Antecedentes de la invención
La presente invención se refiere en general a catéteres de balón y más específicamente a catéteres de balón
5 adecuados para usar en procesos de colocación de stents, perfusión, administración de fármacos, angioplastia, valvuloplastia y endarterectomía. Más particularmente, la presente invención se refiere a un catéter de balón que tiene un balón fabricado exclusivamente de metal y a un método de fabricación de balones metálicos.
El documento WO 89/05609 da a conocer un catéter de balón para la recanalización de estenosis en conductos corporales. El catéter de balón tiene una antena de microondas prevista en el interior del balón, estando dicha
10 antena conectada a un generador de microondas mediante un hilo coaxial. Por la acción de las microondas sobre un líquido de dilatación absorbente de microondas y/o un revestimiento metálico sobre la envuelta del balón, el catéter se calienta y la pared del vaso se somete a un postratamiento térmico para prevenir la restenosis.
El documento US 6231572 da a conocer varias realizaciones de un catéter de electrocirugía adaptado para cortar material corporal definiendo un conducto corporal.
15 El documento US 5704908 da a conocer un aparato de electroporación para introducir moléculas en células en una ubicación seleccionada dentro de una cavidad del cuerpo de un paciente. El aparato incluye un catéter alargado, un balón inflable, electrodos y una fuente de alimentación.
Resumen de la invención

El catéter de balón metálico de la invención consiste generalmente en un catéter que comprende un cuerpo tubular principal, un balón metálico próximo a un extremo distal del cuerpo tubular principal, un anillo central que se extiende a lo largo de un lado longitudinal completo del catéter para acomodar un hilo guía a través del mismo y un anillo de inflado adyacente al anillo central que se extiende a lo largo del eje longitudinal del cuerpo tubular principal y termina
25 en comunicación de circulación de fluido con una cámara de inflado del balón metálico. El catéter de balón metálico puede consistir en un catéter de metal integral unitario en el que el cuerpo tubular principal y el balón se fabrican de metal, o puede consistir en un cuerpo tubular principal polimérico y un balón metálico. Al igual que con los catéteres de balón convencionales, el catéter de balón metálico de la invención tiene conectores estándar para acoplar accesorios de catéter de balón convencionales.
30 El balón metálico de la invención puede adoptar una amplia variedad de geometrías, incluidas sin limitación, bobinas tubulares tales como las que se usan en procedimientos de endarterectomía o como balones de perfusión, balones bifurcados para angioplastia de bifurcaciones vasculares o para la colocación de dispositivos implantables bifurcados, y balones en ángulo que tienen un desplazamiento angular desde el eje longitudinal del catéter. Además, debido a que el balón metálico de la invención está fabricado de metal, puede ser más o menos radiopaco si el balón

Por otra parte, el balón metálico de la invención puede ser utilizado como un conductor de energía eléctrica aplicada directamente o inductivamente energizada por la aplicación externa de energía, tal como por ultrasonido o resonancia magnética. Esta propiedad conductora del balón metálico de la invención es particularmente útil en diatermia, para devolver una señal para la formación de
40 imágenes sin un medio de contraste añadido, o devolver una señal para proporcionar datos referentes al ambiente in vivo.
El balón metálico de la invención se fabrica preferiblemente de un metal biocompatible y se forma como una película de material. El balón metálico de la invención no se limita a las películas de una sola capa, sino que se puede laminar entre sí una pluralidad de películas con el fin de mejorar el material, las propiedades geométricas y/o las
45 propiedades funcionales del balón metálico resultante. Los materiales adecuados para fabricar el balón metálico de la invención se eligen por su biocompatibilidad, sus propiedades mecánicas, es decir, resistencia a la tracción, límite elástico, y su facilidad de deposición, estando incluidos, sin limitación, los siguientes: titanio, vanadio, aluminio, níquel, tántalo, circonio, cromo, plata, oro, silicio, magnesio, niobio, escandio, platino, cobalto, paladio, manganeso, molibdeno y aleaciones de los mismos, tales como aleaciones de zirconio-titanio-tantalio, nitinol y acero inoxidable.
