Abstract
Sistema quirúrgico oftálmico que comprende: un dispositivo de consola de control quirúrgico oftálmico (110) que comprende: un receptor (112); un identificador (102); un controlador (115); una memoria (114), almacenando la memoria unos criterios (116); un componente (100) que va a utilizarse con el dispositivo de consola de control quirúrgico oftálmico (110), comprendiendo el componente: un identificador (102) que incluye unos datos (104, 300, 400), siendo los datos (1125) transmisibles al receptor (112) con el fin de que el componente pueda ser identificado (1130) por el controlador; en el que el controlador (115) está adaptado para determinar si el componente correspondiente a los datos recibidos puede utilizarse con el dispositivo quirúrgico oftálmico por coincidencia (1135) de los datos recibidos con los criterios almacenados (1115) en la memoria, y, si no hay coincidencia, deshabilitar (1140) el componente (100) y, si hay coincidencia, habilitar (1150) el componente para que trabaje con el dispositivo de consola de control quirúrgico (110), en el que el receptor (112) es un transceptor, estando adaptado el transceptor para leer unos datos (104) del identificador del componente y para escribir unos datos (1000) en el identificador (102) del componente (100) relativos a una fecha de utilización, un número de utilizaciones del componente, una duración de utilización del componente o un ajuste de potencia del componente, y en el que el controlador (115) del dispositivo de consola de control quirúrgico oftálmico (110) está adaptado para determinar (1135) si los datos leídos del identificador satisfacen ciertos criterios para habilitar (1155, 1175) condicionalmente una utilización posterior del componente para que trabaje con el dispositivo de consola de control quirúrgico (110), en el que los datos del identificador incluyen unos códigos autorizados (522) que están dispuestos en serie, de modo que un componente (100) se asocie con una tanda o lote (810) de componentes, estando definida la tanda o lote por un componente asociado con un grupo de códigos autorizados en serie (800), y caracterizado porque los datos del identificador (102) incluyen 14 bites, utilizando 2 bites (300) para identificar el componente y utilizando 12 bites (400) para identificar 1012 códigos disponibles, incluyendo códigos autorizados (520) y no autorizados (522).
Description
La presente invención se refiere a dispositivos quirúrgicos oftálmicos y, más particularmente, a la utilización de identificadores electrónicos sobre componentes de dispositivos quirúrgicos oftálmicos que pueden utilizarse para transmitir y recibir datos relacionados con los componentes y la utilización de los mismos con el dispositivo quirúrgico oftálmico.
Los diversos dispositivos quirúrgicos funcionan utilizando componentes que se descartan o se sustituyen por diversas razones. Por ejemplo, pueden descartarse componentes debido a que se contaminan o debido a que han excedido una cierta vida útil. Así, a diferencia del equipo quirúrgico permanente, algunos componentes se utilizan una vez o un número limitado de veces y deben sustituirse después por seguridad y/u otras consideraciones. Las determinaciones referidas a si un componente particular es adecuado para un dispositivo quirúrgico particular se hacen típicamente por un técnico o por un cirujano, por ejemplo sobre la base de si el componente encaja dentro del dispositivo quirúrgico o se sujeta a éste y de las especificaciones de funcionamiento del componente y el dispositivo. Otros aspectos relacionados con la utilización de componentes particulares se dejan también a la discreción del usuario, incluyendo la activación del dispositivo quirúrgico, la calibración del dispositivo para funcionar con un componente particular y la manera en la que el dispositivo funciona con un componente particular. Así, la selección de componentes y el funcionamiento del dispositivo se someten frecuentemente a la discreción del usuario.
Sin embargo, los sistemas y técnicas conocidos para emparejar dispositivos y componentes quirúrgicos pueden ser propensos a error y requerir tiempo y esfuerzos adicionales para asegurar que el componente correcto se sujete al dispositivo quirúrgico correcto. Además, hay problemas asociados a componentes fabricados por terceras partes para sustituir componentes quirúrgicos originales. Los componentes de repuesto de terceras partes pueden tener una serie de inconvenientes. Por ejemplo, pueden ser componentes de menor calidad en comparación con componentes de fabricantes originales. Asimismo, pueden no funcionar como se pretendía y ser menos fiables que los componentes del fabricante original, provocando así preocupaciones de funcionamiento y seguridad. Adicionalmente, los fabricantes del equipo original experimentan una competencia incrementada de terceras partes que fabrican y venden componentes que están destinados a sustituir el equipo original.
Ha habido intentos de abordar algunas de estas preocupaciones, pero sólo con tipos específicos de equipos médicos y con efectividad limitada. Un enfoque ha sido utilizar sistemas de Identificación de Radiofrecuencia (RFID). Los sistemas RFID son bien conocidos y utilizan etiquetas o transpondedores electrónicos para almacenar datos. Algunos sistemas RFID utilizan etiquetas pasivas que se activan cuando se llevan a la proximidad de una señal de radio transmitida, mientras que otros sistemas RFID utilizan etiquetas activas que incluyen una fuente de potencia independiente para funcionar de manera independiente.
Se han utilizados etiquetas (dispositivos) RFID con tipos específicos de equipamiento médico, pero, según el conocimiento de la solicitante, no con dispositivos quirúrgicos oftálmicos. Por ejemplo, un sistema conocido utiliza dispositivos RFID en conexión con componentes de fibra óptica desechables de un sistema láser médico en el que se insertan cordones de fibra óptica en el cuerpo. Los cordones se exponen a fluidos corporales y deben desecharse después de cada utilización o desinfectarse completamente. Otros sistemas conocidos utilizan dispositivos RFID con catéteres que se insertan en el sistema vascular y se dirigen al corazón. Sin embargo, estos sistemas conocidos utilizan dispositivos RFID para dispositivos quirúrgicos particulares y proporcionan una funcionalidad limitada para identificar componentes de repuesto que no son componentes del equipo original.
Otros sistemas conocidos se utilizan para rastrear implementos quirúrgicos. Un sistema sensor registra el tiempo en que se entrega/usa cada implemento quirúrgico. Cuando se ha utilizado el implemento quirúrgico, se le pone sobre el sensor o cerca de éste y se registra la información de entrada. Este sistema se utiliza para rastrear implementos quirúrgicos durante una intervención y para asegurar que no se dejen inadvertidamente implementos médicos dentro de un paciente.
Así, pueden mejorarse los sistemas y técnicas conocidos para identificar un equipo quirúrgico, en particular un equipo quirúrgico oftálmico. Según el conocimiento de la solicitante, la tecnología RFID tiene que aplicarse efectivamente todavía a sistemas y componentes de cirugía oftálmicos. Además, la utilización de etiquetas de RFID en otros dispositivos médicos se limita típicamente a las funciones de identificación básicas y a la habilitación o deshabilitación del equipo. De este modo, los sistemas conocidos no proporcionan otros datos más útiles que conciernen al componente y su funcionalidad y que puede ayudar a los cirujanos cuando se utiliza el equipo. Tal información puede incluir, por ejemplo, datos de calibración y datos relacionados con el historial del componente. En consecuencia, puede mejorarse la manera en la que se utilizan los componentes de los sistemas y los equipos quirúrgicos oftálmicos, incluyendo la integración de dispositivos de transmisión de datos, tales como dispositivos RFID, para realizar identificación y otras funciones que no son proporcionadas por sistemas conocidos, y proporcionando la capacidad de facilitar a los cirujanos información adicional relativa a los componentes.
El documento US-A-2004/0220602 describe un sistema que incluye las siguientes características. Los datos que se programan en lote sobre la pieza de mano son utilizados por la consola de control a la que está conectada la pieza de mano para regular la actuación del motor de la pieza de mano. Los datos escritos en la pieza de mano indican cosas tales como: el tipo de accesorio de corte; las velocidades de funcionamiento inicial, mínima preferida y máxima preferida del accesorio de corte; los códigos de autorización verificados por la consola de control con el fin de determinar si es apropiado o no activar el motor; un tiempo de vida de funcionamiento sugerido para el accesorio de corte; el tipo de accesorio de corte; y el lote de producción en el que se fabricó el accesorio de corte. Los datos anteriores son idénticos para cada accesorio de corte fabricado en único lote de producción.
Los documentos EP-A-1.199.046 y US-A-5.400.267 (sobre los cuales se basa la forma en dos partes) describen también un sistema que utiliza datos codificados en el dispositivo accesorio para simplemente autenticar/aprobar o denegar la utilización del dispositivo después de la verificación por la consola de control.
La reivindicación 1 define un sistema quirúrgico oftálmico. La reivindicación 14 se refiere a un procedimiento de identificar un componente que puede utilizarse con un dispositivo quirúrgico oftálmico.
En las reivindicaciones subordinadas se definen formas de realización preferidas.

