Den foreliggende opfindelse angår N- [ (5R,8S,10R)-2,6-dimeth-ylergolin-8-yl]-2-ethyl-2-methyl-butanamid, en fremgangsmåde til fremstilling deraf, farmaceutiske præparater indeholdende forbindelsen samt anvendelse deraf som et farmaceutikum.
5 GB patentskrift nr. 2 152 507 beskriver en bred klasse af substituerede 8a-acylaminoergoliner, som har prolactin-sekre-tionsinhiberende, luteiniserende hormon-sekretionsinhiberende og apomorphin-antagonistisk virkning.
Det har nu overraskende nok vist sig, at en forbindelse fra 10 denne klasse, som hidtil ikke er blevet specifikt beskrevet, har særligt værdifulde farmakologiske egenskaber. Forbindelsen har kraftig og langvarig neuroleptisk virkning og tolereres godt og har en overraskende lav tilbøjelighed til at give extrapyramidale og især endocrine bivirkninger som 15 indikeret i de nedenfor beskrevne farmakologiske tests.
Den foreliggende opfindelse bygger på denne erkendelse og angår N-[(5R,8S,10R)-2,6-dimethyl-ergolin-8-yl]-2-ethyl-2-methyl-butanamid med formlen I
H <NHCOC(C2H5)2CH3 (οχΎ^3 HN-Lch3 20 i fri baseform eller i syreadditionssaltform.
Den foreliggende opfindelse angår ligeledes en fremgangsmåde til fremstilling af forbindelsen med formlen I, hvilken fremgangsmåde er ejendommelig ved, at 8o;-amino-2,6-dimethyl-ergolin med formlen II

eller et reaktivt derivat deraf, og den resulterende for-5 bindelse med formlen I udvindes i fri baseform eller i syre-additionssaltform.
Fremgangsmåden kan udføres på konventionel måde. Egnede reaktive derivater af forbindelsen med formlen III omfatter fx acylhalogenider, især acylchloridet eller imidazolidet.
10 Omsætning med acylhalogenider kan hensigtsmæssigt udføres i nærværelse af en base såsom triethylamin eller Hiinig-base i et inert opløsningsmiddel såsom methylenchlorid. Omsætning med imidazolidet (vundet fx ved omsætning af forbindelsen med formlen III med Ν,Ν-carbonyldiimidazol) kan hensigtsmæssigt 15 udføres i et inert opløsningsmiddel såsom tetrahydrofuran eller ethanol, fx ved tilbagesvalingstemperatur. Når forbindelsen med formlen III anvendes som sådan, kan omsætningen hensigtsmæssigt udføres i nærværelse af propanphosphonsyrean-hydrid.
20 Udgangsforbindelsen med formlen II er beskrevet i GB patentskrift nr. 2 152 507.
Forbindelsen med formlen I kan udvindes i fri baseform eller i syreadditionssaltform, fx i form af de farmaceutisk acceptable syreadditionssalte deraf. Den frie baseform af for-25 bindeisen med formlen I kan omdannes til syreadditionssalt- DK 169335 B1 3 former på konventionel måde og vice versa. Egnede farmaceutisk acceptable syreadditionssalte omfatter både salte af uorganiske syrer, fx hydrochloridet, og salte af organiske syrer, fx oxalatet eller hydrogenmaleatet. Hydrogenmaleatet 5 er det foretrukne salt.
Forbindelsen med formlen I har farmakologisk virkning og er anvendelig som et farmaceutikum, fx til terapi. Især har forbindelsen med formlen I kraftig og langvarig neuroleptisk virkning, hvilket kan påvises i følgende tests: 10 Forbindelsen med formlen I inhiberer apomorphin-induceret gnavning hos rotter ved en metode, der er baseret på den af P.A. Jansen et al. i Arzneim.-Forsch. (Drug Res.) 10, 1003-1005 (1960) beskrevne:
Grupper på 3-6 rotter (hanner og hunner, 90-160 g, Sprague-15 Dawley, Siiddeutsche Tierfarm, Tuttlingen, Vesttyskland) behandles oralt med testmedikamentet og behandles yderligere efter en forudbestemt tid med 2,0 mg/kg intravenøst apomor-phin-hydrochlorid i vandig opløsning. De anbringes derefter i enkelt-bure, der er foret med crepe-papir. På tidspunkterne 20 10, 20 og 30 minutter efter apomorhinen iagttages hver rotte i 1 minut. Hvis der optræder gnavning i en observationsperiode, bedømmes denne observation som positiv. Der fås på denne måde tre bedømmelser for hver rotte. Den supra-maksimale dosis af apomorphin fremkalder ufravigeligt gnavning hos alle 25 kontroldyr på alle observationstidspunkter. Af det samlede antal observationer per behandlingsgruppe noteres antallet af positive resultater. Den medikamentdosis, som forårsager en 100% inhibering af den apomorphin-inducerede gnavning, anses for at være tærskeldosen. Efter en forbehandlingstid på 3 30 timer ligger tærskeldosen for forbindelsen med formlen I på 0,2 mg/kg peroralt, efter en forbehandlingstid på 6 timer, ligger den også på 0,2 mg/kg peroralt.
