Claims (1)
- 【特許請求の範囲】 1.体内の器官を後退させて隣接組織へのアクセスを確保する装置であって 拡大された状態に膨張されると該器官を後退させる、第一エンベロープを有する 第一膨張手段と、体内の所定位置にある該第一膨張手段を拡大された状態に選択 的に膨張させる手段と、 該第一エンベロープの一部に装着される、該第一膨張手段内部の非加圧チャンバ と、 該第二膨張手段が膨張されるときに該非加圧チャンバを拡大された状態に拡大さ せるため、該非加圧チャンバと作動上関連される第二膨張手段と、該第二膨張手 段を選択的に膨張させる手段とを含む後退装置。 2.請求項1に記載の装置において該第二膨張手段がケージ構造体を有する、装 置。 3.請求項1に記載の装置において該第二膨張手段がトロイダルバルーンを含む 、装置。 4.請求項1に記載の装置であってさらに該第一膨張手段から該非加圧チャンバ 中に物体を通過させることができる実質上気密な通路を与えるポート手段を含む 、装置。 5.請求項4に記載の装置において該ポート手段がさらに、 (a)該通路中に挿入された物体周囲の実質的気密のシールと、 (b)該物体が該通路から除去されるときの実質的気密シールとを与えることに (2)該第一膨張性チャンバ内の非加圧チャンバにして該第一エンベロープの一 部に装着される第二エンベロープを有する非加圧チャンバと、(3)該非加圧チ ャンバに作動上関連された第の膨張性チャンバとを含む膨張性後退器を与える段 階と、(b)該組織に隣接させて、畳んだ状態で該膨張性チャンバを配置する段 階と、 (c)該器官を後退させるべく該第一膨張性チャンバを膨張させる段階と、 (d)該非加圧チャンバを拡大した状態に拡大すべく該第二膨張性チャンバを膨 張させる段階とを含む方法。 11.請求項10に記載の方法において該非加圧チャンバを拡大された状態に拡 大させるため該第二チャンバを膨張させる該段階が 該非加圧チャンバを拡大された状態に膨張させる段階と、 該第二膨張性チャンバを拡大された状態に膨張させる段階と、 該非加圧チャンバを収縮させる段階とを含む、方法。 12.請求項11に記載の方法であってさらに切断器具を与える段階と、 該第一膨張性チャンバと該非加圧チャンバとの間に実質的に気密な通路を与える 段階と、該切断器具を該第一膨張地中に通過させる段階と、該実質的に気密な通 路を通して該切断器具を該非加圧チャンバ中に通過させる段階にして、該実質的 気密通路が該切断器具を実質的に気密なシールを形成する段階と、 膨張させる段階とを含む方法。 24.請求項23に記載の装置において該膨張性後退装置を畳んだ状態で該内視 鏡の遠方端に隣接させて該内視鏡に固着させる段階が、 該内視鏡の遠方端を該非加圧チャンバ中に挿入する段階を含む、方法。 25.請求項23に記載の方法において該非加圧チャンバを拡大された状態に拡 大させるため該第二チャンバを膨張させる該段階が 該非加圧チャンバを拡大された状態に膨張させる段階と、 該第二膨張性チャンバを拡大された状態に膨張させる段階と、 該非加圧チャンバを収縮させる段階とを含む、方法。 26.請求項25に記載の方法であってさらに切断器具を与える段階と、 該第一膨張性チャンバと該非加圧チャンバとの間に実質的に気密な通路を与える 段階と、該切断器具を該第一膨張地中に通過させる段階と、該実質的に気密な通 路を通して該切断器具を該非加圧チャンバ中に通過させる段階にして、該実質的 31.請求項30に記載の方法であってさらに該第一膨張性チャンバと該非加圧 チャンバとの間に実質的に気密な通路を与える段階を含み、該手術器具を該非加 圧チャンバ中に通過させる該段階が該手術器具を実質的に気密のポートを通して 通過させる段階を含む、方法。 32.請求項30に記載の方法であってさらに該非加圧チャンバと該第一エンベ ロープとの間にダクトを与える段階を含み、該手術器具を該非加圧チャンバ中に 該器官を後退下状態に後退させるべく、体内の所定位置にある間、該主膨張性チ ャンバを選択的に膨張させる手段と、 該主膨張性チャンバに作動上関連されて、該主膨張性チャンバが収縮された後、 該器官をその後退位置に維持するための維持手段とを含む後退装置。 37.請求項36に記載の装置において該維持手段が追加の膨張性チャンバを含 む、装置。 44.体内の器官を後退させて隣接する組織へのアクセスを獲得する方法であっ て、 (a)主膨張性チャンバおよび維持器を与える段階と、(b)該維持器および主 膨張性チャンバを組み立てる段階と、 (c)該維持器および主膨張性チャンバ組立体を畳んだ状態で該器官に隣接させ て配置する段階と、(d)該器官を後退した状態に維持すべく該主膨張性チャン バを膨張させる段階と、 (e)該器官をその後退した状態に維持すべく該維持器を拡大する段階と、 (f)該主膨張性チャンバを収縮する段階とを含む方法。 45.請求項44に記載の方法であってさらに該主膨張性チャンバを体外に除去 する段階を含む方法。 46.請求項44に記載の方法において該維持器および該主膨張性チャンバを組 み立てる該段階が、該主膨張性チャンバの内側に該維持器を配置する段階を含み 、 該器官をその後退した状態に維持すべく該維持器を拡大する段階が該維持器を膨 張させる段階を含む、方法。 