Description

Vendaje para herida abdominal.
La presente invención se refiere en general a métodos y dispositivos utilizados para curar heridas causadas por lesión o cirugía. Más específicamente, la presente invención se refiere a métodos y dispositivos para facilitar la curación de heridas abiertas, tales como heridas abdominales que a las que se aplica un cierre temporal que se vuelve a abrir antes del cierre final.
Se han producido avances significativos en la prevención de infecciones post-operatorias. Sin embargo, el tratamiento post-operatorio de pacientes sometidos a varios tipos de cirugías invasoras, especialmente aquellas en las que se ve afectada la cavidad abdominal, se ve mejorado por la posibilidad de poder intervenir nuevamente en la cavidad con el objeto de atender casos de peritonitis y otras infecciones que suelen inhibir la curación final de la herida y de los órganos internos. Si bien el resultado óptimo es que no exista ninguna infección post-operatoria ni otras complicaciones, la realidad es que suelen producirse. Este hecho ha motivado que la práctica habitual en la actualidad sea realizar un cierre temporal de la herida y volver a abrir posteriormente la herida con el objeto de limpiar la cavidad abdominal y tratar aquellas complicaciones que requieran tratamiento antes del cierre final o definitivo.
La descripción de los antecedentes en la patente US 5, 437,683 concedida a Neumann et al. y titulada "Cierre Quirúrgico", presenta una panorámica de las diversas complicaciones asociadas con la cirugía abdominal y el proceso de limpieza post-operatorio de dicha cavidad abdominal. Neumann et al. describe una solución, en cierto modo compleja, para realizar el cierre de una herida que se pueda volver a abrir, es decir un dispositivo de material flexible o lamina de seguridad que cubre la herida, presenta un tubo de succión y se extiende hacia el exterior formando un tubo mayor, sellable, que permite el acceso ulterior según se necesite antes del cierre final.
Se han descrito otras muchas técnicas para el cierre temporal post-operatorio de la pared tras la cirugía abdominal. El estudio de estas técnicas permite establecer criterios objetivos que sirven para evaluar la idoneidad de varias modalidades de cierre temporal propuestas.
Un cierre temporal adecuado deberá ser sencillo, fácil de aplicar, eficaz para contener el contenido abdominal, interferir en grado mínimo con los órganos abdominales y el tejido circundante, promover mínimamente, si acaso, la formación de adhesiones y fístulas y promover el proceso hacia el cierre definitivo.
Un problema que comparten muchos métodos existentes de cierre temporal de heridas es la formación de adhesión entre el tejido y el material en contacto con la herida. Estas adhesiones no son deseables y complican el proceso de preparación de la zona de la herida para proceder al cierre final. El hecho de colocar materiales porosos tales como gasa, toallas quirúrgicas e incluso espuma de celdas abiertas, en contacto directo con la zona de la herida abierta, más bien complica el cierre final al proporcionar una mayor superficie que promueve la formación de adhesiones, con lo cual resulta todavía más difícil quitar este material.
Resulta manifiestamente beneficioso el mantener una presión negativa sobre la zona de la herida sin crear condiciones que hacen más difícil la eliminación del material que cubre la herida temporalmente. Un recubrimiento deseable en capas de la herida permite tanto la aplicación de presión negativa como la eliminación resultante de los fluidos exudados. Se puede acceder nuevamente a esta cobertura con el objeto de limpiar la herida sin causar traumas en la zona de la misma. Este recubrimiento de la herida resulta particularmente ventajoso para heridas quirúrgicas abdominales abiertas. Estos dispositivos y técnicas también se podrían utilizar en otros tipos de heridas abiertas que presentan un proceso de curación difícil.
Un recubrimiento de herida para el tratamiento de una herida mediante la aplicación de presión negativa terapéutica se describe en una solicitud internacional publicada con el número WO 00/07653. Este recubrimiento de herida del estado de la técnica incorpora una lámina de plástico que tiene unos orificios poco espaciados en forma de malla en su centro. La lámina tiene adhesivo en un lateral, que se utiliza en la práctica para sujetar la lámina a una herida. Una almohadilla porosa está situada en el otro lado de la lámina de adhesivo y se mantiene junto a la lámina por medio de un revestimiento importante. Se aplica una presión negativa a un conector que se extiende por el revestimiento, de modo que se pueden sacar los fluidos de la herida a través de la malla de la lámina, pasando por la almohadilla porosa y saliendo del conector. La lámina de este recubrimiento de herida tiene adhesivo en su superficie que se utiliza específicamente para sujetar la lámina a la herida.
La necesidad de un recubrimiento para la herida y un sistema de tratamiento terapéutico que permita la aplicación de presión negativa terapéutica sin causar mayores efectos secundarios negativos en la herida abierta es algo evidente. En la actualidad no existe, que se sepa, ningún recubrimiento de herida de este tipo ni sistema de tratamiento. Esta invención pretende cubrir esta necesidad.