En la siguiente descripción se hace referencia a los dibujos que se acompañan, que forman una parte de la misma y que muestran formas de realización específicas a título ilustrativo. Debe entenderse que pueden realizarse cambios estructurales sin apartarse, por ello, del alcance de las formas de realización.
Las formas de realización del sistema y del procedimiento proporcionan identificadores electrónicos que se integran en un equipo quirúrgico oftálmico. Los identificadores electrónicos pueden realizar diversas funciones, incluyendo identificar un componente o adaptación que puede utilizarse con un dispositivo quirúrgico oftálmico, habilitar y deshabilitar selectivamente el equipo que se utiliza con componentes, identificar componentes no autorizados, tales como componentes que no son compatibles con un dispositivo y componentes de terceras partes fabricantes, proporcionar datos de calibración y proporcionar aplicaciones de lectura/escritura entre un dispositivo quirúrgico oftálmico y un componente utilizado con el dispositivo. Así, las formas de realización permiten determinaciones más precisas referentes a si ciertos componentes o adaptaciones son compatibles con ciertos dispositivos quirúrgicos. Además, las formas de realización proporcionan una funcionalidad mejorada, mejorando así la efectividad y la seguridad de las intervenciones quirúrgicas oftálmicas al asegurar que sólo se utilicen componentes adecuados con el dispositivo. Adicionalmente, las formas de realización permiten que los fabricantes del equipo quirúrgico original conserven un control más estrecho sobre su inventario y la utilización de componentes originales y válidos, proporcionando así prestaciones mejoradas, cumplimiento regulatorio y seguimiento de productos. Estos y otros aspectos de las formas de realización se discuten con más detalle a continuación.
Haciendo referencia a la figura 1, según una forma de realización, un sistema “S” incluye un dispositivo quirúrgico oftálmico 110 y un componente quirúrgico oftálmico o adaptación 100 que se sujeta al dispositivo quirúrgico 110 y se utiliza con éste. El componente 100 se conecta al dispositivo quirúrgico 110 a través de una lumbrera o conector de entrada 120 del dispositivo quirúrgico 110 y un conector 130 del componente 100. Pueden utilizarse también comunicaciones inalámbricas entre ciertos dispositivos y componentes quirúrgicos.
Un identificador 102 está integrado en el componente 100. El identificador 102 transmite datos 104 a un receptor 112 del dispositivo quirúrgico 110. Los datos 104 del identificador pueden utilizarse para determinar si el componente 100 es un componente autorizado que puede utilizarse con el dispositivo quirúrgico 110. Más particularmente, cada componente 100 puede programarse con datos particulares 104. El dispositivo quirúrgico 110 incluye una memoria 114 que almacena criterios, tales como un conjunto de códigos autorizados o datos, o un algoritmo, fórmula u otros criterios predefinidos (generalmente “algoritmos”). La memoria puede ser un elemento de memoria que sea fácilmente accesible o un elemento de memoria que esté integrado dentro de otros componentes del sistema, dependiendo de los requisitos de seguridad.
Un controlador 115, tal como un procesador o un microcontrolador (generalmente “controlador”), se programa con software y/o hardware 117 que procesan los datos 104 recibidos del identificador 102 y los criterios 116 almacenados en la memoria 114 para determinar si el componente 100 deberá aceptarse 140 o denegarse 142 y si el dispositivo quirúrgico 110 deberá habilitarse o deshabilitarse.
En una forma de realización, el dispositivo quirúrgico 110 es una consola de láser oftálmico y el componente 100 es una sonda óptica. Como se ilustra generalmente en la figura 1, una sonda óptica 100 incluye un conector 130 para su fijación a la consola 110 a través de un conector 120 de la consola 110, un alojamiento 132 y una fibra óptica 134. La luz generada por la consola 110 es proporcionada a la sonda 100, que es manipulada por un cirujano de modo que la luz sea suministrada por la fibra óptica 134 al sitio quirúrgico.
Un ejemplo de combinación de la consola de láser 110 y la sonda óptica 100, que puede utilizarse con formas de realización de la invención, incluye el sistema de laser Eyelite, pieza no. 806550001, disponible en Alcon Laboratories Inc., 6201 South Freeway, Fort Worth, Texas 76134. Pueden utilizarse también otras consolas de láser de Alcon Laboratories. Ejemplos de componentes para conexión a una consola de láser 110, tal como el sistema de láser Eyelite, incluyen conectores de sonda de fibra óptica que se utilizan con el sistema LIO-AT Headset, tal como las piezas de conector nos. 8065741019, 8065501003 y 8065501101; una fibra que está provista del sistema LIO-AT Headset, tal como la pieza no. 8065741106; y diversos fibras de endosonda tales como las piezas nos. 8065010719, 8065010503, 8065410219, 8065010203, 8065010739, 8065010419, 8065010319, 8065010403 y 80650100404, que están disponibles también en Alcon Laboratories, Inc.
Los expertos en la materia apreciarán que pueden utilizarse algunas formas de realización con otros dispositivos quirúrgicos oftálmicos 110 y componentes o adaptaciones 100. Como ejemplo adicional, la consola de control 110 puede ser una consola de vitrectomía y los componentes 100 que se sujetan a la consola pueden incluir, pero no se limitan a ellos, una sonda de vitrectomía, unas tijeras alimentadas neumática o eléctricamente, una sonda endoiluminadora y un componente de cirugía retinal. Un ejemplo de consola de vitrectomía 110 es la pieza nº 8065741008, disponible en Alcon Laboratories, Inc. Ejemplos de componentes que pueden sujetarse a la consola de vitrectomía incluyen una sonda de vitrectomía pieza nº 8065741018, unas tijeras neumáticas pieza nº 8065808101 y un endoiluminador pieza nº 8065740264, todos los cuales están disponibles también en Alcon Laboratories, Inc. Para fines de explicación, no de limitación, esta memoria se refiere a un dispositivo quirúrgico oftálmico 110 que es una consola de láser y a un componente o adaptación 100 que es una sonda óptica que está conectada a la consola 110 y se utiliza para suministrar energía láser a un sitio quirúrgico. Sin embargo, los expertos en la materia apreciarán que pueden utilizarse también una consola de vitrectomía y diversos componentes relacionados.
Según una forma de realización, el receptor 112 en la consola 110 y el identificador 102 en la sonda 100 son componentes de Identificación de Radiofrecuencia de (RFID). Un ejemplo de sistema RFID que puede utilizarse con diversas formas de realización incluye la pieza nº MCRF355, disponible en Microchip Technology, Inc., como se describe en la hoja de especificaciones AN707, aplicaciones MCRF 355/360.
La figura 2 ilustra generalmente un ejemplo de sistema RFID 200 que puede utilizarse con algunas formas de realización. Una etiqueta RFID 210 incluye típicamente un circuito integrado (IC), tal como un circuito integrado de aplicaciones específicas (ASIC), que incluye una memoria para almacenar datos. Un transpondedor 212 es activado por una instrucción o señal 224 de radiofrecuencia (RF) procedente del lector 220, que se envía a través de la antena 226 del lector, por ejemplo en respuesta a una unidad de microcontrolador 230, y que se recibe por una antena 216 del transpondedor 212 para escribir de forma inalámbrica datos en la memoria del transpondedor 212 o leer datos de ésta.
Por ejemplo, cuando deba leerse la etiqueta RFID 210, el lector 220 emite un impulso de potencia de 134,2 KHz hacia la antena 226, que dura aproximadamente 50 ms. El campo magnético generado es recogido por la antena 216 en la etiqueta 210, que se sintoniza a la misma frecuencia. Esta energía CA recibida es rectificada y almacenada en un pequeño condensador dentro del transpondedor 212. Después de completar el impulso de potencia, el transpondedor 212 transmite de nuevo sus datos utilizando la energía almacenada en el condensador como fuente de potencia. En total, se transmiten 128 bits (incluyendo la información de detección de error) a lo largo de un periodo de 20 ms. Estos datos son recibidos por la antena 226 y decodificados por la unidad de lectora 220 y el controlador 230. El condensador se descarga después de que los datos se hayan transmitido y el transpondedor 212 se restablece y queda preparado para el siguiente ciclo de lectura.
La configuración RFID descrita anteriormente es “pasiva”, puesto que el transpondedor es motorizado por la potencia almacenada en un condensador, que es generada por la señal RF del lector. Así, un identificador de RFID pasivo está normalmente inactivo y no tiene una fuente de potencia independiente. El sistema RFID puede ser también activo si se proporciona una fuente de potencia o batería independiente. Otros detalles que conciernen a la manera en la que funcionan los sistemas RFID son bien conocidos en la técnica y, por tanto, no se discuten con más detalle en esta memoria. A título explicativo, no limitativo, esta memoria se refiere a componentes RFID que se utilizan para transmitir datos entre una consola 110 y una sonda 100. Sin embargo, los expertos en la materia reconocerán que pueden utilizarse también otros componentes de transmisor, receptor y transceptor.
Cuando se aplican componentes RFID a algunas formas de realización para proporcionar comunicaciones entre los componentes de consola de láser 110 y sonda 100, el identificador 102 en el componente de sonda 100 es una etiqueta RFID o transpondedor 210 y el receptor 112 de la consola 110 es un lector de RFID 220. La etiqueta o identificador de RFID 102 incluye la identificación y, si fuerza aplicable, otros datos relativos al componente. Por ejemplo, la consola 110 puede habilitarse si los datos 104 son aceptados 140 o deshabilitarse si los datos 104 son denegados 142. La consola 110 puede habilitarse también condicionadamente. Por ejemplo, la consola 110 puede funcionar en modo normal o, alternativamente, en un modo seguro a niveles de potencia inferiores. La consola 110 puede habilitarse también condicionadamente cuando, por ejemplo, la misma sonda 100 se haya utilizado antes demasiadas veces. El controlador 115 puede configurar la consola 110 para funcionar de una manera diferente con vistas a la seguridad y otras consideraciones, puesto que la sonda 100 puede no funcionar como se pretende debido a, por ejemplo, especificaciones incompatibles, utilización y desgaste excesivos o expiración de la sonda 100.
Más particularmente, haciendo referencia a la figura 3, los datos 104 transmitidos por la etiqueta RFID 102 incluyen un código de dos bites o datos de identificación 300. El código de dos bites puede representar números 00-99 para identificar 100 tipos diferentes de sondas 100. El controlador 115 puede aplicar los criterios 116 a los datos 300 almacenados en la memoria 114 para determinar si los datos 104 son aceptados 140 o denegados 142 y si la consola 110 deberá habilitarse o deshabilitarse.
Haciendo referencia a las figuras 4 y 5, según la invención, los datos 104 transmitidos por la etiqueta o identificador RFID 102 son un código de 14 bites. Los 14 bites están divididos en dos grupos: dos bites de datos de identificación 300 (como se discute anteriormente con respecto a la figura 3) y 12 bites de datos de seguridad 400. Los datos de seguridad 400 se seleccionan de un grupo mayor de códigos de seguridad potenciales 510. Un subgrupo de los códigos potenciales 510 se selecciona como códigos autorizados 520. Los expertos en la técnica apreciarán que los datos de seguridad 400 y los códigos autorizados 510 pueden ser numéricos, alfanuméricos o de otra representación. Los 12 bites del código de seguridad 400 proporcionan un billón de códigos de seguridad posibles 510, es decir, los números 000000000000 a 999999999999. Esto se representa en la figura 5 como (A+U) 510 o códigos autorizados

