Los dispositivos quirurgicos de aplicacion de sujetador son bien conocidos, en los que estructuras de mordaza opuestas atrapan o agarran y sujetan el tejido seleccionado, que es unido a continuacion por medio de sujetadores quirurgicos. Tfpicamente, uno de los miembros de mordaza incluye un cartucho que aloja una pluralidad de grapas dispuestas en dos o mas filas lineales y el otro miembro de mordaza incluye un yunque que tiene una pluralidad de cavidades para conformar grapas para recibir y conformar las patas de las grapas. Los instrumentos de grapado lineal, por ejemplo, incluyen dos miembros de mordaza alargados, moviles relativamente entre sf, para capturar o sujetar tejido.
Los dispositivos quirurgicos de aplicacion de sujetador que utilizan sujetadores de dos piezas son tambien conocidos. Los sujetadores de dos piezas incluyen un sujetador con puas utilizado en union con una pieza de retencion para sostener el sujetador en su sitio. Tfpicamente, los sujetadores de dos piezas comprenden un espacio de retroceso y dos dientes con puas que son aplicados y bloqueados a una pieza de retencion separada. En uso, el sujetador es presionado al tejido del cuerpo de manera que las puas penetren en el tejido y emerjan desde el otro lado donde son bloqueadas entonces en la pieza de retencion. Los retenedores impiden que el sujetador funcione suelto del tejido. Los sujetadores de dos piezas no pueden ser desbloqueados y no se pueden retirar facilmente. Por esta razon, estan hechos tfpicamente de un material bioabsorbible.
El documento GB 2337934 describe un instrumento quirurgico para suturar segun el preambulo de la reivindicacion independiente 1 adjunta.
El documento US 2003/065336 describe una agrupacion de agujas retractil que se puede montar en un cartucho que comprende una primera sutura continua formada de bucles dependientes, que se extienden a lo largo de los vastagos de las agujas y una sutura adicional que se extiende a traves de dichos bucles.
Resumen
La presente descripcion concierne a un instrumento de sutura quirurgico que incluye un conjunto de empunadura, un miembro alargado, un efector de extremidad, y un mecanismo de accionamiento para aplicar el sistema para suturar en un tejido del paciente. El mecanismo de accionamiento esta ubicado dentro del conjunto de empunadura y esta conectado de forma operativa al efector de extremidad a traves del miembro alargado. El miembro alargado se extiende distalmente desde el conjunto de empunadura y define un eje longitudinal, una extremidad proximal, y una extremidad distal. El efector de extremidad esta sujeto a la extremidad distal del miembro alargado. El efector de extremidad tiene una primera mordaza o mordaza de sutura y una segunda mordaza o mordaza de retencion. La mordaza de sutura tiene un cartucho de sutura con al menos una sutura asegurada de forma que se pueda retirar y la mordaza de retencion tiene un cartucho de retencion con al menos un retenedor asegurado de forma que se pueda retirar. Tanto el cartucho de sutura como el cartucho de retencion son reemplazables y son asegurados de forma que se puedan retirar dentro de las mordazas respectivas.
El cartucho de sutura incluye un cuerpo y una serie de agujas. Cada aguja se extiende desde el cuerpo y tiene una extremidad proximal conica y una garganta o ranura, a traves de la extremidad proximal conica, para recibir y retener de forma que se pueda retirar la sutura en ella. La serie de agujas esta dispuesta de tal manera que la garganta en cada aguja esta en alineacion con la garganta de al menos otra aguja ubicada en una posicion adyacente. Tanto la sutura como el retenedor pueden estar hechos de un material biocompatible que es absorbible por un cuerpo de un paciente. Cada retenedor incluye un cuerpo que define dos aberturas a traves del mismo.
La forma de cada una de las aberturas esta conformada por un paso central, que define un eje longitudinal, y un par de ranuras diametralmente opuestas que se extienden radialmente hacia fuera del paso central. El paso central y las ranuras diametralmente opuestas forman un agujero de seccion transversal en forma de O dentro del cuerpo, definido por un centro sustancialmente circular y dos rectangulos opuestos que se extienden radialmente hacia fuera desde el centro sustancialmente circular. El paso central esta dimensionado para permitir el paso de la aguja a traves del mismo.
