Description
Die Erfindung betrifft ein topisches, Indomethacm enthaltendes Arzneimittel.
Indomethacin ( I -(p-Chlorbenzoyl)-5-methoxy-2-me-thylindol-3-essigsäure) ist ein seit vielen Jahren bewährter Wirkstoff, der wegen der guten entzündungshemmenden, antipyretischen und analgetischen Eigenschaften als Arzneimittel eine grosse Anwendung gefunden hat. Insbesondere zur Bekämpfung von rheumatischen Erkrankungen und von Gicht, aber auch zur Schmerzbekämpfung, Fiebersenkung und zum Abschwellen wird Indomethacin in Form von oralen Verabreichungen, vorzugsweise Kapseln oder Supposito-rien, verwendet. Bei oraler bzw. rektaler Verabreichung geht Indomethacin in den Blutstrom des Körpers über und wird im Körper verteilt.
Da insbesondere bei rheumatischen Erkrankungen Rheumamittel über lange Zeiträume oder sogar ständig eingenommen werden müssen, ist auch bei der Einnahme von Indomethacin eine ständige Überwachung des Patienten und des Blutbildes erforderlich. Gegenüber anderen Arzneimitteln gleicher Wirkungsrichtung, wie Salicylaten, Corticoste-roiden oder Verbindungen auf Basis von Phenylbutazon treten bei der oralen Verabreichung von Indomethacin nur wenig Nebenwirkungen auf, jedoch ist bei längerer Applikation und bei einer gewissen Neigung des Patienten eine Schädigung der Magenschleimhaut und die Ausbildung von Magengeschwüren nicht auszuschliessen.
Da rheumatische Erkrankungen der Gelenke oder Muskeln, sowie Prellungen oder Sportverletzungen im allgemeinen lokalisiert werden können, wäre es vorteilhaft den Wirkstoff Indomethacin lokal an den Entzündungsherd zu applizieren. Ausserdem wäre es vorteilhaft, wenn der Patient selbst Indomethacin lokal auf die Haut auftragen könnte, und Indomethacin in einer solchen Zubereitung vorläge,
dass es bei topischer Verabreichung von der Haut und durch die Haut aufgenommen wird.
Die DE-PS 2 103 833 betrifft ein Antiphlogistikum für äussere Anwendung, das durch einen Gehalt einer wirksamen Menge Indomethacin in einem Methylsalicylat-Träger gekennzeichnet ist.
Das als Träger verwendete Methylsalicylat wird, wie auch in der Patentschrift dort festgestellt wird, seit vielen Jahren in zahlreichen Linimenten wegen seiner gegenentzündlichen Wirkung verwendet. Die entzündungserregende Wirkung von Methylsalicylat ist besonders bei Patienten mit empfindlicher Haut störend oder verhindert sogar die Möglichkeit Indomethacm nach der Lehre des deutschen Patents 2 103 833 in Methylsalicylat gelöst, topisch anzuwenden.
DE-PS I 617 653 betrifft ein Indomethacin zur externen Verabreichung, bei dem Indomethacin in einer Grundlage

aus mindestens einem Diester der allgemeinen Formel ROOC-(CH2)n-COOR, worin R eine aliphatische Al-kylgruppe mit 1 bis 4 Kohlenstoffatomen und n eine ganze Zahl von 1 bis 8 bedeutet, gelöst, enthalten ist. Eine solche, 5 einen Diester der allgemeinen Formel enthaltende Indome-thacinzubereitung ist wegen der Gefahr der Umesterungsre-aktion, die zwischen dem Essigsäurerest und Indomethacin mit den Estergruppen des Diesters ablaufen kann, nur beschränkt lagerstabil. Die Vorschriften für die Zulassung von io Arzneimitteln verlangen jedoch, dass die Wirkstoffe in Arzneimitteln wenigstens 3 Jahre unverändert sind.
