Abstract
Description
The invention relates to medicine, more specifically to surgery, and can be used to perform extensive resections of the liver, accompanied by severe blood loss.
644-652).It is known that there is a method of completely isolating the liver for the prevention of bleeding with extensive resections of the organ, which involves occlusion of the vessels of the portal gates of the liver, compression of the inferior vena cava above and below the liver, and hypothermic perfusion of the liver (Fortner J. et al. "Ann Surg.", 1974, v .180, No. 4, p. 644-652).
The above method has significant disadvantages:
1. Окклюзия в воротах приводит к нарушению оттока крови из органов брюшной полости, окклюзия нижней полой вены приводит к резкому падению центрального венозного давления и уменьшению венозного возврата в правое предсердие;1. Occlusion in the gate leads to a violation of the outflow of blood from the organs of the abdominal cavity, occlusion of the inferior vena cava leads to a sharp drop in central venous pressure and a decrease in venous return to the right atrium;
2. Применение длительных перфузии способствует вымыванию из печени таких важных веществ как гликоген, фосфор, кальций и калий, вследствие чего развивается клеточная гипергидратация тканей, отрицательно влияющая на функции печени в последующем;2. The use of prolonged perfusion promotes leaching of important substances from the liver such as glycogen, phosphorus, calcium and potassium, resulting in the development of cellular tissue hyperhydration, which negatively affects the function of the liver in the future;
3. При углублении гипотермии на каком-то этапе возникает отек ткани, усиливающийся под влиянием перфузии, что нарушает диффузионные процессы, доставку и утилизацию субстратов окисления, тем самым создаются условия гибели клетки;3. When hypothermia deepens at some stage, tissue edema occurs, intensifying under the influence of perfusion, which disrupts diffusion processes, delivery and utilization of oxidation substrates, thereby creating conditions for cell death;
4. The method is technically complicated, as it requires the exact selection of the optimal parameters and type of endocaval balloon catheter, the technique of proper installation of the latter, and the selection of an adequate perfusion regimen.
№2, с.3-7).A known method of “portal” resection of the liver with preliminary ligation of the vascular secretory leg of the removed organ lobe, which consists in sequentially isolating the elements of the hepatoduodenal ligament, ligating them, waiting for a time interval for the outflow of blood into the system of the inferior vena cava, followed by ligation of the hepatic veins proper (Shapkin BC Surgery ”, 1996. No. 2, p. 3-7).
The specified method has the following disadvantages:
1. Не учитывает анатомических особенностей строения сосудистых структур печени;1. Does not take into account the anatomical features of the structure of the vascular structures of the liver;
2. В достаточной мере не снижает интраоперационную кровопотерю и требует дополнительного переливания крови;2. It does not sufficiently reduce intraoperative blood loss and requires additional blood transfusion;
3. The method does not prevent the toxic blood products of cellular metabolism from accumulating under hypoxia as a result of ligation of the elements of the hepatoduodenal ligament into the general bloodstream.
The aim of the present invention is to reduce the magnitude of intraoperative blood loss and the individualization of the technique of left-sided hemihepatectomy with a short left hepatic vein.
This goal is achieved by the fact that when dissecting the leaves of the coronary ligament, the left phrenic vein is isolated, a vascular catheter is inserted through the mouth of the left phrenic vein into the left hepatic vein, the left hepatic vein is ligated at the site of inflow into the inferior vena cava.
The invention "Method of left-sided hemihepatectomy" is new, since it is unknown from the level of medicine, namely in surgery of the hepatopancreatoduodenal zone.
The novelty of the invention lies in the fact that the left hepatic vein is catheterized through the mouth of the left phrenic vein, the latter is ligated at the site of the inflow into the inferior vena cava.
The well-known sources of information in Russia, the CIS and foreign countries do not contain information about a similar method of left-sided hemihepatectomy.
The invention "Method of left-sided hemihepatectomy" is industrially acceptable, as it can be used and repeatedly repeated in the surgical treatment of focal lesions of the left lobe of the liver in medical institutions, especially oncology: oncology dispensaries, research institutes.
The method is as follows. Mobilize the left lobe of the liver (1) by dissecting the ligaments of the liver (see drawing). When dissecting the coronary ligament of the liver for 4 cm, the left diaphragmatic vein (2) is isolated, a ligature (3) is brought under it. A vascular catheter (4) is inserted through the mouth of the left phrenic vein into the left hepatic vein (5) and the phrenic vein is ligated twice on the catheter, the hepatic vein is ligated (5). During subsequent manipulations in the gates of the liver associated with the isolation and ligation of the elements of the hepatoduodenal ligament, blood is removed from the left hepatic vein through a catheter and collected in a container with a preservation solution, to which a 0.033% sodium hypochlorite solution is added in a 1: 1 ratio, withstand 30-60 minutes and returned to the pre-catheterized subclavian vein of the patient. After removal of the lobe, the operation is completed by external drainage of the biliary tract, the wound of the liver is peritonized with a lock of a large omentum.
Example. Patient C-ova, 58 years old, medical history No. 19862/0 was admitted to the clinic with a diagnosis of cancer of the left lobe of the liver with damage to segments II-III, stage III, group II T3NOMO. After preoperative preparation of the patient, an upper median laparotomy was performed, complete mobilization of the left lobe of the liver by dissection of its ligaments was performed. When dissecting the coronary ligament of the liver, the left and middle hepatic veins were isolated, taken on hold. It was revealed that the length of the left hepatic vein is insufficient for catheterization, therefore, the left phrenic vein was isolated, through which a catheter was inserted into the left hepatic vein, the phrenic vein was ligated directly on the catheter, the left hepatic vein was ligated at the site of inflow into the inferior vena cava. The blood coming through the catheter was collected in vials with gluciter, to which a 0.033% sodium hypochlorite solution was added in a 1: 1 ratio, kept for 60 minutes, and in this form the blood returned to the patient’s general bloodstream through the subclavian catheter in an amount of 400 ml. Serial ligation of the elements of the hepatoduodenal ligament was performed. After dissection of the tissue by the method of digitoclasia and ligation of the intrahepatic vessels and ducts, ligation was performed with subsequent intersection of the left hepatic vein at the place of its inflow into the inferior vena cava and ligation with subsequent intersection of the left branches of the middle hepatic vein. The operation was completed by external drainage of the common bile duct with a silicone tube, removed through contrapertures on the skin in the right hypochondrium. The wound of the liver is peritonized with a lock of a large omentum. The abdominal cavity is drained with a polyvinyl chloride tube. The postoperative course is smooth. Drainage from the abdominal cavity was removed on the 9th day. Sutures were removed on the 12th day. The patient on the 14th day in satisfactory condition was discharged home. 1 month after surgery, the patient was removed the drainage tube from the common bile duct.
The technical and economic efficiency of the method lies in the fact that preliminary catheterization of the lumen of the left hepatic vein through the mouth of the left diaphragmatic vein with subsequent ligation of the left hepatic vein helps to reduce the loss of blood deposited in the drug and to reduce hypertension in the hepatic parenchyma, which reduces the risk of intraoperative blood loss. Adding sodium hypochlorite solution to the blood helps prevent toxic products of cellular metabolism from entering the general bloodstream, and returning the patient’s own blood does not require additional transfusion of foreign donor blood. Thus, the proposed method allows to individualize the technique of performing left-sided hemihepatectomies with short hepatic veins, which in turn allows to reduce the number of early postoperative complications, thereby improving the results of surgical treatment of patients with diseases of the hepatopancreatoduodenal organs.
