Cell Res.153:167-172(1984))が根本的に影響される。これによって同時に、内 皮が損傷した場合には、常に(高コレステロール血症による内皮損傷(T.J.Verb euren ら、Circ.Res.58:552-564(1986)、梗塞後段階における内皮損傷(M.R.Si greid ら、Circ.86(Suppl.1):21(1992))内皮機能の病理学的影響が生じ、様 々な結果が生じることが良く分かる。これには、アテローム発生(Atherogenese) の初期段階と考えられる、局所的な血管収縮ないし血管痙攣および血管壁におけ る改造および成長過程が入る。 内皮機能障害は、内皮が仲介する生理学的な血管拡張が制限される、または失 われるのが特徴である。同時に、NOが仲介する血管弛緩、NOにより仲介され る血管保護およびNOにより抑圧された動脈内膜およびMedia における成長過程 の減少または停止が観察されている。内皮機能障害はさらに、有糸分裂生殖の増 加による血管壁における増殖過程、白血球およびマクロファージの内皮密着およ び移行の増加、ならびに内皮に有害な低密度リポタンパク質(LDL)の酸化増 加を特徴とする。この機能障害は、アテローム性動脈硬化症、高血圧、高コレス テロール血症、糖尿病および心不全の枠内における病理生理学的状態で必ず観察 される(Creagerら、J.Cli.Invest.86:228-234(1990); Linderら、Circulatio n 81:1762-1769(1990); Zeiherら、Circulation 83:391-401(1991))。同様に、 低酸素症および解除時の低せん断力も内皮機能障害に適合している。内皮機能障 害は、とりわけ、通常は血圧降下させるアセチルコリンまたはセロトニンの様な 血管活性物質が、それらの直接的な血管収縮効果のために、平滑な血管筋肉で血 管を収縮させ、そのために病気が悪影響を受ける。(Golino ら、N.Engl.J.Me d,324:641-648(1991))。したがって、生理学的な血管運動制御は、内皮機能障 害では、妨害されるだけではなく、反対になる。さらに際だっているのは、血管 内壁のアテローム性動脈硬化症性改造における変化である(Ludmer ら、N.Engl. 、または体外で分離した血管断片に対して)と、あるいは特殊な阻害剤により内 皮のNO形成を抑制すると、GTNまたはPETNの様なニトロ血管拡張剤の血 管拡張作用が強化される(Busseら、Cardiovasc.Pharmacol.14(Suppl.11):S81 -S85(1989); Kojda ら、J.Vasc.Res.29:151(1992A))。内皮のNO合成を薬理 学的に阻害すると、冠静脈に同じ影響が現れる(Kojdaら、Naunyn-Schmiedeberg Arch.Pharmacol.346:R35(1992B))。カルシウム拮抗物質、特に1,4−ジヒド ロピリジン型(DHP)の効果は、内皮を除去した後に弱まることが知られてい る(Kojdaら Bas.Res.Cardiol.86:254-256(1991))。その後の研究により、こ りわけ、グアニル酸シクラーゼに直接的または間接的に作用する化合物であるが 、その際、グアニル酸シクラーゼの間接的な刺激剤としては、グアニル酸シクラ ーゼの刺激剤を放出することができる、またはグアニル酸シクラーゼの酵素有効 濃度を他の方法で増加する、および/またはグアニル酸シクラーゼの阻害剤に対 して拮抗する様に作用する、または阻害剤の酵素有効濃度を他の方法で低下させ る化合物を意味する。グアニル酸シクラーゼの間接的な刺激剤としては、とりわ け、カルシウム拮抗物質の様に内因的なNOの形成または放出を増加させるのに 適した化合物、特に1,4−ジヒドロピリジン型の化合物、例えばニフェジピン 、フェロジピン、ニモジピン、アムロジピン、等、を使用する。同様に、内皮の キニン含有量を高くすることができる化合物の使用も有利である。特にこれは、 合物、ならびに一酸化窒素自体である。ここで、一酸化窒素の放出および/また は移動は、生体内では、薬理学的に活性な代謝物質を経由して行なわれることが 多いので、本発明では基本的にこれらの代謝物質を利用することも同様に好まし い。同時に、上記のすべての化合物の、生理学的に相容性のある誘導体を使用す ることも可能である。とりわけ、一般的に使用されている付加化合物、塩または
