Opfindelsen angår anvendelsen af o<-liponsyre til fremstilling af et lægemiddel til bekæmpelse af sygdomme fremkaldt af retrovirus.
I detaljer angår opfindelsen anvendelsen af 5 <x.-liponsyre til fremstilling af et lægemiddel til bekæmpelse af sygdomme fremkaldt af retrovirus, hvilket lægemiddel er ejendommeligt ved, at doseringsenheden til faste eller halvfaste præparater indeholder 51 mg til 6 g, især 100 mg til 2 g, fortrinsvis 200 mg til 1 10 9 eller også 400 mg eller 500 mg til 1 g a-liponsyre eller et farmaceutisk acceptabelt salt deraf, eller at den, når det drejer sig om injektionsopløsninger, indeholder 26 mg til 500 mg pr. ml, fortrinsvis 50 mg til 200 mg, især 100 mg pr. ml af a-liponsyre eller af et 15 farmaceutisk acceptabelt salt deraf, eller at den, når det drejer sig om drikkelige opløsnings-, suspensionseller emulsionspræparater, indeholder 0,2 mg til 500 mg pr. ml, fortrinsvis l mg til 50 mg pr. ml, især 5 mg til 10 mg pr. ml af α-liponsyre eller af et farmaceu-20 tisk acceptabelt salt deraf.
Ved anvendelsen ifølge opfindelsen indeholder lægemidlet, udover -liponsyre som aktivt stof, desuden hensigtsmæssigt sædvanlige farmceutiske bærere, hjælpestoffer, stabilisatorer, opløsningsfremmende midler 25 og/eller fortyndingsmidler, idet det, hvis der er tale om en opløsning, desuden kan indeholde stabilisatorer og/eller opløsningsfremmende midler. Som stabilisatorer eller opløsningsfremmende midler kan i den forbindelse f.eks. følgende stoffer anvendes: alifatiske C2-C4-al-30 koholer, der indeholder 1, 2 eller 3 hydroxylgrupper, polyethylenglycoler med molvægt mellem 200 og 600, sædvanlige fysiologisk acceptable organiske amider, naturlige α-aminosyrer, alifatiske aminer, hydroxyethyl- 2 DK 173585 B1 theophyllin, trometamol og diethylenglycolmonomethyl-ether.
Lægemidlet, til anvendelse ifølge opfindelsen, kan fremstilles ved, at a-liponsyre oparbejdes til et 5 farmaceutisk præparat eller bringes på en terapeutisk anvendelig form ved hjælp af sædvanlige farmaceutiske bærestoffer og/eller fortyndingsmidler eller andre hjælpestoffer, idet doseringsenheden til faste eller halvfaste præparater, indeholder 51 mg til 6 g, for-10 trinsvis 100 mg til 2 g, især 200 mg til l g eller også 400 mg eller 500 mg til 1 g af a-liponsyre eller af et farmaceutisk acceptabelt salt deraf, eller at den når det drejer sig om α-liponsyre til injektionsopløsninger, indeholder 26 mg til 500 mg pr. ml, fortrinsvis 50 15 mg til 200 mg, især 100 mg pr. ml af α-liponsyre eller af et farmaceutisk acceptabelt salt deraf, eller når det gælder drikkelige opløsnings-, suspensions- eller emulsionspræparater, indeholder 0,2 mg til 500 mg pr. ml, fortrinsvis l mg til 50 mg pr. ml, især 5 mg til 10 20 mg pr. ml af α-liponsyre eller af et farmaceutisk acceptabelt salt deraf.
Lægemidlet, til anvendelsen ifølge opfindelsen, kan også fremstilles ved, at man blander -liponsyre eller et farmaceutisk acceptabelt salt deraf med en el-25 ler flere af de følgende stoffer: stivelse, cyclo- dextrin, urinstof, cellulose, cellulosederivat, lactose, formalincasein, modificeret stivelse, magnesium-stearat, calciumhydrogenphosphat, kiselsyre, talkum, eventuelt granulerer eller pelleterer den opnåede blan-30 ding med en vandig eller alkoholisk opløsning, der som bestanddel kan indeholde gelatine, stivelse, polyvi-nylpyrrolidon, vinylpyrolidon-vinylacetat-copolymeri-sat og/eller polyoxyethylensorbitanmonooleat, eventuelt 3 DK 173585 B1 homogeniserer granulatet eller pillerne med et eller flere af de ovennævnte hjælpestoffer,, og presser blandingen til tabletter eller fylder den i kapsler eller fylder granulatet eller pillerne i beholdere, idet ta-5 bietterne, kapslerne eller beholderne pr. doseringsenhed indeholder 51 mg til 6 g, især 100 mg til 2 g, fortrinsvis 200 mg til 1 g eller også 400 mg eller 500 mg til 1 g af a-liponsyre eller et salt deraf.
Lægemidlet, til anvendelsen ifølge opfindelsen, 10 kan også fremstilles ved, at man homogeniserer og/eller emulgerer -liponsyre eller et farmaceutisk acceptabelt 0 salt deraf ved en temperatur på mellem 20 og 120 C, eventuelt i tilstedeværelse af en eller flere emulgatorer og/eller kompleksdannende midler, med mindst et af 15 de følgende stoffer til en blanding: vand, glycerol, paraffin, vaseline, alifatisk alkohol med 12 til 25 C-atomer, en alifatisk monocarboxylsyre med 15 til 20 C-atomer, sorbitanmonopalmitat, polyoxyethylenpolyol- fedtsyreester, en en- eller flervalent lavmolekylær, 20 alifatisk alkohol, et fedtsyreglycerid, voks, silicone og polyethylenglycol, idet en sådan blanding udgør et strygedygtigt præparat og indeholder 0,1’’til' 50 vægt%, især 0,5 til 20, fortrinvis l til 10 vægt% a-liponsyre eller af et salt deraf på basis af den samlede 25 mængde.
