容易に嚥下し得るアレンドロナート液体組成物を投与することによる、嚥下に困難を有する患者の骨消失を治療又は予防する療法を開示する。該療法を実施するための、アレンドロナートのシロップ、水溶液、及び再構成粉末から形成された溶液からなる医薬剤形も記載する。
Description
【発明の詳細な説明】発明の名称
経口液体アレンドロナート組成物発明の分野
本発明は概して、嚥下に困難を有するヒト患者の骨吸収を阻害するための、ア
レンドロナート、即ち、４−アミノ−１−ヒドロキシブチリデン−１，１−ビス
ホスホン酸一ナトリウム三水和物の使用に関する。発明の背景
正常な骨は、一定の鉱物平衡を維持するという正味作用を有する、カルシウム
の恒常的な吸収及び再沈着を行う生組織である。この二重過程は一般に「骨交替
」と称されている。成長している正常な骨においては、鉱物の沈着が鉱物の吸収
と平衡しているのに対し、特定の病的な状態、例えば、悪性若しくは原発性副甲
状腺機能亢進症又は骨粗鬆症においては、骨吸収が骨沈着を凌駕する。他の病的
状態では、望ましくない量及び領域でカルシウムが沈着し、例えば、異所の骨化
、変形性関節炎、腎臓又は膀胱結石、アテローム性動脈硬化症、及び異常に多い
骨吸収とそれに続く異常なカルシウム沈着の組合わせであるパジェット病になる
。
アレンドロナート、即ち、４−アミノ−１−ヒドロキシブチリデン−１，１−
ビスホスホン酸一ナトリウム三水和物は、骨粗鬆症を含む骨疾患において骨吸収
と闘う薬剤であり、その組成物、使用法及び合成法、並びに他の医薬上許容し得
る塩が、米国特許第４，９２２，００７号及び同第５，０１９，６５１号（共に
Ｍｅｒｃｋに譲渡された）に記載されている。
骨粗鬆症（即ち、骨の希薄化）に対してアレンドロナート療法を受けるている
か又は骨粗鬆症を発症する危険があると考えられる老齢の女性患者が、同時に嚥
下に困難を感じている場合がある。
しかし、現在アレンドロナートは全ての患者に錠剤として経口投与されている
。固形経口剤形のみしか利用できないというのは、錠剤を嚥下しにくい一部の患
者には不利である。さらに、これらの患者はアレンドロナート患者全体の中で有
意な割合を形成している可能性がある。というのは、該薬剤は老齢女性患者向け
でもあるからである。
これらのケースで所望されているのは、嚥下が困難であるという問題を克服す
べく改良された経口組成物を用いて、この老齢女性患者達を最適に治療する有効
な療法である。発明の要旨
本発明は、前記嚥下に困難を有する被験者に、経口液体組成物として医薬上有
効量のアレンドロナートを投与して、該患者の骨消失を治療及び／又は予防する
方法を提供する。液体組成物は、シロップ、水溶液又は水溶液中で再構成される
アレンドロナート粉末の形態であってよく、該溶液のｐＨを調節する緩衝剤及び
アレンドロナートの不溶性複合体の形成を阻止する錯化剤を含んでいる。
本発明の経口液体医薬組成物は、医薬上許容し得る液体担体例えば精製水中医
薬上有効量のアレンドロナート、並びにｐＨを２〜８、好ましくは４〜６に維持
するための緩衝剤例えばクエン酸、及び水性媒体中のアレンドロナートの沈降を
阻止するための錯化剤例えばクエン酸又はＥＤＴＡを含んでいる。また、市販用
に望ましければ着香もし得るシロップを形成する高濃度の糖を含む上記水溶液を
も提供する。
さらに、医薬上許容し得る無水賦形剤中医薬上有効量のアレンドロナートを含
む再構成用粉末、緩衝剤例えばクエン酸、及び前記粉末を水中に溶解させ得る錯
化剤からなる医薬組成物も提供する。発明の詳細な説明及び好ましい実施態様
シロップ、水溶液又は再構成粉末水溶液の形態のアレンドロナート経口溶液は