47.請求項44に記載の方法において該維持器および該主膨張性チャンバを組 み立てる該段階が、該維持器の内側に該主膨張性チャンバを配置する段階を含み 、 該器官をその後退した状態に維持すべく該維持器を拡大する段階が該主膨張性チ ャンバを膨張させる段階を含む、方法。 48.請求項44に記載の方法において該維持器および該主膨張性チャンバを組 み立てる該段階が、該維持器の内側に該主膨張性チャンバを配置する段階を含み 、 該器官をその後退した状態に維持すべく該維持器を拡大する段階が該維持器を膨 張させる段階を含む、方法。 49.膨張性後退装置を膨張させる方法であっって、主膨張性チャンバおよび追 加膨張性チャンバを有する膨張性後退装置を与える段階と、 液体に粒状固体を混ぜたスラリーを与える段階と、主膨張性チャンバを膨張させ る段階と、該追加膨張性チャンバから該液体を除去して後に該粒状固体を残す段 階と、 該粒状固体を固めて実質的に剛性の構造体を形成する段階とを含む方法。 50.請求項49に記載の方法において該粒状固体が予定寸法より大きな粒子を 有し、該追加チャンバから該液体を除去して該粒状固体を残す該段階が 該粒状固体の粒子の概数歩うよりもちいさな網目のフィルターを与える段階と 該追加膨張性チャンバから該フィルターを通して該液体をポンプで汲み出す段階 を含む、方法。 51.請求項49に記載の方法において剛性構造体を形成するため該粒状固体を 固める該段階が該膨張性チャンバを脱気する段階を含む、方法。 52.請求項49に記載の方法において剛性構造体を形成するため該粒状固体を 固める該段階が該粒状固体を加熱する段階を含む、方法。 53.請求項49に記載の方法において剛性構造体を形成するため該粒状固体を 固める該段階が該粒状固体を冷却する段階を含む、方法。 54.体内の組織を観察する装置であって、該組織を観察するための光学的手段 にして実質的に円筒形構造と体内に挿入される遠方端とを有する光学的手段と、 該組織に隣接した器官であって後退させなければ該組織の観察を妨げる器官を後 退させるための後退手段にして、該光学的手段の遠方端に隣接して該光学手段に 装着されると共に、該光学的手段の該遠方端が体内に挿入されるときは折り畳ま れた状態になる後退手段と、体内で該後退手段を拡大する手段とを含む装置。 55.請求項54に記載の装置において該後退手段が膨張性チャンバを含み、 該後退手段を体内で拡大させる該手段が該膨張性チャンバを膨張させる手段を含 む、装置。 56.請求項55に記載の装置において該膨張性チャンバが 中空の円錐台形状を有し、 該光学的手段と実質的に共心的であり、かつ遠方側に向けて増大する直径を有す る、装置。 57.請求項55に記載の装置において該膨張性チャンバが、直径の漸次増大す る重畳された膨張性トロイダルバルーンを含む、装置。 58.請求項55 に記載の装置において該後退手段が 該光学的手段の該遠方端に装着された透明なカラーと、 該光学的手段に滑動可能に装着されたカラーと、該透明なカラーおよび該カラー に付着された複数の細長い可撓性部材とを含み、該後退手段を拡大する該手段が 該透明なカラーに向けて該光学的手段に沿って該カラーを滑動させる手段を含む 、装置。 59.請求項58に記載の装置において該細長い可撓性部材が透明プラスチック 製ストリップである、装置。 60.請求項58に記載の装置において該細長い可能性部材がワイヤである、装 置。 61.近接端および遠方端を有する実質的に円筒形の物体を体内に挿入するため の装置であって、該物体を滑動可能に受容することができ、近接端および遠方端 を有する孔を有する細長い管状部材(細長管)と、 該管状部材の該近接端から該遠方端まで該孔内を通過する紐と、 該紐の該孔内側を通過する部分上にあって、該紐を該物体の近接端に装着する手 段とを含む装置。 62.請求項61に記載の装置において該紐が該孔の外側の該管状部材近接端に 戻る、装置。 63.請求項61に記載の装置であってさらに該管状部材の遠方端における小穴 を含み、該紐が該小穴を通過し、該孔を通って該管状部材の該近接端に戻る、装 置。 (a)(1)近接端お帯び遠方端を有する孔と遠方端とを有する細長い管状部材 と、 (2)該孔内を、該細長い管状部材の該近接端から該遠方端まで通過し、該細長 い管状部材の近接端まで戻る紐とを含む挿入管を与える段階と、(b)該孔内を 通過する該紐を、該物体の近接端に付著させる段階と、 (c)該物体の遠方端を該孔の近接端中に挿入する段階と、 (d)該細長い管状部材の該遠方端を該物体中に挿入する段階と (e)該細長い管状部材の該建設端まで戻る該紐を引いて該物体を孔に通すステ ップとを含む方法。 69.請求項68に記載の方法において該物体が畳んだ状態にされた膨張性後退 装置であり、 該膨張性後退装置が器官を後退させる装置であり、該細長い管状部材の該遠方端 を該物体中に挿入する該段階が、該細長い管状部材の該遠方端を該器官に隣接さ せて配置する段階を含み、 該細長い管状部材の該近接端まで該紐を引き戻す該段階が、該器官に隣接した該 孔から該膨張性後退装置を発出する段階を含む、 方法。