Lægemidlet, til anvendelsen ifølge opfindelsen, kan også fremstilles ved, at man opløser -liponsyre eller et farmaceutisk acceptabelt salt deraf ved temperaturer mellem 20 og 100 C, eventuelt i tilstedevæ-30 relse af et kompleksdannende middel og/eller en emulgator, i vand, en fysiologisk acceptabel alkohol, dime-thylsulfoxid, polyethylenoxid eller olier eller en blanding heraf, og eventuelt supplerer den således op- 4 DK 173585 B1 nåede opløsning op med tilstrækkeligt vand, alkohol, dimethylsulfoxid, polyethylenoxid eller olie, til at slutopløsningen, -suspensionen eller -emulsionen a) i det tilfælde, det drejer sig om injektions-5 opløsninger af a-liponsyre, indeholder 26_mg til 50 mg pr. ml, især 50 mg til 200 mg, fortrinsvis 100 mg pr. ml a-liponsyre eller af et salt deraf; b) i det tilfælde, at det drejer sig om drikkelige opløsnings-, suspensions- eller emulsions- 10 præparater af α-liponsyre, indeholder 0,2 mg til 500 mg pr. ml, fortrinsvis 1 mg til 50 mg pr. ml, især 5 mg til 10 mg pr. ml af α-liponsyre eller af et farmaceutisk acceptabelt salt deraf; Lægemidlet, til anvendelsen ifølge opfindelsen, 15 kan også fremstilles ved, at fremstillingen af dette lægemiddel, der som aktivt stof indeholder <*-liponsyre, desuden udføres under tilsætning af sædvanlige stabilisatorer og/eller opløsningsfremmende midler.
Lægemidlet, til anvendelsen ifølge opfindelsen, 20 kan også fremstilles ved, at der som stabilisatorer eller opløsningsfremmende midler under fremstillingen kan tilsættes følgende stoffer: alifatiske C2-C4-alkoholer, der indeholder 1, 2 eller 3 hydroxylgrupper, polyethy-lenglycoler med en molvægt mellem 200 og 600, sædvanli-25 ge fysiologisk acceptable organiske amider, naturlige α-aminosyrer, alifatiske aminer, hydiro~xyethyltheophyl-lin, trometamol, diethylenglycolmonomethyTéthér.
Det pågældende lægemiddel kan ogs3_-ånvendes i blanding med andre farmakologisk aktive stoffer. Især 30 kan <k-liponsyre kombineres med andre mod retrovirus, især HIV, aktive midler, f.eks. didesoxyinosin, di-desoxycytidin, men imidlertid især med a-interferon og/eller azidothymidin (AZT).
5 DK 173585 B1 Lægemidlerne, til anvendelse ifølge opfindelsen, kan indeholde c<-lionsyre som eneste aktive stof eller ^-liponsyre kombineret med dihydroliponsyre. I det sidste tilfælde er de angivne doseringer baseret på summen 5 af mængderne af de aktive stoffer, dvs. de aktive komponenter «v-liponsyre og dihydroliponsyre. Det samme gælder ligeledes ved kombinationer, f.eks. kombinationer med andre antiretroviralt virksomme stoffer (komponent b). Også her kan der som komponent b anvendes 10 blot en, men også to eller flere (fortrinsvis 2) antiretroviralt virksomme stoffer, idet det også her gælder i det sidste tilfælde, at de angivne doseringer står for summen af de aktuelt tilstedeværende antiretroviralt virksomme stoffer. Udtrykket "doseringsenhed” er 15 også baseret på en enkelt dosis, der også kan administreres flere gange pr. dag. når dosen er angivet i form af enzymenheder, drejer det sig om en dosis, der gælder for en hel dag, idet en sådan dosis kan administreres på en gang, idet den dog fortrinsvis fordeles 20 over en dag (f.eks. i form af infusion). Dosisangivelsen i enzymenheder gælder især for kombinationer med a-interferon.
Midlet til bekæmpelse af retrovirus indeholder f.eks. o<-liponsyre i en sådan mængde, at det ved en el-25 ler flere ganges administrering daglig foreligger i kroppen i en koncentration på mellem 3,5 og 200 mg/kg legemsvægt.
Lægemidlet, til bekæmpelse af retrovirus, kan i en særlig udførelsesform, foruden de sædvanlige farma-30 ceutiske bærere og/eller fortyndingsmidler eller hjælpestoffer, indeholde 6 DK 173585 B1 a) c<-liponsyre og b) et andet antiretroviralt, virksomt stof eller i begge tilfælde et fysiologisk acceptabelt salt af de aktive stoffer, i en mængde til opnåelse af en 5 synergistisk virkning, og foreliggende i en form, der sikrer både en fælles og en adskilt terapeutisk anvendelse af de to aktive stoffer.
Det andet mod retrovirus virksomme stof kan f.eks. være: 10 azidothymidin (AZT), didesoxyinosin (DDI), α-interferon eller didesoxycytidin (DDC).
Det ovenfor nævnte lægemiddel er f.eks. ejendommeligt ved, at der til kombination med en vægtdel o<-liponsyre hver gang anvendes 0,01 til 100 vægtdele af 15 den komponent ifølge b) eller til 1 mg af den aktive komponent a hver gang anvendes 20-200.000 enzymenheder af den anden komponent ifølge b), eller at en doseringsenhed af kombinationen indeholder 50 mg til 6 g, fortrinsvis 200 mg til 2 g af den aktive komponent a) 20 og 10 til 300 mg, fortrinsvis 50 til 200 mg, eller 5 x 105 enzymenheder (Units) til 8 x 106 enzymenheder, fortrinsvis 1 x 106 til 4 x 106 enzymenheder af den anden komponent ifølge b).
Desuden kan et sådant middel også indeholde vi-25 taminer, fortrinsvis vitamin Bx og/eller E.
Lægemidlet, til anvendelse ifølge opfindelsen, kan fremstilles ved, at man oparbejder en vægtdel af den aktive komponent a) og 0,01 til 100 vægtdele eller 20 til 200.000 enzymenheder af den anden komponent 30 ifølge b), idet de aktive stoffer også kan foreligge på saltform, sammen med sædvanlige bærere og/eller fortyndingsmidler eller hjælpestoffer og eventuelt under tilsætning af vitaminer til produkter, som pr. doserings- 7 DK 173585 B1 enhed indeholder 50 mg til 6 g af den aktive komponent a) og 10 til 300 mg eller 5 x 105 til 8 x 106 enzymenheder af den anden komponent ifølge b).
o(-Liponsyre kan også anvendes i form af dets op-5 tiske isomer R(+)- og S-{-)-form (sædvanligvis foreligger o(-liponsyre som racemat) eller som dets diastereo-mere former til fremstilling af farmaceutiske præparater og blandinger og anvendes som beskrevet......