- 520 pueden seleccionarse utilizando, por ejemplo, un generador de números aleatorios u otro algoritmo o criterios de selección adecuados. El algoritmo de selección puede tener diferentes niveles de complejidad para diferentes niveles y aplicaciones de seguridad.
Los identificadores 102 de cada sonda 100 de un fabricante original se programan con uno de los códigos autorizados (A) 520. Los códigos autorizados (A) 520 satisfacen los criterios 116 almacenados en la memoria 114. Por ejemplo, si los criterios 116 son un grupo de datos autorizados que se almacenan en la memoria 114, los datos autorizados 116 pueden ser los mismos que los códigos autorizados (A) 520 o coincidir con estos, mientras que los códigos no autorizados (U) 522 no coincidirían con los criterios de datos autorizados 116. Como ejemplo adicional, si los criterios 116 son algoritmos, entonces los códigos autorizados (A) 520 satisfarían o resolverían el algoritmo, mientras que los códigos no autorizados (U) 522 no resolverían o satisfarían los criterios de algoritmo 116.
En la forma de realización en la que los criterios 116 son algoritmos, no es necesario almacenar un grupo de datos autorizados en la memoria, puesto que el algoritmo se aplica a datos de seguridad recibidos del identificador. Esto puede ser beneficioso cuando las capacidades de memoria y la seguridad son asuntos de interés. Puede programarse un algoritmo con niveles de complejidad variables para proporcionar diferentes niveles de seguridad. Además, puede almacenarse un algoritmo en la memoria de diversos componentes del sistema de modo que los criterios puedan ser difíciles de determinar. En consecuencia, aunque la figura 5 muestra los códigos autorizados (A) 520 como almacenados en la memoria, esto es representativo de los códigos autorizados que satisfacen o resuelven el algoritmo, puesto que no es necesario almacenar los códigos autorizados en la memoria cuando se utiliza un algoritmo.
Así, los datos de seguridad 400 almacenados en la etiqueta RFID 102 de una sonda 100 pueden satisfacer o no los criterios 116 almacenados en la memoria 114. En el caso de que la sonda 100 sea de un fabricante original, el identificador 102 incluye un código autorizado (A) 520 que satisfaría los criterios 116. Sin embargo, si la sonda 100 es de una tercera parte fabricante, el identificador 102 puede incluir o no un código autorizado (A) 520, como se discute con más detalle a continuación.
Cuando una sonda 100 se conecta a la consola de láser 110, la etiqueta RFID 102 transmite el código de identificación 300 de dos dígitos y el código de seguridad 400 de doce dígitos al lector RFID 112. El controlador 115 determina si el código de identificación de dos dígitos y/o el código de seguridad 400 de doce dígitos satisfacen los criterios 116 almacenados en la memoria 114 de la consola 110.
Si, por ejemplo, el código de seguridad 400 satisface los criterios 116, por ejemplo el código de seguridad 400 coincide con uno de los mil millones de códigos autorizados (A) 520 almacenados en la memoria 116 o satisface o resuelve un algoritmo, entonces la consola 110 puede ser habilitada por el controlador 115. En contraste, la consola 100 puede deshabilitarse si el código 400 no satisface los criterios 116 y es uno de los códigos no autorizados (U) 510 o un código no reconocido. Así, como se muestra en la figura 5, la sonda 100A incluye datos de seguridad autorizados 400 y, por tanto, puede utilizarse con la consola 110, mientras que la sonda 100U no puede utilizarse con la consola 110 debido a que está programada con un código no autorizado (U) 522 que no satisface los criterios

