sangre, la valvula mitral debe ser capaz de soportar una presion de retorno considerable cuando el ventriculo izquierdo se contrae.
Las cuspides de la valvula mitral estan ancladas a la pared muscular del corazon mediante cuerdas fibrosas delicadas pero fuertes con el fin de soportar las cuspides durante la contraccion ventricular izquierda. En una valvula mitral sana, la geometria de la valvula mitral asegura que las cuspides se solapan entre sf para impedir el reflujo de la sangre durante la contraccion ventricular izquierda.
El funcionamiento normal de la valvula mitral para impedir el reflujo puede verse afectado por una cardiomioparia dilatada causada por una enfermedad o ciertos defectos naturales. Por ejemplo, ciertas enfermedades pueden causar la dilatacion del anillo de la valvula mitral. Esto puede dar como resultado una deformacion de la geometria de la valvula mitral que causa un cierre inefectivo de la valvula mitral durante la contraccion ventricular izquierda. Tal cierre inefectivo da como resultado una fuga a traves de la valvula mitral y reflujo. Enfermedades tales como inflamaciones bacterianas del corazon o fallo cardiaco pueden causar la distorsion o dilatacion del anillo de la valvula mitral mencionada anteriormente. Ni que decir tiene, el reflujo de la valvula mitral no debe quedar sin corregir.
Un metodo de reparar una valvula mitral que tiene su funcion afectada es reemplazar completamente la valvula. Este metodo se ha encontrado que es particularmente adecuado para reemplazar una valvula mitral cuando una de las cuspides ha sido danada o deformada severamente. Aunque el reemplazamiento de la valvula entera elimina el problema inmediato asociado con un anillo de valvula mitral dilatado, las protesis valvulares cardiacas disponibles en la actualidad no poseen la misma durabilidad que las valvulas cardiacas naturales.
Otros diferentes procedimientos quirurgicos se han desarrollado para corregir la deformacion del anillo de la valvula mitral y asf conservar intacta la funcion de la valvula cardiaca natural. Estas tecnicas quirurgicas implican reparar la forma del anillo de valvula deformado o dilatado. Tales tecnicas, conocidas en general como anuloplastia, requieren restringir quirurgicamente el anillo de valvula para minimizar la dilatacion. Aqrn, ripicamente, una protesis es suturada alrededor de la base de los velos valvulares para remodelar el anillo de la valvula y restringir el movimiento del anillo de valvula durante la apertura y cierre de la valvula mitral.
Muchos tipos diferentes de protesis se han desarrollado para su uso en tal cirugfa. En general, las protesis son miembros de forma anular o parcialmente anular los cuales se ajustan alrededor de la base del anillo valvular. Los miembros con forma anular o parcialmente anular peden estar formados de un material rigido, tal como metal, o de un material flexible.
Aunque los metodos de la tecnica anterior mencionados arriba han sido capaces de alcanzar algun exito en el tratamiento del reflujo mitral, no lo han sido sin problemas y consecuencias adversas potenciales. Por ejemplo, estos procedimientos requieren cirugfa a corazon abierto. Tales procedimientos son caros, son extremadamente invasivos requiriendo un tiempo de recuperacion considerable, y presentan los riesgos de mortalidad concomitantes asociados con tales procedimientos. Ademas, tales procedimientos a corazon abierto son particularmente estresantes para los pacientes con una condicion cardiaca comprometida. Dados estos factores, tales procedimientos se reservan a menudo como un ultimo recurso y por tanto se emplean tardfamente en la progresion del reflujo mitral. Ademas, la efectividad de tales procedimientos es diffcil de evaluar durante el procedimiento y puede no conocerse hasta mucho tiempo despues. Por tanto, la capacidad para hacer ajustes a o cambios en las protesis para obtener una efectividad optima es extremadamente limitada. Correcciones posteriores, si es que se hacen, requieren aun otra cirugfa a corazon abierto. El documento de patente de EE.UU. US 6,210,432 B1 divulga un dispositivo para tratamiento de la dilatacion del anillo mitral, que comprende un cuerpo alargado que tiene dimensiones tales como para poder ser insertado en el seno coronario y que tiene dos estados, en uno primero de los cuales el cuerpo alargado tiene una forma que es se puede adaptar a la forma del seno coronario, y al segundo de los cuales se puede transferir el cuerpo alargado desde dicho primer estado asumiendo un radio de curvatura reducido, por medio de lo cual el radio de curvatura del seno coronario es reducido asf como la circunferencia del anillo de la valvula mitral, cuando el cuerpo alargado esta colocado en el seno coronario. La solicitud PCT publicada de patente internacional WO 2002/062270 divulga un dispositivo para tratamiento de la dilatacion del anillo mitral que comprende un cuerpo

alargado que tiene dos estados. En uno primero de estos estados el cuerpo alargado se puede insertar en el seno coronario y tiene una forma que se adapta a la forma del seno coronario. Cuando esta colocado en el seno coronario, el cuerpo alargado se puede transferir al segundo estado asumiendo un radio de curvatura reducido, por medio de lo cual el radio de curvatura del seno coronario y el radio de curvatura asf como la circunferencia del anillo mitral se reducen.