50 El balón metálico de la invención se fabrica preferiblemente mediante técnicas de deposición en vacío. De acuerdo con la presente invención, las metodologías de deposición preferidas incluyen deposición por evaporación asistida por haz de iones y técnicas de pulverización catódica. En la deposición por evaporación asistida por haz de iones, es preferible emplear evaporación térmica por haz de electrones dual y simultánea con bombardeo iónico simultáneo del sustrato utilizando un gas inerte, tal como argón, xenón, nitrógeno o neón. El bombardeo con un gas inerte, tal
55 como iones de argón, sirve para reducir el contenido de vacío mediante el aumento de la densidad de empaquetamiento atómico en el material depositado durante la deposición. El reducido contenido de vacío en el material depositado es uno de los factores importantes que permiten que las propiedades mecánicas de ese material depositado sean similares a las propiedades del material elemental. Los índices de deposición de hasta 20 nm/s se pueden lograr usando técnicas de deposición por evaporación asistida por haz de iones.
5 Con la técnica de la pulverización catódica, es preferible emplear un blanco de pulverización catódica cilíndrico, una única fuente circunferencial que rodea concéntricamente el sustrato que es soportado en una posición coaxial dentro de la fuente. Otras geometrías de fuente, incluida la esférica, también están contempladas para revestir mejor sustratos con geometrías complejas que incluyen el balón de la invención. Procesos de deposición alternativos que pueden emplearse para formar el balón metálico de acuerdo con la presente invención son deposición por arco
10 catódico, por ablación con láser y deposición directa de haz de iones. Cuando se emplean metodologías de deposición en vacío, la estructura cristalina de la película depositada afecta a las propiedades mecánicas de la película depositada. Estas propiedades mecánicas de toda la película depositada o de una sección diferencial de la película depositada se pueden modificar mediante un tratamiento después del proceso, tal como, por ejemplo, recocido, tratamiento a alta presión o temple de gas.
15 Durante la deposición, la presión de la cámara, la presión de deposición y la presión parcial de los gases de proceso se controlan para optimizar la deposición de las especies deseadas sobre el sustrato. Como es conocido en la fabricación microelectrónica, en la nanofabricación y en la técnica de revestimiento en vacío, tanto los gases reactivos como los no reactivos son controlados y las especies gaseosas inertes o no reactivas introducidas en la cámara de deposición son típicamente argón y nitrógeno. El sustrato puede ser fijo o móvil, puede girar alrededor de
20 su eje longitudinal o moverse por un plano X-Y dentro del reactor para facilitar la deposición o modelado del material depositado sobre el sustrato. El material depositado puede depositarse como una película sólida uniforme sobre el sustrato, o modelarse de la siguiente manera (a) facilitando un modelo positivo o negativo sobre el sustrato, tal como mediante técnicas de atacado o de fotolitografía aplicadas a la superficie del sustrato para crear una imagen positiva
o negativa del modelo deseado o (b) usando una máscara o conjunto de máscaras ya sean fijas o móviles con
25 respecto al sustrato para definir el modelo aplicado al sustrato. El modelado se puede emplear para conseguir que algunas zonas del balón metálico muestren diferentes propiedades funcionales tales como proporcionar zonas plegables que permitan un plegado de perfil bajo del balón metálico para la colocación endoluminal, o diferentes propiedades geométricas del balón metálico, tales como rebajes en la superficie del balón metálico con geometrías de acoplamiento para encajar un stent. Se pueden conseguir geometrías acabadas complejas y propiedades
30 materiales del balón metálico resultante, en el contexto de la orientación espacial del modelo, en espesores de material en diferentes zonas de la película depositada, o diferencias en la estructura cristalina de la película de metal en diferentes zonas de la película de metal mediante el empleo de técnicas de deposición en vacío y un tratamiento térmico de la película de metal después del proceso.