La selección de códigos autorizados 520 de un grupo suficientemente grande de códigos posibles 510 facilita la capacidad de los fabricantes de componentes de sonda originales 100 para asegurar que se utilicen solamente componentes ordinales y que sea probable que los componentes de repuesto de terceras partes bloqueen el funcionamiento de la consola 110. Esto puede realizarse incluso sin tener que utilizar técnicas de encriptado/desencriptado y el hardware y software de encriptación/desencriptación adicionales, que pueden incrementar los costes y complejidades del sistema. Por el contrario, los códigos autorizados (A) 520 pueden permanecer no encriptados y se proporciona suficiente seguridad por el hecho de que el subgrupo de códigos autorizados (A) se seleccione de un fondo suficientemente grande de códigos potenciales (A+U) 510 para que la probabilidad de que una tercera parte fabricante seleccione un código autorizado 520 sea suficientemente baja. Por ejemplo, en la forma de realización en la que se seleccionan mil millones de códigos autorizados diferentes (A) 520 de un billón de códigos potenciales (A+U) 510, el número total de códigos posibles 510 es al menos tres órdenes de magnitud mayor que el número seleccionado de códigos autorizados. En esta forma de realización, habría solamente una probabilidad de 1/1000 de que el componente de repuesto de terceras partes satisfaga los criterios 116 y sea aceptado por el controlador 115 de la consola 100, habilitando así la consola 100 para trabajar con el componente. La probabilidad puede incrementarse o reducirse ajustando el número de bites en el código de seguridad 400, el número de códigos autorizados 520 y/o el número de códigos no autorizados 522. En consecuencia, el ejemplo que implica mil millones de códigos autorizados (A) 520 y un billón de códigos potenciales totales (A+U) 510 no está destinado a ser limitativo.
Es posible que una tercera parte utilice repetidamente una sonda 100 que se haya aceptado por la consola 100, puesto que la sonda aceptada 100 incluye un código autorizado (A) 520. Una tercera parte fabricante puede comprar también un pequeño número de sondas de equipo originales y utilizar entonces los datos de seguridad 400 de esas sondas para programar otros componentes de repuesto. Sin embargo, pueden configurarse algunas formas de realización para deshabilitar la consola 100 después de un cierto número de utilizaciones o apariciones de un código de seguridad particular 400 o después de una cantidad predeterminada de tiempo tras el primer caso de un número. Por ejemplo, cuando los criterios 116 son un algoritmo, el algoritmo 116 puede configurarse o programarse para permitir un cierto número de utilizaciones o una utilización dentro de un periodo de tiempo.
Por ejemplo, haciendo referencia a la figura 6, cuando la etiqueta RFID 102 de una sonda original 100 transmite el código de seguridad 400 al lector de RFID 112, los datos de seguridad 400 coinciden con los criterios 116 o los satisfacen o resuelven. Así, la consola 110 será habilitada para funcionar con ese componente 100. Puede haber intentos de reutilizar la misma sonda, puesto que tiene un código autorizado (A) 520 que satisface los criterios 116. En algunos casos, sin embargo, pueden ser adecuados múltiples utilizaciones de una sonda 100, pero, con el fin de asegurar que se sigan directrices regulatorias y de seguridad adecuadas, es aconsejable que cada sonda autorizada 100 se utilice una sola vez. Para abordar estos asuntos, el controlador 115 puede programarse para deshabilitar la consola 110 después de un número predeterminado de utilizaciones de la misma sonda 100 o después de un número predeterminado de casos del mismo código de seguridad 400. Así, la consola 110 se habilita en circunstancias normales, pero puede deshabilitarse después de un número predeterminado de utilizaciones repetidos de la misma sonda 100.
En la forma de realización ilustrada, se habilita la consola 110 para permitir que la misma sonda 100 se utilice una segundan vez, pero se la deshabilita cuando se haga un intento de utilizar la misma sonda una tercera vez. Efectivamente, el controlador 115 puede programarse para permitir diferentes números de utilizaciones repetidas de la misma sonda 100. Como resultado, se requerirá una nueva sonda 100, puesto que la misma sonda 100 ya no puede utilizarse con la consola 110. Alternativamente, o además, el controlador 115 puede programarse para generar un aviso o mensaje visual o audible destinado al usuario, indicando que la sonda 100 se ha utilizado antes, mediante la utilización de una pantalla 600 o un altavoz 610. El controlador 115 puede configurarse también para indicar el número de utilizaciones anteriores de la sonda 100.
Como ejemplo adicional, haciendo referencia a la figura 7, es posible que la misma sonda 100 pueda utilizarse múltiples veces dentro de un periodo de tiempo predefinido. El periodo de tiempo predefinido puede ser, por ejemplo, horas, días, semanas o meses. La sonda 100 puede utilizarse una segunda vez o una vez posterior si la segunda utilización o la utilización posterior ocurren durante un periodo de tiempo predeterminado después de una utilización anterior o previa de la sonda 100. Sin embargo, si la sonda 100 se utiliza fuera de esta ventana autorizada, entonces el controlador 115 puede deshabilitar la consola 110, requiriendo así una nueva sonda 100. El controlador 115 puede programarse también para generar un aviso o un mensaje como se discute con referencia a la figura 6.
Otro ejemplo es asociar un código autorizado con una consola 110 y/o componente 100 que son fabricados o utilizados en un área geográfica, país o grupo de países predefinidos. Así, cuando los datos recibidos satisfacen los criterios 116, indicando que se satisfacen los requisitos geográficos, el controlador 115 permite que la consola 110 se utilice con el componente 100.
Haciendo referencia a la figura 8, según la invención, se disponen en serie en 800 los códigos de seguridad autorizados 520, de modo que puedan identificarse lotes o grupos particulares 810 de sondas 100 sobre la base de los códigos en serie. Esto puede ser útil si, por ejemplo, hay una retirada de un grupo de sondas 100. Las sondas 100 que deben retirarse pueden identificarse rápidamente utilizando los datos de código de seguridad en serie 800 y los lotes o grupos definidos 810 de sondas.
En otra forma de realización, haciendo referencia a la figura 9, la etiqueta RFID 102 puede incluir los datos de identificación 300 y los datos de seguridad 400, como se discute anteriormente, y, además, los datos de calibración 900 que sean necesarios para calibrar la consola 110 con el fin de que trabaje con una sonda 100 o componente particular. Por ejemplo, los datos de calibración 900 pueden ser instrucciones o ajustes de parámetros, incluyendo, por ejemplo, ajustes de calibración de láser para una sonda o adaptación particular. Cada sonda o adaptación puede requerir la utilización de una fibra óptica diferente y única que cambie el acoplamiento óptico a la sonda o adaptación y, por tanto, dé como resultado una calibración particular.