Según un aspecto de la invención, se ofrece un cierre separable para herida, que comprende una almohadilla porosa, permeable a los fluidos, que se coloca sobre o dentro de una herida, almohadilla porosa que contiene una espuma polímera que tiene células de interconexión y una lámina de plástico flexible que tiene una pluralidad de orificios distribuidos sobre su superficie, lámina de plástico que está en contacto con una superficie de la almohadilla porosa, de modo que cuando se utiliza, la lámina de plástico se dispone entre la superficie de la herida y la almohadilla, un recubrimiento en forma de lámina impermeable a los líquidos para colocar sobre la almohadilla porosa y un dispositivo de conexión que se extiende por el recubrimiento laminar y en comunicación hidráulica con la almohadilla porosa para conectar a una fuente de presión negativa con el fin de estimular un flujo de fluidos exudados de la herida, donde el revestimiento laminar tiene un perímetro adhesivo para adherirse a un área de piel que rodea la herida, y los orificios distribuidos por la superficie de la lámina constituyen menos del 10% del área efectiva de la lámina.
De preferencia, los orificios en la lámina de plástico flexible comprenden unas hendiduras o ranuras.
Convenientemente, la lámina flexible de plástico es de material laminar elastómero.
Ventajosamente, el dispositivo de conexión comprende una parte de brida dispuesta entre el revestimiento laminar y la almohadilla porosa y una parte tubular que se extiende por el revestimiento para conectar con la fuente de presión negativa.
De preferencia, la parte de brida del dispositivo de conexión incluye unos canales para dirigir el flujo de fluido desde la almohadilla porosa a la parte tubular.
Se ha dispuesto convenientemente una segunda hoja de lámina flexible entre el revestimiento laminar y la almohadilla porosa, estando esta segunda hoja de lámina flexible adherida a la primera hoja de lámina flexible en torno a sus perímetros respectivos para formar una envoltura y la segunda hoja de lámina flexible presenta unos orificios de modo que cuando se conecta el dispositivo de conexión a una fuente de presión negativa, se extrae exudado de la herida a través de la almohadilla porosa y sale de la almohadilla a través de los orificios en la segunda hoja flexible.
Ventajosamente, los perímetros de las láminas flexibles están sellados juntos para formar una brida de lámina unida, que se extiende hacia el exterior, más allá del perímetro de la almohadilla porosa.
Convenientemente, los orificios en la primera hoja flexible están descentrados con respecto a los orificios en la segunda lámina flexible.
Ventajosamente, los orificios en la primera hoja flexible están alineados con unos orificios dentro de una primera parte contigua de la almohadilla porosa y los orificios en la segunda están alineados con unos orificios en una segunda parte contigua de la almohadilla porosa.
De preferencia, la almohadilla porosa comprende dos o más capas de espuma polímera que tienen células de interconexión.
Convenientemente, la almohadilla porosa comprende una forma polímera que tiene al menos 90% de células de interconexión.
De preferencia, se dispone una almohadilla porosa adicional entre la segunda hoja de lámina flexible y el revestimiento laminar.
La invención también se refiere a un vendaje para herida separable, especialmente como el que se aplicaría a una gran herida abierta que requiere el drenaje de fluido de la misma, vendaje que comprende:
una primera almohadilla porosa, permeable a los fluidos, para poner en contacto con la herida, comprendiendo:
una almohadilla de espuma que comprende una espuma a base de polímero que tiene unas células de interconexión y presenta una pluralidad de orificios transversales dispersos; y
unas hojas de lámina elastómera, selladas en su periferia, donde la primera hoja de lámina elastómera tiene una pluralidad de orificios dispersos en la misma, y la segunda tiene una pluralidad de orificios dispersos en la misma, estando los orificios dispersos de la segunda hoja alineados con los orificios dispersos de la primera hoja;
un revestimiento laminar, impermeable a fluidos, para colocar sobre la segunda almohadilla porosa mencionada y la primera almohadilla porosa colocada sobre la herida, comprendiendo el revestimiento laminar un perímetro adhesivo para adherir dicho revestimiento a una zona de la piel que rodea dicha herida; y
un conector tubular que atraviesa dicho revestimiento laminar y en comunicación hidráulica con la almohadilla porosa mencionada, para conectar a una fuente de presión negativa e iniciar un flujo de fluidos exudados de dicha herida.
Convenientemente, el vendaje puede comprender adicionalmente una envoltura elastómera que contiene:
una primera y segunda hoja de lámina elastómera, selladas en su periferia, y que encierran la mencionada almohadilla de espuma, donde la primera hoja de lámina elastómera tiene una pluralidad de orificios dispersos en la misma, orificios dispersos que están descentrados y no alineados con los orificios dispersos en la almohadilla de espuma, teniendo la citada segunda hoja de lámina elastómera una pluralidad de orificios dispersos alineados, los citados orificios dispersos en esta segunda hoja con los mencionados orificios dispersos en la almohadilla de espuma; y
una segunda almohadilla porosa, permeable a los fluidos, para colocar sobre y en contacto con dicha primera almohadilla porosa, donde la mencionada segunda almohadilla porosa comprende una espuma polímera que tiene células de interconexión.