Las ranuras diametralmente opuestas se extienden en el sentido longitudinal del cuerpo y definen una seccion de trayecto tortuoso. La seccion de trayecto tortuoso permite el paso de la sutura en una primera direccion e inhibe el paso de la sutura en una segunda direccion. El trayecto tortuoso esta definido por al menos un saliente que se extiende desde una pared lateral de la ranura. Al menos el saliente tiene una superficie distal, una superficie

proximal, y un vertice. La superficie distal del saliente tiene un area mayor que la de la superficie proximal del saliente.
Como se ha descrito aqm, la seccion de trayecto tortuoso tiene dos salientes estando los vertices desplazados entre sf, de tal manera que un vertice esta longitudinalmente mas alto que otro vertice. Se ha concebido tambien que los vertices esten adyacentes entre si y en una proximidad inmediata. La seccion de trayecto tortuoso tambien puede contener o bien un unico saliente o bien mas de dos salientes.
Como se ha descrito aqm, la sutura tiene una forma cilmdrica convencional, sin embargo se ha concebido que la sutura incluya una serie de secciones conicas configuradas para ayudar en la retencion de la sutura dentro de la seccion de trayecto tortuoso del retenedor de sutura.
El penmetro exterior del cuerpo tiene generalmente la forma de un ovalo, un drculo, un triangulo, o un rectangulo. Se ha concebido que el cuerpo tenga un penmetro exterior que enlaza una o mas de estas formas juntas.
Se ha concebido tambien que el paso central tenga una forma sustancialmente conica.
El cartucho de retencion incluye un cuerpo con una serie de agujeros. Los retenedores estan dispuestos alrededor de los agujeros para permitir que una de las agujas pase a traves del retenedor y al agujero.
Breve descripcion de los dibujos
Los dibujos adjuntos, que estan incorporados en esta memoria y constituyen una parte de la misma, ilustran sistemas para suturar ejemplares y realizaciones de la descripcion, y junto con una descripcion general de la exposicion dada anteriormente, y la descripcion detallada de los sistemas para suturar ejemplares y de las realizaciones dadas a continuacion, sirven para explicar los principios de la exposicion.

Descripcion detallada de las realizaciones
Las realizaciones del instrumento de sutura quirurgico descritos actualmente son descritas en detalle con referencia a los figuras 11 a 19, en los que numeros de referencia similares designan elementos identicos o correspondientes en cada una de las diferentes vistas. Como se ha utilizado aqrn, el termino "distal" se refiere a esa porcion del instrumento quirurgico para suturar, o componente del mismo, mas lejos del usuario mientras que el termino "proximal" se refiere a esa porcion del instrumento quirurgico para suturar o componente del mismo, mas proximo al usuario.
El instrumento quirurgico de sutura con el que la unidad de carga desechable de la presente exposicion es descrita seran descritos en terminos tanto de procedimientos convencionales como endoscopicos. Se ha contemplado que el sistema para suturar y el instrumento quirurgico de sutura descrito aqrn pueden encontrar uso en procedimientos en los que el acceso esta limitado a una pequena incision incluyendo pero no estando limitados a procedimientos de artroscopia y/o laparoscopia, y en otros procedimientos no endoscopicos (por ejemplo abiertos).
Haciendo referencia ahora a los dibujos, en los que numeros de referencia similares identifican elementos estructurales similares del sistema e instrumento en cuestion, se ha ilustrado en la fig. 1 un sistema para suturar ejemplar, designado generalmente por el numero de referencia 5. El sistema para suturar 5 ejemplar incluye una cantidad de sutura 10, al menos una aguja 20, y al menos un retenedor 30. Tanto la porcion de sutura 10 como el retenedor 30 estan hechos de un material biocompatible. El material biocompatible puede ser absorbible en un cuerpo de un paciente.
Haciendo referencia ahora a la fig. 2, cada una de las agujas 20 ejemplares tiene una extremidad proximal conica 21 y una garganta 22, transversal a la extremidad proximal conica 21, para recibir y retener de forma que se pueda retirar la porcion de la sutura 10 en ella. Las agujas 20 estan dispuestas de tal manera que la garganta 22 en cada aguja 20 esta en alineacion con la garganta 22 de al menos otra aguja 20 que esta ubicada en una posicion adyacente.