α-Liponsyre er vidt udbredt i form. af racema-10 tet (ThioctsåureR) i planter og dyr. Det virker i mange enzymatiske reaktioner som coenzym, udgør en vækstfaktor for mange bakterier og protozoer og anvendes ved knoldebladsvampeforgiftning. Desuden udviser a-lipon-syreracematet antiflogistisk, antinociceptiv (analge-15 tisk) og cytoprotektive egenskaber. Thioctsyre er på markedet som lægemiddel til følgende indilcatiohéri
Fedtlever og fedtchirrhosis, især ved kronisk alkoholleversygdom, til leverbeskadigelse forårsaget af svampefor-20 giftning, diabetisk neuropathi, alkoholisk neuropathi. ”
Dihydroliponsyre er 6,8-dimercapto-octansyre.
Ved dyreundersøgelser er det kendt, at dihydroliponsy-25 re inaktiverer slangegift. Disse undersøgelser udførtes f.eks. på rotter og mus, idet opløsninger i vand eller fysiologisk kogesaltopløsning anvendtes, hvilke indeholdt slangegift og dihydroliponsyre.
I de hidtil kendte præparater er α-liponsyre og 30 dihydroliponsyre til stede i relativt lave mængder.
Anvendelen af lægemidlerne ifølge opfindelsen med højere mængder af α-liponsyre og eventuelt dihydroliponsyre er ny. Det var desuden ikke nærliggende, at 8 DK 173585 B1 de aktive stoffer i højere dosis ville udvise de fordelagtige farmaceutiske virkninger, f.eks. ved behandling af AIDS.
Fremstillingen af lægemidlet anvendt følge 5 opfindelsen udføres på kendt måde, idet de kendte og sædvanlige farmaceutiske hjælpestoffer og yderligere sædvanlige bærere og fortyndingsmidler kan anvendes.

Den farmaceutiske og galeniske behandling af 20 o<-liponsyre udføres ved sædvanlige standardmetoder. Eksempelvis blandes de aktive stoffer" og hjælpe- eller bærestofferne godt ved omrøring eller homogenisering (f.eks. ved hjælp af sædvanligt blanderapparatur), idet der i almindelighed arbejdes ved temperaturer mellem 20 O 0 25 og 50 C, fortrinsvis 20 til 40 C, især ved stuetemperatur. Til fremstilling af opløsninger eller suspensioner kan man f.eks. anvende vand eller fysiologisk acceptable organiske opløsningsmidler, som f.eks. alkoholer (ethanol, propanol, isopropanol, l,2-propylengly-15 col, polyglycoler og derivater deraf, fedtalkoholer el ler fede alkoholer, og partielle estere af glyceroler), olier (f.eks. jordnøddeolie, olivenolie, sesamolie, mandelolie, solsikkeolie, sojabønneolie, ricinusolie og "klovodolie"), paraffiner, dimethylsulfoxid, trigly-20 cerider og lignende.
Til injicerbare opløsninger eller suspensioner kan man f.eks. anvende ikke-toksiske, parenteralt acceptable fortyndingsmidler eller opløsningsmidler, som f.eks.: vand, 1,3-butandiol, ethanol, 1,2-propylengly-25 col, polyglycoler i blanding med vand, glycerol, Rin ger's opløsning, isotonisk kogesaltopløsning eller hærdede olier, herunder syntetiske mono- eller diglyceri-der eller fedtsyrer, såsom oliesyre.
Ved fremstilling af præparaterne kan kendte og 30 sædvanlige opløsningsfremmende midler eller emulgatorer anvendes. Som opløsningsfremmende midler og emulgatorer kan man f.eks. anvende: polyvinylpyrrolidon, sorbitan-fedtsyreestere, såsom sorbitantrioleat, phosphatider, 13 DK 173585 B1 såsom lecithin, acacia, traganth, polyoxyethyleret sor-bitanmonooleat og andre ethoxylerede fedtsyreestere af sorbitan, polyoxyethylerede fedtstoffer, polyoxyethyle-rede oleotriglycerider, linoliseret oleotriglycerid, 5 polyethylenoxid-kondensationsprodukter af fedtalkoholer, alkylphenoler, fedtsyrer eller fede syrer, eller l-methyl-3-(2-hydroxyethyl)imidazolidon-(2). Polyoxyethyleret betyder i denne forbindelse, at de pågældende stoffer indeholder polyoxyethylenkæder, hvis polymeri-10 sationsgrad i almindelighed ligger mellem 2 og 40, især mellem 10 og 20.
Sådanne polyoxyethylerede stoffer kan f.eks. opnås ved omsætning af hydroxylgruppeholdige forbindelser (f.eks. mono- eller diglycerider eller umættede forbin-15 delser, som f.eks. sådanne forbindelser, der indeholder oliesyrerester) med ethylenoxid (f.eks. 40 mol ethylen-oxid pr. mol glycerid). Eksempler på oleotriglycerider er olivenolie, jordnøddeolie, ricinusolie, sesamolie, bomuldsfrøolie og majsolie. Se også Dr. H.P. Til vandige injektionsopløsninger og drikkeopløsninger anvendes især følgende materialer som stabilisatorer eller opløsningsfremmende midler: lavere ali-25 fatiske mono- eller polyvalente alkoholer med 2-4 C- atomer, såsom ethanol, n-propanol og glycerol, poly-ethylenglycoler med molvægte mellem 200-600 (f.eks. i en 1 til 40%'s vandig opløsing), diethylenglycolmono-ethylether, 1,2-propylenglycol, organiske amider, 30 f.eks. amider af alifatiske C^-Cg-carboxylsyrer med ammoniak eller primære, sekundære eller tertiære C C4-aminer eller C^-C^-hydroxyaminer, såsom urinstof, urethan, acetamid, N-methylacetamid, N,N-diethylacet- 14 DK 173585 B1 amin, Ν,Ν-dimethylacetamid, lavere alifatiske aminer og diaminer med 2-6 C-atomer, såsom ethylendiamin, hy-droxyethyltheophyllin, trometamol (f.eks. som 0,1 til 20%'s vandig opløsning), og alifatiske aminosyrer. Derudover er det muligt at tilsætte konserve ringsmidler; stabilisatorer, puffermaterialer, smagskorrigerende midler, sødemidler, farvestoffer, anti- 15 DK 173585 B1 oxidanter og kompleksdannende midler._og lignende. Som kompleksdannende midler kan man f.eks. anvende: chelat-dannende midler, såsom ethylendlamino-tetraeddikesyre, nitriloeddikesyre, diethylentriaminpentaeddikesyre og 5 salte deraf. Endvidere kommer også sådanne midler, der indeslutter R- eller S-a-liponsyre i et hulrum, på tale. Eksempler herpå er urinstof, thiourinstof, cyclo-dextrin og amylose. Ligeledes kan pH-værdien indstilles til en værdi i området fra 6 til 9 med fysiologisk ac-10 ceptable baser eller puffere til stabilisering af det aktive molekyle. I almindelighed foretrækkes en så neutral til svagt sur (indtil pH 8) pH-værdi som muligt.