La invención, en otro aspecto, comprende un vendaje para herida separable, especialmente como el que se aplicaría a una gran herida abierta que requiere el drenaje de fluidos de la misma, vendaje que comprende:
una primera almohadilla porosa, permeable a los fluidos, para poner en contacto con la herida, primera almohadilla porosa que comprende:
una almohadilla de espuma que comprende una espuma a base de polímero que contiene unas células de interconexión y una pluralidad de orificios transversales dispersos; y
una envoltura elastómera que comprende una primera y segunda hoja de lámina elastómera, selladas en su periferia, y que encierran dicha almohadilla de espuma, donde la primera hoja de lámina elastómera mencionada tiene una pluralidad de orificios dispersos en la misma, orificios dispersos que están descentrados y no alineados con los orificios dispersos en la citada almohadilla de espuma, donde la segunda hoja de lámina elastómera mencionada tiene una pluralidad de orificios dispersos en la misma, los cuales están alineados, en esta segunda hoja, con los orificios dispersos de la almohadilla de espuma mencionada;
una segunda almohadilla porosa, permeable a los fluidos, para colocar sobre y en contacto con dicha primera almohadilla porosa, segunda almohadilla porosa que comprende una espuma de polímero que tiene células de interconexión;
un revestimiento laminar, impermeable a los fluidos, para colocar sobre dicha segunda almohadilla y la primera almohadilla porosa colocada sobre dicha herida, comprendiendo el revestimiento laminar un perímetro adhesivo para adherir dicho revestimiento a una zona de piel que rodea dicha herida; y
un conector tubular colocado dentro de dicho revestimiento laminar y en comunicación hidráulica con la segunda almohadilla porosa, para conectar a una fuente de presión negativa e iniciar un flujo de fluidos exudados de dicha herida.
Por consiguiente una realización preferida de la invención presenta un vendaje en capas para herida, que sirve de recubrimiento temporal para una herida abierta y permite la aplicación de presión negativa para la eliminación de fluidos exudados de la herida.
Una realización preferida de la presente invención presenta un vendaje para herida que permite la aplicación de presión negativa para eliminar los fluidos de la zona de la herida, de modo que facilite la curación de la misma y minimice la formación de adhesiones entre el vendaje de la herida y el tejido subyacente.
Otra realización preferida de la presente invención ofrece un vendaje para herida que permite la aplicación de presión negativa para la eliminación de fluidos de la zona de la herida, de modo que no promuevan la formación de adhesiones entre el vendaje de la herida y el tejido subyacente y proporcione sin embargo un medio poroso a través del cual pueden pasar los fluidos exudados sin experimentar un bloqueo excesivo que degradaría la eficacia de la presión negativa.
Otra realización de la invención ofrece un vendaje para herida abdominal, que sirve de recubrimiento temporal de una herida abierta y permite la aplicación de presión negativa para eliminar los fluidos exudados de la herida, que se puede quitar fácilmente de la zona de la herida con el objeto de permitir un acceso repetido a la cavidad abdominal para proceder a la limpieza y/o al tratamiento.
Las realizaciones de la invención pueden ofrecer también un vendaje para herida abdominal y unos métodos para su uso que permiten la aplicación de presión negativa a la zona de la herida, con el objeto de promover la curación de la zona, limitando al mismo tiempo la formación de adhesiones que impedirían la eliminación del vendaje.
Los orificios en la lámina flexible de plástico deben constituir una proporción suficientemente pequeña de la superficie de la lámina en contacto con la herida de forma que el encarnamiento del tejido fibroso dentro de la hoja sea limitado. Así, la lámina no se adhiere a la herida y sigue siendo relativamente fácil quitarla. De preferencia, los orificios en la lámina deben constituir menos del 10%, especialmente menos del 5% e incluso de 1 a 2% o menos de la superficie eficaz de la lámina. Las aberturas en forma de hendiduras o ranuras se prefieren ya que constituyen una parte muy pequeña de la superficie de la lámina.
Las hendiduras o ranuras pueden ser por lo general de 0,5 a 2 cm, p. ej. a 1,5 cm de longitud y estar alineadas en filas y separadas por espacios de 10 a 40 milésimas de pulgada, p. ej. 20 a 30 milésimas de pulgada.
La lamina flexible deberá ser lo suficientemente rígida para conservar su tamaño. Un material adecuado es el PVC plastificado de 30 a 80 micras de espesor, p. ej. 40 a 60 micras.