Una terapia mejorada para tratar el reflujo mitral sin recurrir a la cirugfa a corazon abierto ha sido propuesta recientemente. Esto se hace posible por el entendimiento de que el seno coronario de un corazon esta cerca de y al menos parcialmente rodea al anillo de la valvula mitral y luego se extiende en un sistema venoso que incluye la vena cardiaca magna. Segun se usa aqm, el termino “seno coronario” es tiene un significado que se refiere no solo al propio seno coronario sino, ademas, al sistema venoso asociado con el seno coronario que incluye la vena cardiaca magna. La terapia contempla el uso de un dispositivo introducido en el seno coronario para remodelar y afectar ventajosamente la geometria del anillo de la valvula mitral.
El dispositivo incluye un miembro resiliente que tiene unas dimensiones en seccion transversal para ser recibido dentro del seno coronario del corazon y una dimension longitudinal que tiene una configuracion arqueada distendida cuando se coloca en el seno coronario. El dispositivo rodea parcialmente y ejerce una presion hacia dentro de la valvula mitral. La presion hacia dentro constrine el anillo de la valvula mitral, o al menos una porcion de el, para restaurar esencialmente la geometna de la valvula mitral. Esto provoca una accion de cierre hermetico efectivo de la valvula y elimina el reflujo mitral.
El dispositivo puede ser implantado en el seno coronario usando solo tecnicas percutaneas similares a las tecnicas usadas para implantar derivaciones cardiacas tales como derivaciones de marcapasos. Un sistema propuesto para implantar el dispositivo incluye un introductor alargado configurado para ser acoplado de manera liberable al dispositivo. El introductor es, preferiblemente, flexible para permitirle hacer avanzar al dispositivo al interior del corazon y al interior del seno coronario a traves del ostium del seno coronario. Para favorecer el guiado, primero se avanza una vaina alargada al interior del seno coronario. Entonces, el dispositivo y el introductor son movidos a traves de un lumen de la vaina hasta que el dispositivo esta en posicion dentro del seno coronario. Debido a que el dispositivo esta formado de material resiliente, se conforma a las curvaturas del lumen cuando es avanzado a traves de la vaina. La vaina es, entonces, retrafda parcialmente para permitir al dispositivo que asuma su configuracion arqueada distendida. Una vez que el dispositivo esta colocado apropiadamente, el introductor es, entonces, desacoplado del dispositivo y retrafdo a traves de la vaina. El procedimiento se completa entonces por la retraccion de la vaina. Como resultado, el dispositivo es dejado en el interior del seno coronario para ejercer la presion hacia dentro sobre la valvula mitral para restaurar la geometna de la valvula mitral.