Som antioxidanter kan man f.eks. anvende natriumsulfit, natriumhydrogensulfit, natriummetabisul-15 fit, ascorbinsyre, ascorbylpalmitat, -myristat eller -stearat, gallussyre, gallussyre-alkylester, butyl-hydroxyanisol, nordihydroguajaretsyre, tocopheroler og synergistikse materialer (stoffer, der binder tungmetaller ved kompleksdannelse, f.eks. lecithin, acsorbin-20 syre, phopshorsyre, ethylendiamintetraeddikesyre. citrat og tartrat). Tilsætningen af synergistiske materialer øger antioxidanternes antioxiderende virkning væsentligt.
Som konserveringsmiddel kan man f.eks. anvende 25 sorbinsyre, p-hydroxybenzoesyreester (f.eks. en lavere alkylester), benzoesyre, natriumbenzoat, trichloriso-butylalkohol, phenol, kresol, benzethoniumchlorid, chlorhexidin og formaldehydderivater.
ce-Liponsyre har især en væksthæmmende. yirkning_på 30 retrovirus, d.sær humanimmundeficiensvirus (HIV, f.eks.
HIV-1 og HIV-2) og en aktiverende ..og vækstfremmende virkning på perifere, mononucleare _blodc.eller. Eksempelvis forsinker eller hæmmer forbindelserne I formeringen af viruset.

Som retrovirus, mod hvilke forbindelserne/pro-dukterne ifølge opfindelsen er virksomme, kommer f.eks.: HIV-virus, oncornavirus og spumavirus, på tale.
Forbindelserne med formlen I har f.eks. en god 5 væksthæmmende virkning på Hiv (type 1 og 2), hvilket kan vises in vitro, f.eks. ved de følgende virologiske-cellebiologiske testmetoder: 1. 15 Ved en enkel tilsætning af 0,035 mg/ml a-lipon- syre som racemat sænkes antallet af infektiøse virus (f.eks. HIV-l) i kultursupernatanten, således f.eks. fra 100% i den positive kontrol til 0%. En virusinhibe-rende virkning kan ved den testmetode allerede påvises 20 ved meget lave doser, f.eks. 0,001 mg/ml.
Som alment dosisområde til opnåelse af virkning (forsøg som ovenfor) gælder f.eks. følgende: 0,0035 til 0,091 mg/ml, især 0,035-0,070 mg/ml.
Ved in vitro-forsøgene anvendes det aktive stof 25 med formlen I f.eks. i benzylalkohol som opløsningsmiddel.
Til in vitro-undersøgelserne af replikationsfor-holdene for retrovirus, især HIV, kan man f.eks. anvende følgende substrater: 17 DK 173585 B1 1. Virusholdigt RPMI 1640 medium, f.eks. IX flydende 041-01875 (syntetisk kulturmedium fra Gibeo ifølge Moore, Gerner og Franklin, H.A. (1967), J.A.M.A.

Celletype: human T-celleleukæml. Vækstmedium: RPMI 1640, 90%; føtalt kalveserum, 10%. Indfrysningsmedium: kulturmedium, 90%; dimethylsulfoxid, 10%. Andel levende celler: 80%. Hver 2.-3.dag foretages passage af 15 cellerne. Celleantallet skal holdes mellem 105 og 106 celler/ml. Morfologi: lymfocytagtig. Karyologi: ikke angivet. Sterilitet: bakterier og mycoplasma negativ. Revers transkriptase: negativ. Specielle egenskaber: denne klon af Jurkat-FHCRC (Dr. Kendall Smith, Dart-20 mouth) danner store mængder af interleukin-2 (IL-2) efter passende stimulering. Kulturegenskaber som den oprindelige cellelinie. Morfologi: lig den oprindelige cellelinie. Karylogi: ikke angivet. Sterilitet: bak terier og mycoplasma negativ. Revers transkriptase: 25 negativ. Særlige egenskaber: inden den retrovirusfor- midlede genoverføring med CD4c:DNA eksprimerer disse celler intet overflade-CD4, og de er ikke følsomme for AIDS-virusinfektionen. Den hæmmende virkning vises på en cellelinie, der er inficeret permanent med HIV-1.
Fremstilling af en cellelinie, der er permanent infice-5 ret med HIV-1: 1. Negative (ikke-inficerede) celler (f.eks. Molt 4) udtages fra kulturkolben med kulturmedium RPMI 1640 og centrifugeres herefter i egnet beholder. Som negati- 10 ve celler kan principielt celler af enhver CD4-recep-tor-positiv T-cellelinie anvendes. Eksempelvis kommer følgende kommercielt tilgængelige cellelinier på tale: H9, Hut78, SupTl, Jurkat og Molt 4. Molt 4 er f.eks. en monoklonal T-cellelinie fra det perifere blod af en pa- 15 tient med kredsløbssygdomme. T-Cellelinier hidrører fra humane tumorer, f.eks. ved bestemte leukæmiformer. Sådanne cellelinier vokser permanent så længe de holdes i et passende næringsmedium ved en passende C02-koncen-tration i et inkubationsskab ved 37°C. Disse cellelini- 20 er kan erhverves fra det såkaldte American Type Culture Collection (ATCC).
2. Disse Molt4 celler resuspenderes dernærst i kulturmedium (RPMI 1640 + 10% føtalt kalveserum) med højt 25 virusindhold, hældes i en kulturflaske og inkuberes i ca. 12 timer i inkubationsskab ved 37°C og 5% C02. Herved inficeres en del af cellerne (alt efter koncentrationen af de inficerende virus).
30 3. Efter 12 timer centrifugeres cellerne igen fra, og de resuspenderes i frisk medium (RPMI 1640, ikke-virusholdig) og dyrkes i varmeskab i ca. 4 uger.
20 DK 173585 B1 4. I den forbindelse udskifter man hver uge mediet fuldstændigt en gang (fracentrifugering, resuspension, osv.) og negative, ikke-inficerede celler tilsættes.