Como ejemplo adicional, cuando se utiliza una consola de vitrectomía, los datos de calibración 900 pueden incluir instrucciones o ajustes de parámetros relativos a transmisividad de la sonda de láser, puntos de presión neumática de actuación de la sonda de vitrectomía y transmisividad del endoiluminador.
Efectivamente, puede utilizarse otra calibración dependiendo del dispositivo y componente quirúrgicos oftálmicos que se utilicen. La consola 110 puede calibrarse automáticamente ella misma sobre la base de los datos de calibración recibidos 900. Alternativamente, la consola 110 puede ser calibrada por un cirujano utilizando los datos de calibración recibidos 900.
Además, los datos de identificación y/o calibración pueden utilizarse para diversas finalidades, incluyendo, por ejemplo, habilitar o deshabilitar parámetros de funcionamiento que son compatibles con el componente identificado, e invocar ciertas características de seguridad asociadas con el componente identificado. Por ejemplo, el sistema puede configurarse para limitar rangos de potencia y/o rangos de exposición dados a un tipo particular de componente. Adicionalmente, el sistema puede configurarse para determinar si están presentes accesorios de seguridad, tales como filtros, de modo que los componentes identificados puedan utilizarse sin lesionar al cirujano. Además, los datos de identificación y/o calibración pueden utilizarse para generar una interfaz de usuario que está relacionada con el componente particular y sea presentada sobre una pantalla de visualización para un cirujano.
En otra forma de realización, haciendo referencia a la figura 10, los lectores y etiquetas RFID del sistema y el componente pueden configurarse en aplicaciones de dos vías o de lectura/escritura 1000. Esta aplicación puede requerir mayores capacidades de datos de la etiqueta RFID 102, por ejemplo >100 bites. En esta realización, el lector (o transceptor) puede reescribir diferentes tipos de datos en la etiqueta RFID 102 para una utilización o referencia futuros. Por ejemplo, el lector de RFID 112 puede escribir información tal como los datos de utilización de una sonda 100, un número de veces que se utilizó una sonda 100, y la cantidad de tiempo en que la sonda 100 estaba activa, el ajuste de potencia de la sonda 100 cuando estaba activa, etc. Esta información puede ser particularmente útil cuando la sonda 100 se utiliza en un momento posterior debido, por ejemplo, a que un cirujano puede revisar la información y hacer una evaluación relativa al desgaste y la vida útil de la sonda 100. Por ejemplo, la vida útil de una sonda 100 puede reducirse cuando la sonda 100 está activa durante un periodo de tiempo más largo, se la utiliza a un ajuste de potencia más alta y se la usa múltiples veces.
Considerando la descripción de las formas de realización del sistema, un procedimiento para identificar y controlar dispositivos y componentes quirúrgicos oftálmicos se resume en los diagramas de flujo mostrados en las figuras 11A-B. Comenzando con la etapa 1100, se seleccionan un grupo de códigos de identificación potenciales y un grupo de códigos de seguridad potenciales. El grupo de códigos potenciales incluye códigos autorizados y no autorizados que satisfarán o no un grupo de criterios. En la etapa 1105, se selecciona un grupo de códigos autorizados del grupo mayor de códigos potenciales. En la etapa 1110, los identificadores o etiquetas RFID de componentes autorizados, por ejemplo componentes de fabricantes originales, se programan con uno de los códigos autorizados. En la etapa 1115, si el controlador ejecuta un criterio sobre la base de comparar datos autorizados con datos recibidos de un identificador, los códigos autorizados son almacenados en la memoria del sistema de láser oftálmico, tal como un dispositivo quirúrgico de láser oftálmico o vitreorretinal. Si se utiliza un algoritmo, el algoritmo puede almacenarse en la memoria sin almacenar datos autorizados.
En la etapa 1120, se proporciona un componente para que sea conectado al dispositivo de láser. En la etapa, los datos procedentes de la etiqueta RFID del componente se transmiten al receptor o lector de RFID en el dispositivo de láser. Los datos recibidos de la etiqueta RFID se utilizan para identificar el componente en la etapa 1130. Los datos procedentes del dispositivo RFID pueden utilizarse para diversas finalidades, incluyendo, por ejemplo, generar una interfaz de usuario para presentación sobre una pantalla de visualización, habilitar o deshabilitar parámetros de funcionamiento que son compatibles con el componente identificado, e invocar ciertas características de seguridad asociadas al componente identificado. Por ejemplo, los valores de ciertos parámetros, tales como potencia y exposición, pueden limitarse para asegurar que el dispositivo no se haga funcionar fuera de los rangos especificados de los parámetros de funcionamiento. Además, el sistema puede configurarse para comprobar el equipo de seguridad, tal como un filtro, que se utiliza con el componente.
En la etapa 1135, se procesan los datos procedentes de la etiqueta RFID para determinar si satisfacen ciertos criterios. En la etapa 1140, se deshabilita el dispositivo quirúrgico oftálmico si los datos recibidos no satisfacen los criterios. En la etapa 1145, el controlador determina si el componente se ha utilizado antes. Si el componente está siendo utilizado por primera vez, el dispositivo quirúrgico oftálmico puede habilitarse para que trabaje con el componente en la etapa 1150. Si el componente se ha utilizado antes, en la etapa 1155, puede habilitarse condicionalmente el dispositivo quirúrgico oftálmico.
Por ejemplo, el dispositivo quirúrgico oftálmico puede habilitarse si la sonda se ha utilizado previamente un cierto número de veces 1160 o se ha utilizado previamente dentro de un cierto periodo de tiempo 1165. El controlador puede generar también un mensaje o aviso 1170 para el usuario con el fin de indicar que la sonda se ha utilizado previamente o ajustar el dispositivo de láser para que funcione en un modo seguro 1175, por ejemplo a potencia más baja.
Los expertos en la materia apreciarán que existen diversas modificaciones que pueden realizarse sin apartarse, por ello, del alcance de las formas de realización. Pueden utilizarse algunas formas de realización con otros tipos de equipo quirúrgico oftálmico. Además, pueden utilizarse algunas formas de realización para diferentes finalidades, incluyendo finalidades de identificación calibración, compatibilidad y bloqueo. Así, los ejemplos ilustrativos expuestos anteriormente no están destinados a ser limitativos.
5 Aunque se ha hecho referencia en la descripción anterior a diversas formas de realización, los expertos en la materia reconocerán que pueden realizarse modificaciones, alteraciones y sustituciones poco importantes a las formas de realización descritas sin apartarse, por ello, de la invención según se reivindica en las reivindicaciones adjuntas.

