Et topisk farmasøytisk preparat for adminisfrering av en tungtiøselig PDE 4-inhibitor er beskrevet. En overraskende god systemisk biotilgjengehghet observeres med derme doseringsform.
Description
Teknisk felt
Den foreliggende oppfinnelse vedrører feltet for farma-søytisk teknologi og beskriver et topisk appliserbart farmasøytisk preparat omfattende som aktiv ingrediens Roflumilast. Oppfinnelsen vedrører i tillegg fremgangsmåter for å fremstille det topisk appliserbare farmasøytiske preparat og anvendelsen for behandlingen av dermatoser.
Kjent teknikk
Cykliske nukleotid-fosfodiesterase (PDE)-inhibitorer (spesielt type 4) er for tiden av spesiell interesse som en ny generasjon av aktive ingredienser for behandling av inflammatoriske forstyrrelser, spesielt forstyrrelser i luftveiene slik som astma eller luftveisobstruksjoner (slik som, for eksempel, KOLS = kronisk obstruktiv lungesykdom). En rekke PDE 4-inhibitorer undergår for tiden advansert klinisk testing, inklusive en doseringsform for oral administrasjon omfattende den aktive ingrediens N-(3,5-di-klorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenz-amid (INN: roflumilast). Denne og andre forbindelser med en benzamidstruktur og deres anvendelse som cykliske nukleotid-f osf odiesterase (PDE)-inhibitorer er beskrevet i WO95/01338. Disse aktive ingredienser er foreslått i WO95/01338 også for behandlingen av visse forstyrrelser i huden (slik som, for eksempel, dermatoser). WO 00/53182 foreslår anvendelsen av roflumilast eller dens N-oksid for behandlingen av multippel sklerose.
For behandling av forstyrrelser i huden er det ønskelig å gi den aktive farmasøytiske ingrediens i et farmasøytisk preparat egnet for topisk applikasjon. Som fagmannen er klar over, kan imidlertid fremskaffelsen av doseringsformer for topisk applikasjon vise seg å være ekstremt vanskelig eller er umulig hvis intensjonen er å administrere en aktiv ingrediens som har en svært lav løselighet. Således er, for eksempel, vannløseligheten funnet for PDE 4-inhibitoren N-
(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetok-sybenzamid (INN: roflumilast), som er beskrevet i WO 95/01338, bare 0,53 mg/l ved 21 °C.
Beskrivelse av oppfinnelsen
Det har nå overraskende blitt funnet at topisk appliserbare farmasøytiske preparater omfattende den tungtløselige PDE 4-inhibitor roflumilast viser en svært god effekt i behandlingen av dermatoser ved lokal, dermal applikasjon. Dessuten er det fullstendig overraskende funnet, ved siden av den lokal effekt, en utmerket systemisk effekt som er sammenlignbar med den til en oral doseringsform.
Et første aspekt av oppfinnelsen er derfor anvendelse av en forbindelse valgt fra gruppen bestående av roflumilast, salter av roflumilast, N oksidet av pyridinresiduet av roflumilast og salter derav for fremstilling av et topisk appliserbar farmasøytisk preparat for dermal administrasjon for behandlingen av en dermatose valgt fra gruppen psoriasis (vulgaris), toksisk og allergisk kontakteksem, atopisk eksem, seboreisk eksem, lichen simplex, solbrenthet, pruritus i det genitoanale område, alopecia areata, hypertrofiske arr, diskoid lupus erythematosus, follikulære og ekstensive pyodermaer, endogen og eksogen akne, acne rosacea eller andre proliferative, inflammatoriske og allergiske hudforstyrrelser.
Heri beskrives et farmasøytisk preparat som kan administreres topisk og omfatter en aktiv farmasøytisk ingrediens sammen med en eller flere farmasøytiske bærere og/eller eksipienser egnet for topisk administrasjon, den aktive farmasøytiske ingrediens er en forbindelse valgt fra gruppen bestående av roflumilast, salter av roflumilast, N-oksidet av pyridinresiduet av roflumilast og salter derav.