Forbindelsen med formlen I bindes yderligere til dopamin-receptorer, hvilket er karakteriseret ved udskiftning af 3H- DK 169335 B1 4 ligander fra deres respektive bindingssteder i hjernevævsho-mogenisat. Affiniteten til D-l-receptorsteder [jævnfør W. Billard et al. i Life Sci., 35, 1885-1893 (1984)] under anvendelse af 3H-(R)-(+)-8-chlor-7-hydroxy-3-methyl-5-phenyl-5 2,3,4,5-tetrahydro-lH-3-benzazepin (3H-SCH 23390) som ligand bestemmes på følgende måde:
Friskt striatalvæv fra kalvehjerne homogeniseres i et 20 gange så stort rumfang Tris-HCl-puffer (50 mM, pH 7,4 ved 25°C) under anvendelse af en Polytron vævshomogenisator.
10 Homogenisatet centrifugeres i 10 minutter ved 50.000 x g (4°C), og supernatanten kasseres. Pellets resuspenderes i samme puffer som ovenfor. Suspensionen inkuberes i 20 minutter ved 37°C og centrifugeres derefter igen som ovenfor anført. De resulterende pellets oplagres frosset ved -20°C, 15 indtil de skal anvendes i bindingsassay. Til bindingsassay resuspenderes pellets i Tris-HCl-puffer (50 mM, pH 7,4 ved 37°C, indeholdende 120 mM NaCl) på en sådan måde, at slutrum-fanget på 2 ml indeholder membraner svarende til ca. 5 mg originalt vævsvægt. 3H-SCH 23390 tilsættes til dannelse af en 20 slutkoncentration på 0,1 nM. Analyserne til bestemmelse af ikke-specifik binding indeholder yderligere 1 μΜ ikke-mærket SCH 23390. Testforbindelsen tilsættes til dannelse af 5-9 forskellige koncentrationer. Prøverne inkuberes i 50 minutter ved 37°C, hvorefter de vakuumfiltreres gennem Whatman GF/B-25 filtre. Filtrene skylles to gange med 5 ml iskold Tris-HCl-puffer. Filtrene analyseres for radioaktivitet ved væskescin-tillationstælling. IC50-værdien (dvs. den koncentration af testmedikamentet, som inhiberer specifik binding af 3H-SCH 23390 med 50%) bestemmes ved lineær regressionsanalyse ud fra 30 Hill-plots. IC-50-værdien for forbindelsen med formlen I er ca. 50 nM.
Forbindelsens høje affinitet for D-2-receptorsteder kan bestemmes ved 3H-spiperon-bindingsforsøg [jævnfør S. Urwyler & D. Coward i Naunyn-Schmiedeberg's Arch. Pharmacol., 335, 35 115-122 (1987)]. Striatal-membraner fra kalv fremstilles som ovenfor beskrevet til 3H-SCH 23390-bindingsforsøget. Pellets DK 169335 B1 5 resuspenderes i 50 mM Tris-HCl-puffer (pH 7,7) indeholdende 120 mM NaCl, hvorved der fås en membrankoncentration svarende til 4 mg originalt vævsvægt per 4 ml analysevolumen. 0,5 μΜ cinanserin tilsættes for at forhindre bindingen af 3H-spi~ 5 peron til 5HT2-receptorer. Koncentrationen af 3H-spiperon er 0,1 nM; prøverne til måling af den ikke-specifikke binding tilsættes yderligere 5 μΜ haloperidol. Prøverne inkuberes i 40 minutter ved stuetemperatur, hvorefter de filtreres, og der udføres væskescintillationstælling som beskrevet ovenfor.
10 Forbindelsen med formlen I har en IC50-værdi på ca. 20 nM.
Forbindelsen med formlen I tolereres godt hos mus op til 100 mg/kg peroralt. Det ikke-toxiske dosisniveau hos hunde efter 4 ugers behandling er 3 mg/kg/dag peroralt.
En typisk uønsket bivirkning af de fleste neuroleptika på de 15 endocrine funktioner er en forøget prolactin-frigørelse.