Haciendo referencia ahora a las figs. 3 y 4, el retenedor 30 ejemplar incluye un cuerpo 31 que define una abertura 32 a traves del mismo. La forma de la abertura 32 es definida por un paso central 33, que define un eje longitudinal "x" y un par de ranuras 34 diametralmente opuestas que se extienden radialmente hacia afuera desde el paso central 33. La abertura 32 forma un agujero de seccion transversal en forma de O en el cuerpo 31 definido por un centro sustancialmente circular y dos rectangulos opuestos que se extienden radialmente hacia fuera desde el mismo. El paso central 33 tiene una forma cilmdrica sustancialmente constante dimensionada para permitir el paso de la aguja 20 a traves del mismo.
Una seccion de trayecto tortuoso 35 es definida por dos salientes 40a, 40b que se extienden desde las ranuras 34. Cada saliente 40a, 40b se extiende desde una de las paredes laterales de las ranuras 34. Cada uno de los dos salientes 40a, 40b tiene una superficie proximal 42, una superficie distal 43, y un vertice 44. La superficie proximal
42 forma un angulo con respecto al eje longitudinal "x" que es mayor que un angulo formado por la superficie distal
43 que se corta con el eje longitudinal "x". Como resultado, el area de la superficie distal 43 es mayor que el area de la superficie proximal 42. La seccion de trayecto tortuoso 35 permite el paso de la porcion de sutura 10 en una primera direccion "a" e inhibe el paso de la porcion de sutura 10 en una segunda direccion "b".
El penmetro exterior del cuerpo 31 esta descrito como de forma generalmente circular, sin embargo esto no deberia ser tomado como una limitacion. El cuerpo puede ser tambien de forma ovalada, rectangular, o triangular. Se ha concebido tambien que el cuerpo tiene un penmetro exterior que enlaza una o mas de estas formas juntas y que mas de una sutura puede ser retenida dentro de un unico cuerpo.
Como se ha descrito aqrn e ilustrado en la fig. 4, la seccion de trayecto tortuoso 35 tiene dos salientes 40a, 40b con vertices desplazados 44, de tal manera que un vertice es longitudinalmente mas alto que otro vertice. Se ha concebido que los vertices son adyacentes entre si, como se ha mostrado en otra realizacion del retenedor en la fig. 4a. Los vertices adyacentes 44 estan en proximidad inmediata. La seccion de trayecto tortuoso 35 puede contener tambien un unico saliente o mas de dos salientes.
Como se ha descrito aqrn, la porcion de sutura 10 tiene una forma cilmdrica convencional sin embargo se ha

concebido que la porcion de sutura 10 incluye una serie de secciones conicas configuradas para ayudar en la retencion de la porcion de sutura 10 dentro de la seccion de trayecto tortuoso 35 del retenedor de sutura 30.
Se ha concebido tambien que el paso central 33 tiene una forma sustancialmente conica.
Con referencia ahora a las figs. 5-10, en uso los retenedores 30 ejemplares son posicionados sobre un lado del tejido y las agujas 20 ejemplares son posicionadas sobre el otro lado del tejido. Las agujas 20 son alineadas con el eje longitudinal "x" de los retenedores 30. Cuando una fuerza F es aplicada a las agujas 20, las agujas 20 y la porcion de sutura 10 son forzadas a penetrar a traves del tejido, haciendo que las agujas 20 se extiendan al paso central 32 de los retenedores 30.
La aplicacion continuada de la fuerza F mueve la extremidad proximal conica 21 de la aguja 20 con la garganta 22 mas alla del retenedor 30. Como resultado, la porcion de sutura 10 es forzada a las secciones de trayecto tortuoso 35 de los retenedores 30 y a traves de ellas. La estrecha proximidad de los vertices 44 pellizca o aprieta la porcion de sutura 10 para retenerla en posicion dentro de los retenedores 30 y permitir que las agujas 20 sean retiradas.
Se ha descrito tambien un instrumento quirurgico 100 de sutura, mostrado en la fig. 11. Distinta realizaciones de la presente exposicion se han descrito a continuacion con referencia a los dibujos adjuntos. Las funciones o construcciones bien conocidas no se han descrito en detalle para evitar oscurecer la presente exposicion con detalles innecesarios. Los expertos en la tecnica comprenderan que la presente exposicion puede ser adaptada para utilizar bien con un instrumento endoscopico o bien con un instrumento abierto.