5 5. Efter ca. 4 uger er 80-90% af cellerne infice ret = permanent inficeret cellelinie. Inficeringen bestemmes f.eks. ved hjælp af immunfluorescens. De således inficerede celler danner HIV i et titerområde fra ca. 105 - 106 infektionsenheder (PFU). Den således per-10 manent inficerede tumorcellelinie (Molt 4) behandles i 3 uger med 70 yg α-liponsyre/ml. Herunder fornyedes kulturmediet og a-liponsyren hver tredje dag, og virusaktiviteten bestemtes ved revers transkriptase og pla-quetest. Ved den reverse transkriptasetest måltes den 15 relative mængde af dannet virus (også af infektionsdefekte partikler), mens man ved plaquetesten kun kan bestemme indholdet af infektløse virusenheder. Det valgte koncentrationsområde på 70 yg/ml ligger på ca.
10-20 gange over den plasmakoncentration, der måltes 20 ved normal oral, bivirkningsfri dosering. Prolifera-tionshastigheden eller letaliteten af perifere blod-lumfocytter påvirkes ikke væsentligt ved denne dosering. Som det fremgår af kurven i fig. 1 falder antallet af infektiøse virusenheder allerede efter 3 dage 25 til næsten 0, hvilket svarer til en næsten 100%'s reduktion. Antallet af producerede virusenheder falder først fra dag 6 og når efter 3 uger en reduktion på 90%. Disse resultater viser den stærke antivirale virkning af a-liponsyre. Som et andet vigtigt resultat kan 30 det fastslås, at der ved en koncentration på 7 0 yg/ml ikke har vist sig eller er tegn på nogen toleransud-vikling efter 3 uger. Det der især er betydning for en langtidsbehandling af inficerede personer.

Hvis man ønsker at sammeligne in vitro-virknin-gen af a-liponsyre med andre midler, der allerede med resultater anvendes terapeutisk mod AIDS, er det nærliggende at bruge α-interferon. Den antivirale virkning 5 af dette materiale udøves på den posttranslationale side, det vil sige, at man diskuterer en hæmmende virkning på den såkaldte Budding- eller udslusningsproces af viruset. Ligesom a-liponsyre virker a-interferon således på de allerede inficerede celler. Til sammenlig-10 ning af de to forbindelser inficeredes frisk splejsede Jurkat celler med HIV (θ x 103 PFU), hvorefter rekombi-nant α-interferon (rIF) (70 enheder/ml) eller 35 mg/ml a-liponsyre pipetteredes dertil. Forsøget afsluttedes efter 7 dage, især til bedømmelse af væksten af den 15 første virusgeneration. Begge materialer udviste en sammenlignelig hæmning af den virale replikation i de inficerede, ikke-forbehandlede celler (fig. 2 og 3). Skønt rIF trods alt udviser en stærkere inhiberende virkning ved revers transkriptasetesten (måler den re-20 lative mængde af dannede virioner, fig. 2), er hæmningen ved plaquetesten tydeligere for α-liponsyre (giver det nøjagtige antal af infektiøse virioner, fig. 3). Kombinationen af de to materialer udviser en additiv virkning.
25 α-Liponsyre kan til behandling af AIDS især også kombineres med AZT, idet AZT har en anden virkningsmekanisme. AZT hæmmer den reverse transkriptase og virker dermed overvejende på ikke-inficerede celler. Er en celle en gang inficeret, kan AZT imidlertid i 30 modsætning til α-liponsyre ikke mere hæmme væksten af virus.

Disse virusgenerationer kan på disse dage kvantificeres {plaque- og revers-transkriptasetest) og sammenlignes.

Fremgangsmåde
Bestemmelse af p24-antigenkoncentrationen i plasma ved hjælp af en kommerciel ELISA 30 p24 er betegnet som for et strukturprotein af HIV-virus ELISA (forkortelse for enzymlinket immunsorbent-assay) er en indenfor virologien hyppigt anvendt test- 23 DK 173585 B1 metode til bestemmelse af proteiner, antigener, osv.
Virusisolering med sammendyrkning:
Patientlymfocytter dyrkes sammen med donor-5 lymfocytter. Det af disse celler dannede virus (p24) måles efter 21 dage ved en ELISA (standardmetode).
Virusisolering uden sammendyrkning: Den dannede mængde af virus (p24) ud fra 1 x 106 patientlymfocytter 10 bestemmes kvantitativt efter 11 dage ved hjælp af en ELISA. Ved denne standardiserede fremgangsmåde tilsættes ingen donorlymfocytter. Den dannede mængde virus kan angives i pg*p24/ml/200000 celler. (Ny udviklet fremgangsmåde).

Celletitrering: Virusisoleringen gennemføres idet man går ud fra et faldende antal patientlymfocytter (f.eks. ΙΟ6, ΙΟ5, 104).

Resultater 25 De følgende resultater opnåedes ved 4 patienter på Walter-Reed-stadium 6. Administrering af a-liponsyre udførtes ved infusion af en α-liponsyreopløsning med følgende sammensætning: 10 ml vandig opløsning indeholdende 250 mg a-liponsyre i form af ethylendiaminsaltet 30 (323 mg salt) og 1 g 1,2-propylenglycol og 100 mg ben- zylalkohol.
Walter-Reed-stadier: Inddeling af HIV-inficerede patienter efter graden af deres kliniske symptombille 24 DK 173585 B1 de. Ved stadium O kan intet HIV-virus påvises. Stadium 6 betyder slut AIDS-stadium.
Patient 1 (K.M., 64 kg; Walter-Reed-stadium 6) 5
Terapeutisk fase 58 dage, dog afbrudt 2 x 36 timer under ferier. Den samlede dosis udgjorde 258,3 g, svarende til en gennemsnitlig daglig dosis på 4,7 g/dag. På 19 dage administreredes doser fra 3 til min-10 dre end 6 g, og på 18 dage doser på 6-8 g/dag.