Claims (36)
- REIVINDICACIONES1. Sistema quirúrgico oftálmico que comprende:un dispositivo de consola de control quirúrgico oftálmico (110) que comprende:un receptor (112);un identificador (102);un controlador (115);una memoria (114), almacenando la memoria unos criterios (116);un componente (100) que va a utilizarse con el dispositivo de consola de control quirúrgico oftálmico (110), comprendiendo el componente:un identificador (102) que incluye unos datos (104, 300, 400), siendo los datos (1125) transmisibles al receptor(112) con el fin de que el componente pueda ser identificado (1130) por el controlador;en el que el controlador (115) está adaptado para determinar si el componente correspondiente a los datos recibidos puede utilizarse con el dispositivo quirúrgico oftálmico por coincidencia (1135) de los datos recibidos con los criterios almacenados (1115) en la memoria, y, si no hay coincidencia, deshabilitar (1140) el componente (100) y, si hay coincidencia, habilitar (1150) el componente para que trabaje con el dispositivo de consola de control quirúrgico (110),en el que el receptor (112) es un transceptor, estando adaptado el transceptor para leer unos datos (104) del identificador del componente y para escribir unos datos (1000) en el identificador (102) del componente (100) relativos a una fecha de utilización, un número de utilizaciones del componente, una duración de utilización del componente o un ajuste de potencia del componente, y en el que el controlador (115) del dispositivo de consola de control quirúrgico oftálmico (110) está adaptado para determinar (1135) si los datos leídos del identificador satisfacen ciertos criterios para habilitar (1155, 1175) condicionalmente una utilización posterior del componente para que trabaje con el dispositivo de consola de control quirúrgico (110),en el que los datos del identificador incluyen unos códigos autorizados (522) que están dispuestos en serie, de modo que un componente (100) se asocie con una tanda o lote (810) de componentes, estando definida la tanda o lote por un componente asociado con un grupo de códigos autorizados en serie (800), ycaracterizado porque los datos del identificador (102) incluyen 14 bites, utilizando 2 bites (300) para identificar el componente y utilizando 12 bites (400) para identificar 1012 códigos disponibles, incluyendo códigos autorizados(520) y no autorizados (522).