Denne forbindelse har det kjemiske navn N-(3,5-diklorpyrid-4-yl)-3-cyklopropylmetoksy-4-difluormetoksybenzamid (INN: roflumilast). N-oksidet av roflumilast har det kjemiske navn 3-cyklopropylmetoksy-4-difluormetoksy-N-(3,5-di-klorpyrid-4-yl 1-oksid)benzamid.
Denne forbindelse med formel I, dens salter, N-oksidet av pyridinresiduet, dets salter og anvendelsen av disse forbindelser som fosfodiesterase (PDE) 4-inhibitorer er beskrevet i den internasjonale patentsøknad WO 95/01338.
Salter passende for forbindelser med formel I - avhengig av substitusjonen - er alle syreaddisjonssalter, men spesielt alle salter med baser. Spesielt kan det nevnes de farmakologisk akseptable salter av de uorganiske og organiske syrer og baser normalt anvendt i farmasøytisk teknologi. Farmakologisk uakseptable salter som, for eksempel, kan være de innledende produkter i fremgangsmåtene for fremstilling av forbindelsene som beskrives her i den indus-trielle skala, omdannes til farmakologisk akseptable salter ved prosesser kjent for fagmannen. De som er egnet er på den ene side vannløselige og vannuløselige syreaddisjonssalter med syrer slik som, for eksempel, saltsyre, hydro-bromsyre, fosforsyre, salpetersyre, svovelsyre, eddiksyre, sitronsyre, D-glukonsyre, benzosyre, 2-(4-hydroksy-benzoyl)benzosyre, smørsyre, sulfosalisylsyre, maleinsyre, laurinsyre, eplesyre, fumarsyre, ravsyre, oksalsyre, vinsyre, embonsyre, stearinsyre, toluensulfonsyre, metan-sulfonsyre eller 3-hydroksy-2-naftosyre, syrene anvendes for å fremstille saltene i det ekvimolare mengdeforhold, eller ett som skiller seg derfra - avhengig av om syren er monobasisk eller polybasisk og avhengig av hvilket salt som er ønsket.
På den annen side er salter med baser også spesielt passende. Eksempler på basiske salter som kan nevnes er litium-, natrium-, kalium-, kalsium-, aluminium-, mag-nesium-, titan-, ammonium-, meglumin- eller guanidi-niumsalter, igjen er basene som anvendes for å fremstille saltene i det ekvimolare mengdeforhold eller ett som skiller seg derfra.
Andelen (i vektprosent basert på vekten av det ferdige farmasøytiske preparat; vekt/vekt) av aktiv farmasøytisk ingrediens i det farmasøytiske preparat for anvendelse ifølge oppfinnelsen er vanligvis fra 0,001 til 50 vekt%. Andelen av aktiv farmasøytisk ingrediens er fortrinnsvis opp til 1 vekt%.
De farmasøytiske bærere og/eller eksipienser egnet for topisk administrasjon er i henhold til oppfinnelsen konvensjonelle bærere og/eller eksipienser kjent for fagmannen i forbindelse med farmasøytiske preparater for dermal administrasjon (= dermatologika). Eksempler som kan nevnes er bærere og/eller eksipienser som er egnet for å fremstille støvende pulvere, emulsjoner, suspensjoner, sprayer, oljer, salver, fete salver, kremer, pastaer, geler, skum eller løsninger, og transdermale terapeutiske systemer.
Det topiske farmasøytiske preparat for anvendelse ifølge oppfinnelsen kan fremstilles ved prosesser velkjente for fagmannen.
Konvensjonelle dermatologika og deres fremstilling, og foretrukne bærere og/eller eksipienser for de individuelle farmasøytiske preparater er beskrevet, for eksempel, i læreboken "Pharmazeutische Technologie" (Sucker, Fuchs, Speiser, Georg Tierne Verlag,1978 fra side 629).