El instrumento quirurgico de sutura incluye la unidad de carga, por ejemplo, una unidad de carga de un solo uso ("SULU") o unidad de carga desechable ("DLU") en el tejido de un paciente y el metodo de uso del instrumento. Por simplicidad, de aqu en adelante, SULU y DLU seran referidos como "SULU", pero debena comprenderse que incluyen cualquiera o ambas de una DLU o de una SULU.
La sutura 210, la aguja 220, y el retenedor 230 son sustancialmente similares a la sutura 10, la aguja 20, y el retenedor 30, respectivamente, y seran descritos solamente en lo que se refiere a las diferencias entre ellos.
Muchos componentes del instrumento quirurgico 100 de sutura son sustancialmente como se ha descrito en la patente Norteamericana N° 6.032.849. Se ha contemplado que las realizaciones actualmente descritas del efector de extremidad 140 pueden ser utilizadas en asociacion con otros dispositivos de grapado conocidos de construccion tanto endoscopica como abierta. Estos dispositivos incluyen dispositivos articulados y no articulados, asf como, dispositivos reutilizables y no reutilizables.
El instrumento de sutura quirurgico 100 incluye un conjunto de empunadura 110, un miembro alargado 120, y un efector de extremidad 140. El conjunto de empunadura 110 incluye un miembro de agarre estacionario 111, un disparador pivotable 112, y un boton moleteado giratorio 113. Un mecanismo de accionamiento 114 esta situado dentro del conjunto de empunadura 110 y esta conectado operativamente al disparador pivotable 112. El mecanismo de accionamiento 114 se extiende a traves del miembro alargado 120 y se conecta operativamente con el efector de extremidad 140. El disparador pivotable 112 es accionado a traves de una carrera o carreras de accionamiento para hacer avanzar el mecanismo de accionamiento 114 dentro del efector de extremidad 140.
El miembro alargado 120 se extiende distalmente desde el conjunto de empunadura 110 y define un eje longitudinal "y", una extremidad proximal 121, y una extremidad distal 122. Una herramienta 130 (por ejemplo, SULU) esta fijada a la extremidad distal 122 del miembro alargado 120. La HERRAMIENTA 130 esta adaptada para ser fijada de forma que se pueda retirar al miembro alargado 120 e incluye una porcion de cuerpo proximal 131 y el efector de extremidad 140. El efector de extremidad 140 incluye una mordaza de retencion 150 y una mordaza de sutura 160, que son utilizadas respectivamente para capturar y sujetar el tejido entre ellas. La mordaza de sutura 160 y la mordaza de retencion 150 son pivotables una con relacion a la otra entre una posicion abierta y una posicion sujeta o aproximada.
El avance del mecanismo de accionamiento 114 mueve la mordaza de sutura 160 en relacion a la mordaza de retencion 150 entre una posicion abierta y una posicion sujeta y para eyectar las suturas 210 del cartucho de sutura 161. Alternativamente, la carrera de accionamiento puede hacer pivotar la mordaza de retencion 150 hacia una mordaza de sutura estacionana 160.
Con referencia a las figs. 12-15, la mordaza de sutura 160 incluye un cartucho de sutura 161 que contiene al menos una sutura 210 asegurada de forma que se pueda retirar y la mordaza de retencion 150 incluye un cartucho de retencion 151 complementario que contiene al menos un retenedor 230 asegurado de forma que se pueda retirar. Tanto el cartucho de sutura 161 como el cartucho de retencion 151 son reemplazables y son asegurados de forma que se pueden retirar dentro de las mordazas respectivas. Se ha contemplado que tanto el cartucho de sutura 161 como el cartucho de retencion 151 contienen una o mas filas complementarias de suturas 210 y retenedores 230, respectivamente.
Ahora con referencia a las figs. 14-15, el efector de extremidad 140 esta ilustrado de acuerdo con una realizacion de la presente exposicion. El cartucho de sutura 161 esta ubicado dentro de un canal 166 de cartucho de sutura de la

mordaza de sutura 160. El canal 166 del cartucho de sutura esta dimensionado y configurado para recibir el cartucho de sutura 161 en el. El cartucho de sutura 161 incluye apendices 162 formados a lo largo de cada lado del cartucho de sutura 161 dimensionados y configurados para asegurarse en un rebaje de apendice correspondiente 168 formado a lo largo de cada lado del canal 166 de cartucho de sutura. El rebaje 168 de apendice esta configurado para retener, posicionar, y alinear los apendices 162 del cartucho de sutura 161 en el. Cualquier otro metodo convencional puede ser utilizado tambien para asegurar el cartucho de sutura 161 dentro del canal 166 del cartucho de sutura.