- Sistema según la reivindicación 1, en el que el receptor (112) es un lector (220) de identificación de radiofrecuencia (RFID) configurado para comunicación bidireccional de lectura/escritura, y el identificador (102) es una etiqueta (210) de identificación de radiofrecuencia (RFID) con capacidad de almacenamiento de datos.

- Sistema según la reivindicación 1, en el que los datos (104) recibidos del identificador (102) están sin encriptar.

- Sistema según la reivindicación 1, en el que un número de códigos disponibles (510) es al menos tres órdenes de magnitud mayor que un número de códigos autorizados (520).

- Sistema según la reivindicación 1, en el que los códigos autorizados (520) se seleccionan aleatoriamente de entre los códigos disponibles (510).

- 6.
- Sistema según la reivindicación 1, en el que el dispositivo quirúrgico oftálmico (110) es un láser y el componente
(100) es una sonda óptica que está sujeta al láser. -
- 7.
- Sistema según la reivindicación 1, en el que el dispositivo quirúrgico oftálmico (110) es un dispositivo quirúrgico vitreorretinal y el componente (100) es cualquiera de entre una sonda de vitrectomía, unas tijeras alimentadas neumática o eléctricamente o una sonda de endoiluminador que se sujetan al dispositivo quirúrgico vitreorretinal.

- Sistema según la reivindicación 1, en el que el controlador (115) está configurado para determinar un número de veces que se ha utilizado el componente (100) sobre la base de un número de veces que se reciben los mismos datos del identificador (102).

- Sistema según la reivindicación 8, en el que, cuando el componente (100) se utiliza más de una vez y los mismos datos recibidos satisfacen los criterios (116), el controlador (115) está adaptado para deshabilitar el dispositivo quirúrgico oftálmico (110) impidiendo que sea utilizado con el componente después de un número predeterminado de utilizaciones del componente.

- Sistema según la reivindicación 8, en el que, cuando el componente (100) se usa más de una vez y los mismos datos recibidos satisfacen los criterios (116), el controlador está adaptado para generar un mensaje destinado a un usuario indicando que se ha utilizado previamente el componente.

- Sistema según la reivindicación 1, en el que el controlador (115) está adaptado para deshabilitar el dispositivo quirúrgico oftálmico (110) impidiendo que sea utilizado con el componente después de una cantidad de tiempo predeterminada tras una utilización del componente (100).

- Sistema según la reivindicación 1, en el que los datos (104) del identificador incluyen datos de calibración (900), indicando los datos de calibración la configuración del dispositivo quirúrgico oftálmico (110) para su utilización con el componente (100).

- 13.
- Sistema según la reivindicación 1, en el que los criterios (116) están asociados con un área geográfica predefinida de tal manera que, cuando los datos recibidos satisfacen los criterios, el controlador (115) está adaptado para habilitar el dispositivo quirúrgico oftálmico (110) permitiendo que sea utilizado con el componente (100) en el área geográfica predefinida.

