Description
本発明は医療技術の分野に関する。特に、本発明は、医療装置、医療器具および医療方法に関し、特にインプラント、埋込み(implantation)用の器具および埋込み方法に関する。
ねじが生体の骨組織に係止される場合、しばしば、骨の安定性が不十分になったり、骨への係止の安定性が不十分になったりするという問題が生じる。特に、骨梁組織においては、ねじに作用する如何なる負荷もほんのわずかな骨梁にしか伝わらず、ねじと骨との接続箇所の耐荷重能力と、長期間の安定性とに対して悪影響を及ぼす。これは、特に、骨粗鬆症もしくは骨減少症の骨組織または脆くなった骨組織の場合には深刻である。
− ソノトロードを備えたコンバータをばね力に対抗して手動で引込めるための操作レバー。このような操作レバーはハンドグリップなどを含み得る。
− インプラントにインプラント軸部分およびインプラント頭部を設けるステップを含み、インプラント頭部は回転非対称であり、インプラント軸部分は、長手方向ボアと、長手方向ボアから外側に至る少なくとも1つの開口部とを有し、当該方法はさらに、
− 頭部が骨組織から突出るように軸部分を骨組織に押込むステップと、
− 軸部分を骨組織に押込んだ後、熱可塑性材料が長手方向の開口部にあり、インプラント頭部の向きが保持構造によって器具のケーシングに対して固定された状態で、上述のタイプの器具を頭部に連結するステップとを含み、当該方法はさらに、
− コンバータを作動させて、ソノトロードが機械的振動を熱可塑性材料に結合させるようにしつつ、ソノトロードを遠位方向に向かって押込み、これにより、熱可塑性材料の部分を液化させ、少なくとも1つの孔から押出して骨組織の構造に押込むステップと、
− コンバータを停止させ器具を取外すステップとを含む。
以下において、本発明および実施例を実行するための方法を、添付の図面を参照しつつ説明する。図面は大部分が概略的に図示される。図面においては、同じ参照番号は同じまたは類似の要素を指す。
図1〜図6に概略的に図示された装置41は、硬組織および/または硬組織置換材料において係止される外科手術用インプラントである。より特定的には、装置41はペディクルスクリューである。
、次いで、配向管の雄ねじをねじ頭部の雌ねじに係合させるよう回転グリップ67を回転移動させることによって、器具に装着可能である。
図16に示されるようなレバー91によってではなく、顎部93および94は、他の機構によって、たとえば図18に示される機構などによって、ねじ頭部と係合させられてもよい。図18は、図17の矢印によって示される線に沿った断面を非常に概略的に示す。顎部は、たとえば、偏向リング98を矢印の方向に向かって軸方向に移動させることによって弾性力に対抗して逸らすことができる。クリッピング機構(図示せず)などは、移動させるとその位置で偏向リングを保持し得る。図18の実施例は2つ以上の顎部を含み得る。
Claims (14)
- インプラントの自動的な埋込みまたは硬組織および/もしくは硬組織置換材料の自動的な補強プロセスのための器具であって、インプラントは、近位端から延在する長手方向ボアと、長手方向ボア内の液化可能材料とを有する被覆要素(41)を備え、
被覆要素(41)は、被覆要素(41)の壁に少なくとも1つの孔を含み、機械的振動が長手方向ボアにおいて液化可能な材料に結合され、同時に押圧力が遠位方向に加えられることにより、液化可能な材料の部分が液化され、少なくとも1つの孔を通って長手方向ボアから押出され、
前記器具は、
ケーシング(56)と、
機械的振動を発生させるよう動作可能なコンバータ(61)とを含み、前記コンバータ(61)は、ケーシング(56)に対して長手方向にずらすことができるようにケーシング(56)内に装着され、前記器具はさらに、
コンバータ(61)の遠位端に連結されたソノトロード(62)と、
ケーシング(56)に回転結合され、被覆要素(41)の近位端付近を保持するために設けられた保持構造(65.1,93,94)を備え、ケーシング(56)の遠位端と被覆要素(41)の近位端との間に設けられた軸部分(65)とを含み、保持構造(65.1,93,94)は、被覆要素(41)を軸部分(65)の遠位端上に位置決めし、前記器具はさらに、
被覆要素(41)を軸部分(65)の遠位端上に位置決めした状態で、被覆要素(41)を軸部分(65)に固定してケーシング(56)を被覆要素(41)にロックする軸継手(64,95)とを含み、
軸部分(65)、軸継手(64,95)およびソノトロード(62)は、ソノトノード(62)が軸部分(65)内を案内され、かつ、被覆要素(41)がケーシング(56)にロックされると、ソノトロード(62)の遠位端がケーシング(56)に対するコンバータ(61)のずれによって被覆要素(41)の長手方向の開口部(13)に差込まれ得るように、相互に配置される、器具。 - 張力がかけられたときにコンバータ(61)およびソノトロード(62)を遠位方向に移動させることのできるばね機構(71,72)をさらに含む、請求項1に記載の器具。
- コンバータ(61)は、第1の摺動軸受(73)および第2の摺動軸受(74)によってケーシング(56)に装着され、第2の摺動軸受(74)は、コンバータ(61)およびケーシング(56)の両方に対して摺動式に装着され、ばね機構(71,72)は、張力がかけられると、コンバータ(61)またはコンバータ(61)に固定して装着された要素と第2の摺動軸受(74)との間に第1のばね力を加えるよう配置された第1の螺旋ばね(71)を含み、ばね機構(71,72)は、張力がかけられると、第2の摺動軸受(74)とケーシング(56)またはケーシング(56)に固定して装着された要素との間に第2のばね力を加えるよう配置された第2のばね(72)を含む、請求項2に記載の器具。
- 軸部分(65)はソノトロード(62)の一部を囲み、ケーシング(56)、軸部分(65)および被覆要素(41)のアセンブリがソノトロード(62)を完全に覆う、請求項1から3のいずれかに記載の器具。
- 回転可能な管(64)をさらに含み、前記回転可能な管(64)の遠位端には軸継手が配置されており、回転可能な管(64)はケーシング(56)に対して回転可能である、請求項1から3のいずれかに記載の器具。