Weiterhin ist aus DE-OS 2 827 018 eine indomethacin-haltige analgetische Salbe bekannt, bei welcher Indomethacin in einem glykolhaltigen, z.B. Polyethylenglykol, Medium 15 das ausserdem Zellulose und/oder ein Carboxyvinylpolyme-res, das mit einem Amin neutralisiert worden ist, neben einer ausreichenden Menge Wasser vorliegt. Die Sorptionsfähigkeit, d. h. die Aufnahme des Wirkstoffes durch die Haut, ist aber noch nicht voll befriedigend.
20 Es besteht somit ein Bedürfnis nach einem methylsalicy-latfreien, topischen, Indomethacin enthaltenden Arzneimittel mit guter Lagerstabilität und guter Sorption durch die Haut.
Dieses Bedürfnis wird durch eine Zubereitung gemäss 25 dem Patentanspruch 1 befriedigt.
Die Erfindung betrifft somit ein topisches Arzneimittel, entsprechend der im Anspruch 1 angegebenen Kombination von Merkmalen - vorteilhafte Weiterbildungen ergeben sich aus den abhängigen Ansprüchen.
30 Polyalkylenglykole sind z. B. Polyethylenglykol oder Po-lypropylenglykol. Bevorzugte Polyalkylenglykole sind Poly-ethylenglykole der Formel

wobei n zwischen etwa 3 und 200 variieren kann. Polyethy-lenglykole sind bei Molekulargewichten bis etwa 600 bei Zimmertemperatur flüssig und bei Molekulargewichten von etwa 1000 bis 11 000 weich bis hartwachsähnlich. 40 Sie sind farblos, geruchsfrei, ungiftig und haben bereits vielfältig Verwendung als Glyzerinersatz in kosmetischen Zubereitungen und auch als Bestandteil von Salben und Cremes gefunden. Auch als Bindemittel bei der Herstellung von Tabletten sind Polyethylenglykole verwendet worden. 45 Ein bevorzugter PolyethylengIykol(Ci 0-C14alkyl)ether istPolyethylenglykoIdodecylether, der Iokalanästhetisehe und endoanästhetische Wirkung hat. Dieser Ether hat vorzugsweise ein Molekulargewicht von 580 bis 7QO.
Die Konzentration von Indomethacin in dem Träger aus so einem Gemisch aus Polyalkylenglykol und Polyethylengly-koI(Ci 0-C14alkyl)ether ist nicht kritisch und hängt von dem Grad und der Art der zu behandelnden Krankheit, der Häufigkeit der topischen Anwendung und weiteren Faktoren ab. Rezepturen* die mit Vorteil angewendet werden können, ent-55 halten vorzugsweise etwa 1 bis 12, insbesondere 2 bis 6 Gewichtsteile Indomethacin auf 100 g Salbe.
Das Trägermaterial aus Polyalkylenglykol und Polyethy-IengIykol(CIO-C14.aIkyI)ether enthält die Polyalkylenglykole in überwiegenden Mengen, gegenüber der Menge an Poly-6oethylengIykol(C10-C14.alkyI)ether. Vorzugsweise liegt das Gewichtsverhältnis von PolyalkyIengIykol(Ci0-CI4alkyl)-ether zu Polyalkylenglykol bei 0,5 zu 20 bis I zu IG. Polyalkylenglykol liegt somit in einer 40 bis lOfachen Menge* bezogen auf Polyethylenglykol cIO bis cI4 Alkylether vor. 65 Die erfindungsgemässe topische Zubereitung kann übliche Zusatzstoffe, wie oberflächenaktive Mittel, Wasser oder Parfüme, enthalten. Eine typische erfindungsgemässe Rezeptur für eine Salbe enthält:

Das Polyethylenglykol besteht vorzugsweise aus einem Gemisch von Polyethylenglykolen unterschiedlicher Kettenlänge. Bei der oben gezeigten Rezeptur macht Polyethylenglykol 300 (Molekulargewicht 300, n = 4) den Hauptteil aus, während durch Polyethylenglykol 1000 bzw. 1500 (n = 13 bzw. 20) die Salbe eine festere Konsistenz erhält. Die Herstellung der Salbe kann in üblicher Weise erfolgen. Beispielsweise kann man für die Herstellung von 1 kg Fertigprodukt folgenden Ansatz durchführen:
(1) Ansatz I
50 g Indomethacin und Polyethylenglykoldodecylether werden bei Raumtemperatur in 650 g Polyethylenglykol 300 unter Rühren gelöst.