La terapia que antecede tiene muchas ventajas sobre la aproximacion de cirugfa a corazon abierto tradicional. Puesto que el dispositivo, sistema y metodo pueden ser empleados en un procedimiento comparativamente no invasivo, el reflujo de la valvula mitral puede ser tratado en una etapa temprana de la progresion del reflujo mitral. Ademas, el dispositivo puede ser colocado con relativa facilidad por cualquier cardiologo mmimamente invasivo. Aun mas, puesto que el corazon permanece completamente intacto a lo largo de todo el procedimiento, la efectividad del procedimiento puede ser determinada facilmente. Ademas, si se consideran deseables ajustes, tales ajustes pueden ser hechos durante el procedimiento y antes de que el paciente sea enviado a recuperacion.
Otra aproximacion para tratar el reflujo mitral con un dispositivo en el seno coronario se basa en la observacion de que la aplicacion de una fuerza localizada contra una porcion discreta del anillo de la valvula mitral puede terminar con el reflujo mitral. Esto sugiere que la dilatacion de la valvula mitral puede ser localizada y no uniforme. Por tanto, el dispositivo aplica una fuerza a una o mas porciones discretas de la pared auricular del seno coronario para proporcionar remodelado localizado del anillo de la valvula mitral en vez de remodelado generalizado del anillo de la valvula mitral. Tal terapia localizada tendna todos los beneficios de la terapia generalizada. Ademas, un dispositivo para terapia localizada puede ser mas facil de implantar y ajustar.
Otra aproximacion mas para tratar el reflujo mitral desde el seno coronario del corazon contempla un dispositivo que tiene un primer anclaje configurado para ser colocado en el interior y fijado al seno coronario del corazon adyacente al anillo de la valvula mitral dentro del corazon, un cable fijado a el primer anclaje y que se extiende proximalmente desde el primer anclaje dentro del corazon, un segundo anclaje configurado para ser colocada en y fijada en el corazon proximal a el primer anclaje y dispuesta para recibir de manera deslizante el cable, y un elemento de bloqueo que bloquea el cable sobre el segundo anclaje. Cuando los primer y segundo anclajes estan fijadas dentro del corazon, el cable puede el cable puede ser traccionado proximalmente y bloqueado sobre el segundo anclaje. La geometna de la valvula mitral es, de este modo, afectada. Esta aproximacion proporciona flexibilidad por que el segundo anclaje puede se colocada y fijada en el seno coronario o, como alternativa, el segundo anclaje puede ser colocada y fijada en la auricula derecha. Esta aproximacion permite, ademas, ajustes en la tension del cable despues del implante. La presente invencion proporciona aun otra alternativa mas para tratar el reflujo mitral con un dispositivo colocado en el seno coronario adyacente al anillo de la valvula mitral.

La presente invencion proporciona un dispositivo (30) que afecta a la geometna del anillo (20) de la valvula mitral de un corazon (10), que comprende:
un primer anclaje (32) configurado para ser anclado dentro del seno coronario (14) del corazon (10) adyacente al anillo (20) de la valvula mitral dentro del corazon (10);

cuando los primer y segundo anclajes estan dentro del corazon con el primer anclaje anclado en el seno coronario, el segundo anclaje puede ser desplazado proximalmente para afectar a la geometna del anillo de la valvula mitral y soltado para mantener el efecto sobre la geometna de la valvula mitral,
caracterizado por que el primer anclaje se curva hacia atras y forma un lazo (33), y el segundo anclaje que se curva hacia atras y forma un lazo (35).
En este documento se describe un dispositivo que afecta a la geometna del anillo de la valvula mitral de un corazon. El dispositivo incluye un primer anclaje configurado para ser anclado dentro del seno coronario del corazon adyacente al anillo de la valvula mitral dentro del corazon y un segundo anclaje configurado para ser colocada dentro del corazon proximal al primer anclaje y adyacente al anillo de la valvula mitral dentro del corazon. El dispositivo incluye, ademas, un miembro de conexion que tiene una longitud fija unido de manera permanentemente a los primer y segundo anclajes, por medio del cual cuando los primer y segundo anclajes estan dentro del corazon con el primer anclaje anclado en el seno coronario, el segundo anclaje puede ser desplazado proximalmente para afectar a la geometna del anillo de la valvula mitral y soltado para mantener el efecto sobre la geometna de la valvula mitral. El segundo anclaje puede estar configurado, cuando esta desplegado, para anclarse contra el movimiento distal pero poder moverse proximalmente para permitir que el segundo anclaje sea desplazado proximalmente dentro del seno coronario.