、固体経口剤形の嚥下に困難を有する患者に対して、投与が容易であり且つより
受け入れやすいという利点を提供する。さらに、再構成用粉末は、療養所、薬局
、病院及び倉庫での貯蔵スペースを最小限にするというさらなる利点をも提供す
る。これらの組成物は、所望なら、用量滴定を可能にするという利点を有してい
る。
本発明の方法を用いて、ヒト特に閉経後の女性を骨形成上有効量のアレンドロ
ナートで治療して、そのような治療を要する骨吸収を阻害し得る。そうした治療
の必要性は、骨折、偽関節、骨欠損などの場合に局所的に発生し、また、骨粗鬆
症、変形性関節症、パジェット病、骨軟化症、多発性骨髄腫及び他の形態の癌、
ステロイド療法並びに老化関連骨量消失におけるような全身的な骨疾患の場合に
も発生する。
本明細書に用いられている「骨吸収の阻害」という用語は、骨消失の治療及び
予防、特に、破骨細胞の形成又は活性を直接間接に変性させることにより骨塩相
及び／又は有機基質相から現存する骨が消失するのを阻害することを指
す。従って、本明細書に用いられている「骨吸収阻害剤」という用語は、破骨細
胞の形成又は活性を直接間接に変性させて骨消失を子防し、治療を受けている患
者達の骨量を増大させ得る薬剤を指す。
本明細書に用いられている「骨形成上有効」という用語は、成熟骨の交替をも
たらす量を意味する。本明細書に用いられている骨形成上有効な量は、「医薬上
有効」でもある。
本明細書に用いられている「治療」又は「治療する」という用語は、（１）被
験者に、予防的に作用して弱体化及び／又は不健康な症状の発症を予防するに十
分な量のアレンドロナートを投与し、及び／又は、（２）被験者に、病的状態及
び／若しくは病的状態の徴候や弱体化及び／若しくは不健康な状態を軽減若しく
は排除するに十分な量のアレンドロナートを投与することを意味する。
投与用のアレンドロナートを含む本発明の医薬組成物は一般に、骨成長を促進
する骨形成上有効量のアレンドロナートに加えて、医薬上許容し得る賦形剤を含
む。該組成物は、スクロース及びラクトースを含む糖、スターチ及び誘導体、セ
ルロース及び誘導体、ガム、脂肪酸及びその塩の
ような粉末形態用無水不活性担体と共に調剤するのが好ましい。
液体組成物に適当な液体賦形剤／担体は精製水及び塩水溶液である。
他の適当な賦形剤及び他の補助添加剤は以下の通りである：溶媒

さらに、水溶液のｐＨを２〜８、好ましくは４〜６の範
囲に維持するには緩衝剤の存在が必要である。
緩衝剤系は一般に、弱酸とその可溶性塩の混合物、例えば、クエン酸ナトリウ
ム／クエン酸；又は二塩基酸のモノカチオン又はジカチオン、例えば、酒石酸水
素カリウム、酒石酸水素ナトリウム、フタル酸水素ナトリウム、フタル酸水素カ
リウム、リン酸二水素カリウム及びリン酸水素二ナトリウムである。
用いられる緩衝剤系の量は、所望のｐＨ及びアレンドロナートの量に依存する
。一般に、組成物のｐＨを２〜９の範囲に維持するのに用いられる緩衝剤の量は
、モル比０．５〜５０：１の緩衝剤：アレンドロナートであり、一般に、存在す
るアレンドロナートに対する緩衝剤のモル比は１：１０である。
有用な緩衝剤の１種は、１ｍｌ当たり１〜１５ｍｇのクエン酸に対して１ｍｌ
当たり５〜５０ｍｇのクエン酸ナトリウムの範囲のクエン酸ナトリウム／クエン
酸である。
ガラス容器若しくはゴム栓から滲出するか又は通常の水道水中に存在し得る溶
解金属イオン、例えば、Ｃａ、Ｍｇ、Ｆｅ、Ａｌ、Ｂａとの金属複合体の形成に
よるアレンドロナートの沈降を阻害するために錯化剤も存在する。錯化剤
は、アレンドロナートとの競合的錯化物質として作用して可溶性金属複合体を生