En la práctica, la lámina se corta a un tamaño de modo que se solape con el borde de la herida. Se corta una espuma de polímero absorbente, p. ej. una espuma de poliuretano reticulada a un tamaño tal que encaja dentro de los límites de la herida. La almohadilla de espuma cortada se coloca sobre la lámina cortada, de modo que los bordes de la lamina se extiendan más allá del perímetro de la espuma, p. ej. unos 20 a 80 mm, de preferencia 40 a 60 mm, por lo general en torno a 50 mm. Se coloca un revestimiento quirúrgico de material laminar elastómero sobre la almohadilla de espuma y la lámina flexible de plástico, con el fin de encapsular y proporcionar un cierre hermético al aire sobre el vendaje de la herida. Se dispone un tubo de succión o conector que se extiende por el revestimiento quirúrgico hasta el interior de la espuma de polímero o muy cerca de la misma. El tubo de succión o conector se sella con el revestimiento quirúrgico, de modo que el espacio por debajo del revestimiento se puede mantener a una presión inferior a la atmosférica conectando el tubo de succión o conector a una fuente de presión negativa. En la patente GB 2329127 y la US 6,216,701, se describe un cabezal preferido para tubo de succión o conector.
Los revestimientos quirúrgicos suelen comprender una lámina elastómera fina recubierta totalmente por una cara de un adhesivo pegajoso, sensible a la presión. Por lo general, una hoja protectora tratada con un agente de desmoldeo está en contacto con la superficie adhesiva y se suele quitar justo antes de utilizarlo para dejar al descubierto la superficie adhesiva. A los efectos de la presente invención, el conjunto formado por la lámina elastómera adhesiva y la lámina protectora se cortan a su tamaño y, si es preciso, se hace un orificio para el tubo de succión o conector antes de quitar la hoja protectora.
Como láminas elastómeras adecuadas, se pueden citar el poliuretano y el polibutadieno. Los revestimientos quirúrgicos disponibles en el mercado incluyen los comercializados por la sociedad 3M bajo la marca "Tegaderm".
La espuma puede comprender varias capas. Se dispone un grifo para el tubo de succión o conector sobre la superficie superior de la espuma para conectar a una fuente de presión negativa. En lugar de utilizar una hoja de plástico flexible en contacto con la herida y una parte separada de espuma polímera, la espuma puede estar encerrada en una envoltura de lámina de plástico. En esta realización, la lámina de plástico puede ser de naturaleza elastómera y la lámina no tiene que extenderse más allá del perímetro de la espuma. No obstante, es conveniente proporcionar una brida de lámina que se extiende más allá del perímetro de la espuma, aprox. 20 a 80 mm, por lo general en torno a 40-60 mm, habitualmente en torno a 50 mm. Como materiales adecuados para la lámina elastómera se pueden citar el poliuretano; PTFE, o láminas de polibutadieno, similares a las utilizadas para revestimientos quirúrgicos. De preferencia, la lámina elastómera tiene un grosor de aprox. 30 a 70 micras, particularmente 40 a 60 micras, por lo general en torno a 50 micras. Se cortará cierto número de mechas de espuma de polímero de tamaños diferentes, se configurarán y encerrarán dentro de una envoltura de material elastómero en lámina. Puede ser conveniente soldar la lámina para formar la envoltura utilizando soldadura por ultrasonido o térmica. Por lo general, las heridas abdominales suelen tener forma elíptica y la almohadilla de espuma será también de preferencia de forma elíptica. Los tamaños habituales para las almohadillas de espuma pueden ser de aprox. 280, 320 y 380 mm para la dimensión mayor y aprox. 140, 175 y 250 mm para la dimensión menor. Resulta adecuada una espuma de poliuretano que tiene un grosor de aprox. 5 a 10 mm, p. ej. en torno a 6 mm. Se prefieren las espumas de poliuretano que tienen un tamaño de poro de aprox. 12 a 20, p. ej. 16 poros por cm, (30 a 50, p. ej. 40 poros por pulgada). Se pueden utilizar alternativamente espumas de poliéter o de alcohol de polivinilo. Si se utilizan múltiples capas de espuma, se pueden usa espumas diferentes en las diferentes capas. Por ejemplo, la capa en contacto con la lámina inferior puede ser de espuma de alcohol de polivinilo y la o las capas de espuma superiores pueden ser de poliuretano.
En la realización multicapa, por lo menos una capa inferior de espuma puede estar encerrada en material elastómero y colocada en contacto directo con el tejido dentro de la herida abierta. En otra realización, solo se utiliza una capa de espuma, y la envoltura de material elastómero que contiene la capa de espuma se coloca en contacto directo con la herida abierta.
Los fluidos son extraídos por presión negativa a través de los orificios en el material flexible o elastómero y luego a través de la espuma. En la realización multicapa, los fluidos son extraídos por presión negativa a través de los orificios situados en la envoltura elastómera a través de la/las capas inferiores de espuma, nuevamente a través de unos orificios en la envoltura elastómera y luego a través de la capa superior de espuma situada sobre la o las capas inferiores encerradas.
