Abstract

Un anticuerpo para uso en el tratamiento de un individuo con un tumor mediado por erbB, para administrar a dicho individuo en combinación con radiación anticáncer, en donde el tumor se caracteriza por a) homodímeros erbB que son homodímeros de EGFR mutantes u homodímeros de p185 y/o b) heterodímeros de erbB que son heterodímeros de p185/EGFR, heterodímeros de p185/EGFR mutantes, heterodímeros de p185/erbB3, heterodímeros de p185/erb4 o heterodímeros de EGFR/EGFR mutante, en donde dicho anticuerpo es un anticuerpo anti-p185 humanizado o fragmento del mismo o un anticuerpo anti- EGFR humanizado, o fragmento del mismo, y en donde el anticuerpo o fragmento de la mismo interrumpe la actividad de quinasa asociada con los dímeros y hace sensibles a la radiación, las células tumorales previamente resistentes a la radiación.

De acuerdo con algunas realizaciones preferidas, la composición que comprende un agente activo el cual provoca la interrupción de la actividad de la quinasa asociada con el conjunto receptor multimérico es administrado por cualquier ruta de administración que se pueda utilizar para administrar el agente al tumor. En algunas realizaciones, la composición se administra por vía intravenosa, intraarterial, intramuscular, intradérmica, subcutánea, parenteral, o administración intratumoral. De acuerdo con algunas realizaciones preferidas, el individuo ha tenido una cirugía para extirpar masa tumoral masiva antes de la administración de la composición.
De acuerdo con aspectos de la presente invención, después de la administración de la composición que comprende un agente activo el cual provoca la interrupción de la actividad de la quinasa asociada con el conjunto receptor multimérico; El individuo es entonces expuesto a una cantidad terapéutica de radiación gamma. La terapia de radiación se puede iniciar en cualquier momento después de que haya transcurrido una cantidad suficiente de tiempo para que el agente activo cause la interrupción de la actividad de la quinasa asociada con el conjunto receptor multimérico. En general, el individuo es expuesto a la radiación en algunos casos 1-10 minutos después, en algunos casos 1-10 horas después, y en algunos casos hasta 24-72 horas después de la administración del agente activo. En algunos casos, la radiación se pen una dosis única mientras que en algunas realizaciones, se administran dosis múltiples durante varias horas, días y/o semanas. El agente activo hace sensibles a la radiación las células tumorales resistentes a la radiación. Así, una vez que el agente activo inhibe la actividad de la quinasa, la exposición a la radiación puede continuar el proceso. La radiación gamma es suministrada de acuerdo con los protocolos radioterapéuticos estándar usando dosificaciones y regímenes estándar. La administración del agente activo hace que la radiación sea más efectiva en la erradicación de las células tumorales.
De acuerdo con aspectos de la presente invención, después de la administración de la composición que comprende un agente activo el cual provoca la interrupción de la actividad de la quinasa asociada con el conjunto receptor multimérico; al individuo se le administra entonces un agente quimioterapéutico citotóxico, además de la exposición a una cantidad terapéutica de radiación gamma. Como en el caso de la terapia de radiación, la quimioterapia puede comenzar en cualquier momento después de que haya transcurrido una cantidad suficiente de tiempo para que el agente activo cause la interrupción de la actividad de la quinasa asociada con el conjunto receptor multimérico. En general, al individuo se le administra el quimioterapéutico en algunos casos 1-10 minutos después, en algunos casos 1-10 horas después, y en algunos casos hasta 24-72 horas después de la administración del agente activo inhibidor de la quinasa. En algunos casos, el quimioterapéutico se provee en una dosis única mientras que en algunas realizaciones, se administran dosis múltiples durante varias horas, días y/o semanas. El agente activo hace que las células tumorales sean más sensibles a los agentes citotóxicos. Así, una vez que el agente activo inhibe la actividad de la quinasa, la administración del quimioterapéutico puede continuar el proceso. Los quimioterapéuticos son suministrados de acuerdo con los protocolos radioterapéuticos estándar utilizando agentes, dosificaciones y regímenes estándar. En algunas realizaciones, el quimioterapéutico se selecciona del grupo que consiste de: cisplatino, doxirrubicina, danurrubicina, tamoxifen, taxol y metotrexato. En algunas realizaciones, los tratamientos de quimioterapia y radiación se emplean ambos después de la administración del agente activo. En tales realizaciones, las combinaciones estándar de las dos modalidades terapéuticas se utilizan en conjunción con la administración del agente activo inhibidor de la quinasa.
La presente invención no pretende limitarse por ninguna teoría. La presente invención se ilustra adicionalmente mediante los siguientes ejemplos, los cuales no pretenden ser limitantes de ninguna manera.
EJEMPLOS
Ejemplo 1: Construcción de mutantes, vectores de expresión y creación de líneas celulares
Métodos detallados para la construcción de las especies de p185 mutantes, los vectores de expresión y líneas celulares se han descrito previamente ((Qian et al., Proc. Natl. Acad. Sci. USA, 1994; 91, 1500; y Weiner et al., Oncogene, 1989, 4; 1175, cada uno de los cuales se incorpora aquí como referencia).