I en første utførelsesform av oppfinnelsen er det topiske farmasøytiske preparat for anvendelse ifølge oppfinnelsen en semifast doseringsform. Eksempler som kan nevnes er, spesielt, salver (f.eks. løsningssalve, suspensjonssalve), kremer, geler eller pastaer.
01je-i-vann eller vann-i-olje emulsjoner refereres normalt til som kremer. Fremfor alt anvendt for oljefasen er fettalkoholer, f.eks. lauryl-, cetyl- eller stearylalkohol, fettsyrer, f.eks. palmitinsyre eller stearinsyre, flytende eller faste parafiner eller ozokeritt, flytende til faste vokser, f.eks. isopropylmyristat, naturlig eller delvis syntetisk fett, f.eks. kokosnøttfettsyre-triglyserid, herdede oljer, f.eks. hydrogenert peanøtt- eller ricinusolje, eller fettsyre delvise estere av glyserol, f.eks. glyserolmonostearat eller glyseroldistearat. Passende emulgatorer er overflateaktive substanser, f.eks. ikke-ioniske surfaktanter, f.eks. fettsyreestere av polyalkoholer eller etylenoksidaddukter derav, slik som poly-glyserolfettsyreestere eller polyoksyetylen-sorbitan-fettsyreestere (Tween®: ICI,) sorbitan-fettsyreestere (Span®: ICI), slik som, for eksempel, sorbitanoleat og/eller sorbitanisostearat, steroler, også polyoksy-etylenfettalkoholetere eller fettsyreestere, eller anioniske surfaktanter slik som alkalimetallsalter av fettalkoholsulfater, f.eks. natriumlaurylsulfat, natrium-cetylsulfat eller natriumstearylsulfat, som normalt anvendes i nærvær av nevnte fettalkoholer, f.eks. cetyl-alkohol eller stearylalkohol. Det er mulig å tilsette til den vandige fase blant annet midler som forhindrer at kremen tørker ut, f.eks. polyalkoholer slik som glyserol,
sorbitol, propylenglykol og/eller polyetylenglykoler, også konserveringsmidler, dufter etc.
Salver kan være vannfrie og inneholder som basis parafinene som er egnet for topisk anvendelse og er flytende ved kroppstemperatur, spesielt parafin med lav viskositet, også de nevnte naturlige eller delvis syntetiske fett, f.eks. kokosnøttfettsyre-triglyserid, herdede oljer, f.eks. hydrogenert peanøtt- eller ricinusolje, fettsyre delvise estere av glyserol, f.eks. glyserolmonostearat og distearat, silikoner, f.eks. polydimetylsiloksaner, f.eks. heksa-metyldisiloksan eller oktametyltrisiloksan, og, for eksempel, fettalkoholene nevnt i forbindelse med de vannholdige kremer og som øker vannopptakskapasiteten, og steroler, ullvokser, andre emulgatorer og/eller andre additiver.
I tilfellet av geler, gjøres en forskjell mellom vannholdige, vannfrie og geler med lavt vanninnhold, som består av svellbart, geldannende materiale. Passende er først og fremst transparente hydrogeler basert på uorganiske eller organiske makromolekyler. Makromolekylære uorganiske komponenter med geldannende egenskaper er overveiende vannholdige eller vannabsorberende silikater slik som aluminiumsilikater, f.eks. bentonitt, magnesium-aluminiumsilikater, f.eks. Veegum® - Vanderbilt Exp. Corp., eller kolloidal silika, f.eks. Aerosil® - Degussa. Eksempler på anvendte makromolekylære organiske substanser er naturlige, semisyntetiske eller syntetiske polymerer. Naturlige og semisyntetiske polymerer avledes, for eksempel, fra polysakkarider med forskjellige karbohydratenheter, slik som cellulose, stivelse, tragant, gummi arabicum, agar-agar, gelatin, alginsyre og salter derav, f.eks. natrium-alginat og derivater derav, lavere alkylcellulose, for eksempel metyl- eller etylcellulose, karboksy- eller hydroksy-lavere-alkylcellulose, f.eks. karboksymetyl- eller hydroksypropylcellulose. Enhetene i syntetiske, geldannende polymerer er, for eksempel, umettede, substituerte ali-fatiske forbindelser slik som vinylalkohol, vinylpyrroli- don, akryl- eller metakrylsyre. Eksempler som skal nevnes på slike polymerer er polyvinylalkoholderivater slik som Polyviol® - Wacker, polyvinylpyrrolidoner, slik som Kol-lidon® - BASF eller Polyplasdon® - Vanlig anilin, poly-akrylater og polymetakrylater, slik som Rohagit S® - Rohm und Haas. Det er mulig å tilsette konvensjonelle additiver slik som konserveringsmidler eller dufter til gelene.