En la realización multicapa, la o las capas inferiores de espuma pueden ser de porosidad más fina, mientras que la capa superior de espuma puede ser de porosidad más gruesa. En todas las realizaciones descritas de la presente invención, una hoja elastómera adhesiva, p. ej. un revestimiento quirúrgico, cubre todo el vendaje de la herida y sella los bordes a la piel alrededor de la herida. Se acopla un dispositivo de vacío adecuado al grifo del tubo de succión o conector para proporcionar presión negativa terapéutica.
La figura 1 es una vista en sección del dispositivo de una primera realización de la presente invención, que muestra la utilización de una combinación de una sola capa de espuma dentro de una envoltura y un sobrepuesto de una segunda capa de espuma.
La figura 2 es una vista en sección de una segunda realización de la presente invención similar a la figura 1, pero que muestra la utilización de una doble capa de espuma dentro de una envoltura.
La figura 3 es una tercera realización que comprende una hoja flexible y una capa de espuma sobre la misma.
Un vendaje para heridas según la presente invención puede permitir su uso junto con técnicas quirúrgicas que requieren un acceso repetido a la cavidad abdominal tras la cirugía. La presente invención está pensada para proporcionar un vendaje separable y beneficioso que facilita tanto la curación de la herida como su cierre final.
Se hace referencia a la solicitud de patente US 08/951.832, así como a la US 09/078,223 y a la patente GB 2307180 (WO 94/20041 y WO 97/18007) para la descripción de un sistema de presión negativa adecuado para aplicar el vacío necesario para utilizar las estructuras de vendaje y el método de tratamiento aquí descrito. La presión negativa se define aquí como un vacío de menos de una atmósfera de presión. En las referencias antes citadas también se describe el control de la presión negativa y la recogida y eliminación de fluidos exudados. La forma de utilizar estos sistemas junto con la estructura del vendaje aquí descrita será algo evidente para los expertos en la materia.
Se hace referencia a la figura 1 para una descripción de una primera realización de la presente invención, en la que se utiliza una sola capa inferior de espuma en la capa del vendaje en contacto con la herida. Tanto la figura 1 como la 2 son vistas parcialmente detalladas de un vendaje de la invención. Durante la utilización, las capas de espuma y las hojas de material elastómero que intervienen estarán en estrecho contacto y algunas veces selladas entre sí.
El vendaje completo para heridas 10 mostrado en la figura 1 comprende una capa de espuma superior 12 y una capa de espuma inferior 36. La capa de espuma inferior 36 está encerrada dentro de dos hojas de material elastómero 38 como el que se utiliza para las membranas de los revestimientos quirúrgicos. Las hojas elastómeras 38 pueden ser de preferencia a base de lámina de poliuretano, que proporciona la resistencia y la elasticidad necesarias para su colocación en contacto directo con la herida. Otro material adecuado es el politetrafluoretileno (PTFE). Las hojas elastómeras 38 se sellan en su periferia, p. ej. mediante soldadura o adhesivo, de forma que rodeen la capa inferior de espuma 36. Las hojas elastómeras 38 resisten la formación de adherencias a la herida y resultan por tanto adecuadas para colocar en contacto directo con las superficies del tejido dentro y en torno a la periferia de la herida.
El hecho de colocar este material laminar no poroso en contacto con la abertura de la herida, en lugar de espuma celular abierta o material de malla reduce enormemente la aparición de adhesiones que, con el tiempo, harían que la eliminación del vendaje resultase difícil o peligrosa. La envoltura de la capa inferior flexible se inserta fácilmente sobre y/o dentro de la herida. La envoltura de la capa inferior puede, si se desea, colocarse sobre la herida, en cuyo caso los bordes de la capa están en contacto con la piel que rodea la herida o, de preferencia, dentro de la herida dejando la superficie superior de la envoltura de la capa inferior al descubierto sobre la herida.
Todo el vendaje 10 tiene un perfil generalmente plano según se muestra en sección en las figuras 2 y 3. La forma del vendaje puede variar notablemente según el tamaño y forma de la herida a la que se tenga que aplicar. Una forma rectangular o cuadrada con dimensiones del orden de 20 cm por 20 cm (8 pulgadas por 8 pulgadas) hasta 45 cm por 45 cm (18 pulgadas por 18 pulgadas) puede ser la adecuada para utilizar junto con las heridas abdominales descritas anteriormente. Otras dimensiones y formas, como circular, oval o incluso oblonga pueden resultar adecuadas para otros tipos de heridas. Las dimensiones habituales para grosores y dimensiones planas de mechas de espuma elípticas u ovaladas son las que se han dado antes. Esta estructura básica se puede aplicar fácilmente con cualquiera de estas formas o tamaños o incluso con otros diferentes.