Forbindelsen med formlen I påvirker kun serumprolactinniveau-erne hos rotter ved høje doser (a 10 mg/kg) bestemt ved følgende testmetode efter subkutan administration [jævnfør E. Fluckiger et al. i Experientia, 34, 1330-1331 (1978)]: 20 OFA-hanrotter med en legemsvægt på ca. 250 g anbringes i forsøgslokalet 24 timer før det pågældende forsøg. De holdes i passende bure med 10 i hvert bur. Efter forsøgsbehandlingen holdes de individuelt. Foder og vand er frit tilgængeligt. Forskellige doser af forbindelsen eller bærestoffet admini-25 streres subkutant til grupper på 5 dyr. Ved dette standardforsøg decapiteres dyrene 4 timer efter behandlingen. Sera fra de enkelte dyr dybfryses, indtil de analyseres. Prolactin måles derefter i alikvoter ved radioimmunoassay. Serumprolac-tinniveauerne udtrykkes i ng/ml ved en prolactinstandard 30 NIAMD-RPrl-RPl. Efter subkutan administration af 10 mg/kg af forbindelsen med formlen I optræder der kun en moderat forøgelse af serumprolactinniveauet hos hanrotter 4 timer efter administrationen.

Ifølge de biologiske teste, som beskrives i GB patentskrift nr. 2 152 507, påvirker de herfra kendte 8a-acylaminoergolin-er imidlertid serumprolactinniveauet allerede ved administration af 0,0005 til 0,5 mg/kg af forbindelserne.
5 Oral administration af forbindelsen med formlen I giver også kun svage virkninger på prolactinniveauet. Testen udføres på følgende måde:
Hanrotter (120-140 g, Sprague-Dawley, Siiddeutsche Tierfarm, Tuttlingen, Vesttyskland) holdes i en 12:12 lys-mørkecyclus 10 (lyset tændes kl. 06.00, slukkes kl. 18.00). Aftenen før forsøget mærkes dyrene til identifikationsformål og sættes i bure i grupper på 4 i makrolonbure (type 3) . Næste morgen får rotterne i hver gruppe 10 mg/kg peroralt testforbindelse eller placebo (normal saltopløsning + 2 dråber HCl) 2, 4, 8, 15 16 eller 24 timer, før de dræbes ved decapitering. Blodprøver opsamles i plastikglas, centrifugeres til dannelse af serum, som derefter oplagres ved -20°C, indtil de analyseres for prolactin. Prolactin i serum måles ved radioimmunoassay ifølge A. Håusler et al. i J. Ultrastruct. Res., 64, 74 20 (1978). Forbindelsen med formlen I sænker signifikant serum prolactinniveauet mellem 4 og 8 timer efter administration af 10 mg/kg peroralt. Efter 16 timer indtræder der en signifikant forøgelse, som senere falder (24 timer) mod normalværdien.
25 Den svage effekt på serumprolactinniveauerne reducerer sandsynligheden for uønskede endocrine bivirkninger, fx galactorrhea eller gynecomasti.
Forbindelsen med formlen I har endvidere farmakologiske egenskaber, der antyder en lav tilbøjelighed til at inducere 30 extrapyramidale bivirkninger. Fx har forbindelsen kun svag cataleptogen aktivitet i den test, der er baseret på den af G. Stille et al. i Arzneim.-Forsch. (Drug res.), 21, 252-255 (1971) angivne test. I denne test får grupper på 4-8 rotter (120-170 g, Sprague-Dawley, hanner og hunner, Siiddeutsche DK 169335 B1 7
Tierfarm, Tuttlingen, Vesttyskland) teststoffet indgivet oralt. På bestemte tidspunkter efter behandlingen undersøges hver rottes catalepsi ved at anbringe forpoterne på en 7 cm høj blok. Den tid, i hvilken dyrene forbliver i denne unatur-5 lige stilling, måles i op til maksimalt 45 sekunder. Tærskeldosis er den endelige dosis, som stadigvæk forårsager en catalepsi-median på > 10 sekunder. Forbindelsen med formlen I har en tærskeldosis på 5 mg/kg peroralt, bestemt under en 8 timers målingsperiode. Dette er en dosis, som er mindst 25 10 gange større end den, som er nødvendig til at antagonisere apomorphin-induceret gnavning. Yderligere viser forbindelsen dopaminagonist-lignende egenskaber, hvilket vises ved, at den fremkalder motoriske stimulerende virkninger på dyr med læderet dopaminerg neurotransmission. Fx inducerer forbindel-15 sen med formlen I i rotter med en unilateral 6-0HDA-(6-hy-droxydopamin)-induceret læsion i substantia nigra [dvs. Ungerstedt-rotte, jævnfør J.M. Vigouret et al. i Pharmacology, 16, (suppl. 1), 156-173 (1978)] en langvarig roterende adfærd, der er modsat rettet læsionen, ved relativt lave 20 doser (0,5 mg/kg peroralt: 950 rotationer inden for 7 timer).