Pastaer er kremer eller salver med bestanddelene nevnt i det foregående og sekresjonsabsorberende støvende pulver-bestanddeler slik som metalloksider, f.eks. titanoksid eller sinkoksid, også talk og/eller aluminiumsilikater, som har oppgaven å binde fuktighet eller sekreter.
I en utførelsesform som beskrives her er det topiske farmasøytiske preparat av oppfinnelsen et semifast farmasøytisk preparat, hvor én av eksipiensene er polyetylenglykol, spesielt polyetylenglykol 400.
I en ytterligere utførelsesform av oppfinnelsen er det topiske farmasøytiske preparat for anvendelse ifølge oppfinnelsen et transdermalt terapeutisk system (TTS), for eksempel et system som beskrevet i Pharmazeutische Technologie: Moderne Arzneiformen, Wissenschaftliche Verlagsgesellschaft mbH Stuttgart 1997, side 81 et sec. TTSer er i prinsippetkarakterisert veden definert tilførsel av medisinske substaner til huden, en total dose av den medisinske substans i TTS, et totalt areal og et areal som muligens er forskjellig derfra for frigivelse av den medisinske substans, et dekkark (bakside lag) som er ugjennomtrengelig for den medisinske substans, et medisinsk substansreservoar, et kontrollelement som kontrollerer tilførselen av medisinsk substans til huden, et (trykksensitivt) adhesivt lag og et avtakbart beskyttende lag. Det er mulig ved behov for at mer enn en funksjon oppfylles av ett og det samme element, f.eks. reservoar, kontroll og adhesive funksjoner av en passende adhesiv matriks. Fra synspunktet for farmasøytisk teknologi, katagoriseres TTSer i henhold til måten kontrollfunksjonen oppnås, det vil si hvordan den kontrollerer tilførselen av medisinsk substans til huden. Eksempler som kan nevnes her er TTSer med membrangjennomtrengningskontrollert frigivelse (membranmoderert legemiddellevering), TTSer med matriks-diffusjonskontrollert frigivelse og TTSer med mikroreservoar-løsningskontrollert frigivelse.