成するのに対し、アレンドロナートは通常、不溶性金属複合体を形成する。錯化
剤には、緩衝剤／錯化剤又はＥＤＴＡとして作用するクエン酸緩衝液が含まれる
。ＥＤＴＡを用いる場合、組成物の０．００５〜０．１重量％、１重量部のアレ
ンドロナートに対して０．００５〜２重量部のＥＤＴＡの量、好ましくは組成物
の約０．０１重量％の量で用いる。クエン酸緩衝液を単独で用いるのが好ましい
。
アレンドロナートの３種の経口剤形の例は以下の通りである。経口水溶液
アレンドロナートのバルク薬剤を水又は適切な補助溶媒に溶解して所望の濃度
を得る。必要に応じて、着香剤、着色剤、増粘剤、保存剤、安定剤及び緩衝剤を
加える。該溶液を複数回又は単回用量のパッケージに充填する。
水溶液はそのままビンから直接用いる。一般配合
アレンドロナート ０．５〜１０．０ｍｇ
クエン酸ナトリウム ５〜５０ｍｇ
クエン酸 １〜１５ｍｇ
精製水 １ｍｌまでの量
補助溶媒、着香剤、着色剤、保存剤、安定剤及び緩衝剤のような追加の薬剤は
以下のような配合で特定的に添加し得る：特定配合
アレンドロナート ０．５〜１０．０ｍｇ
クエン酸ナトリウム ５〜５０ｍｇ
クエン酸 １〜１５ｍｇ
野生のサクランボ（粉末）a １０〜２００ｍｇ
ＦＤＣ Ｒｅｄ ３３番b ０．１〜１．０ｍｇ
ソルビン酸 ０．０５〜０．２％
サッカリンc １〜１００ｍｇ
プロピレングリコール ５〜２０％精製水 １ｍｌまでの量 a
野生のサクランボ以外の着香剤を用いてもよい。b
他の着香剤とマッチさせるためにＦＤＣ ３３番以外
の着色剤を選択してもよい。c
あるいは、甘味剤としてスクロース又はアスパルテー
ムを用いてもよい。経口シロップ剤
アレンドロナートのバルク薬剤をスクロース（１０〜８５％）溶液に加えて所
望の濃度を得る。必要に応じ、グリセリン、ソルビトール、着香剤、着色剤、増
粘剤、保存剤、安定剤及び緩衝剤のような追加の薬剤を加える。最終溶液
を複数回又は単回用量パッケージに充填する。
シロップはそのままビンから直接又は嚥下しやすいように少量の水道水に加え
て用いてよい。一般配合
アレンドロナート ０．５〜１０．０ｍｇ
クエン酸 １〜１５ｍｇ
クエン酸ナトリウム ５〜５０ｍｇ
スクロース １０〜８５％
精製水 １ｍｌまでの量
グリセリン、ソルビトール、着香剤、着色剤、保存剤、安定剤及び緩衝剤のよ

アレンドロナートのバルク薬剤を、着香剤、着色剤、保存剤、安定剤の中の１
種以上及び特に緩衝剤と均質混合する。次いで、粉末ブレンドを複数回用量容器
又は単回用量パケットに充填する。
粉末は、普通の水道水に溶解して水溶液としてアレンドロナートを再構成し得
る。再構成用粉末配合

容器はビン（水を加え得る）又はサッシェパケットであっ
てよい。あるいは、組成物は複数回用量容器中にバルクと
して装入し得る。
必要な厳密用量は、被験者の年齢、体格、性別及び症状、治療すべき疾患の種
類及び重篤度などに応じて異なる。従って、厳密な有効量は前もって特定するこ
とは不可能であり、治療者により決定される。しかし、適切な量は、以下に記載
の動物モデルを用いた日常的な実験により決定し得
る。一般に、任意の経口液体組成物中のアレンドロナートの有効用量は、体重１
ｋｇ当たり約１．５〜３，０００μｇ、好ましくは約１０〜約２００μｇである
。
本発明の方法及び組成物は、骨折、骨欠損、及び骨粗鬆症、骨髄腫、変形性関
節症、パジェット病、骨灰分欠乏、骨軟化症、多発性骨髄腫及び他の癌形態に起
因する骨消失、他の医療処置（例えば、ステロイド療法）の副作用に起因する骨
消失、並びに老化関連骨量消失といった病理学的症状に起因する疾患の治療に有
用である。