El primer anclaje y el segundo anclajes, preferiblemente, son autodesplegables al soltarlos en el seno coronario o pueden ser desplegados despues de la colocacion. Ademas, el miembro de conexion, al ser de longitud fija, tiene una longitud maxima extendida y como tal puede ser un miembro ngido, tener una configuracion inicial arqueada, incluir un muelle, tener una longitud maxima o ser flexible pero no extensible.
Tambien se describe en este documento un dispositivo para afectar a la geometna del anillo de la valvula mitral de un corazon. El dispositivo incluye primeros medios de anclaje para anclarse en el seno coronario del corazon adyacentes al anillo de la valvula mitral y segundos medios de anclaje para ser desplegados dentro del corazon proximales a los primeros medios de anclaje y adyacentes al anillo de la valvula mitral y medios de conexion que tienen una longitud fija y que conectan de manera permanentemente los primeros medios de anclaje a los segundos medios de anclaje. Como resultado, cuando los primeros y segundos medios de anclaje estan dentro del corazon con los primeros medios de anclaje anclados en el seno coronario, los segundos medios de anclaje pueden ser desplazados proximalmente para cooperar con los primeros medios de anclaje y los medios de conexion para afectar a la geometna del anillo de la valvula mitral y ser soltados para mantener el efecto sobre la geometna de la valvula mitral.
Tambien se describe en ese documento un sistema que afecta a la geometna del anillo de la valvula mitral. El sistema incluye un dispositivo para valvula mitral que incluye un primer anclaje configurado para ser colocada dentro de y anclada al seno coronario del corazon adyacente al anillo de la valvula mitral dentro del corazon, un segundo anclaje configurado para ser colocado dentro del corazon proximal al primer anclaje y adyacente al anillo de la valvula mitral dentro del corazon, y un miembro de conexion que tiene una longitud fija unido de manera permanente a los primer y segundo anclajes.
El sistema descrito incluye, ademas, un cateter que tiene un extremo distal, un extremo proximal y un lumen que recibe el dispositivo, siendo el cateter susceptible de ser guiado al interior del seno coronario adyacente al anillo de la valvula mitral, y que despliega los primer y segundo anclajes del dispositivo dentro del seno coronario adyacentes al anillo de la valvula mitral, y un cordon tensor acoplado de manera liberable al segundo anclaje y que se extiende proximalmente a traves del lumen y hacia fuera del extremo proximal del cateter. Como resultado, cuando el primer anclaje es desplegado mediante el cateter en el seno coronario, el segundo anclaje puede ser desplazado proximalmente mediante tirar proximalmente del cordon tensor para afectar la geometna del anillo de la valvula mitral y despues de ello ser soltada para desplegarse para mantener el efecto sobre la geometna de la valvula mitral.

Tambien se describe en este documento un dispositivo que afecta a la geometna del anillo de la valvula mitral de un corazon. El dispositivo incluye un primer anclaje configurado para ser colocado dentro de y anclado al seno coronario del corazon adyacente al anillo de la valvula mitral dentro del corazon, un segundo anclaje configurado para ser colocado dentro del corazon proximal al primer anclaje y adyacente al anillo de la valvula mitral dentro del corazon, y un miembro de conexion unido entre los primer y segundo anclajes. Al menos uno de los primer y segundo anclajes, anclandose contra el movimiento en un primer direccion y pudiendo moverse en un segundo direccion opuesta a el primer direccion.
El al menos un anclaje puede ser el primer anclaje en el que la primera direccion es una direccion proximal y en el que la segunda direccion es una direccion distal. El al menos un anclaje puede ser el segundo anclaje en el que la primera direccion es una direccion distal y en el que la segunda direccion es una direccion proximal. En una realizacion preferida, el primer anclaje se ancla contra el movimiento en una direccion proximal y se puede mover en una direccion distal y el segundo anclaje se ancla contra el movimiento en la direccion distal y se puede mover es la direccion proximal.
