Abstract
【解決手段】本発明は、組織の焼灼および切除に関する。一対の電極が、対向する組織表面上に配置され、高周波数電力が電極を通じて適用され、電極の間の組織部分が焼灼される。焼灼が実施された後、焼灼された領域内の平面に沿って、最小限の出血または出血することなく、組織を切除することができる。次いで、組織部分を取り除くことができる。

現時点で好ましい実施例においては、電源装置は、定電圧または定電流設計ではなく、一定出力の電力設計から成る。本発明の設計においては、電力出力は、システムにかかる負荷に基づいて設定される。従って、システムに非常に高いインピーダンスの負荷がかかる場合、アーク放電を避けるため、電圧は妥当なレベルに維持される。電力発生器が使用される用途(すなわち電気焼灼)においては、インピーダンスの範囲は、組織の焼灼中、例えば、2オームから150オームの間で変化しうる。本発明の電力源は、一定の電力を適用することによって、最初に組織が焼灼されるときの初期乾燥を達成するため、低いインピーダンスにおいて大きな電流を提供し、焼灼が進行するにつれて、組織の封鎖プロセスを完了するために、より高い電圧を印可する。このように、本発明は、焼灼プロセスの開始時には、より大きな電流と、より低い電圧とを提供し、プロセスの封鎖段階においては、より高い電圧と、より小さい電流とを提供する。このような電力発生器を制御するうえで要求されることは、システムが電力をモニタすることのみである。

Claims (17)
- 対向する一対のジョーを備えるプローブであって、少なくとも1個の当該ジョーが、組織部分の処置表面と係合する複数の電極を備える、プローブと、
少なくとも1個の当該ジョーに結合されている少なくとも1個の関節接合素子であって、当該関節接合素子が、ピボットを介してジョーが結合されている補助ジョー・アームを備える、関節接合素子と、
を備える、組織を焼灼する装置であって、
当該プローブが、更に、
少なくとも2個のハンドル素子とハンドル係合表面を備えるハンドルと、
両方のジョーを当該ジョーの実質的な末端部で回転可能に合体させるピボット・ポイントであって、当該ジョーが、前記ジョーの間の角度を減少させ、かつ当該組織部分に接触している前記ジョーの表面に沿って力を均一に分布させる第三種のレバーとして、構成されている、ピボット・ポイントと、
アクチュエータ表面を備えるアクチュエータとを、
備え、
当該少なくとも1個のハンドル素子の当該ピボット軸についての回転が、当該係合表面を当該アクチュエータ表面に係合させ、当該アクチュエータを当該プローブの手前側端部の方に向かわせ、その結果当該レバー機構が動作して、当該ジョーを互いに向かせる、
組織を焼灼する装置。 - 当該少なくとも1個の関節接合素子が、当該ジョーの何れかまたは両方に結合している、請求項1に記載の装置。
- 何れかまたは両方の当該ジョーに結合している複数の関節接合素子をさらに備える、請求項1に記載の装置。
- 刃と、
刃ホルダーと、
当該ジョーの間に保持されている組織に渡って当該刃を前進させかつ後退させるように、当該刃ホルダーが、移動可能なトラックと、
前記ジョーが開放位置にあるときに前記刃が前進することを阻止するインタロック機構と協働するカムを備える当該刃に結合する突起と、
を備える切断機構を、更に備える、請求項1に記載の装置。 - 当該刃が、V字形状を有する2個の切断エッジを備え、前記刃が前進する際に、前記組織部分の上部と底部の両方を切断するために、両方のエッジが出会うポイントに前記組織部分を押し付ける、請求項4に記載の装置。
- 対向する一対のジョーを備えるプローブであって、少なくとも1個の当該ジョーが、組織部分の処置表面と係合する複数の電極を備えるプローブと、
少なくとも2個のハンドル素子とハンドル係合表面を備えるハンドルと、
当該ハンドル素子の動作が当該ピボット軸について少なくとも1個の当該ハンドル素子を回転させる、ピボット軸と、
アクチュエータ表面を備えるアクチュエータと、
4部品クロスビーム組立体と、
を備え、
当該少なくとも1個のハンドル素子の当該ピボット軸についての回転が、当該係合表面を当該アクチュエータ表面に係合させ、当該アクチュエータを当該プローブの手前側端部の方に向かわせ、その結果、当該4部品クロスビーム組立体が動作し、当該ジョーが平行状態で互いに向き、かつ当該組織部分と接触する前記ジョーの表面に沿って力が均一に分布する、
組織を焼灼する装置。 - 当該ジョーの何れかまたは両方に結合している少なくとも1個の関節接合素子を備える、請求項6に記載の装置。
- 何れかまたは両方の当該ジョーに結合している複数の関節接合素子をさらに備える、請求項6に記載の装置。
- 当該関節接合素子が、ジョーがピボットを介して結合されている補助ジョー・アームを備える、請求項7に記載の装置。
- 刃と、
刃ホルダーと、
当該ジョーの間に保持されている組織に渡って当該刃を前進させかつ後退させるように、当該刃ホルダーが、移動可能なトラックと、

- 少なくとも1個のジョーに結合する少なくとも1個の関節接合された素子に設けられた複数の個々の電極または電極アレーをさらに備える、請求項1に記載の装置。
- 各関節接合された素子が、さらに当該電極に対する電力発生器にフィードバックを与えるロードセルをさらに備える、請求項12に記載の装置。
- 当該電極が、弾性材料上にフォトリソグラフィ、蒸着、スプレイ、または浸漬被覆型プロセスにより形成される、請求項12に記載の装置。
- 当該処置表面間の組織を焼灼するためにそこに高周波電力を選択的に印加する当該電極に接続可能な電源と、
当該電極および/または当該組織の近傍の当該高周波電力をモニタし、かつ電流、電圧、インピーダンス、エネルギ、電力、時間、および温度の内の任意の2個に基づいて、当該高周波電力を変調する手段と、
を備える、請求項12に記載の装置。 - 当該少なくとも1個の電極が、適合性の、導電表面を備える、請求項12に記載の装置。