TTSer med membrangjennomtregningskontrollert frigivelse karakteriseres ved en polyrnermembran sammensatt av en PVA-VA-kopolymer (Chronomer®) som kontrollerer gjennom-tregningen av den medisinske substans fra reservoaret inn i huden. Den medisinsk substans er i begynnelsen i form av faste partikler eller som en dispersion eller løsning i reservoaret. Polymermembranen kan knyttes til reservoaret på forskjellige måter (ekstrusjon, innkapsling, mikroinn-kapsling). TTSer med matriks-diffusjonskontrollert frigivelse har en forholdsvis enkel struktur. De inneholder intet separat kontrollelement. Frigivelsen av medisinsk substans kontrolleres av en lipofil eller hydrofil poly-mermatriks og/eller det adhesive lag. Det er mulig å skille, i henhold til matriksens karakteristika, mellom TTSer med en matriks i gelform og TTSer som representerer faste polymerlaminater. Det medisinske substansreservoar dannes av den medisinske substans løst i matriksen (mono-littisk system) eller en homogen dispersjon av faste medisinske substanspartikler. Et matriks-TTS kan fremstilles ved å blande de medisinske substanspartikler med en viskøs flytende eller semifast polymer ved romtemperatur, etterfulgt av kryssbinding av polymerkjedene. En ytterligere mulighet er også å blande den medisinske substans ved hevet temperatur med myknet polymer (varmsmelte teknikk), eller de to komponenter (løst i et organisk løsningsmiddel) blandes sammen og løsningsmidlet fjernes deretter in vacuo (løsningsmiddelfordamping). Formgivning er mulig ved å helle i passende former, spredning med spesielle anordninger (kniver) eller ved ekstrusjon. I tilfellet av TTSer med mikroreservoar-løsningskontrollert frigivelse (mikroforseglet legemiddellevering, MDD-prin-sipp), innesluttes tallrike mikroseksjoner inneholdende den aktive ingrediens og 10-200 um i størrelse i en matriks som representerer både reservoar og leveringskontrollelement. På grunn av matriksen er disse TTSer faktisk tilordnet til matrikssystemene. For fremstilling dispergeres den medisinske substans i begynnelsen sammen med vann og 40 % polyetylenglykol 400 i isopropylpalmitat, som virker som gjennomtrengningsfremmer. Den resulterende dispersjon innarbeides ved å anvende en spesiell høyenergi-disper-sjonsteknikk inn i en viskøs silikonelastomer som samtidig undergår katalytisk polymerisasjon. Den medisinske sub-stansinneholdende matriks kan formes spesielt ved smelte-eller ekstrusjonsteknikker før den kombineres med bæreren på en allerede beskrevet måte. Avhengig av de fysiko-kjemiske egenskaper til de medisinske substanser og den tiltenkt frigjøring, er det mulig å dekke matriksen med et lag av en biokompatibel polymer for således å modifisere mekanismen og frigjøringshastigheten.

Fremstilling av doseringsformene
Eksempel 1
550 gram inneholder
Fremstilling skjer ved å løse den aktive ingrediens i den angitte mengde polyetylenglykol ved ca 60-70 °C. Ca 90 g renset vann tilsettes og blandes homogent, og Karbopol 934 dispergeres homogent deri med en hurtigblander. Under langsom omrøring tilsettes natriumhydroksidløsning inntil en pH på 6,5-7,5 nås. Resten av vannet tilsettes opp til sluttvekten og blandes homogent.
Eksempel 2
550 gram inneholder
Fremstilling skjer ved å behandle de to polyetylenglykoler til 70 °C for å gi en klar smelte. Den aktive ingrediens tilsettes på samme vis for å gi en klar løsning. Preparatet avkjøles til romtemperatur under langsom omrøring.
Eksempel 3
550 gram inneholder
Fremstilling skjer ved å lage en klar løsning av den nøytrale olje, cetostearylalkoholen og Tego Care 150 ved ca 70 °C. Polyetylenglykolen, hvor roflumilasten har blitt løst, røres likeledes inn ved å anvende en hurtigblander. Vannet varmet til 70 °C tilsettes til lipidfasen. En Turrax anvendes for homogenisering. Preparatet omrøres deretter inntil kaldt (romtemperatur).

100 gram inneholder
Fremstilling skjer ved å varme alle komponentene (bortsett fra vann) sammen til ca 70-80 °C for å gi en klar løsning. Vannet tilsettes deretter under omrøring, og preparatet fremstilt på denne måte avkjøles til romtemperatur under omrøring.

Fremstilling skjer ved å lage en klar smelte av den flytende parafin, ullvoksen og den hvite petrolatum ved ca 80 °C. Den mikroniserte aktive ingrediens tilsettes, og preparatet omrøres inntil det har avkjølt til romtemperatur .