Tambien se describe en este documento un dispositivo que afecta a la geometna del anillo de la valvula mitral de un corazon y que permite que una derivacion cardiaca sea implantada en el lado izquierdo del corazon. El dispositivo incluye un primer anclaje configurado para ser colocado dentro de y anclado al seno coronario del corazon adyacente al anillo de la valvula mitral dentro del corazon, un segundo anclaje configurado para ser colocado dentro del corazon proximal a el primer anclaje y adyacente al anillo de la valvula mitral dentro del corazon, y un miembro de conexion unido entre los primer y segundo anclajes. El primer anclaje descrito esta configurada para ocupar menos que todo el seno coronario para permitir que una derivacion cardiaca sea pasado por el primer anclaje.
El primer anclaje puede incluir un lazo a traves del cual puede ser pasado la derivacion cardiaca. El segundo anclaje puede ser susceptible de ser colocado dentro del seno coronario y estar configurada para ocupar menos que todo el seno coronario para permitir que el derivacion cardiaco sea pasado por el segundo anclaje. El segundo anclaje puede incluir tambien un lazo a traves del cual puede ser pasado la derivacion cardiaca.

Descripcion detallada de la invencion
Haciendo referencia ahora a la figura 1, esta es una vista superior de un corazon humano 10 con la auricula quitada para mostrar la valvula mitral 12, el seno coronario 14, la arteria coronaria 15 y la arteria circunfleja 17 del corazon 10 para prestar un mejor entendimiento a la presente invencion. Tambien se muestran de forma general en la figura 1 la valvula pulmonar 22, la valvula aortica 24 y la valvula tricuspide 26 del corazon 10.
La valvula mitral 12 incluye una cuspide anterior 16, una cuspide posterior 18 y un anillo 20. El anillo rodea las cuspides 16 y 18 y mantiene su espaciamiento para proporcionar un cierre completo durante una contraccion

ventricular izquierda. Como es bien conocido, el seno coronario 14 rodea parcialmente la valvula mitral 12 adyacente al anillo 20 de la valvula mitral. Como tambien es conocido, el seno coronario es parte del sistema venoso del corazon y se extiende a lo largo del surco AV entre la auricula izquierda y el ventnculo izquierdo. Esto coloca al seno coronario esencialmente en el mismo plano que el anillo de la valvula mitral haciendo al seno coronario disponible para la colocacion en el del dispositivo terapeutico para valvula mitral de la presente invencion.
La figura 2 muestra un dispositivo 30 terapeutico de la valvula mitral que materializa la presente invencion. Como puede notarse en la figura 2, el dispositivo 30 incluye un primer anclaje 32, un miembro de conexion 34 y un segundo anclaje 36. Los anclajes 32 y 36 y el miembro de conexion 34 pueden estar formados del mismo material para proporcionar una estructura integral.
El primer anclaje 32 esta situado en el extremo distal del dispositivo 30. El anclaje 32 tiene forma de gancho para ser autodesplegable cuando es soltado en el seno coronario 14. Mas espedficamente, el dispositivo 30 puede estar formado de casi cualquier material biocompatible tal como acero inoxidable, Nitinol, una aleacion mquel/titanio del tipo bien conocido en la tecnica que tiene memoria de forma o plastico. La configuracion en forma de gancho del anclaje 32 se expande, asf, cuando es soltado para acunarse contra la pared interna del seno coronario 14 para anclar o fijar el anclaje 32 contra al menos el movimiento proximal. El anclaje 32 puede, sin embargo, permitir el movimiento distal. Preferiblemente, el anclaje 32 se coloca justo proximalmente al punto de cruce 19 del seno coronario 14 y una arteria circunfleja 17.
