TEHNIČKA OBLAST
[0001] Predmetni pronalazak se odnosi na sredstva za isporučivanje tečnosti namenjena za isporučivanje terapeutskih tečnosti pacijentu, konkretnije na infuzionu pumpu za isporučivanje terapeutskih tečnosti pacijentu.
OSNOVNE INFORMACIJE
[0002] Sredstva za isporučivanje tečnosti imaju brojne primene, kao što je supkutano isporučivanje tečnog leka ili druge terapeutske tečnosti pacijentu. Na primer, ambulantne infuzione pumpe su kod pacijenata sa dijabetesom melitusom korišćene za isporučivanje insulina pacijentu. Ove ambulantne infuzione pumpe mogu da obezbede prefinjene profile isporučivanja tečnosti, uključujući promenljivu bazalnu brzinu i bolusne potrebe. Sposobnost pažljivog kontrolisanja isporučivanja leka može dovesti do poboljšane delotvornosti leka i terapije i manje toksičnosti za pacijenta.
[0003] Neke postojeće ambulantne infuzione pumpe uključuju rezervoar koji sadrži tečni lek i koriste elektomehaničke tehnologije za pumpanje ili doziranje kako bi se tečni lek putem cevčice isporučio u iglu i/ili mekanu kanilu koja je supkutanu umetnuta u pacijenta. Jedan primer za takvu pumpu je insulinska infuziona pumpa OMNIPOD<®>koju proizvodi kompanija Insulet Corporation. DETALJAN OPIS
[0008] Sredstvo za isporučivanje tečnosti, prema otelotvorenjima predmetnog otkrića, može da se koristi za isporučivanje terapeutske tečnosti (npr. tečnog leka) pacijentu putem alata za transkutani pristup, kao što je igla/trokar i/ili kanila. Mehanizam za inserciju alata za transkutani pristup može da se koristi za primenu alata za transkutani pristup, na primer, umetanjem i povlačenjem igle/trokara jednim neprekidnim pokretom. Mehanizam za inserciju takođe može da obezbedi rastuću silu za inserciju prilikom pomeranja igle/trokara u smeru insercije. Prema predmetnom pronalasku, sredstvo za isporučivanje tečnosti takođe uključuje mehanizam kvačila koji olakšava punjenje rezervoara i aktivaciju pogonskog mehanizma za pokretanje tečnosti iz rezervoara. U određenim otelotvorenjima, sredstvo za isporučivanje tečnosti može da obuhvata ambulantno sredstvo za infuziju insulina.
[0009] U drugim otelotvorenjima, sredstvo za isporučivanje tečnosti može da se koristi za isporučivanje terapeutske tečnosti pacijentu uz integrisano praćenje, kao što je kontinualno merenje glukoze (CGM). U ovim otelotvorenjima, sredstvo za isporučivanje tečnosti može da obuhvata alat za transkutani pristup konfigurisan za uvođenje test trake za praćenje u pacijentovu kožu, na primer, koristeći jednu ili više igala, kanila i/ili trokara.
[0010] Uz upućivanje na SL.1-6 prikazano je i opisano jedno otelotvorenje sredstva za isporučivanje tečnosti 100. U jednom reprezentativnom otelotvorenju, sredstvo za isporučivanje tečnosti 100 se koristi za supkutano isporučivanje tečnosti, kao što je tečni lek (npr. insulin), nekoj osobi ili životinji. Stručnjacima za ovu oblast će biti jasno da sredstvo za isporučivanje tečnosti 100 može da se koristi za isporučivanje drugih vrsta tečnosti. Sredstvo za isporučivanje tečnosti 100 može da se koristi za isporučivanje tečnosti na kontrolisani način, na primer, u skladu sa profilima isporučivanja tečnosti koji zadovoljavaju bolusne potrebe, kontinuiranu infuziju i varijabilnu brzinu isporučivanja.