Las hojas elastómeras 38 presentan una pluralidad de orificios 32 y 34 en las paredes superior e inferior 28 y 30 de la envoltura que definen. Estos orificios pueden ser del orden de 3 mm (1/8 pulgadas) a 6 mm (1/4 pulgadas) de longitud, diámetro o dimensión máxima y pueden dispersarse por la superficie de la envoltura definida en centros de 25 a 75 mm (1 pulgada a 3 pulgadas). De preferencia, los orificios tienen forma de hendidura o ranura. Las posiciones de los orificios pueden asociarse con orificios situados en el interior o por dentro de la capa de espuma inferior 36, tal como se describe con más detalle en lo que sigue. Si se han dispuesto orificios en la espuma, pueden ser de tamaños y formas diferentes.
La capa de espuma inferior 36 es de preferencia a base de poliuretano con un elevado grado de reticulación y buena permeabilidad bajo aspiración. Las espumas que tienen por lo menos 90% y de preferencia 95% de células de interconexión son las preferidas. De preferencia, las espumas tienen de 8 a 24 poros por cm (20 a 60 poros por pulgada). De preferencia, las espumas de poliuretano se preparan haciendo reaccionar un poliéterpoliolhidrófilo con un tolileno diisocianato en presencia de agua. Se libera en la reacción dióxido de carbono que proporciona un agente soplador gaseoso. El tamaño del poro se puede controlar agitando y mediante la temperatura. La reticulación se potencia manteniendo la espuma en la cámara en la que dicha espuma se forma inicialmente y haciendo que el agente de soplado se siga expandiendo y rompiendo las paredes de la célula.
Según se una indicado anteriormente, la capa de espuma inferior 36 presenta una pluralidad de orificios que atraviesan totalmente la capa desde su superficie superior hasta la inferior. Como se podrá ver por su colocación con respecto a los orificios 32 y 34 en la envoltura elastómera, los orificios en la capa de espuma inferior 36 están dispuestos en forma de "sumideros" dentro de los cuales se puede llevar los fluidos exudados procedentes de la herida. Los orificios 34 en la superficie inferior de la envoltura elastómera 38 pueden colocarse directamente sobre los orificios de la capa de espuma 36.
Los orificios 32 en la pared superior 28 de la de la envoltura elastómera 38 están situados de preferencia de forma que no estén alineados con los orificios en la capa de espuma 36 y los orificios en la superficie inferior 30 de la envoltura 38. De este modo, se retiran fluidos de la herida (mediante una presión negativa adecuada) y se llevan al interior de los orificios 34 situados en la superficie inferior 30 de la envoltura 38 y hacia el interior de los agujeros "sumideros" en la capa de espuma inferior 36. Los fluidos son arrastrados entonces al interior de las células de la capa de espuma inferior 36 donde se dispersan antes de ser extraídos nuevamente por unos orificios situados en la envoltura elastómera 38, esta vez en la pared superior 28 de la envoltura 38. La colocación de la envoltura/capa de espuma inferior sobre o dentro de la herida, requiere por tanto una orientación adecuada de los orificios, según se ha mostrado.
Uno de los problemas que se encuentra en el estado de la técnica consiste en el deterioro del flujo causado por la obstrucción del medio a través del cual deben circular los fluidos exudados y los residuos acompañantes. Estos flujos son arrastrados hacia la fuente de la presión negativa y se atascan por lo tanto más rápidamente a lo largo del tramo directo que conduce a la fuente de vacío, en este caso, el tubo de aspiración. La presente invención dispersa el vacío con el objeto de deslocalizar el flujo. Las capas de espuma y los orificios espaciados en las hojas elastómeras producen esta dispersión. Incluso sin una capa intercalada de espuma, los orificios descentrados cumplen este propósito. Las flechas oscuras en las figuras 1 y 2 muestran el flujo de fluido exudado a través del vendaje.
La capa de espuma superior 12 proporciona el mecanismo inicial para dispersar el vacío que arrastra los fluidos hacia arriba desde la herida. En la realización actualmente preferida, que se muestra en la figura 1, la capa de espuma superior 12 es una espuma de poliuretano o a base de poliéster más basta que tiene una mayor rigidez que la capa inferior de espuma de poliuretano. Por ejemplo, la capa inferior de espuma 36 puede tener de 1,5 a 2 veces más poros por pulgada que la capa de espuma más basta. Por lo general, la capa basta de espuma puede tener de 20 a 30 poros por pulgada. La capa de espuma superior 32 deberá ser en cierto modo menos flexible con el objeto de mantener su forma bajo la influencia de una presión negativa incluso relativamente intensa. La capa superior de espuma 12 se coloca en contacto directo con la pared superior 28 de la envoltura elastómera 38 que contiene la capa inferior de espuma 36. La capa superior de espuma de cobertura 12 y el resto del vendaje es un revestimiento elastómero 14 que, en la realización preferida, está hecho a base del mismo tipo de lámina de poliuretano que la envoltura elastómera 38.