El cartucho de sutura 161 contiene una pluralidad de filas lineales de agujas 220 y aberturas 164 de aguja complementarias. Cada abertura 164 de aguja esta enlazada a otra abertura 164 de aguja por un paso de sutura 165. Una ranura de cuchilla 163 separa al menos dos filas lineales de agujas 220 y permite el paso de una cuchilla 115 a traves de ella, como se ha visto en la fig. 17. Una ranura longitudinal 169 puede extenderse longitudinalmente a lo largo de la parte superior del canal 166 de cartucho de sutura para proporcionar un punto adicional de fijacion e indicacion visual de la posicion de la cuchilla 115 con respecto al canal 166 del cartucho de sutura.
Cada fila de agujas 220 es paralela al eje longitudinal "y", dentro del cartucho de sutura 161, mientras que cada aguja individual 220 es perpendicular con respecto al eje longitudinal "y". Cada fila de agujas 220 esta dispuesta de tal manera que la garganta en cada aguja 220 esta en alineacion con la garganta de al menos otra aguja 220 que esta ubicada en una posicion adyacente. Cada aguja 220 es conectada a una base 167 de aguja. Multiples agujas 220 pueden ser conectadas a una unica base 167 de aguja.
El cartucho de sutura 161 contiene un miembro de leva 116 para actuar sobre la base 167 de aguja para mover secuencialmente las agujas 220 perpendiculares al eje longitudinal "y" para extender las agujas 220 desde el cartucho de sutura 161. Cuando las agujas 220 son extendidas fuera del cartucho de sutura 161, las agujas 220 y la sutura 210 son forzadas a traves del tejido y al retenedor.
Con referencia ahora a las figs. 12-13, el cartucho de retencion 151 incluye una serie de retenedores 230 asegurados de forma que se pueden retirar dentro de una serie de rebajes retenedores 152, y una serie de agujeros 153 alineados con las aberturas de los retenedores 230. La serie de agujeros 153 complementa las filas de agujas 220 en el cartucho de sutura 161. Los retenedores 230 tienen dos aberturas y estan alineadas con los agujeros 153 para permitir que al menos una de las agujas 220 y la porcion de la sutura 210 pasen a traves del retenedor 230 y al agujero 153. Las suturas 210 son ajustadas en las agujas 220 con holgura entre las agujas 220 para acomodar distintos grosores de tejido.
Se ha concebido tambien que las agujas 220 y los retenedores 230 esten alineados a lo largo de las mordazas curvadas.
Con referencia ahora a las figs. 16 y 19, la presente exposicion se refiere tambien a un metodo quirurgico ejemplar para utilizar el instrumento quirurgico 100 de sutura.
Durante la diseccion del tejido, el efector de extremidad 140 es adaptado para acceder a un sitio quirurgico a traves de un dispositivo de acceso que es conocido en la tecnica. Para conseguir esto, el efector de extremidad 140 que incluye la mordaza de retencion 150 y la sutura 160 es mantenido en una posicion sujeta o aproximada cuando el miembro alargado 120 es insertado a traves de una canula (no mostrada). Despues de eso, las mordazas 150, 160 puede ser abiertas y el tejido puede ser unido y/o cortado sujetando y accionando el instrumento quirurgico 100 de sutura.
Como se ha mostrado en la fig. 16, el tejido es colocado entre la mordaza de retencion 150 y la mordaza de sutura
160. Cuando el disparador pivotable 112 es accionado, el mecanismo de accionamiento 114 es accionado de forma distal a lo largo de la mordaza de sutura 160, ilustrada en la fig. 17, mientras empuja la leva 116 y la cuchilla 115 para cortar el tejido. Cuando la leva 116 avanza, lo largo del eje longitudinal "y", la leva 116 actua sobre la base 167 de aguja para mover las agujas 220 y las suturas 210 perpendicularmente desde el cartucho de sutura 161. Las agujas 220 y la porcion de sutura 210 se extienden a traves del tejido y a los retenedores 230, y parcialmente a traves de los retenedores 230.
