Abstract
【解決手段】 前十字靭帯(ACL)を移植片で置換する方法である。その方法は大腿骨150および脛骨130に骨トンネル280,290を穿孔する過程を含む。置換移植片には第1の端部および第2の端部が設けられている。生物分解性の複合物のねじ5が提供される。そのねじは生物分解性のポリマーおよび生体セラミックたまはバイオガラスから作られている。移植片200の少なくとも一方の端部は生物分解性の複合物のねじを用いて骨トンネル内にしっかりと取り付けられている。

Description
生物分解性の複合物の骨ピン1が通常の方式で準備されて実験用哺乳動物の大腿骨内に植え込まれた。その骨ピン1は以下の3種類の組成、すなわちA)15体積%/85体積%のβ−燐酸三カルシウムおよび85体積%/15体積%のポリラクチドコグリコリド(poly(lactide co-glycolid))の複合物、B)ポリラクチド、およびC)15体積%/85体積%のβ−燐酸三カルシウムおよび85体積%/15体積%のポリラクチドの複合物であった。植え込み後約24時間が経過したとき、実験用哺乳動物が安楽死させられて組織構造の部分が採取された。図11Aに示されているように、組成A)の骨ピン500は矢印505で示されたもともとの直径と比較して有意な程度の吸収および有意な組織(骨)の内殖を示した。さらに、最小の組織の反応が観察された。図11Bおよび図11Cに示されているように、組成B)の骨ピン510および骨ピン520は各々矢印515および矢印525で示されたもともとの直径と比較して最小の吸収を示した。図11Dに示されているように、組成C)の骨ピン530は矢印535で示されたもともとの直径と比較して最小の吸収を示した。さらに、図11Eに示されているように、組成A)の骨ピン540は矢印545で示されたもともとの直径と比較して有意の程度の吸収および有意の組織(骨)の内殖を示した。最小の組織の反応が観察された。
(1)移植片の第2の端部を脛骨トンネル内に挿入する過程と、
生物分解性の締まり嵌めねじを上記脛骨トンネル内の上記脛骨トンネルの内側面と上記移植片の上記第2の端部との間に挿入する過程と、
上記締まり嵌めねじを、上記締まり嵌めねじが上記脛骨トンネル内に実質的に収容され、上記移植片の上記第2の端部が上記締まり嵌めねじと上記脛骨トンネルの上記内側面の一部との間の所定の位置に固定されるように回転させる過程と
をさらに有する、請求項1記載の方法。
(2)生体セラミックが、モノ−燐酸カルシウム、ジ−燐酸カルシウム、トリ−燐酸カルシウム、α−トリ−燐酸カルシウム、β−トリ−燐酸カルシウム、テトラ−燐酸カルシウム、ハイドロキシアパタイト、硫酸カルシウム、酸化カルシウム、炭酸カルシウム、燐酸カルシウムマグネシウムからなる集合から選択された生体セラミックからなる、請求項1記載の方法。
(3)生体セラミックがβ−トリ−燐酸カルシウムからなる、上記実施態様(2)記載の方法。
(4)生物分解性のポリマーが、実質的に85モル%から実質的に95モル%までのポリ乳酸および実質的に5モル%から実質的に15モル%までのポリグリコール酸を含むポリ乳酸およびポリグリコール酸のコポリマーからなる、請求項1記載の方法。
(5)生物分解性のポリマーが、実質的に85モル%のポリ乳酸および実質的に15モル%のポリグリコール酸のコポリマーからなる、上記実施態様(4)記載の方法。
(7)複合物の締まり嵌めねじが、実質的に15.0体積%の生体セラミックを含む、請求項1記載の方法。
(8)移植片が一方の端部に取り付けられた骨ブロックを有する、請求項1記載の方法。
(9)移植片の両端に骨ブロックが取り付けられている、請求項1記載の方法。
(10)大腿骨トンネルおよび脛骨トンネルの内側面と骨ブロックとの間をねじ切りして上記大腿骨トンネルおよび上記脛骨トンネルの上記内側面と上記骨ブロックとの間にねじ山が設けられた空間を形成する過程をさらに有する、請求項1記載の方法。

Claims (1)
- 膝関節の前十字靭帯を置換する方法であって、
第1の端部および第2の端部を有する移植片を提供する過程と、
内側壁を有する脛骨トンネルを脛骨に穿孔する過程と、
内側壁を有する大腿骨トンネルを大腿骨に穿孔する過程と、
上記移植片の上記第1の端部を上記大腿骨トンネル内に取り付ける過程と、
上記移植片の上記第2の端部を上記脛骨トンネル内に取り付ける過程と、
ポリ乳酸およびポリグリコール酸のコポリマーからなる生物分解性のポリマーと生体セラミックとを含む生物分解性の複合物の締まり嵌めねじを提供する過程と、
上記生物分解性の締まり嵌めねじを上記大腿骨の上記大腿骨トンネルの内側面と上記移植片の上記第1の端部との間に挿入する過程と、
上記締まり嵌めねじを、上記締まり嵌めねじが上記大腿骨トンネル内に実質的に収容され、上記移植片の上記第1の端部が上記締まり嵌めねじと上記大腿骨トンネルの上記内側面の一部との間の所定の位置に固定されるように回転させる過程と
を有する、膝関節の前十字靭帯を置換する方法。