Fremstilling: En klar smelte av cetylalkoholen, parafinen med høy viskositet og den hvite petrolatum fremstilles ved ca 70 °C. Den mikroniserte roflumilast (90 % av partiklene under 10 ym) røres inn, og en homogen dispersjon fremstilles ved å anvende en Ultra-Turrax. Suspensjonen av-kjøles til romtemperatur under omrøring og anvendes for å fylle passende rør.

Sammensetning av en dråpeløsning i form av en emulsjon
(mengde for 1 000 milliliter)
Fremstilling: Først løses roflumilasten og deretter lecitinet i triglyseridene med medium kjedelengde og glyserolen ved 30 °C-40 °C. Under kraftig omrøring tilsettes det rensede vann og homogeniseres deretter inntil dråpestørrelsen av den dispergerte fase er under 500 nm. Tiomersalen løses ved omrøring. Emulsjonen filtreres gjennom et 0,45 pm filter og dispergeres i passende beholdere.

Sammensetning av en nesesalve (mengde for 1.000 gram)
Fremstilling: En klar smelte av cetylalkoholen, parafinen med høy viskositet, ullvoksen og den hvite petrolatum fremstilles ved ca 70 °C. Den mikroniserte roflumilast (90 % av partiklene under 10 p. m) røres inn, og en homogen dispersjon fremstilles ved å anvende en Ultra-Turrax. Suspensjonen avkjøles til romtemperatur under omrøring og anvendes for å fylle passende rør.

Eksempel A
Et preparat som tilsvarer eksempel 7 og et preparat som tilsvarer eksempel 8 inneholdende [<14>C]roflumilast ble applisert på barberte områder av rottehud (5 hann Wistar-rotter) 4 cm<2>i størrelse. Radioaktivitetskonsentrasjonene ble målt i plasmaet etter 1 time, 4 timer, 8 timer, 24 timer og i urinen (0-24 timer) (n = 5). Dosen var 1,7 mg/kg.

Konklusjon:
Etter perkutan administrasjon av 1,7 mg/kg [14C] rof lumilast til rotter transporteres den totale radioaktivitet godt gjennom huden og når et maksimum plasmanivå på 0,214 mg ekv./l etter 4 timer, uansett preparatet anvendt. Basert på den totale radioaktivitet er AUCene og ekskresjonene med urinen etter perkutan administrasjon ubetydelig forskjellige fra de etter oral administrasjon.
Eksempel B
Et preparat som tilsvarer eksempel 5 inneholdende [<14>C]roflumilast ble applisert på et barbert område av rottehud (hann Wistar-rotte) 4 cm<2>i størrelse. Radioaktivitetskonsentrasjonene ble målt i plasmaet etter 1 time, 4 timer, 8 timer, 24 timer og i urinen (0-24 timer)

Konklusjon:
Disse data viser at roflumilast absorberes fra preparatet i eksempel 5 enda noe bedre enn fra preparatene som tilsvarer eksempel 7 eller 8. Ekskresjonen med urinen i de 24 timer etter administrasjon er 22 %, som også er i området av ekskresjonen med urinen etter dermal administrasjon av preparatene som tilsvarer eksempel 7 eller 8. Sammenligning med oral administrasjon viser at, uten hensyn til sammen-setningen av det topiske preparat, oppnås lignende Cmax og AUCer og lignende ekskresjoner med urinen.
Industriell anvendelighet
De farmasøytiske preparater for anvendelse ifølge oppfinnelsen er spesielt egnet for behandlingen av dermatoser valgt fra gruppen psoriasis (vulgaris), toksisk og allergisk kontakteksem, atopisk eksem, seboreisk eksem, lichen simplex, solbrenthet, pruritus i det genitoanale område, alopecia areata, hypertrofiske arr, diskoid lupus erythematosus, follikulære og ekstensive pyodermaer, endogen og eksogen akne, acne rosacea og andre proliferative, inflammatoriske og allergiske hudforstyrrelser. Anvendelsen av de farmasøytiske preparater av oppfinnelsen i behandlingen av psoriasis og atopisk eksem kan nevnes.