Infusionen af a-liponsyre udførtes via et centralt venekateter over 24 timer. På grund af at man i starten ikke kunne udelukke interaktioner med Zovirax (på grund af Herpes Zoster) blev Infusionen med a-li-15 ponsyre afbrudt under Zovirax-indgivelsen (3 gange af hver 1-2 timer/dag).
p24-Antigenkoncentrationen i plasmaet bestemtes, og virusisoleringen gennemførtes. p24-Antigenkoncentra-tionen i plasmaet bevægede sig i måneden inden den te-20 rapeutiske behandling påbegyndtes i et område omkring 0,350 ng/ml. 14 Dage efter påbegyndelsen af den terapeutiske behandling faldt denne værdi til 0,05 ng/ml, hvorefter den faldt yderligere indtil 29. dagen (til 0,025 ng/ml), hvorefter den igen steg til en værdi på 25 0,25 ng/ml ved afslutningen af den terapeutiske be handling. virusisoleringen (standardmetode) var positiv under hele den terapeutiske behandling.
Patient 2 (Ro., 50 kg; Walter-Reed-stadium 6) 30
Den terapeutiske fase varede 21 dage, idet der kun administreredes medikament på i alt 17 dage. Den samlede dosis udgjorde 32 g. Doseringen udgjorde 1 g på 2 dage, 3 g på 8 dage og 4 g på 1 dag.
25 DK 173585 B1 p24-Antigenkoncentrationbestemmelse i plasma: Virusisolering med celletitrering og plasmatitrering gennemførtes ved denne patient. p24-Antigenkoncentra-tionen i plasmaet lå inden behandlingen konstant ved 5 0,4 ng/ml. Denne værdi faldt først til knap 1/10. Cel- letiteren lå inden behandlingen ved 1000, dvs. at virus kun kunne isoleres ud fra 103 celler (standardmetode).
I løbet af den terapeutiske behandling steg denne værdi med to logaritmiske trin, dvs., at viruset kun kunne 10 podes ud fra 105 celler. Inden den terapeutiske behandling lå plasmatiteren ved 100. Denne værdi faldt allerede kort efter og begyndelsen af den terapeutiske behandling til 0.
Mod aflsutningen og efter afslutningen af den 15 terapeutiske behandling skete et drastisk fald af samtlige virusmarkører. Selv virusisoleringen var gentagne gange negativ.
Patient 3 (Pal.; Walter-Reed-stadium 6) 20
En samlet dosis af a-liponsyre på 104 g på 26 dage af den 27 dages terapeutiske fase. På 2 dage administreredes 2 g/dag, på 2 dage 3 g/dag, på 1 dag 4 g/dag, på 2 dage 5 g/dag og på 3 dage 6 g/dag. Efter 1 25 dags pause uden terapeutisk behandling, 2 dage med 6 g/dag, efterfølgende dosisreduktion til 1 g i 4 dage på grund af tiltagende trombopeni. Dernæst l dag med 3 g, 6 dage med 6 g/dag, dernæst dosisreduktion til l g i 3 dage på grund af fornyet fald i antal thrombocytter 30 (daraprim-bivirkning) og endelig 1 dag med 2 g/dag. Dernæst planmæssig afslutning af den terapeutiske behandling. På 11 dage i alt administreredes den maksimale dosis på 6 g/dag.

Ved denne patient måltes p24-antigenkoncentra-tionen i plasmaet (hver gang negativ), virusisolering samt plasmatitrering (hver gang negativ). Inden påbegyndelsen af den terapeutiske behandling lå produktio-5 nen af p24-antigen i pg/ml/200000 celler på ca. 10. Denne værdi fordobledes ved 5. dagen af den terapeutiske behandling og faldt derefter kontinuerligt indtil 28. dagen til 0. Efter den terapeutiske behandling steg denne værdi igen langsomt.

Terapeutisk fase 19 dage. Samlet dosis af a-li-ponsyre: 82,75 g. Terapeutisk behandling for 22.05.
15 1990 med a-liponsyre-infusion med en varighed på 20- 24 timer/dag. Doser: 2 dage med 2 g, 9 dage med 4 g, 1 dag med 3 g, 1 dag med 2,75 g, 3 dage med 6 g, l g med 7 g og 2 dage med 6 g. Indgivelse som 20-24 timers langtidsinfusion med l gangs pause på Vi dag.
20 Alle parametre (p24-antigenkoncentration i plas ma) , virusisolering med celletitrering (plasmatitrering) og ved denne patient positive og måltes under den terapeutiske behandlingsforløb.
p24-Antigenkoncentrationen i plasmaet, der bevæ- 25 gede sig omkring 0,070 ng/ml kort inden og indtil påbegyndelsen af den terapeutiske behandling, faldt indtil 18. dagen af den terapeutiske behandling til 0,005 ng/ml. Efter afslutningen af den terapeutiske behandling steg koncentrationen igen til begyndelsesværdien.
30 p24-Produktionen/ml/200000 celler lå kort inden den terapeutiske behandling på 8 pg/ml. indtil afslutningen af den teapeutiske behandling faldt denne værdi med 75%, og den steg væsentligt efter afslutningen af den 27 DK 173585 B1 terapeutiske behandling. Til dette tidspunkt var"påti-enten desuden under terapeutisk behandling med AZT. Plasmatiteren, der inden påbegyndelsen af den terapeutiske behandling var 10, faldt under den terapeutiske 5 behandling til 0 og indstillede sig dernæst vé3 afslutningen og efter dne terapeutiske behandling til i. Inden påbegyndelsen af den terapeutiske behandling kunne virus entydigt påvises fra 2 x 106 celler. Ved afslutningen af den terpeutiske behandling kunne ingen" virus 10 nu påvises (i tilsvarende områder). Efter afslutning af den terapeutiske behandling forringedes resultaterne igen.
Sammenfattende opnåedes følgende resultater: Under thiooctacid-terapi faldt koncentrationen af posi-15 tive p24-antigen i plasma (tilfælde i og-2')_ "tydeligt indenfor et kort tidsrum (ca. 14 dage) til mindst 1/10 af dens udgangsværdi.
Ved virusisoleringen udfra patientlymfocytter kunne en tydelig ringere virusproduktion i tre tilfælde 20 påvises under den terapeutiske behandling. Ved to patienter (tilfælde 2 og 3) var resultatet sågar helt negativt. Positive plasmatitere faldt ligeledes under den terapeutiske behandling til 0 (tilfælde 2 og 4).
Disse resultater viser, at man „ved patienter, 25 der viser tydelige, målelige, virologiske markører, under thiooctacid-behandling kan opnå en udpræget forbedring af disse parametre således ved en længere varende behandling med a-liponsyre kan regne med en positiv virkning af det samlede sygdomsbillede.