El miembro de conexion 34, al estar formado de Nitinol, es relativamente ngido y esta predispuesto para tener una configuracion arqueada para corresponderse generalmente con la forma del anillo 20 de la valvula mitral. El miembro de conexion 34 es de una longitud fija y esta unido de manera permanentemente a los primer y segundo anclajes 32 y 36. Aqrn, se notara que el segundo anclaje esta colocado dentro del seno coronario justo distal al ostium 21 del seno coronario 14. El segundo anclaje 36 puede tener una configuracion en forma de gancho similar y, preferiblemente, tambien es autoexplansible para ser autodesplegable. La forma de gancho del anclaje 36 ancla o fija el anclaje 36 contra el movimiento distal pero permite que se tire proximalmente del anclaje. Este es un aspecto particularmente significativo del dispositivo 30 porque permite que el dispositivo sea ajustado despues de que los anclajes 32 y 36 sean desplegados primero.
Cuando el dispositivo 30 esta desplegado segun se muestra en la figura 2, el primer anclaje 32 esta fijado contra el movimiento proximal dentro del seno coronario 14. El miembro de conexion 34, entonces, se extiende proximalmente desde el primer anclaje 32 hasta el segundo anclaje 36. El segundo anclaje 36 es entonces colocado en su localizacion deseada dentro del seno coronario 14 proximal al primer anclaje 32 y se permite que se autoexpanda para ser anclado contra el movimiento distal. Entonces, se tira del segundo anclaje 36 proximalmente mientras que el primer anclaje 32 es mantenido en su posicion fija. Esto crea tension en el miembro de conexion 34 para afectar a la geometna del anillo 20 de la valvula mitral. Una vez que se aplica una cantidad deseada de tension al miembro de conexion 34, el segundo anclaje 36 es liberado mas movimiento y es redesplegado contra el movimiento distal. Con el miembro de conexion 34 ahora bajo tension mantenida, la geometna del anillo 20 de la valvula mitral afectada ventajosamente es ahora conservada. La tension en el cable es ajustada preferiblemente tirando del segundo anclaje 26 mientras que se monitoriza un parametro indicativo del reflujo mitral, tal como un eco Doppler.
El miembro de conexion 34 puede estar provisto de una envoltura (no mostrada). La envoltura puede, preferiblemente, estar formada de un material compresible para servir para amortiguar las fuerzas del miembro de conexion aplicadas contra la pared interna del seno coronario 14.
Las figuras 3 a 5 muestran una manera en la cual el dispositivo 30 puede ser desplegado mediante un conjunto de despliegue 50. Como se notara en la figura 3, el conjunto de despliegue 50 incluye un cateter 52 y un cordon tensor 54. El cateter 52 tiene un lumen 56 dimensionado para recibir de manera deslizante el dispositivo 30 en su estado predesplegado con el cordon tensor 54 arrollado alrededor del segundo anclaje 36 y extendiendose hacia fuera del extremo proximal del cateter 52.
Como se notara en la figura 3, el primer anclaje 32 ha sido desplegado mientras que el segundo anclaje permanece en el lumen 56 del cateter. Esto puede estar acompanado por alimentar al cateter 52 en el seno coronario hasta que el primer anclaje este en una posicion deseada. Ahora, el cateter 52 puede ser movido proximalmente mientras que se mantiene el primer anclaje 32 contra el movimiento. El movimiento proximal del cateter 52 soltara el anclaje 32. Cuando el anclaje es soltado, se autexpandira para autodesplegarse y fijarse contra el movimiento proximal.
Segun se muestra en la figura 4, el cateter 52 es retrafdo mas para soltar el segundo anclaje 36 para permitirle autoexpandirse y autodesplegarse. El segundo anclaje 36 esta fijado ahora contra el movimiento distal pero tiene permitido moverse proximalmente. El cordon tensor 54 continua extendiendose hacia fuera del extremo proximal del cateter 52.