Al terminar el avance de la leva 116 a traves de las bases 167 de aguja, el mecanismo de accionamiento 114 es retrafdo distalmente. Cuando el mecanismo de accionamiento 114 es retrafdo, la leva 116 y la cuchilla 115 son retrafdas tambien distalmente permitiendo que las agujas 220 se retraigan perpendicularmente al cartucho de sutura
161. Cuando las agujas 220 son retrafdas, las suturas 210 son liberadas de las agujas 220 y bloqueadas en su sitio dentro de los retenedores 230.
Los retenedores 230 son separados del cartucho de retencion 161 cuando el efector de extremidad 140 libera el tejido. Despues de que el mecanismo de accionamiento 114 es completamente retrafdo, el instrumento quirurgico 100 de sutura puede ser retirado del sitio quirurgico. Ajustar la anchura de hueco entre la mordaza de retencion 150 y la mordaza de sutura 160 permite el uso del instrumento quirurgico 100 de sutura con una variedad de grosores de tejido.
Se comprendera que pueden hacerse distintas modificaciones en las realizaciones descritas aqrn. Por ejemplo, el
tamano, angulos y/o curvas de la superficie o superficies del conjunto de cartucho pueden ser modificados para adaptarse mejor a un procedimiento quirurgico particular. Por lo tanto, la descripcion anterior no debena ser construida como limitativa, sino meramente como ejemplificaciones de distinta realizaciones. Los expertos en la tecnica consideraran otras modificaciones dentro del objeto de las reivindicaciones adjuntas.
Claims (7)
- 51015202530REIVINDICACIONES1. Un instrumento de sutura quirurgico (100), que contiene:un conjunto de empunadura (110);un miembro alargado (120) que se extiende distalmente desde el conjunto de empunadura (110) y que define un eje longitudinal (y) y una extremidad distal (122);un efector de extremidad (140) fijado a la extremidad distal del miembro alargado (120), teniendo el efector de extremidad (140) una mordaza de retencion (150) y una mordaza de sutura (160);un mecanismo de accionamiento (114) conectado de manera operativa a traves del miembro alargado (120) al efector de extremidad (140); caracterizado por quela mordaza de retencion (150) incluye un cartucho de retencion (151) reemplazable que contiene una pluralidad de retenedores (230) asegurados de forma que se puedan retirar, cada retenedor tiene dos aberturas configuradas para recibir una sutura en una primera direccion e impedir la eliminacion de la sutura en una segunda direccion, yla mordaza de sutura (160) incluye un cartucho de sutura (161) reemplazable que contiene una pluralidad de suturas (210) aseguradas de forma que se puedan retirar; y donde el cartucho de sutura (161) contiene una pluralidad de filas lineales de agujas (220) y de aberturas (164) de aguja complementarias.
- 2. El instrumento de sutura de la reivindicacion 1, en el que la forma de la abertura esta definida por un paso central que define un eje longitudinal (x) y un par de ranuras diametralmente opuestas que se extienden radialmente hacia afuera desde el paso central, estando dimensionado el paso central para permitir el paso de la aguja (220) a traves del mismo, donde las ranuras se extienden longitudinalmente a lo largo del cuerpo y definen una seccion de trayecto tortuoso que permite el paso de la sutura (210) en una primera direccion e inhibe el paso de la sutura (210) en una segunda direccion.
- 3. El instrumento de sutura de la reivindicacion 2, en el que la seccion de trayecto tortuoso esta definida por dos salientes con vertices desplazados entre sf, de manera que un vertice esta longitudinalmente mas alto que el otro vertice.
- 4. El instrumento de sutura de la reivindicacion 3, en el que los vertices estan adyacentes entre sf.
- 5. El instrumento de sutura de la reivindicacion 4, en el que los vertices adyacentes estan en proximidad inmediata.
- 6. El instrumento de sutura de la reivindicacion 1, en el que las aberturas (164) de aguja estan enlazadas entre sf por un paso de sutura (165).
- 7. El instrumento de sutura de la reivindicacion 1 o de la reivindicacion 6, en el que cada fila de agujas (220) es paralela al eje longitudinal (y) y cada aguja (220) es perpendicular con respecto al eje longitudinal (y).