[0011] Prema jednom otelotvorenju, sredstvo za isporučivanje tečnosti 100 može da uključuje jednu ili više baterija 110 koje obezbeđuju izvor napajanja, rezervoar za tečnost 130 u kome se tečnost nalazi, pogonski mehanizam za tečnost 150 za pokretanje tečnosti van rezervoara 130, mehanizam za sprovođenje tečnosti 170 koji prima tečnost iz rezervoara 130 i sprovodi tečnost do odredišta putem alata za transkutani pristup 172, i mehanizam za inserciju alata za transkutani pristup 180 za primenu alata za transkutani pristup 172. Sredstvo za isporučivanje tečnosti 100 može da obuhvata štampanu ploču 101 sa kontrolnim kolima za upravljanje sredstvom i postolje 102 koje obezbeđuje mehaničku i/ili električnu vezu između komponenata sredstva za isporučivanje tečnosti 100. Sredstvo za isporučivanje tečnosti 100 takođe može da uključuje kućište 104 koje prima štampanu ploču 101, postolje 102 i komponente 110, 130, 150, 170, 180.

[0015] Jedno otelotvorenje povezujućeg mehanizma sa kosom oprugom 182 uključuje spiralnu torzionu oprugu 181 i prvu i drugu spojnicu 183a, 183b spojene između torzione opruge 181 i prvog kliznog elementa 184. Energija koja je uskladištena u torzionoj opruzi 181 primenjuje silu na spojnice 183 a, 183b, koje primenjuju silu na prvi klizni element 184 radi pomeranja prvog kliznog elementa 184 u smeru umetanja kao i u smeru izvlačenja. Kada se klizni elementi 184, 186 oslobode, energija uskladištena u torzionoj opruzi 181 dovodi do rotacije prve spojnice 183a (npr. u smeru kazaljke na satu, kao što je prikazano), koja primenjuje silu na prvi klizni element 184 preko druge spojnice 183b, što dovodi do pomeranja prvog kliznog elementa 184 sa iglom/tokarom 174 (sa drugim kliznim elementom 186) u smeru umetanja. Tako, kao što je prethodno opisano, kontinualna neprekidna rotacija prve spojnice 183a u smeru kazaljke na satu putem energije torzione opruge 181 omogućava mehanizmu za inserciju alata za transkutani pristup 180 da umetne i izvuče iglu/trokar 174 u jednom neprekidnom pokretu.
[0016] Povezujući mehanizam sa kosom oprugom 182 omogućava da jedna opruga i pokret ostvare umetanje kao i izvlačenje, i relativno je malih dimenzija. Povezujući mehanizam sa kosom oprugom 182 takođe smanjuje statičko naprezanje izazvano zaključavanjem i zadržavanjem kliznih elemenata 184, 186 i obezbeđuje glatkije i ugodnije umetanje igle/trokara zbog načina na koji spojnice 183a, 183b vektorišu sile primenjene na klizne elemente 184, 186. Statičke sile na kliznim elementima 184, 186 su relativno male u položaju pre primene, kada su spojnice 183a, 183b u potpunosti povučene. Kada se započne primena i spojnice 183a, 183b počnu da se produžavaju, sila umetanja raste jer se vektori sile povećavaju u smeru umetanja sa produžavanjem spojnica 183 a, 183b sve dok se ne ostvari maksimalna sila umetanja kada su potpuno ispružene, u srednjem položaju. Zahvaljujući postepenom povećavanju sile umetanja, umetanje i izvlačenje igle/trokara je glatkije, mirnije i bezbolnije.
[0017] Još jedno otelovorenje mehanizma za inserciju 280 je detaljnije prikazano na SL.7-10. Klizni elementi 284, 286 se klizno prihvataju u okvir 290 i pokreće ih povezujući mehanizam sa kosom oprugom 282 koji obuhvata torzionu oprugu 281 i spojnice 283a, 283b. U ovom otelotvorenju, krak brega 292 (npr. koji se proteže sa okvira 290) pomera se na poziciju ispod jednog ili oba klizna elementa 284, 286 kako bi zaključao klizne elemente u povučenom položaju ili položaju pre primene (SL.7). U ovom položaju pre primene, krak brega 292 zadržava se na kliznim elementima 284, 286 pomoću graničnika 296 koji može da se pomera (rotira) kako bi se omogućilo da se krak brega 292 pomeri i oslobodi klizne elemente 284, 286 (SL.8). [0020] Uz upućivanje na SL.11-16, pogonski mehanizam za tečnost 150 prema predmetnom pronalasku koristi mehanizam kvačila 160 da omogući punjenje rezervoara 130 i aktivaciju pogonskog mehanizma za tečnost 150 za izbacivanje tečnosti van rezervoara 130. Pogonski mehanizam za tečnost 150 uključuje prvi element sa navojem u obliku izdužene osovine kao što je pogonski klip sa navojem ili vodeći vijak 152, sa spoljašnjim navojima koji se protežu od klipa 136 koji se nalazi u rezervoaru 130 i zaptiven je zaptivkom 137 na unutrašnjoj površini rezervoara 130. Vodeći vijak 152 i klip 136 mogu biti neodvojiv sklop oblikovan brizganjem plastike. Drugi element sa navojem u obliku izdužene osovine, kao što je cevasta navrtka 154 sa unutrašnjim navojima, putem navoja zahvata vodeći vijak 152 i može da se pokreće pogonskim točkom 156 preko mehanizma kvačila 160.
