Abstract
1. Polaczenie zawierajace taxotére z co najmniej jedna substancja terapeutycznie przydatna do leczenia chorób nowotworowych, znamienne tym, ze jako substancje terapeu- tycznie przydatne do leczenia chorób nowotworowych zawiera substancje wybrane sposród srodków alkilujacych takich jak cyklofosfamid; epidofilotoksyn takich jak etopozyd; anty- metabolitów takich jak 5-fluorouracyl, alkaloidów rózanecznika takich jak winkrystyna lub vinavelbina; antybiotyków takich jak doksorubicyna i/lub inhibitorów topoizomerazy takich jak pochodne substancji o nazwie camptotecyna tzn. kamptotecyna-11 (CPT-11), przy czym polaczenie to zawiera od 10 do 90% taxotére. Description
Niniejszy wynalazek dotyczy połączeń taxotere oraz substancji użytecznych terapeutycznie w leczeniu chorób nowotworowych.
Taxotere, który jest również znany pod nazwą docetaksel, a stanowi związek z grupy taksanów, wykazuje godne uwagi właściwości przeciwnowotworowe i przeciwbiałaczkowe i jest szczególnie przydatny w leczeniu raka jajnika, sutka i płuc.
Wytwarzanie taxotere i jego pochodnych jest przedmiotem przykładowo europejskich opisów patentowych EP 0253 738 i EP 0253 739 oraz międzynarodowego zgłoszenia patentowego PCT WO 9209589.
Dawki zazwyczaj stosowane wynoszą, w zależności od charakterystyki leczonego pacjenta, od 1 do 10 mg/kg przy podawaniu dootrzewnym lub od 1 do 3 mg/kg przy podawaniu dożylnym.

Poza tym aktywność produktów zależy od użytych dawek i jest możliwe zastosowanie wyższych dawek oraz zwiększenie aktywności przy ograniczeniu zjawiska toksyczności lub opóźnieniu jej wystąpienia, włączając do połączenia czynniki wzrostu typu krwiotwórczego, takie jak G-CSp lub GM-CSF albo pewne interleukiny.
Połączenia lub asocjacje według wynalazku pozwalają na uniknięcie względnie opóźnienie zjawiska odporności pleiotropowej lub multi-drug resistance.
W szczególności wynalazek dotyczy asocjacji Taxotere z alkaloidami różanecznika, cyklofosfamidem, 5-fluorouracylem, doksorubiną, cisplatyną i etopozydem.

Połączenie przejawia synergizm terapeutyczny, jeśli ma działanie lecznicze silniejsze niż każdy ze składników, użytych w dawce optymalnej [T.H.Corbett i in., Cancer Treatment Reports, 66, 1187 (1982)].
Dla uzyskania skuteczności połączenia może być potrzebne porównanie tolerowanej maksymalnej dawki połączenia z tolerowaną maksymalną dawką każdego ze składników odrębnie w danym badaniu.

Produkty tworzące połączenia, można podawać równocześnie, oddzielnie lub w sposób rozłożony w czasie tak, aby uzyskać maksimum skuteczności połączenia; każde podanie może mieć różny czas trwania od szybkiego podania całkowitego do ciągłego wlewu.
Wynika z tego, że w znaczeniu niniejszego wynalazku połączenia nie ograniczają się jedynie do fizycznej asocjacji składników, ale obejmują również połączenia, pozwalające na podawanie oddzielne, w sposób równoczesny lub rozłożony w czasie.
Środki farmaceutyczne zawierające połączenia według wynalazku są korzystnie środkami nadającymi się do podawania pozajelitowego. Jednakże środki te można podawać też doustnie lub dootrzewnowo w wypadku leczenia miejscowego.
Środki do podawania pozajelitowego są zwykle jałowymi roztworami lub zawiesinami, farmaceutycznie dopuszczalnymi, które ewentualnie można wytworzyć bezpośrednio w momencie użycia. Do wytworzenia roztworów lub zawiesin niewodnych można zastosować naturalne oleje roślinne, takie jak oliwa z oliwek, olej sezamowy lub olej parafinowy, albo organiczne estry nadające się do wstrzyknięć, takie jak oleinian etylu. Jałowe roztwory wodne można uzyskać z rozpuszczenia produktu w wodzie. Wodne roztwory nadają się do podawania, o ile mają odpowiednio ustalone pH i są izotoniczne np. przez dodanie odpowiedniej ilości chlorku sodu lub glukozy. Sterylizacji można dokonać przez ogrzewanie lub każdym innym sposobem, który nie zmienia środka. Połączenia mogą również występować w postaci lipozomów lub w postaci asocjacji z nośnikami, takimi jak cyklodekstryny lub polietylenoglikole.
Środki do podawania doustnego lub dootrzewnowego są korzystnie roztworami lub zawiesinami wodnymi.
W połączeniach według wynalazku, których składniki można aplikować równocześnie, oddzielnie lub rozkładając w czasie, szczególnie korzystne jest to, że ilość pochodnej taksanu stanowi od 10 do 90% wagowych połączenia, przy czym ta zawartość może się zmieniać zależnie od charakteru dołączonej substancji, poszukiwanej skuteczności i natury leczonego raka.
Połączenia według wynalazku są szczególnie przydatne do leczenia raka sutka, jajnika lub płuc. Dają one szczególnie korzystną możliwość użycia składników w dawkach znacznie mniejszych, niż dawki składników stosowanych pojedynczo.

Połączenia według wynalazku objaśnia następujący przykład.
P r z y k 1 a d. Wytwarza się zwykłymi metodami do podania dożylnego ampułki o pojemności 10 cm3, zawierające 100 mg środka o nazwie taxotere. Wytwarza się zwykłymi metodami do podania dożylnego ampułki o pojemności 5 cm3, zawierające 10 mg etopozydu. Roztwory te podaje się równocześnie po odpowiednim rozcieńczeniu przez wlew. Zabieg można powtórzyć kilka razy dziennie lub w ciągu tygodnia, aż do częściowej lub całkowitej remisji albo do wyzdrowienia.

- Zastrzeżenia patentowe1. Połączenie zawierające taxotere z co najmniej jedną substancją terapeutycznie przydatną do leczenia chorób nowotworowych, znamienne tym, że jako substancje terapeutycznie przydatne do leczenia chorób nowotworowych zawiera substancje wybrane spośród środków alkilujących takich jak cyklofosfamid; epidofilotoksyn takich jak etopozyd; antymetabolitów takich jak 5-fluorouracyl, alkaloidów różanecznika takich jak winkrystyna lub vinavelbina; antybiotyków takich jak doksorubicyna i/lub inhibitorów topoizomerazy takich jak pochodne substancji o nazwie camptotecyna tzn. kamptotecyna-11 (CPT-11), przy czym połączenie to zawiera od 10 do 90% taxotere.
- 2. Połączenie według zastrz. 1, znamienne tym, że zawiera ponadto czynniki wzrostu typu krwiotwórczego.