Oppfinnelsen vedrører anvendelsen av roflumilast, salter av roflumilast, N-oksidet av pyridinresiduet av roflumilast eller salter derav for fremstilling av et topisk farmasøytisk preparat for dermal administrasjon for behandlingen av en dermatose valgt fra gruppen psoriasis (vulgaris), toksisk og allergisk kontakteksem, atopisk eksem, seboreisk eksem, lichen simplex, solbrenthet, pruritus i det genitoanale område, alopecia areata, hypertrofiske arr, diskoid lupus erythematosus, follikulære og ekstensive pyodermaer, endogen og eksogen akne, acne rosacea og andre proliferative, inflammatoriske og allergiske hudforstyrrelser.
Den gode systemiske tilgjengelighet overraskende observert ved topisk administrasjon gjør dessuten de farmasøytiske preparater for anvendelse ifølge oppfinnelsen egnet for systemisk behandling og således for behandlingen av spesielt de overnevnte sykdommer.
Her beskrives derfor også videre anvendelsen av roflumilast, salter av roflumilast, N-oksidet av pyridinresiduet av roflumilast eller salter derav for fremstilling av et topisk farmasøytisk preparat for dermal administrasjon for den systemiske behandling avovernevnte sykdommer
De farmasøytiske preparater for anvendelse ifølge oppfinnelsen er dessuten spesielt egnet for adminstration til pasientgrupper som lider av de overnevnte sykdommer og har problemer med å ta farmasøytiske preparater som skal administreres oralt, slik som, for eksempel, sengeliggende pasienter, pasienter i intensive medisinsk pleie, pasienter med svelgproblemer og barn.
Doseringsformene som beskrives her omfatter den aktive farmasøytiske ingrediens i dosen vanlig for behandlingen av den spesielle sykdom. Doseringen av den aktive ingrediens er i størrelsesorden alminnelig for PDE-inhibitorer, det er mulig å administrere den daglige dose i en eller flere doseringsenheter. Alminnelige doseringer er beskrevet for eksempel i WO 95/01338. Den normale dose ved systemisk terapi (oral) er mellom 0,001 og 3 mg per kilogram og dag. Doseringsformer som beskrives her for topisk administrasjon inneholder fra 0,005 mg til 5 mg roflumilast, fortrinnsvis fra 0,01 mg til 2,5 mg, spesielt foretrukket 0,1 mg til 0,5 mg roflumilast per doseringsenhet. Eksempler på farmasøytiske preparater for anvendelse ifølge oppfinnelsen inneholder 0,01 mg, 0,1 mg, 0,125 mg, 0,25 mg og 0,5 mg roflumilast per doseringsenhet.
Claims (9)
1. Anvendelse av en forbindelse valgt fra gruppen bestående av roflumilast, salter av roflumilast, N-oksidet av pyridinresiduet av roflumilast og salter derav for fremstilling av et topisk appliserbar farmasøytisk preparat for dermal administrasjon for behandlingen av en dermatose valgt fra gruppen psoriasis (vulgaris), toksisk og allergisk kontakteksem, atopisk eksem, seboreisk eksem, lichen simplex, solbrenthet, pruritus i det genitoanale område, alopecia areata, hypertrofiske arr, diskoid lupus erythematosus, follikulære og ekstensive pyodermaer, endogen og eksogen akne, acne rosacea eller andre proliferative, inflammatoriske og allergiske hudforstyrrelser.

5. Anvendelse ifølge krav 1, hvor den topisk appliserbare farmasøytisk preparat er en semifast farmasøytisk preparat, hvor én av eksipiensene er polyetylenglykol.
6. Anvendelse ifølge krav 5 hvor polyetylenglykolen er polyetylenglykol 400.
7. Anvendelse ifølge krav 1, hvor den topisk appliserbare farmasøytisk preparat er en salve og omfatter en parafin som er egnet for topisk anvendelse og er flytende ved kroppstemperatur.