[0021] Kada je rezervoar 130 prazan (SL.11 i 12), klip 136 je pozicioniran na jednom kraju rezervoara 130 tako da je klip 136 ispružen, a mehanizam kvačila 160 je neaktivan. U nekim otelotvorenjima, rezervoar 130 može biti napunjen tečnošću, naročito insulinom, putem otvaranja ulaznog priključka za rezervoar 130 i pumpanjem insulina pod dovoljnim hidrauličnim pritiskom za povlačenje klipa 136 u rezervoar 130. Kada je rezervoar 130 napunjen i klip 136 se pomera na suprotni (uvučeni) kraj rezervoara 130 (SL.13), mehanizam kvačila 160 ostaje neaktivan kako bi omogućio da cevasta navrtka 154 pređe u izduženi cilindrični otvor (duž pogonske ose) glavčine pogonskog točka 156. Mehanizam kvačila 160 zatim može da se aktivira (SL.
14-16) tako da rotacija pogonskog točka 156 dovodi do toga da mehanizam kvačila 160 okreće cevastu navrtku 154, usled čega vodeći vijak 152 gura klip u rezervoar 130 kako bi se isporučila tečnost iz rezervoara 130. U alternativnim otelotvorenjima, rezervoar 130 može biti napunjen kada je klip 136 već povučen.
[0022] U ilustrovanom otelotvorenju, mehanizam kvačila 160 uključuje oprugu kvačila 162 (npr. spiralnu torzionu oprugu) koja se nalazi u provrtu na jednom kraju pogonskog točka 156, pored rezervoara 130. U ilustrovanom otelotvorenju, pre punjenja rezervoara 130, opruga kvačila 162 zadržana je u napregnutom, neaktivnom položaju putem zasuna opruge 164 koji je u interakciji sa pogonskim točkom 156 (SL.11-13). Nakon što je rezervoar 130 napunjen, opruga kvačila 162 na taj način može da se aktivira rotacijom pogonskog točka 156 sve dok zasun opruge 164 ne oslobodi oprugu kvačila 162 (SL.14) što omogućava da se opruga kvačila 162 oslobodi i stegne cevastu navrtku 154 (SL.15 i 16), nakon čega tečnost može da se dozira iz rezervoara 130 uz kontinualno okretanje pogonskog točka 156.

[0025] Kada se koristi mehanizam kvačila, interakcija između vodećeg vijka i navrtke se vrši pri sklapanju, te tako nije potrebna rotacija da bi navrtka stupala u interakciju sa vodećim vijkom tokom rada sredstva. Ovo smanjuje broj primarnih impulsa putanje tečnosti za pripremu pumpe i osigurava kompletnu i odgovarajuću pripremu putanje tečnosti pre postavljanja na telo. Mehanizam kvačila takođe omogućava promenu koraka navoja za primenu drugih lekova bez potrebe da se menja dizajn navrtke s nagibom koja se koristi u mehanizmima za pogon tečnosti u drugim postojećim pumpama. Komponente mehanizma kvačila se takođe lakše pregledaju od sklopa navrtke s nagibom.

Tako, prva igla/trokar 374 kao i druga igla/trokar 375 mogu istovremeno da se uvedu u pacijenta, naročito kako bi se smanjio bol od uzastopnog umetanja.