30 virkningsmåden af o(-Hponsyre anvendt ifølge op findelsen med hensyn til deres antirerovirale virkning kan først og fremmest sammenlignes med virkningen af det kendte lægemiddel immuthiol (natrium-diethyl-di- 28 DK 173585 B1 thiocarbonat), idet den cytopatogene virkning (CPE) f.eks. til forskel fra ved disse undertrykkes væsentligt tydeligere med <=(-liponsyre, især når stofferne gives efter inficering af cellerne.
5 Som indikationer for «^-liponsyre kommer følgende f.eks. i betragtning: terapeutisk behandling af HIV inficerede, asymptomatisk eller symptomatisk syge personer under alle stadier af erhvervet immundeficiens-syndrom (AIDS) efter den internationalt sædvanlige 10 klassificering. Der er ingen kontraindikationer.
Doseringsmængde og anvendelsesformer, der udover de allerede angivne kommer på tale, især ved anvendelse ved bekæmpelse af retrovirus (især AIDS) er:
De farmaceutiske præparater indeholder til denne 15 anvendelse i almindelighed mellem 5 0 mg og 3 g som en enkeltdosis, fortrinsvis 100 mg til 1 g, især 400 mg eller 500 mg til 1 g <x-liponsyre. Den opnåede koncentration af aktivt stof/kg legemvægt skal ligge mellem 3,5 og 200 mg, fortrinsvis mellem 7 og 100 mg, især 20 mellem 35 og 70 mg/kg legemvægt. «K-Liponsyre skal afgives langsomt fra præparaterne. Dette gælder også for komponenterne b ved kombinationspræparater.
Administreringen kan f.eks. foretages i form af tabletter, kapsler, piller, drageer, aerosolpræparater 25 eller i væskeformige præparater.
Som væskeformige anvendelsesformer kommer f.eks. følgende på tale: alkoholiske eller vandige opløsninger, suspensioner og emulsioner.
Fortrukne anvendelsesformer er f.eks. tabletter, 30 der indeholder mellem 100 mg og 2 g af det aktive stof eller opløsninger, der indeholder mellem 10 mg og 0,2 g/ml væske af aktivt stof.
Enkeltdosen af c*-liponsyre kan f.eks. være: 29 DK 173585 B1 a) ved orale lægemiddelformer mellem 100 mg og 3 g, fortrinsvis 200 mg og l g, især 400 mg og l g.
b) ved parenterale lægemiddelformer (f.eks. intra- 5 venøse eller intramuskulære) mellem 100 mg og 12 g, fortrinsvis 200 mg og 6 g, især 500 mg og 6 9· c) ved lægemiddelformer til inhalation (opløsnin- 10 ger eller aerosoler) mellem 100 mg og 2 g, for trinsvis 200 mg og lg
Doserne ifølge a) til c) kan f.eks. administreres 1 til 6 gange, fortrinsvis 1 til 4 gange dagligt, 15 eller som en vedvarende infusion, f.eks. ved hjælp af infusoniater (infusionsapparat til nøjagtig dosering af et opløst, aktivt stof hver time). Den daglige dosis af -liponsyre for mennesker ligger f.eks. for den parenterale administrering i området fra 40 til 80 mg, for-20 trinsvis 40 til 60 mg, pr. kg legemsvægt (f.eks. intravenøs vedvarende infusion over 24 timer). Ved længere anvendelse kan dosen eventuelt øges, f.eks. til 160 mg pr. kg legemsvægt indenfor 24 timer. Ved oral administrering kan den daglige dosis f.eks. ligge mellem 40 25 og 120 mg/kg legemsvægt (ved længere anvendelse endog stigninger til 160 mg eller 200 mg pr. kg legemsvægt dagligt). Enkeltdosen ligger f.eks. ved 60-20 mg pr. kg legemsvægt, idet denne dosis hensigtsmæssigt indgives 4 gange pr. dag. Fortrinsvis udgør dagsdosen af det akti-30 ve stof med formlen I 4-6 g: lægemidlet indeholder således fortrinsvis 1-1,5 g cx-liponsyre i en galenisk formulering, idet en sådan dosis fortrinsvs administreres 4 gange.

Til behandlingen kan f.eks. anbefales l til 4 tabletter med et indhold på 50 mg til 2 g, fortrinsvis 400 mg eller 500 mg til 2 g aktivt stof eller en intravenøs injektion 1 til 4 gange dagligt fra en ampul/in-5 fusionsflaske med et indhold på 1 til 500 mg med 200 mg til 6 g, især 500 mg til 6 g aktivt stof, f.eks. anbefales. Ved oral administrering af den minimale, daglige dosis f.eks. 300 mg. Den maksimale, daglige dosis ved oral indgivelse må ikke overkride 12 g.
10 Det angivne dosismængder er baseret på den frie syre af o<-liponsyre. Hvis c<-liponsyre i form af dets salt anvendes, skal de angivne doseringer/doserings-områder forhøjes svarende til den tilsvarende molvægt.
Til kombinationspræparater af det aktive stof 15 a-liponsyre med komponenten b, f.eks. AZT, kan de to komponenter f.eks. hver gang blandes i et forhold på l til 100 eller 100 til 1 ækvimolære andele af det virksomme stof, især i et forhold på l til 10 til 10 til 1, fortrinsvis i et forhold fra 1 til 3 til 3 til 1 ande- 20 le. Hvis det drejer sig om en kombination af det aktive stof a-liponsyre og a-interferon, kan de to komponenter f.eks. foreligge i de følgende forhold: 50 mg - 6 g forbindelse I (komponent a) til 8 x 106 enzymenheder til 1 x 105 enzymenheder α-interferon, især 0,5 - 3 g 25 komponent a til 1-4 x 106 enzymenheder a-interferon.
I kombinationspræparater af «k-liponsyre og andre komponenter ifølge b) kan de to komponenter foreligge som en blanding. I almindelighed foreligger komponenter dog adskilt fra hinanden i en galenisk formulering, 30 idet de hertil velkendte, galeniske formuleringer kommer på tale: f.eks. en komponent som tablet eller lakeret tablet, den anden komponent som et pulver, begge komponenter i en kapsel og overtrukket; en komponent i 31 DK 173585 B1 form af pellets, den anden som pulver, drageer eller tabletter, der er overtrukket, idet de to former f.eks. kan foreligge i en kapsel; eller i form af flerlagede tabletter eller tabletter med en kappe. Der henvises i 5 denne forbindelse f.eks. til bogen af Karl Thoma, ArzneimittelstabilitSt, Frankfurt 1978, f.eks. side 207 ff.