- 14.
- Procedimiento para identificar un componente (100) que puede utilizarse con un dispositivo quirúrgico oftálmico (110), que comprende:
proporcionar un dispositivo quirúrgico oftálmico y un componente que está sujeto al dispositivo quirúrgico oftálmico;establecer unos criterios (116) y almacenar los criterios en una memoria (114) del dispositivo quirúrgico oftálmico;transmitir (1125) unos datos (104) de un identificador (102) en el componente a un receptor (112) en el dispositivo quirúrgico oftálmico;determinar (1135) si el componente correspondiente a los datos recibidos puede utilizarse con el dispositivo quirúrgico oftálmico por coincidencia (1135) de los datos recibidos con los criterios almacenados (1115) en la memoria y, si no hay coincidencia, deshabilitar (1140) el componente (100); si hay una coincidencia, habilitar (1150) el componente para que trabaje con el dispositivo de consola de control quirúrgico (110); y si hay una coincidencia, determinar (1145) además si el componente identificado satisface criterios de utilización relativos al componente identificado, y, sobre la base de dicha etapa de determinación, habilitar (1155) condicionalmente el componente para que funcione con el dispositivo de consola de control quirúrgico (110);escribir datos (1000) en el identificador (102) del componente (100), comprendiendo esto escribir una fecha de utilización, un número de utilizaciones del componente (100), una duración de utilización del componente o un ajuste de potencia del componente en el identificador (102) del componente, yen el que los datos del identificador incluyen unos códigos autorizados (522) que están dispuestos en serie, de modo que un componente (100) esté asociado con una tanda o lote (810) de componentes, definiéndose la tanda o lote por un componente asociado a un grupo de códigos autorizados serializados (800),caracterizado porque los datos del identificador (102) incluyen 14 bites, utilizando 2 bytes (300) para identificar el componente y utilizando 12 bites (400) para identificar 1012 códigos disponibles, incluyendo códigos autorizados(520) y no autorizados (522). -
- 15.
- Procedimiento según la reivindicación 14, que comprende además identificar el tipo de componente (100) sobre la base de los datos (300) del identificador.

- 16.
- Procedimiento según la reivindicación 15, que comprende además generar (1170) una interfaz de usuario para la presentación en una pantalla de visualización (600) sobre la base del componente identificado (100).

- 17.
- Procedimiento según la reivindicación 15, que comprende además habilitar (1150) parámetros de funcionamiento que sean compatibles con el componente identificado (100).
-
- 18.
- Procedimiento según la reivindicación 15, que comprende además deshabilitar (1140) parámetros de funcionamiento que sean incompatibles con el componente identificado (100).

- 19.
- Procedimiento según la reivindicación 15, que comprende además limitar un valor de un parámetro de funcionamiento (1155) sobre la base del componente identificado (100).
-
- 20.
- Procedimiento según la reivindicación 15, en el que la limitación del valor del parámetro de funcionamiento (1155) comprende limitar un valor de potencia o un rango de exposición.
-
- 21.
- Procedimiento según la reivindicación 15, que comprende además determinar la presencia de un componente de
seguridad que está asociado con el componente identificado (100). -
- 22.
- Procedimiento según la reivindicación 21, en el que la determinación de la presencia del componente de seguridad comprende determinar la presencia de un filtro.

- 23.
- Procedimiento según la reivindicación 15, en el que la transmisión (1125) de datos del identificador (102) al receptor (112) comprende transmitir datos a un lector (220) de identificación de radiofrecuencia (RFID).

- 24.
- Procedimiento según la reivindicación 23, en el que la transmisión de datos del identificador (102) comprende transmitir datos de una etiqueta RFID (210).

- 25.
- Procedimiento según la reivindicación 14, que comprende además seleccionar (1105) códigos autorizados (520) de entre un grupo mayor de códigos disponibles (510).
-
- 26.
- Procedimiento según la reivindicación 14, en el que la selección (1105) de códigos autorizados (520) comprende la selección de un número de códigos autorizados de entre un número de códigos disponibles (510) que es al menos tres órdenes de magnitud mayor que un número de códigos autorizados.

- 27.
- Procedimiento según la reivindicación 14, en el que la transmisión (1125) de datos del identificador (102) en el componente (100) al receptor (112) en el dispositivo quirúrgico oftálmico (100) comprende transmitir datos no encriptados.

- 28.
- Procedimiento según la reivindicación 14, en el que el dispositivo quirúrgico oftálmico (110) es un láser y el componente (100) es una sonda óptica que está sujeta al láser.

- 29.
- Procedimiento según la reivindicación 14, en el que el dispositivo quirúrgico oftálmico (110) es un dispositivo quirúrgico vitreorretinal y el componente (100) es cualquiera de entre una sonda de vitrectomía, unas tijeras alimentadas neumática o eléctricamente o una sonda de endoiluminador que se sujetan al dispositivo quirúrgico vitreorretinal.

- Procedimiento según la reivindicación 14, que comprende además determinar (1145) un número de veces que se ha utilizado un componente (100).

- 31.
- Procedimiento según la reivindicación 30, que comprende además deshabilitar el dispositivo quirúrgico oftálmico
(110) impidiendo que sea utilizado (1160) con el componente (100) después de un número predeterminado de utilizaciones del componente. -
- 32.
- Procedimiento según la reivindicación 30, que comprende además generar (1170) un mensaje indicando que se ha utilizado previamente el componente.

- 33.
- Procedimiento según la reivindicación 14, que comprende además determinar una duración de tiempo después de una utilización del componente (100).

- 34.
- Procedimiento según la reivindicación 33, que comprende además deshabilitar el dispositivo quirúrgico oftálmico
(110) impidiendo que sea utilizado (1165) con el componente (100) después de una cantidad de tiempo predeterminada tras una utilización del componente. -
- 35.
- Procedimiento según la reivindicación 14, en el que los datos del identificador (102) del componente incluyen unos datos de calibración (900), comprendiendo además calibrar el dispositivo quirúrgico oftálmico para su utilización con el componente sobre la base de los datos de calibración.

- 36.
- Procedimiento según la reivindicación 14, que comprende además codificar el identificador (102) en el componente (100) con datos con un código autorizado (520) que satisface los criterios (116), y transmitir (1125) datos comprendiendo la transmisión de los datos codificados del identificador en el componente al receptor (112) en el dispositivo quirúrgico oftálmico (110).