Descripción detallada de las realizaciones preferidas
5 Con referencia en particular a las figuras 1 y 2, el catéter de balón metálico 10 de la invención consiste generalmente en un elemento de cuerpo de catéter tubular principal 12 y un balón 14 situado en un extremo distal del catéter de balón metálico 10. Un extremo proximal del catéter de balón metálico 10 (no mostrado) está provisto de accesorios convencionales para acoplarse con accesorios de control de catéter de balón convencionales. El elemento de cuerpo 12 y el balón 14 pueden fabricarse de metal y/o de metales biocompatibles, que pueden seleccionarse de
10 entre el grupo que consiste en titanio, vanadio, aluminio, níquel, tántalo, circonio, cromo, plata, oro, silicio, magnesio, niobio, escandio, platino, cobalto, paladio, manganeso, molibdeno y aleaciones de los mismos, tales como aleaciones de zirconio-titanio-tantalio, nitinol y acero inoxidable. 25 Los catéteres de balón convencionales suelen requerir un gran número de orificios de inflado 22 con el fin de cumplir los requisitos normativos gubernamentales para los tiempos de inflado y desinflado. Sin embargo, se ha encontrado en la presente invención que mediante la fabricación del balón 14 a partir de un metal biocompatible que tiene un espesor de pared de entre 0.1μ y 25μ y diámetros exteriores de inflado de entre 0,1 mm y 40 mm, se cumplen los requisitos reglamentarios para los tiempos de inflado y desinflado con un único orificio de inflado 22.
30 Si se fabrica el balón 14 de un metal biocompatible, se pueden obtener espesores de pared de entre 3μ y 12μ, mostrando el balón metálico resultante 14 tolerancia cero a una resistencia a la tracción muy alta. Otra ventaja resultante del balón metálico de la invención 14 es que ciertos metales, tales como el nitinol, muestran propiedades lubricantes de superficie que hace innecesario el uso de lubricantes de superficie como los que se encuentran en balones poliméricos convencionales. Además, en la realización en la que el balón metálico de la invención está
35 hecho de un material superelástico tal como nitinol, el balón metálico puede fabricarse de manera que la configuración de perfil bajo esté asociada con el estado de deformación más bajo del balón de manera que después del inflado, el balón vuelva a adoptar la configuración de perfil bajo en virtud de sus propias propiedades superelásticas. En la realización en la que el balón metálico de la invención está hecho de un material con memoria de forma tal como nitinol, el balón metálico puede fabricarse de manera que la configuración de perfil bajo esté
40 asociada al estado de alta temperatura y deformación más baja del balón de manera que después del inflado, el balón vuelva a adoptar la configuración de perfil bajo al aplicar calor.
Volviendo a la figura 3, se ilustra una realización de administración de fármaco 30 del catéter de balón metálico de la invención. El catéter de balón metálico de administración de fármaco 30 de la invención consiste generalmente en un elemento de cuerpo de catéter 32 generalmente tubular que define un lumen de inflado 33 y se comunica con al 45 menos un orificio de inflado 34, un primer balón metálico 36 y un segundo balón metálico 38 con una relación concéntrica espaciada coaxial entre ellos, y un lumen anular 42 entre el primer balón metálico 36 y el segundo balón metálico 38, que está en comunicación de circulación de fluido con un lumen introductorio 46. El segundo balón metálico 38 tiene una pluralidad de poros 40 que pasan a través del mismo, que están en comunicación de circulación de fluido con el lumen anular 42. El primer balón metálico 36 tiene un espesor de pared sólido. Un agente 50 bioactivo, tal como un medicamento farmacéutico, se introduce en el lumen introductorio 46 y pasa al lumen anular
42. El número y el tamaño de la pluralidad de poros 40 son tales que el agente bioactivo y su portador no pasarán a través de los poros 40, excepto por la influencia de una presión positiva. Un fluido, tal como una solución salina, se introduce en el lumen de inflado 44 a través de la lumen de inflado 33, y ejerce una presión positiva en el primer balón 36 que comunica esa presión positiva a cualquier agente bioactivo presente en el lumen anular 42 y en el
55 segundo balón metálico 38, y causa la dilatación del primer balón metálico 36 y del segundo balón metálico 38 y fuerza al agente bioactivo en el lumen anular 42 para que pase a través de la pluralidad de poros 40 al segundo balón metálico 38.
Un catéter de balón metálico de perfusión 50 se ilustra en la figura 4. El catéter de balón metálico de perfusión 50 de la invención consta generalmente de un elemento de cuerpo de catéter 54 y un balón metálico 52 que tiene una pluralidad de orificios de perfusión 56 que atraviesan el balón metálico. Al igual que con catéteres de perfusión convencionales, fluidos corporales, tales como sangre, fluyen hacia y a través de los orificios de perfusión 56 y son
5 perfundidos con un fluido introducido a través del elemento de cuerpo de catéter 54.