Segun puede verse mas ahora en la figura 5, se aplica entonces tension al miembro de conexion 34 tirando proximalmente del cordon tensor 54 y, por tanto, del segundo anclaje 36, mientras que el primer anclaje 32 resiste al movimiento proximal. Cuando se coloca la tension deseada sobre el miembro de conexion 34, el segundo anclaje 36

es soltado para volver a auto desplegarse. Cuando esto se completa, el primer anclaje 32 y el segundo anclaje 36 estan fijados en posicion con una tension en el miembro de conexion 34. El cateter 52 y el cordon tensor 54 pueden, entonces, ser retirados para completar el proceso de despliegue. Aunque el anclaje 36 proximal esta mostrado para estar desplegada finalmente en el seno coronario, se apreciara por los expertos en la tecnica que el anclaje 36 proximal, despues de ser desplazada proximalmente, puede ser desplegada finalmente dentro de la auricula derecho justo proximal al ostium 21 del seno coronario 14. Por tanto, cualquier posicion final del anclaje 36 proximal al anclaje 32 distal y dentro del corazon se contempla de acuerdo con la presente invencion.
De acuerdo con la presente invencion, el dispositivo 30 puede ser desplegado de una manera ligeramente diferente que la descrita arriba. Asf que, el primer anclaje 32 puede ser desplegado segun de describe arriba y el segundo anclaje 36 dejado en el cateter 52 cuando es movida proximalmente. Cuando el segundo anclaje 36 alcanza una posicion deseada, entonces, se puede tirar hacia atras del cateter 52 para soltar y desplegar el segundo anclaje 36. Como resultado, de acuerdo con esta realizacion alternativa, el segundo anclaje, cuando esta desplegado, puede anclarse contra el movimiento tanto distal como proximal.
La figura 6 muestra otro dispositivo 70 para valvula mitral que materializa la presente invencion. El dispositivo 70 es similar al dispositivo 30 descrito previamente excepto en que su miembro de conexion 74 incluye una configuracion con muelle 75. El muelle 75 tiene una longitud maxima y sirve para mantener energicamente la tension aplicada sobre el anillo 20 de la valvula mitral. A este fin, el dispositivo 70 incluye un primer anclaje 72, el miembro de conexion 74 y un segundo anclaje 76.
Los primer y segundo anclajes 72 y 76 estan, de nuevo, configurados de forma que cuando son soltados, se autoexpanden, para acunarse contra la pared interna del seno coronario 14. De nuevo, el primer anclaje resiste el movimiento proximal y el segundo anclaje 76 resiste el movimiento distal. En todos los demas aspectos, el dispositivo 70 puede ser identico a y ser desplegado de la misma manera que el dispositivo 30.
Los dispositivos de estimulacion cardiaca implantables son bien conocidos en la tecnica. Tales dispositivos pueden incluir, por ejemplo, marcapasos y desfibriladores cardiacos implantables. Los dispositivos son implantados generalmente en la region pectoral del pecho por debajo de la piel de un paciente dentro de lo que se conoce como una bolsa subcutanea. Los dispositivos implantables funcionan generalmente en asociacion con uno o mas electrodos que llevan derivaciones las cuales son implantadas dentro del corazon. Los electrodos son colocados usualmente dentro del lado derecho del corazon, bien dentro del ventriculo derecho o de la auricula derecha, o de ambos, para hacer contacto electrico con su respectiva camara cardiaca. Conductores en el interior de las derivaciones y un conector proximal llevado por las derivaciones acoplan los electrodos al dispositivo para habilitar el dispositivo para detectar la actividad electrica cardiaca y entregar la terapia deseada.
Tradicionalmente, la entrega de la terapia ha sido limitada al lado venoso o derecho del corazon. La razon para esto es que los electrodos implantados pueden causar formacion de coagulos de sangre en algunos pacientes. Si un coagulo de sangre fuera liberado arterialmente desde el corazon izquierdo, como por ejemplo el ventnculo izquierdo, podria pasar directamente al cerebro dando como resultado potencialmente un derrame cerebral paralizante o fatal. Sin embargo, un coagulo de sangre liberado desde el corazon derecho, como desde el ventnculo derecho, pasana a los pulmones en donde la accion de filtrado de los pulmones impedina un embolismo fatal o debilitante en el cerebro.