Kombinationspræparaterne kan også foreligge som produkter, ved hvilke de to aktive enkeltstoffer 10 foreligger i formuleringen, der er fuldstændigt adskilt fra hinanden, idet især komponenten b, men også de to komponenter (a og b) kan foreligge i ampuller og/eller infusionsflasker, således at en adskilt eller tidsforskudt administrering er mulig.
15 Hvis der foreligger sådanne fuldstændigt adskil te formuleringer, er disse afstemt efter hinanden, og de indeholder de enkelte, aktive stoffer i doseringsenhederne i den samme mængde og i tilsvarende vægtforhold , som de kan foreligge i i det kombinerede blan-20 dingspræparat.
Ved et produkt til den adskilte anvendelse er det også muligt, at aministrere de to kombinationsparter tidsforskudt. I sådanne tilfælde kan of-liponsyre f.eks. administreres som en langvarig infusion (dosis 25 f.eks. 2-5 g pr. dag) og den anden komponent b kan gives samtidig (dosis f.eks. 50-800 mg eller 1-8 x 106 enzymenheder, fortrinsvis intramuskulær) eller som langtidsinfusion pr. dag, eller c<-liponsyre kan f.eks. gives 4 gange pr. dag (enkeltdosis f.eks. 0,52 g), og 30 den anden komponent b kan gives samtidig (dosis f.eks. 50-200 mg eller 0,5-3 x 10^ enzymenheder). I denne forbindelse kan der f.eks. følge l til 3 yderligere dosis af komponent b (f.eks. mellem 50 og 200 mg eller 0,5 og 32 DK 173585 B1 3 x 106 enzymenheder) med et mellemrum på 6 og/eller 12 timer hver gang.
Præparaterne/produkterne indeholder fortrinsvis også tilsatte vitaminer, især vitamin B1 og/eller 5 vitamin E.
Den akutte toksicitet af a-liponsyren på mus (udtrykt som LD 50 mg/kg; fremgangsmåde efter Miller og Tainter; Proc. Soc. Exper. Biol. a. Med. 57 (1944) 261) ligger eksempelvis ved oral indgivelse over 400 mg/kg 10 kropsvægt for dihydroliponsyre over 200 mg/kg kropsvægt i mus.
Eksempler 15 Eksempel i
Tabletter med 50 mg S- eller R-a-liponsyre 250 g S-a-liponsyre findeles jævnt med 750 g 20 mikrokrystallinsk celleulose. Efter sigtning af blandingen blandes 250 g stivelse (Starch 1500/Colorcon), 762,5 g lactose, 15 g magnesiumstearat og 2,5 g høj-dispers siliciumdioxid i, og blandingen presses til tabletter med en vægt på 400 mg.
25 En tablet indeholder 50 mg S-a-liponsyre.
På samme måde kan tabletter med 50 mg S-a-liponsyre fremstilles, idet der i stedet for 250 g S-a-liponsyre anvendes det samme mængde R-a-liponsyre.
Ligeldes kan tabletterne forsynes med et mave-30 saftopløseligt eller mavesaftpermeabelt filmovertræk ved sædvanlige fremgangsmåder.

Ampuller med 50 mg S- eller R-a-liponsyre som trometa-molsalt i 2 ml.
5 250 g S-a-liponsyre opløses sammen med 352,3 g trometamol (2-amino-2-(hydroxymethyl)-1,3-propandlol) i en blanding af 9 liter vand til injektions formål og 200 g 1,2-propylenglycol under omrøring. Opløsningen 10 fyldes op til 10 liter med vand til injektionsformål og filtreres dernæst gennem et membranfilter med en porestørrelse på 0,2 μπι med et glasfiberforfilter. Filtratet fyldes under aseptiske betngelser op til 2 ml i steriliserede 2 ml-ampuller.
15 En ampul indeholder 50 mg S-a-liponsyre som tro metamol s alt i 2 ml injektionsopløsning.
På samme måde kan ampuller med R-a-liponsyre fremstilles, idet der i stedet for 250 g S-a-liponsyre anvendes den same mængde R-a-liponsyre.
Claims (4)
1. Anvendelse af α-liponsyre til fremstilling af et lægemiddel til bekæmpele af sygdomme fremkaldt af retrovirus, kendetegnet ved, at doseringsenheden til 5 faste eller halvfaste præparater indeholder 51 mg til 6 g, især 100 mg til 2 g, fortrinsvis 200 mg til 1 g a-liponsyre eller et farmaceutisk acceptabelt salt deraf, eller at den, når det drejer sig om injektionsopløsninger, indeholder 26 mg til 500 mg pr. ml, fortrinsvis 50 mg til 200 mg, især 10 100 mg pr. ml af of-liponsyre eller af et farmaceutisk acceptabelt salt deraf, eller at den, når det drejer sig om drikkelige opløsnings-, suspensions- eller emulsionspræparater, indeholder 0,2 mg til 500 mg pr. ml, fortrinsvis 1 mg til 50 mg pr. ml, især 5 mg til 10 mg pr. ml af 15 a-liponsyre eller af et farmaceutisk acceptabelt salt deraf.
2. Anvendelse af a-liponsyre til fremstilling at et lægemiddel ifølge krav 1, kendetegnet ved, at lægemidlet indeholder α-liponsyre som aktivt stof og 20 desuden sædvanlige farmaceutiske bærere, hjælpestoffer, stabilisatorer, opløsningsfremmende midler og/eller fortyndingsmidler.
3. Anvendelse af α-liponsyre til fremstilling af et lægemiddel ifølge krav 2, kendetegnet ved, at 25 opløsninger, der som aktivt stof indeholder a-liponsyre, indeholder stabilisatorer og/eller opløsningsfremmende midler.
4. Anvendelse af α-liponsyre til fremstilling af et lægemiddel ifølge krav 3, kendetegnet ved, at 30 der som stabilisatorer eller opløsningsfremmende midler anvendes følgende stoffer: alifatiske c2-C4-alkoholer, der indeholder 1, 2 eller 3 hydroxylgrupper, polyethylenglyco-ler med molvægt mellem 200 og 600, sædvanlige fysiologisk acceptable organiske amider, naturlige α-aminosyrer, 35 alifatiske aminer, hydroxyethyltheopyllin, trometamol eller diethylenglycolmonomethylether.