På grund af dens høje affinitet for dopamin D-l- og D-2-receptorsteder og dens inhiberende virkning på apomorphin-induceret gnavning og de øvrige tests som nævnt ovenfor er forbindelsen med formlen I anvendelig som et godt tålt neuro-25 leptikum, fx til behandling af psykotiske lidelser, fx schi-zofreni, psykose induceret af antiparkinson-medikation eller aldersbetinget psykiatriske lidelser, der hyppigt er knyttet til demens (paranoia). Yderligere er forbindelsen med formlen I, i lyset af de resultater, der fremgår af Ungerstedt-rotte-30 testen, også anvendelig til behandling af schizofreni, hvor der udvises negative symptomer. På grund af den dopaminago-nist-lignende virkning, som er vist i Ungerstedt-rottetesten, er forbindelsen yderligere nyttig til behandling af Parkinson's syge.
35 Til disse anvendelser vil den passende dosis naturligvis variere alt afhængigt af fx administrationsmåden og arten og DK 169335 B1 8 alvoren af den lidelse, der behandles. En indikeret daglig dosis ligger normalt i området fra ca. l. til ca. 50 mg, fortrinsvis fra 5 til 40 mg, af forbindelsen, bekvemt administreret i deldoser op til 4 gange dagligt, i enhedsdosisform 5 eller i retard-form.
Forbindelsen med formlen I kan administreres i fri baseform eller i farmaceutisk acceptabel syreaddditionssaltform.
Sådanne salte kan fremstilles på konventionel måde og har det samme aktivitetsniveau som den frie base.
10 Den foreliggende opfindelse angår ligeledes farmaceutiske præparater, som er ejendommelige ved, at de indeholder forbindelsen med formlen I eller et farmaceutisk acceptabelt syreadditionssalt deraf sammen med mindst ét farmaceutisk bærestof eller fortyndingsmiddel.
15 Forbindelsen med formlen I kan administreres ad en hvilken som helst konventionel vej, især enteralt, fortrinsvis oralt, fx i form af tabletter eller kapsler, eller parenteralt, fx i form af injektionsopløsninger eller -suspensioner. Til fx oral administration, fx i form af tabletter eller kapsler, 20 kan forbindelsen med formlen I eller et farmaceutisk acceptabelt syreadditionssalt deraf blandes med konventionelle farmaceutisk acceptable excipienser, fx inerte fortyndings-midler såsom lactose, mannitol, calciumsulfat, mikrokrystal-linsk cellulose; sprængmidler, fx stivelse, natriumcarboxyme-25 thylcellulose, natriumcarboxymethylstivelse, algininsyre, crospovidon,; bindemidler såsom cellulosederivater (methyl-, hydroxymethyl-, hydroxypropylmethyl-), povidon, gelatine; smøremidler, fx siliciumdioxid, stearinsyre, magnesium- eller calciumstearat; hydrogenerede olier såsom riccinusolie, 30 glycerolestere, fx palmitostearat og/eller smagsstoffer, farvestoffer og sødemidler. Tabletterne kan være ucoatede eller coatede i henhold til kendte teknikker for at forsinke desintegration og absorption i mavetarmkanalen og derved give en vedvarende virkning i en længere periode. Til parenteral DK 169335 B1 9 administration kan der anvendes egnede sterile vandige eller ikke-vandige opløsninger eller suspensioner.
Enhedsdosisformer indeholder fx fra ca. 0,25 til ca. 25 mg af forbindelsen med formlen I eller et farmaceutisk acceptabelt 5 syreadditionssalt deraf.
De farmaceutiske præparater kan fremstilles i henhold til konventionelle teknikker.
Til fremstilling af tabletter kan forbindelsen med formlen I blandes med lactose og granuleres med vand, 0,5% natriumalginat-10 eller 5% hydroxypropylmethylcellulose-opløsning. Det tørrede granulat komprimeres til tabletter i nærværelse af ca. 20% majsstivelse og 1% magnesiurastearat. På denne måde fås fx tabletter med nedenstående sammensætning:

Disse tabletter, som er forsynet med en delekasrv, kan admini-25 streres oralt i en dosis på fra en halv til 1 tablet 1-4 gange dagligt.
Kapsler kan indeholde det aktive middel alene eller i blanding med en inert excipiens i fast form, fx som nævnt ovenfor.