Recientemente, se han propuesto nuevas estructuras de derivacion y metodos e incluso practicado para entregar terapia de regulacion del ritmo cardiaco al corazon izquierdo. Estas estructuras de derivacion y metodos evitan la colocacion directa de electrodos dentro de la auricula izquierda y el ventriculo izquierdo del corazon mediante la implantacion de la derivacion dentro del seno coronario del corazon. Como se menciono previamente, la expresion “seno coronario” se refiere a no solo el propio seno coronario sino, ademas, al sistema venoso asociado con el seno coronario que incluye la vena cardiaca magna.
Se ha demostrado que los electrodos colocados en la region del seno coronario del corazon pueden ser usados para la estimulacion auricular izquierda, la estimulacion ventricular izquierda o cardioversion y desfibrilacion. Estos avances posibilitan dispositivos de estimulacion cardiaca implantables para abordar las necesidades de una poblacion de pacientes con disfuncion ventricular izquierda y/o insuficiencia cardiaca congestiva los cuales se beneficiarian de estimulacion del lado izquierdo del corazon, bien solo o en conjunto con estimulacion del lado derecho del corazon (estimulacion bicameral) y/o desfibrilacion.
Incluso aunque el dispositivo de la presente invencion es implantable en el seno coronario del corazon, esta configurado de acuerdo con otros aspectos de la presente invencion para permitir que una derivacion cardiaca pase a traves del seno coronario para funcionar como se describio arriba. A este fin, y como se ve mejor en la figura 7, los anclajes 32 y 36 del dispositivo 30 ocupar solo una pequena porcion de, y por tanto menos de todo el espacio interior, del seno coronario 14. Esto permite que una derivacion cardiaca 80 sea avanzado al interior del seno coronario 14 para implantarlo en el lado izquierdo del corazon.
Mas espedficamente, los anclajes 32 y 36 toman la forma de lazos 33 y 35 respectivamente, los cuales son, entonces, curvados hacia atras en el dispositivo para formar las formas de gancho a las que se hizo referencia previamente, para su autodespliegue. Los lazos 33 y 35 permiten, asf, que la derivacion cardiaca 80 sea pasado a traves de ellos para su implantacion en el corazon izquierdo. Esto es particularmente deseable porque muchos 5 pacientes que sufren de reflujo mitral pueden tambien ser candidatos a terapias de regulacion del ritmo cardiaco del corazon izquierdo.
Claims (6)
- 510152025REIVINDICACIONES1. un dispositivo (30) que afecta a la geometna del anillo (20) de la valvula mitral de un corazon (10), que comprende:un primer anclaje (32) configurado para ser anclado dentro del seno coronario (14) del corazon (10) adyacente al anillo (20) de la valvula mitral dentro del corazon (10);un segundo anclaje (36) configurado para ser colocado dentro del corazon (10) proximal a el primer anclaje (32) y adyacente al anillo (20) de la valvula mitral dentro del corazon (10); yun miembro de conexion (34) que tiene una longitud fija unido de manera permanentemente a los primer (32) y segundo (36) anclajes, por medio del cualcuando los primer y segundo anclajes estan dentro del corazon con el primer anclaje anclado en el seno coronario, el segundo anclaje puede ser desplazado proximalmente para afectar a la geometna del anillo de la valvula mitral y soltada para mantener el efecto sobre la geometna de la valvula mitral,caracterizado por que el primer anclaje se curva hacia atras y forma un lazo (33), y el segundo anclaje que se curva hacia atras y forma un lazo (35).
- 2. Un dispositivo (30) de la reivindicacion 1, en el que el segundo anclaje (36), cuando esta desplegado, esta configurado para anclarse contra o es anclado contra el movimiento distal y se puede mover en una direccion proximal.
- 3. Un dispositivo (30) de la reivindicacion 1, en el que el primer anclaje (32) se autodespliega al ser soltado en el seno coronario (14).
- 4. Un dispositivo (30) de la reivindicacion 1, en el que el segundo anclaje (36) se autodesplega al ser soltado en el seno coronario (14).
- 5. Un dispositivo (30) de la reivindicacion 1, en el que el miembro de conexion (34) es un miembro ngido.
- 6. Un dispositivo (30) de la reivindicacion 1, en el que el miembro de conexion (34) es flexible y no extensible.