Construcción de mutante N757
El mutante Nneu K757M de enlazamiento a ATP (N757) fue derivado a partir de pSV2TneuK757M (Weiner et al., Oncogene, 1989, 4, 1175, el cual se incorpora aquí como referencia) mediante técnicas de subclonación. Este constructo fue preparado por mutagénesis dirigida al sitio para sustituir un Met por Lys757. Un experto en la técnica entenderá fácilmente la preparación de tal mutante mediante mutagénesis dirigida al sitio. Brevemente, un fragmento de Xba1 de pSV2neuT que corresponde a una banda de 1.2 kb que abarca el sitio de unión a ATP probable de la secuencia de nucleótidos publicada se clonó en M13mp18 y se transfectó en cepa de E. coli CJ236 (dot, ung-) pUC13 de tal manera que el sitio HindIII del polienlazador cayó en el extremo 5' de las secuencias insertadas. La mutagénesis se realizó como se describe utiliazndo un cebador en el cual el codón AAG, que codifica para Lys, fue reemplazado por el codón AUG correspondiente a Met (Bargmann et al., Nature 1986, 319, 226, que se incorpora aquí como referencia). Las mutaciones puntuales así creadas se verificaron mediante secuenciación de ADN. El plásmido que porta la nueva mutación se escindió con Xba1 que liberó el fragmento original. para el receptor erbB en una metodología de terapia génica para la neoplasia humana. Oncoproteínas ∆EGFR se expresan diferencialmente en muchos neoplasmas epiteliales humanos y pueden representar un objetivo específico de tumor, sin embargo, estos receptores no están regulados por el enlazamiento del ligando, son constitutivamente fosforilados, e interiorizar pobremente. Estas características pueden limitar los esfuerzos para inhibir la señalización de los receptores ∆EGF en tumores humanos. La tendencia termodinámica preferida para la formación de heterodímeros de erbB en ausencia de ligando sugiere que direccionando las oncoproteínas erbB con el ectodominio de p185neu, novedosos productos farmacéuticos, o miméticos de péptidos relevantes para la formación de dímero serían más efectivos en lograr la inhibición del crecimiento que la inhibición de la activación inducida por ligando con anticuerpos monoclonales o antagonistas de enlazamiento al ligando.
Materiales y métodos
Construcción del vector
El mutante de eliminación neu T691 de detención neu fue derivado a partir del ADNc de neu oncogénico de rata, pSV2Tneu, que contiene un único punto de mutación en la región de transmembrana. La mutagénesis dirigida al sitio se utilizó para introducir un codón de detención en la posición Thr-691 en el endodominio. Esta forma de eliminación citoplásmica de p185neu, que carece del dominio catalítico de quinasa y los sitios de autofosforilación del terminal carboxilo, a continuación, se insertaron entonces en un vector de expresión de mamífero. Un fragmento que contiene el gen de higromicinar de pHYG se subclonó en APtag-1, un vector de expresión de la fosfatasa alcalina (AP) bajo el control del promotor del virus Maloney de la leucemia y LTR (MuLVLTR). Mantenimiento de células y desarrollo de líneas celulares transfectadas de forma estable.
La línea celular de glioblastoma humano progenitor U87MG y el subclón de glioblastoma humano U87MG.∆EGFR previamente informado que contiene receptores de ∆EGF humanos (Nishikawa et al., 1994) se obtuvieron de Dr. Webster Cavenee (Ludwig Cancer Institute, San Diego, California). Para transfecciones celulares estables, diez microgramos del constructo pMuLVLTR/T691 de detención/Hygr se transfectaron en células U87MG.∆EGFR a través del reactivo lipofectamina (Gibco/BRL, Gaithersberg, MD) bajo condiciones determinadas por transfecciones utilizando el constructo informador de CMV-β (β-galactosidasa bacteriana) (Clontech). La eficiencia de transfección óptima se determinó por quimioluminiscencia como se detectó por un luminómetro (Tropix). Todas las células se cultivaron en medio Eagle modificado de Dulbecco (DMEM, Bio-Whittaker, Walkersville, MD) con 10% de suero bovino fetal (Hyclone, Ogden, UT), 100 U de penicilina, 50 µg/ml de estreptomicina, y L-glutamina 2 mM (GIBCO BRL). Las células cultivadas se mantuvieron a 37 ºC en 95% de aire/5% de CO2. Las células U87MG.∆EGFR se suplementaron con .4mg/ml de G418 (geneticina, GIBCO BRL) para el mantenimiento de la expresión transgénica de ∆EGFR.
Para el desarrollo de transfectantes dobles derivados de U87MG que expresan proteínas ∆EGFR y neu T691 de detención, el medio se suplementó tanto con sulfato de G418 como con higromicina B. Después de 2-3 semanas en medio de selección que contenía 70ug/ml de higromicina B (Boehringer Manheim) y .4mg/ml de sulfato de G418 (geneticina, Gibco BRL), clones U87MG.∆EGFR establecidos que expresan neu T691 de detención (designados células U87MG.∆EGFR/T691) fueron aislados y seleccionados por análisis de citometría de flujo utilizando mAb
7.1.64 contra el ectodominio de neu. El medio para subclones transfectados de forma estable se suplementó con .4mg/ml de sulfato de G418 y 35-70ug/ml de higromicina B para el mantenimiento de la expresión transgénica. Las líneas celulares transfectadas de forma estable fueron revisadas periódicamente mediante análisis de citometría de flujo con mAb 7.16.4 para documentar los niveles estables de expresión transgénica de neu T691 de detención.
Marcado metabólico de las células, seguido por inmunoprecipitación.
Se siembran durante la noche células subconfluentes (1 x 106) en placas de 10 cm en medio de cultivo completo (10% de FBS-DMEM).
