- 【特許請求の範囲】 1.虚血により損傷を受けた器官の修復を、該器官の機能障害が反転し得る程 度にまで誘導するための方法であって: 前記器官を、生理的緩衝溶液を用いて約28℃〜約37℃の温度でフラッシュする ことにより、血液および血流喪失の間に前記器官に蓄積したアシドーシス生成物 を除去することと; 前記器官を、該器官内の血管を拡張するための手段、細胞の完全性および細胞 機能を再確立することにより器官の機能を修復するための手段、および前記器官 を酸素添加された環境に再適用する際に該器官内の酸化的代謝を再確立するため の手段を更に含む生理的緩衝溶液を用いて、約28℃〜約37℃の温度で潅流するこ ととを具備した方法。 2.請求項1に記載の方法であって、前記器官を潅流するための生理的緩衝用 液が、更に、前記器官に血液循環が再生される際の器官組織の損傷を防止する手 段を含んでいる方法。 3.請求項1に記載の方法であって、前記酸化的代謝を再確立するための手段 が、酸素添加器および酸素輸送剤からなる群から選択されたモードにより分子状 酸素を導入することを含む方法。 4.請求項1に記載の方法であって、前記潅流は、前記生理学的緩衝用液を供 給する層流ポンプシステムまたは脈動ポンプシステムを含む装置を用いて行われ 、該装置は更に、潅流および潅流圧を提供および制御するための手段と、温度制 御のための手段と、呼吸ガスの導入および換気を提供および制御するための手段 と、既に前記器官を通して潅流された生理的緩衝用液を試験して、pH、種々の 圧力、流速、血管抵抗性、化学的成分、酸素添加、二酸化炭素濃度および酸素消 費のような一以上の機能的特性をモニターおよび測定する試験または採取のため の手段とを具備する方法。 5.請求項4に記載の方法であって、前記潅流は、前記第一の装置と組み合わ せて、前記器官から放出される器官生成物を試験するために試験または採取する 第二の装置を用いて行われ、その後の前記器官生成物のパラメータの測定は、器 官の完全性および機能に関連する方法。 6.虚血性損傷を防止するため、および虚血により損傷を受けた器官の修復を 、該器官の機能障害が反転し得る程度にまで誘導するための方法であって: (a)前記器官を、血液および血流喪失の間に前記器官に蓄積したアシドーシ ス生成物を除去する生理的緩衝用液(これは更に、前記器官を生理的pHに修復 するための手段、前記器官の微小血管を拡張するための手段、進行中の嫌気的代 謝を指示する手段、建機的代謝から酸化的代謝への反転を開始させるための手段 、細胞の完全性および細胞機能を回復させるための代謝物を含む)を用いてフラ ッシュすることと、 (b)前記器官を、生理学的緩衝用液(これは更に、器官の酸素添加、器官の 温度、および器官のpHを正常化するための手段、器官の血管を拡張するための 手段、細胞の完全性および細胞機能を回復するための代謝物を含む)を用いて潅 流することとを具備した方法。 7.請求項6に記載の方法であって、前記器官を潅流するための生理学的緩衝 溶液は、更に、前記器官への血液循環の回復の際に、器官の組織損傷を防止する ための手段を含有している方法。 8.請求項6に記載の方法であって、前記酸化的代謝を再確立するための手段 が、酸素添加器または酸素輸送剤からなる群から選択されるモードにより分子状 酸素を導入することを含む方法。 9.請求項6に記載の方法であって、前記潅流は、前記生理学的緩衝用液を供 給する層流ポンプシステムまたは脈動ポンプシステムを含む装置を用いて行われ 、該装置は更に、潅流および潅流圧を提供および制御するための手段と、温度制 御のための手段と、呼吸ガスの導入および換気を提供および制御するための手段 と、既に前記器官を通して潅流された生理的緩衝用液を試験して、pH、種々の 圧力、流速、血管抵抗性、化学的成分、酸素添加、二酸化炭素濃度および酸素消 費のような一以上の機能的特性をモニターおよび測定する試験または採取のため の手段とを具備する方法。 10.請求項9に記載の方法であって、前記潅流は、前記第一の装置と動作的 に関連した、前記器官から放出される器官生成物を試験するために試験または採 取する第二の装置を用いて行われ、その後の前記器官生成物のパラメータの測定 は、器官の完全性および機能に関連する方法。 11.虚血による損傷を阻止するため、および虚血により損傷を受けた器官の 修復を、該器官の機能障害が反転し得る程度にまで誘導するための蘇生溶液であ って:該蘇生溶液は更に、 前記器官の血管を拡張するための生理学的に有効な量の血管拡張剤と; 器官の血流喪失の間に失われた酸化的代謝を再確立するための、生理学的有効 量の化学エネルギー基質と; 器官の血流喪失の間に失われた細胞機能を再確立する一以上の細胞修復プロセ スを促進するための、生理学的有効量の栄養素とを含有する蘇生溶液。 12.請求項11に記載の蘇生溶液であって、前記血管拡張剤は、内皮細胞媒 介性血管拡張の基質、微小血管拡張の基質、およびカルシウムチャンネルブロッ カーからなる群から選ばれる成分の組み合わせを含む蘇生溶液。 13.請求項12に記載の蘇生溶液であって、前記内皮細胞媒介性血管拡張の 基質はアセチルコリンおよびアルギニンを含み、前記微小血管拡張のための基質 はプロスタサイクリン(prostacycline)およびMg+を含み、また前記カルシウム チャンネルブロッカーはアデノシンおよびベラパミル(verapamil)を含む蘇生溶 液。 