Pasando a las figuras 5 y 6, se ilustra una realización del catéter de balón metálico 60 de la invención en el que la topografía de superficie del balón metálico 62 está configurada para incluir una pluralidad de haces o salientes longitudinales 64 que sobresalen por encima de la superficie del balón metálico 62. Al proporcionarse los salientes 64, las propiedades mecánicas de la película de metal que comprende el balón metálico 62 se alteran para crear 10 zonas relativamente más fuertes a lo largo del eje longitudinal de los salientes 64 y zonas relativamente más débiles entre pares adyacentes de salientes 64. En esta configuración, las zonas relativamente más débiles crean líneas de plegado para el balón metálico 62 durante el inflado y desinflado del balón metálico 62. Alternativamente, la topografía de superficie del balón metálico puede estar configurada de manera que para proporcionar los salientes 64 en un modelo que corresponda al modelo geométrico de un dispositivo implantable, tal como un stent, el
15 dispositivo implantable es capaz de encajar en el balón metálico 62 entre los salientes 64 durante la colocación endoluminal.
Finalmente, con referencia a la figura 7, se ilustra una realización 70 del catéter de balón metálico de la invención en el que el balón metálico 72 está recubierto con un revestimiento ultradelgado de un elastómero biocompatible 74. El elastómero 74 añade un componente compatible con el balón metálico 72 y sirve para encapsular el balón metálico y
20 protegerlo de la fragmentación en caso de fatiga y/o agrietamiento metálico del balón metálico 72.
De acuerdo con el método de la presente invención, se emplean de manera preferible métodos de deposición en vacío, como se conocen en la microelectrónica y en la técnica de la nanofabricación. Es preferible emplear pulverización catódica o deposición por evaporación asistida por haz de iones para depositar al menos una película de metal de un metal biocompatible sobre un sustrato cilíndrico sacrificial. El sustrato cilíndrico sacrificial tiene una 25 geometría que corresponde a la geometría deseada para el balón metálico de la invención, y al menos una de una pluralidad de capas de película de metal se deposita sobre el sustrato cilíndrico sacrificial. Después de depositar una película que tiene un espesor deseado de entre 0,1μm y 25μm, el sustrato y la película depositada se retiran de la cámara de deposición y el sustrato sacrificial se retira con medios adecuados para el sustrato seleccionado. Por ejemplo, se puede emplear un sustrato de cobre, luego se sacrifica retirándolo mediante atacado químico. Cualquier 30 modelo de zonas de encaje para un stent y/o salientes para crear líneas de plegado para el balón puede facilitarse ya sea mediante la deposición de especies metálicas a través de una máscara o mediante atacado de zonas de una película depositada. El balón metálico completo o zonas seleccionadas del balón metálico pueden someterse a un recocido después de la deposición para modificar la estructura cristalina de la película de metal y efectuar cambios en las propiedades del material de la película de metal, tales como la alteración de la temperatura de transición de
35 las zonas recocidas, así como para crear configuraciones ventajosas de tensión-deformación cero, tales como pliegues de perfil bajo.
Las figuras 8 y 9 ilustran el catéter de balón metálico de la invención fabricado mediante pulverización catódica de una aleación de níquel-titanio sobre un mandril de cobre, atacando el mandril de cobre para extraer el balón metálico depositado, y adhiriendo el balón metálico sobre un cuerpo de catéter polimérico utilizando un adhesivo
40 biocompatible de cianoacrilato para unir partes proximales y distales del balón metálico.
