Description
本願は、自然の血管から血栓を除去するための回転式血栓摘出ワイヤに関する。
特定の場合では、使用者によって連結される分離可能な血栓摘出ワイヤ及びモータを提供するのが有利である。これにより、ワイヤの挿入を容易にでき、様々なモータ及び/又はバッテリーと交換できる。
ガイドワイヤを大腿動脈に導入する工程と、
ガイドワイヤを血管系を通して大脳動脈に挿入する工程と、
カテーテルチューブをガイドワイヤ上で大脳動脈に挿入する工程と、
ガイドワイヤを除去する工程と、
カテーテルチューブの近位端に導入器を配置する工程と、
カバーで覆われたコイル状チップを持つ血栓摘出ワイヤを、導入器を通して、カテーテルチューブ内に挿入する工程と、
血栓摘出ワイヤを大脳動脈まで前進する工程と、
モータを血栓摘出ワイヤの近位端に作動的に連結する工程と、
モータを作動して血栓摘出ワイヤを回転し、大脳動脈内の血栓を浸軟する工程とを含む方法を提供する。
本開示の好ましい実施例を、添付図面を参照して説明する。
図1の血栓摘出装置には、全体に参照番号10が付してある。この装置は、ハウジング12及びこのハウジングから延びる血栓摘出ワイヤ30を含む。
ワイヤ260をシース内(導入器シース内、又は他の実施例において装置ハウジングから延びるシース内)に一杯に引っ込めたとき、ワイヤ260のカバーが設けられた領域が圧縮され、遠位領域276が実質的に直線状をなした非展開形状で収容される。ワイヤ260のこのカバーにより、導入器シースを通した挿入及び血管構造内での操作が容易になる。可撓性シースを軸線方向近位方向に移動することによって引っ込めたとき、又はワイヤをシースに関して前進したとき、又はワイヤ及びシースの両方を互いに関して移動したとき、このような相対的移動によりワイヤ260の遠位領域276を露呈し、ワイヤ260を、血管の内腔内でその長さ方向軸線を中心として回転するため、図7に示す実質的に直線状でない正弦波形状に戻す。シース及びワイヤの相対的移動とは、これらの構成要素のうちの一方の移動及びこれらの構成要素の両方の移動を含むということに着目されたい。
コイル状チップが形状記憶材料で形成された実施例では、記憶された形状は、図7に示す正弦波形状又はS字形状、又は図6に示すJ字形状である。シース内での比較的柔らかな状態では、ワイヤは、実質的に直線状形状をなしている。この状態は、ワイヤを手術箇所まで送出するために使用される。ワイヤが比較的高い体温に露呈されたとき、チップはオーステナイト状態になり、記憶されたS字形状をとる。コイル状チップは、別の態様では、「S字形状」に予備成形された放射線不透過性コイル/ポリマーで形成されていてもよい。

Claims (27)
- 血管血栓又は他の障害物を破壊するための血栓摘出装置において、
回転式血栓摘出ワイヤを含み、
前記回転式血栓摘出ワイヤは、
近位部分及び遠位部分を持ち、前記遠位部分が前記近位部分よりも小径のコアと、前記コアに連結され、前記コアから遠位方向に延びる、第1カバー材料がその外側に被せて位置決めされたケーブルと、前記ケーブルの遠位部分に取り付けられた第1コイルとを含み、前記第1コイルは、前記ケーブルの直径よりも大径であり、第2カバー材料がその外側に被せて位置決めされており、前記ワイヤはモータによって回転できる、血栓摘出装置。 - 請求項1に記載の血栓摘出装置において、更に、
前記コアの前記遠位部分の一領域に被せて位置決めされた、前記第1コイルから近位方向に間隔が隔てられた第2コイルを含む、血栓摘出装置。 - 請求項1又は2に記載の血栓摘出装置において、
前記第1コイルは第1長さを有し、前記第1コイルは前記ワイヤの長さ方向軸線に対して所定角度で位置決めされている、血栓摘出装置。 - 請求項3に記載の血栓摘出装置において、
前記第1コイル及び前記第1コイルの下にある前記ケーブルの一部が正弦波形状を有する、血栓摘出装置。 - 請求項3に記載の血栓摘出装置において、
前記ワイヤは、Jチップで終端する、血栓摘出装置。 - 請求項1乃至5のうちのいずれか一項に記載の血栓摘出装置において、
前記第1カバー材料はポリマージャケットを含み、前記第2カバー材料は、熱収縮性チューブを含む、血栓摘出装置。 - 請求項1乃至6のうちのいずれか一項に記載の血栓摘出装置において、