14.請求項11に記載の蘇生溶液であって、前記化学エネルギー基質は、ア デニン化合物プールの成分、クエン酸サイクルの成分、および結合電子輸送鎖の 成分からなる群から選ばれる成分の組み合わせを含む蘇生溶液。 15.請求項14に記載の蘇生溶液であって、前記化学エネルギー基質は、ピ ルビン酸塩、グルコース、ATP、AMP、補酵素A、β-ニコチンアミドアデ ニンジヌクレオチド(Nad+)、フラビンアデニンジヌクレオチド(FAD) 、チアミン、塩化ピロリン酸塩(コカルボキシラーゼ)、UTP、塩素陰イオン 、マグネシウム、およびこれらの組み合わせからなる群から選ばれる蘇生溶液。 16.請求項11に記載の蘇生溶液であって、前記栄養素は、アミノ酸、マグ ネシウム、核酸誘導体、リボヌクレオシド、産生繊維芽細胞増殖因子(FGF) 、塩基性FGF、ヘパリン、硫酸コンドロイチン、およびこれらの組み合わせか らなる群から選ばれる蘇生溶液。 17.請求項11に記載の蘇生溶液であって、該溶液は更に、生理学的有効量 の、抗酸化剤、酸素運搬剤およびこれらの組み合わせからなる群から選ばれる成 分を含有する蘇生溶液。 18.請求項11に記載の蘇生溶液であって、前記溶液は更に、薬学的有効量 の神経保護薬を含有する蘇生溶液。 19.虚血により損傷を受けた器官の修復を、該器官の機能障害が反転し得る 程度にまで誘導するための方法であって: 該方法は約28℃〜約37℃の温度で行われ、前記器官をフラッシュすること、お よび請求項11に記載の蘇生溶液を用いて前記器官を潅流することを具備した方 法。 20.虚血により損傷を受けた器官の修復を、該器官の機能障害が反転し得る 程度にまで誘導するための方法であって: 該方法は約28℃〜約37℃の温度で行われ、前記器官をフラッシュすること、お よび請求項17に記載の蘇生溶液を用いて前記器官を潅流することを具備した方 法。 21.虚血性損傷を防止するため、および虚血により損傷を受けた器官の修復 を、該器官の機能障害が反転し得る程度にまで誘導するための方法であって: 該方法は約28℃〜約37℃の温度で行われ、前記器官をフラッシュすること、お よび請求項11に記載の蘇生溶液を用いて前記器官を潅流することを具備した方 法。 22.虚血性損傷を防止するため、および虚血により損傷を受けた器官の修復 を、該器官の機能障害が反転し得る程度にまで誘導するための方法であって: 該方法は約28℃〜約37℃の温度で行われ、前記器官をフラッシュすること、お よび請求項17に記載の蘇生溶液を用いて前記器官を潅流することを具備した方 法。 23.虚血性損傷を防止するため、および虚血により損傷を受けた器官の修復 を、該器官の機能障害が反転し得る程度にまで誘導するための溶液を調製する方 法であって:生理学的緩衝用液に添加剤を加えることを具備し、 該添加剤は、前記器官の血管を拡張するための生理学的有効量の血管拡張剤 と;器官の血流喪失の間に失われた酸化的代謝を再確立するための生理学的有効 量の化学エネルギー基質と;器官の血流喪失の間に失われた細胞機能を再確立す る一以上の細胞修復プロセスを促進するための、生理学的有効量の栄養素とを含 み、 前記溶液は、約6.5〜7.5の範囲のpHおよび330mOsmを超える浸透圧を有 するように調節される方法。 24.請求項23に記載の方法であって、前記血管拡張剤は、内皮細胞媒介性 血管拡張の基質、微小血管拡張の基質、およびカルシウムチャンネルブロッカー からなる群から選ばれる成分の組み合わせを含む方法。 25.請求項24に記載の方法であって、前記内皮細胞媒介性血管拡張の基質 はアセチルコリンおよびアルギニンを含み、前記微小血管拡張のための基質はプ ロスタサイクリン(prostacycline)およびMg+を含み、また前記カルシウムチャ ンネルブロッカーはアデノシンおよびベラパミル(verapamil)を含む方法。 26.請求項23に記載の方法であって、前記化学エネルギー基質は、アデニ ン化合物プールの成分、クエン酸サイクルの成分、および結合電子輸送鎖の成分 からなる群から選ばれる成分の組み合わせを含む方法。 27.請求項26に記載の方法であって、前記化学エネルギー基質は、ピルビ ン酸塩、グルコース、ATP、AMP、補酵素A、β-ニコチンアミドアデニン ジヌクレオチド(Nad+)、フラビンアデニンジヌクレオチド(FAD)、チ アミン、塩化ピロリン酸塩(コカルボキシラーゼ)、UTP、塩素陰イオン、マ グネシウム、およびこれらの組み合わせからなる群から選ばれる方法。 28.請求項23に記載の方法であって、前記栄養素は、アミノ酸、マグネシ ウム、核酸誘導体、リボヌクレオシド、産生繊維芽細胞増殖因子(FGF)、塩 基性FGF、ヘパリン、硫酸コンドロイチン、およびこれらの組み合わせからな る群から選ばれる方法。 29.請求項23に記載の方法であって、該溶液は更に、生理学的有効量の、 抗酸化剤、酸素運搬剤およびこれらの組み合わせからなる群から選ばれる成分を 含有する方法。 30.請求項23に記載の方法であって、前記溶液は更に、薬学的有効量の神 経保護薬を含有する方法。