(2) Ansatz 2
Eine Mischung aus 50 g Polyethylenglykol 1000, 50 g Polyethylenglykol 1500 und 75 g einer Mischung aus 95 Gew.-% Glyzerinmonostearat und 5 Gew.-% Polyethylenglykolalkylether wird bis zum Erhalt einer klaren Lösung bei 60 °C auf einem Wasserbad erwärmt.
Ansatz 1 wird absatzweise unter gleichmässigem Rühren zu Ansatz 2 gegeben und zu dieser Mischung werden 74 g gereinigtes Wasser von 49 °C zugefügt.
Der Gesamtansatz wird bis zur beginnenden Gelbildung gerührt und dann wird 1 g Parfümöl zugegeben. Anschliessend wird der Ansatz bis zur Abfüllung auf Raumtemperatur gleichmässig gerührt.
Eine als Gel formulierte Zubereitung hat z.B. folgende Zusammensetzung:
Indomethacin 2 g
Polyethylenglykoldodecylether 2 g
Polyacrylsäure 1 g
Polyethylenglykol 300 20 g s Wasser 73 g p-Hydroxybenzoesäureethylester 0,05 g
Propylgallat 0,002 g
Triethanolamin ca. 2 g
~ 100,0 g io
Die Herstellung erfolgt, indem man Lösung I zu Lösung II gibt und zu dieser Mischung soviel Triethanolamin unter ständigem Rühren zusetzt, bis sich ein klares Gel gebildet hat. Danach werden 0,05 g p-Hydroxybenzoesäureethylester is von 0,002 g Propylgallat zugesetzt.
Lösung I
2 g Indomethacin und 2 g Polyglykolmonododecylether werden bei Raumtemperatur in 30 g Polyethylenglykol 300 20 gelöst und diese Lösung mit ca. 2 g Triethanolamin auf einen pH von etwa 7 eingestellt.
Lösung II
1 g Polyacrylsäure wird bei Raumtemperatur in 73 g de-25 stilliertem Wasser unter kräftigem Rühren suspendiert und anschliessend wird die Suspension unter weiterem kräftigen Rühren mit Triethanolamin auf einen pH von etwa 7 eingestellt.
Indomethacin ist in dem Gemisch aus Polyethylenglykol 30 und PolyethylenglykoI(Ci0-C14alkyl)ether löslich. Die topischen Zubereitungen werden zweckmässigerweise so formuliert, dass sie thixotrop sind, wobei bei Hauttemperatur die strukturviskosen und -plastischen Eigenschaften der Salbe erhalten bleiben sollen.
35 Die erfindungsgemässe topische Zubereitung ist besonders vorteilhaft zur Behandlung von Prellungen, Stauchungen oder ähnlichen Sportverletzungen, bei denen keine Verletzung der Haut vorliegt. Durch die gezielte topische Anwendung kommt man mit den erfindungsgemässen topi-40 sehen Zubereitungen mit geringeren Mengen des Wirkstoffes aus gegenüber oraler oder rektaler Applikation, bei der der Wirkstoff Indomethacin in den Blutkreislauf abgegeben und von dort an den Krankheitsherd geführt wird.
Claims (3)
1. Topisches, eine wirksame Menge Indomethacm in Po-lyalkylenglykol als Träger enthaltendes Arzneimittel, dadurch gekennzeichnet, dass es als Träger eine überwiegende Menge Polyalkylenglykol und eine gegenüber Polyalkylen-glykol geringere Menge Polyethylenglykol-(C10-C14.alkyl)-ether enthält.
2. Arzneimittel gemäss Anspruch 1, dadurch gekennzeichnet, dass der Polyethylenglykol(Ci0-C14alkyl)ether Po-lyethylenglykoldodecylether ist.
3. Arzneimittel gemäss Ansprüchen 1 oder 2, dadurch gekennzeichnet, dass es in Form einer thixotropen Salbe oder eines Gels vorliegt.