Las figuras 10A a 10D ilustran el balón metálico 110 de la invención en su estado inflado (figura 10A) que tiene secciones cónicas proximal 112 y distal 114 y una sección tubular alargada intermedia 118. De acuerdo con una realización de la invención, el balón metálico 110 se puede proporcionar con una geometría desinflada, como se representa en la figura 10B, en la que la sección intermedia 118 y las secciones cónicas proximal 112 y distal 114 se 45 desinflan para formar una configuración con una pluralidad de valvas 120 que sobresalen radialmente hacia fuera desde el eje longitudinal del balón metálico 110. La figura 10C es una vista extrema de la figura 10B. La figura 10D representa el plegado de las valvas 120 con el fin de recibir la colocación o retirada endoluminal del balón metálico

La geometría desinflada que se muestra en la figura 10B puede proporcionarse con una amplia variedad de medios
50 que incluyen, sin limitación, memoria de forma o propiedades superelásticas del material metálico, doblar o marcar líneas a lo largo del balón metálico 110 que definen zonas plegables para las valvas 120, o zonas engrosadas del balón metálico 110 entre las valvas 120 que ofrecen una mayor resistencia al plegado al desinflarse el balón metálico 110.
De acuerdo con la realización preferida de fabricación del dispositivo implantable metálico microporoso de la
55 invención, en la que el dispositivo se fabrica a partir de un tubo de nitinol depositado en vacío, un sustrato de cobre desoxigenado cilíndrico se forma en una configuración geométrica que corresponde a un balón de angioplastia inflado que tiene secciones cónicas proximal y distal. El sustrato se pule mecánicamente y/o se electropule para proporcionar una topografía de superficie sustancialmente uniforme para recibir la deposición metálica. Se empleó un dispositivo de pulverización catódica con magnetrón hueco cilíndrico, en el que el cátodo estaba en el exterior y el
sustrato se colocó a lo largo del eje longitudinal del cátodo. Se proporciona un blanco cilíndrico que consiste en una aleación de níquel-titanio que tiene una relación atómica de níquel a titanio de aproximadamente 50-50% y que se puede ajustar mediante soldadura por puntos de hilos de níquel o titanio al blanco, o un cilindro de níquel que tiene una pluralidad tiras de titanio soldadas por puntos a la superficie interior del cilindro de níquel, o un cilindro de titanio 5 que tiene una pluralidad de tiras de níquel soldadas por puntos a la superficie interior del cilindro de titanio. Es conocido en la técnica de deposición por pulverización catódica, refrigerar un blanco dentro de la cámara de deposición manteniendo un contacto térmico entre el blanco y una camisa de refrigeración dentro del cátodo. De acuerdo con la presente invención, se ha encontrado útil reducir la refrigeración térmica aislando térmicamente el blanco de la camisa de refrigeración dentro del cátodo proporcionándose todavía contacto eléctrico con la misma. Al 10 aislar el blanco de la camisa de refrigeración, se permite que el blanco se caliente dentro de la cámara de reacción. Se emplearon dos métodos para aislar térmicamente el blanco cilíndrico de la camisa de refrigeración del cátodo. En primer lugar, una pluralidad de hilos con un diámetro de 0,0381mm fueron soldados por puntos alrededor de la circunferencia exterior del blanco para proporcionar un espaciamiento equivalente entre el blanco y la camisa de refrigeración del cátodo. En segundo lugar, un manguito aislante cerámico tubular se interpuso entre la
15 circunferencia exterior del blanco y la camisa de refrigeración del cátodo. Además, debido a que los rendimientos de la pulverización catódica de Ni-Ti pueden depender de la temperatura del blanco, se prefieren métodos que permitan que el blanco se caliente de manera uniforme.
La cámara de deposición se evacuó a una presión inferior a o de aproximadamente 2-5 x 10-7 Torr y la limpieza previa del sustrato se llevó a cabo en vacío. Durante la deposición, la temperatura del sustrato se mantiene 20 preferiblemente dentro del intervalo de entre 300 y 700 grados centígrados. Es preferible aplicar una tensión de polarización negativa de entre 0 y -1000 voltios al sustrato, y preferiblemente de entre -50 y -150 voltios, que es suficiente para hacer que las especies energéticas lleguen a la superficie del sustrato. Durante la deposición, la presión del gas se mantiene entre 0,1 y 40 mTorr, aunque preferiblemente entre 1 y 20 mTorr. La pulverización catódica preferiblemente se produce en presencia de una atmósfera de argón. El gas argón debe ser de alta pureza 25 y se pueden emplear bombas especiales para reducir la presión parcial de oxígeno. Los tiempos de deposición variarán dependiendo del grosor deseado de la película tubular depositada. Después de la deposición, la pluralidad de microperforaciones se forma en el tubo retirando zonas de la película depositada mediante atacado, tal como atacado químico, ablación, tal como mediante láser de excímeros o mediante mecanizado por descarga eléctrica (EDM), o similares. Una vez formada la pluralidad de microperforaciones, la película microporosa formada se retira
30 del sustrato de cobre dejando expuesto el sustrato y la película a un baño de ácido nítrico durante un periodo de tiempo suficiente para eliminar por disolución el sustrato de cobre.