Kapsler indeholdende de nedenfor angivne bestanddele kan 30 fremstilles ved konventionelle teknikker og administreres i en dosis på 1 kapsel 1-4 gange dagligt.

Nedenstående injektionsopløsning formuleres med den indikerede mængde aktivt middel under anvendelse af konventionelle 10 teknikker. Injektionsopløsningen er egnet til administration 1 gang dagligt.

Opløsningerne kan filtreres gennem et 0,2 μιη sterilt filter og fyldes på ampuller under aseptiske forhold. Ampullerne fyldes med carbondioxidgas.
Den foreliggende opfindelse angår ligeledes en forbindelse 25 med formlen I og farmaceutisk acceptable syreadditionssalte deraf til anvendelse som farmaceutikum, fx til anvendelse som neuroleptikum og til anvendelse som et antiparkinson-medika-ment.
Opfindelsen angår endvidere en forbindelse med formlen I og 30 et farmaceutisk acceptabelt syreadditionssalt deraf til DK 169335 B1 11 fremstilling af et farmaceutisk præparat til anvendelse i behandling af psykotiske lidelser.
Opfindelsen belyses nærmere i nedenstående eksempel, hvor alle temperaturer er ukorrigerede, [a]D20-værdierne er lige-5 ledes ukorrigerede.
EKSEMPEL
N- [ (5R, 8S,10R) -2, 6-dimethyl-ergolin-8-yl] -2-ethyl-2-methyl -butanamid
En suspension af 68 g 2,6-dimethyl-8a-aminoergolin i 1,5 10 liter dichlormethan forafkøles til 4°C og behandles med 78 ml triethylamin. Blandingen behandles under omrøring i løbet af 25 minutter dråbevis med 40,5 g 2-methyl-2-ethyl-butyryl-chlorid. Blandingen omrøres i 30 minutter, hældes ud i 3 liter vand, og den organiske fase tørres (Na2S04) og kon-15 centreres. Remanensen chromatograferes på 800 g silicagel under anvendelse af dichlormethan/methanol (98:2) til dannelse af titelforbindelsen, som omkrystalliseres af diethyl-ether, smeltepunkt 191-192°C.
Til fremstilling af hydrogenmaleatet opløses 57,3 g af basen 20 i 500 ml ethanol, og blandingen behandles med en opløsning af 18,09 g maleinsyre i 250 ml ethanol. Den begyndende krystallisation færdiggøres ved afkøling til 4°C. Krystallerne filtreres og tørres, hvorved hydrogenmaleatet af titelforbindelsen fås, smeltepunkt 232-233°C. [a]D20 = -14,5° (c = 25 1,0 i dimethylformamid).

1. Fremgangsmåde til fremstilling af N-[(5R,8S,10R)-2,6-dimethyl-ergolin-8-yl]-2-ethyl-2-methyl-butanamid med formlen I ^nhcoc(C2h5)2ch3 'JL NCH,
2. Forbindelse med formlen I og syreadditionssalte deraf. DK 169335 B1 13
3. Forbindelse med formlen I ifølge krav 2, kendetegnet ved, at den er i form af et farmaceutisk acceptabelt syreadditionssalt deraf.
4. Forbindelse ifølge krav 3, 5 kendetegnet ved, at den er i form af hydrogen-maleatet deraf'.
5. Forbindelse ifølge krav 2 og farmaceutisk acceptable syreadditionssalte deraf til anvendelse som farmaceutikum.
5. I (oTJh HN-LcH3 eller et syreadditionssalt deraf, kendetegnet ved, at 8a-amino-2,6-dimethylergolin med formlen II H ^NH2 11 HN-LcHj 10 omsættes med forbindelsen med formlen III CH3(C2H5)2CCOOH III eller et reaktivt derivat deraf, og den resulterende forbindelse med formlen I udvindes i fri baseform eller i syre-15 additionssaltform.
6. Forbindelse ifølge krav 5 til anvendelse som neurolepti-10 kum.
7. Forbindelse ifølge krav 5 til anvendelse som et antipar-kinsonmedikament.
8. Farmaceutisk præparat, kendetegnet ved, at det indeholder en forbindel-15 se ifølge krav 2 eller et farmaceutisk acceptabelt syreadditionssalt deraf sammen med et farmaceutisk bærestof eller fortyndingsmiddel.
9. Anvendelse af forbindelsen ifølge krav 2 eller et farmaceutisk acceptabelt syreadditionssalt deraf til fremstilling 20 af et farmaceutisk præparat til anvendelse i behandling af psykotiske lidelser.