El revestimiento 14 es una sola hoja cortada suficientemente grande para cubrir totalmente la capa superior de espuma 12 y una superficie adicional de piel que rodee la zona de la herida. Se reviste un adhesivo sensible a la presión, como un adhesivo acrílico permanentemente pegajoso, sobre por lo menos la periferie del revestimiento 14 para crear un sello impermeable al aire contra la piel, que cierra de forma eficaz el vendaje y la herida que se encuentra debajo del mismo.
El conector 16 se coloca sobre un orificio cortado en el revestimiento 14 y se sujeta al mismo utilizando un parche de sellado 22. El parche de sellado 22, que es circular en la realización preferida, se adhiere a una parte inferior de la brida 20 del conecto 16 así como el revestimiento 14. De este modo, se obtiene una presión negativa en la capa superior de espuma 12 cuando se acopla un tubo de aspiración adecuado al conector de tubo 18 colocado sobre el conector 16. La parte de brida 20 puede tener una superficie inferior que presenta canales para que pueda fluir directamente el fluido procedente de la almohadilla porosa hacia la parte tubular 18 del conector 16. En los documentos GB 2329127 y US 6,216,701 (WO 99/13793) se muestra un conector adecuado del cabezal de aspiración.
Otra realización de la presente invención se presenta en la figura 3. En esta realización, los principios de funcionamiento son idénticos, pero el vendaje está formado por una sola capa de espuma, en cuyo interior se prescinde de la lámina no porosa.
La espuma utilizada es a base de poliuretano de células abiertas, de color negro, comercializado actualmente en el producto V.A.C. del cesionario.
Con referencia a la figura 3, se coloca una hoja flexible 38 de material plástico semirígido, p. ej. a base de PVC plastificado, en contacto con la herida, de forma que los bordes recortados se alineen a lo largo de las paredes de la herida. La hoja presenta unos orificios 34, de preferencia hendiduras o ranuras estrechas. Se coloca una espuma de plástico reticulado sobre la hoja 38. Un conector de cabezal de aspiración 16 se coloca con su parte de brida 20 en contacto con la superficie superior de la almohadilla de espuma y se pone un revestimiento quirúrgico 14 sobre la espuma y la hoja 38 para sellar el vendaje en la herida.
En lugar de proporcionar un parche separado de sellado 22 para sellar el conector al vendaje, el parche puede formar parte del revestimiento quirúrgico, tal como se describe e ilustra en los documentos GB 2329127 y US 6,216,701.
La estructura del vendaje de la presente invención se pudo aplicar en cierto número de circunstancias junto con una variedad de tipos de heridas diferentes. En el caso de una herida abdominal abierta, como la que se presenta en la cirugía, la capa de espuma inferior contenida dentro de la envoltura elastómera estaría colocada dentro de la herida con la periferia extendiéndose posiblemente por debajo de la fascia y el peritoneo de la pared abdominal. La capa de espuma superior se cortaría entonces a un tamaño adecuado para colocar sobre la superficie superior descubierta de la envoltura de la capa inferior. El revestimiento con el grifo colocado adecuadamente se puede poner sobre la capa de espuma superior y la herida como un todo, extendiéndose dos pulgadas o más por encima de la piel que rodea la herida y manteniendo de forma adhesiva el vendaje en su sitio. La aplicación de una tubería de aspiración, según lo descrito anteriormente, hace que todo el vendaje sea aplicado sobre la herida y sirve par iniciar el flujo de efluente a través del vendaje.
El grosor de las capas de espuma superior e inferior queda determinado en parte por un deseo de tener un perfil plano del vendaje colocado. La capa de espuma superior más rígida puede tener, en la realización preferida, un grosor del orden de 25 a 75 mm (1 pulgada a 3 pulgadas) sin comprimir. Su rigidez deberá limitar la compresión a 80%-90% bajo las presiones negativas normalmente asociadas con la aspiración de la herida y similares. La capa de espuma inferior más flexible, si se utiliza, deberá tener un grosor del orden de 5 a 15 mm, p. ej. de 5 a 10 mm (aprox. un 1/4 a 3/4 de pulgada) para poderse insertar fácilmente dentro de la herida.
La estructura de la presente invención se puede utilizar de forma similar sobre la superficie de la piel cuando la herida abierta no está asociada con una cavidad interna o similar. En tal caso, la periferia de la envoltura inferior elastómera puede estar en contacto con la piel que rodea la herida. Esto resulta adecuado mientras la periferia del revestimiento que cubre la capa de espuma superior se extiende adecuadamente más allá de la envoltura inferior para adherirse a la piel y sellar el vendaje. Es evidente por tanto, que la envoltura elastómera inferior puede fabricarse previamente en una variedad de tamaños y formas, mientras que la capa de espuma superior y el revestimiento se pueden cortar en el momento en que se coloca el vendaje sobre la herida, al tamaño y a la configuración adecuados.