Aunque la presente invención ha sido descrita con referencia a sus realizaciones preferidas, los expertos en la técnica entenderán y apreciarán que las variaciones en materiales, dimensiones, geometrías y métodos de fabricación pueden ser o llegar a ser conocidas en la técnica, y aun así permanecer dentro del alcance de la
35 presente invención que está limitada sólo por las reivindicaciones adjuntas a la misma.
Claims (10)
- REIVINDICACIONES1. Catéter de balón que comprende:a. un elemento de cuerpo de catéter (12, 32, 54) que tiene un lumen de inflado (20); 5 caracterizado por que comprende ademásb. un balón metálico (14, 36, 38, 52, 62, 72) diametralmente extensible desde el elemento de cuerpo de catéter por la influencia de un fluido que pasa a través del lumen de inflado y llega hasta el balón metálico.
- 2. Catéter de balón de acuerdo con la reivindicación 1, en el que el balón metálico se selecciona del grupo queconsiste en titanio, vanadio, aluminio, níquel, tántalo, circonio, cromo, plata, oro, silicio, magnesio, niobio, escandio, 10 platino, cobalto, paladio, manganeso, molibdeno y aleaciones de los mismos.

- Catéter de balón de acuerdo con la reivindicación 1, en el que el balón metálico tiene un espesor de pared de entre aproximadamente 3μm y 10μm.

- Catéter de balón de acuerdo con la reivindicación 1, en el que el balón metálico se desinfla por la influencia de al menos uno de una memoria de forma, una propiedad superelástica o una propiedad elástica de su metal.
- 15 5. Catéter de balón de acuerdo con la reivindicación 1, en el que el balón metálico en un estado desinflado comprende una pluralidad de valvas (120) que sobresalen radialmente hacia fuera desde el elemento de cuerpo de catéter.
- 6. Catéter de balón de acuerdo con la reivindicación 5, en el que las valvas (120) están configuradas para doblarse y recibir la colocación o la retirada endonuminal del balón metálico.
- 20 7. Catéter de balón de acuerdo con la reivindicación 1, en el que el balón metálico comprende una pluralidad de capas de metal.
- 8. Catéter de balón de acuerdo con la reivindicación 1, en el que el balón metálico comprende un primer balón (36), un segundo balón (38) y un lumen (42) entre el primer balón y el segundo balón, y el segundo balón comprende una pluralidad de poros (40) en comunicación de circulación de fluido con el lumen (42).
- 25 9. Catéter de balón de acuerdo con la reivindicación 8, en el que el segundo balón está en una relación concéntrica, espaciada, coaxial con el primer balón.
- 10. Catéter de balón de acuerdo con la reivindicación 1, en el que el balón metálico comprende una pluralidad de orificios de perfusión (56) que pasan a través del balón metálico (52).
- 11. Catéter de balón de acuerdo con la reivindicación 1, en el que un revestimiento (74) encapsula el balón metálico 30 (72) para protegerlo contra la fragmentación del balón metálico.

- Catéter de balón de acuerdo con la reivindicación 1, en el que el balón metálico (62) comprende una topografía de superficie que está configurada para incluir una pluralidad de salientes longitudinales (64) que sobresalen por encima de la superficie del balón metálico.

- Catéter de balón de acuerdo con la reivindicación 1, que comprende además un lumen de hilo guía (18).

Método de fabricación de catéter de balón de acuerdo con la reivindicación 1, comprendiendo el método las etapas de:a. proporcionar un mandril generalmente cilíndrico que tiene una superficie exterior que presenta una geometría deseada para el balón,b. depositar en vacío un metal sobre el mandril generalmente cilíndrico, formando de ese modo una película de 40 metal que tiene una geometría que corresponde a la geometría deseada del balón, yc. extraer el balón formado del mandril generalmente cilíndrico.FIGURA