La figura 2 muestra una realización preferida alternativa de la presente invención, en la que la única capa inferior de espuma mostrada en la figura 1 se sustituye por una doble capa de espuma. En esta realización, la envoltura elastómera 38 encierra una primera capa de espuma inferior 40 y una segunda capa de espuma inferior 42. La ventaja que se obtiene con esta disposición reside en la posición descentrada de los orificios dispersados en ambas capas de espuma. Los orificios 34 que presenta la pared inferior 30 de la envoltura elastómera 38 permanecen alineados con los orificios "sumideros" situados en la segunda capa (inferior) de espuma 42. Los orificios 32 de la pared superior 28 de la envoltura elastómera 38 están, según se describe anteriormente, descentrados y no alineados con los orificios 38 de la superficie inferior de la envoltura, aunque están alineados con los orificios "sumideros" colocados en la primera capa (superior) de espuma 40. Con esta estructura, se produce una dispersión interior del flujo de fluido con una reducción consiguiente de localización de residuos de obstrucción y la prolongación de la vida útil del vendaje. Los componentes restantes, aplicaciones y métodos de uso del vendaje son los descritos anteriormente en relación con la figura 1.
La invención se ha descrito en relación con ciertas realizaciones preferidas, Los expertos en la materia podrán prever realizaciones alternativas y otras aplicaciones de la invención sin apartarse de los conceptos y principios básicos de la invención. Algunos materiales se han descrito como adecuados para las capas específicas de espuma y lámina, aunque también resultarían adecuados toda una serie de materiales similares. Ciertas geometrías y dimensiones han sido descritas y se consideran adecuadas bajo diversas condiciones y con diversas aplicaciones del vendaje, especialmente en relación con heridas abdominales abiertas y a las que se puede acceder ulteriormente. Estas geometrías y dimensiones no pretenden limitar sino que se describen como representativas de las diversas formas y tamaños posibles utilizando la estructura fundamental en capas del vendaje. El ámbito de la invención no queda por tanto limitado por la descripción anterior y queda definido de forma óptima por las reivindicaciones específicas que siguen.

2. Cierre (10) según la reivindicación 1, en el que los orificios (32, 34) en la lámina flexible de plástico (28) comprenden unas hendiduras o ranuras.
3. Cierre (10) según la reivindicación 1 o 2, en el que la lámina flexible de plástico (38) es un material laminar elastómero.
4. Cierre (10) según cualquiera de las reivindicaciones anteriores, en el que el dispositivo de conexión (18) comprende una parte de brida (20) dispuesta entre el revestimiento laminar (14) y la almohadilla porosa (12) y una parte tubular (16) que se extiende por el revestimiento (14) para conectar a la fuente de presión negativa.
5. Cierre (10) según la reivindicación 4, en el que la parte de brida (20) del dispositivo de conexión (18) comprende unos canales para dirigir el flujo de fluido desde la almohadilla porosa (12) a la parte tubular (16).
6. Cierre (10) según cualquiera de las reivindicaciones anteriores, en el que se ha dispuesto una segunda hoja (28) de lámina flexible (38) entre el revestimiento laminar (14) y la almohadilla porosa (12), estando esta segunda hoja (28) de lámina flexible (38) adherida a la primera hoja (30) de lámina flexible (32) en torno a sus perímetros respectivos para formar una envoltura y la segunda hoja (28) de lámina flexible (38) presenta unos orificios (22) de modo que cuando se conecta el dispositivo de conexión (18) a una fuente de presión negativa, se extrae exudado de la herida a través de la almohadilla porosa (36) y sale de la almohadilla (36) a través de los orificios (32) en la segunda hoja flexible (28).
7. Cierre (10) según la reivindicación 6, en el que los perímetros de las láminas flexibles (28, 30) están sellados juntos para formar una brida de lámina unida que se extiende hacia el exterior, más allá del perímetro de la almohadilla porosa (26).
8. Cierre (10) según la reivindicación 6 o 7, en el que los orificios (34) en la primera hoja flexible (30) están descentrados con respecto a los orificios (32) en la segunda lámina flexible (28).
9. Cierre (10) según cualquiera de las reivindicaciones 6 a 8, en el que los orificios (34) en la primera hoja flexible (30) están alineados con unos orificios dentro de una primera parte contigua de la almohadilla porosa (33) y los orificios (32) en la segunda hoja flexible (28) están alineados con unos orificios en una segunda parte contigua de la almohadilla porosa (36).
10. Cierre (10) según la reivindicación 9, en el que la almohadilla porosa (36) comprende dos o más capas (40, 42) de espuma polímera que tienen células de interconexión.
11. Cierre (10) según cualquiera de las reivindicaciones anteriores, en el que la almohadilla porosa (40, 42) comprende una espuma polímera que tiene al menos 90% de células de interconexión.
12. Cierre (10) según cualquiera de las reivindicaciones 6 a 11, en el que se ha dispuesto una almohadilla porosa adicional (12) entre la segunda hoja (28) de lámina flexible (38) y el revestimiento laminar (14).
