Una forma humanizada de 4D5 conocida como trastuzumab (comercializado como Herceptin® por Genentech, Inc.) fue desarrollada y aprobada para tratar cánceres relacionados con la expresión en exceso o amplificación genética de HER2/neu, que incluyen el cáncer de mama. (Cobleigh et al. (1999) J. Clin. Oncol. 17:2639-2648). El trastuzumab 5 inhibe la división mediada por APMA de HER2/neu en las porciones ECD y p95 in vitro y se cree que funciona in vitro a través de diferentes mecanismos, que incluyen la posible inhibición de la diseminación de HER2/neu. 15 La interacción de complejos de antígenos/anticuerpos con células del sistema inmunitario tiene como consecuencia una gran variedad de respuestas, que van de funciones efectoras tales como citotoxicidad dependiente de anticuerpos, desgranulación de mastocitos y fagocitosis hasta señales inmunomoduladoras tales como la regulación de proliferación de linfocitos y secreción de anticuerpos. Todas estas interacciones se inician mediante la unión del dominio de Fc de anticuerpos o complejos inmunitarios con los receptores de Fc, que son receptores especializados
20 de la superficie celular de células hematopoyéticas. La diversidad de respuestas celulares desencadenadas por anticuerpos y complejos inmunitarios resulta de la heterogeneidad estructural de los receptores de Fc. Los receptores de Fc comparten dominios de unión de ligandos relacionados estructuralmente que quizá intervengan en la señalización intracelular.
Los receptores de Fc, miembros de la superfamilia de los genes de inmunoglobulina de las proteínas, son

Cada receptor de Fc (“FcR”) es una glicoproteína de membrana integral, que tiene dominios extracelulares relacionados con un conjunto C2 de dominios relacionados con inmunoglobulina, un único dominio transmembrana y un dominio intracitoplasmático de longitud variable. Existen cuatro FcR conocidos, denominados FcRI (CD64),
5 FcRII (CD32), FcRIII (CD 16) y FcRIV. Los receptores se codifican por distintos genes; sin embargo, la amplia homología entre los miembros de esta familia sugiere que surgen de un progenitor común, quizá mediante duplicación genética.
Tanto las señales de activación como las inhibidoras se transducen a través del FcRs después de la unión. Estas funciones diametralmente opuestas resultan de las diferencias estructurales entre las diferentes isoformas de los 10 receptores. Dos dominios distintos dentro de los dominios citoplasmáticos de señalización del receptor denominados motivos de activación de inmunorreceptores a base de tirosina (ITAMs) o motivos inhibidores de inmunorreceptores a base de tirosina (ITIMS) son responsables por las diferentes respuestas. El reclutamiento de diferentes enzimas citoplasmáticas a estas estructuras determina el resultado de las respuestas celulares mediadas por FcR. Los complejos de FcR que contienen ITAM incluyen FcRI, FcRIIA, FcRIIIA y FcRIV, mientras que los complejos que
15 contienen ITIM solo incluyen FcRIIB.
El FcRI muestra alta afinidad para la región constante del anticuerpo y especificidad restringida para los isotipos (Hulett y Hogarth (1994) Adv Immunol 57:1-127). Las proteínas FcRII son glicoproteínas de membrana integrales de 40 KDa que solo se unen con IgG compleja debido a una baja afinidad con la Ig monomérica (106 M-1). Este receptor es el más expresado en FcR, está presente en todas las células hematopoyéticas, que incluyen monocitos, 20 macrófagos, linfocitos B, células NK, neutrófilos, mastocitos y plaquetas. El FcRII solo tiene dos regiones similares a inmunoglobulina en su cadena de unión con inmunoglobulina y, por lo tanto, una afinidad mucho menor para IgG que el FcRI. Existen tres genes de FcRII humanos conocidos (FcRII-A, FcRII-B, FcRII-C), todos los cuales se unen con IgG en agregados o complejos inmunitarios. Los neutrófilos humanos expresan el gen de FcRIIA. El gen de FcRIIB se expresa en linfocitos B; su dominio extracelular es 96 % idéntico a FcRIIA y se une a complejos de

Diferencias definidas dentro de los dominios citoplasmáticos de FcRII-A y FcRII-B crean dos respuestas funcionalmente heterogéneas para la unión de receptores. La isoforma FcRII-A inicia la señalización intracelular que produce la activación celular, tal como fagocitosis y estallido respiratorio, mientras que la isoforma FcRII-B inicia señales inhibidoras, por ejemplo, inhibe la activación de linfocitos B. humanizados). La unidad estructural básica de las inmunoglobulinas de origen natural (por ejemplo, IgG) es un tetrámero que tiene dos cadenas ligeras y dos cadenas pesadas. Por lo general, la inmunoglobulina de origen natural se expresa como una glicoproteína de aproximadamente 150 KDa, aunque también puede producirse IgG en una forma no glicosilada. La porción amino-terminal (“N”) de cada cadena incluye una región variable de aproximadamente 100 a 110 o más aminoácidos responsables principalmente del reconocimiento de antígenos. La porción carboxi-terminal (“C”) de cada cadena define una región constante, con cadenas ligeras que tienen un solo dominio constante y cadenas pesadas que por lo general tienen tres dominios constantes y una región en bisagra. Por lo tanto, la estructura de las cadenas ligeras de una molécula de IgG natural es N-VL-CL-C y la estructura de las cadenas pesadas de IgG es N-VH-CH1-H-CH2-CH3-C (donde H es la región en bisagra). Las regiones variables de una molécula de IgG consisten en las regiones de determinación de complementariedad (CDR), que contienen los restos en contacto con el antígeno y segmentos distintos de CDR, denominados segmentos de estructura, que mantienen la estructura y determinan la posición de los bucles de CDR. Por lo tanto, los dominios de VL y VH tienen la estructura N-FR1-CDR1-FR2-CDR2-FR3-CDR3-FR4-C.
Como se utiliza en la presente memoria, el término ácido nucleico “heterólogo” indica ADN, ARN, etc. introducido en una célula hospedadora. El ácido nucleico puede derivarse de cualquiera de una diversidad de fuentes que incluyen ADN genómico, ARNm, ADNc, ADN sintético y fusiones o combinaciones de estos. El ácido nucleico puede incluir un polinucleótido de la misma célula o tipo celular que la célula hospedadora o célula receptora o un polinucleótido de un tipo celular diferente, por ejemplo, de un mamífero o planta, y puede, de manera opcional, incluir genes marcadores o de selección, por ejemplo, genes con resistencia a antibióticos, genes con resistencia a la temperatura, etc.
La expresión “región en bisagra” por lo general se define como una región que se extiende desde Glu216 a Pro230 de IgG1 humana. Las regiones en bisagra de otros isotipos de IgG pueden alinearse con la secuencia de IgG1 al colocar el primer y último resto de cisteína que forma los enlaces de S-S entre la cadena pesada en las mismas posiciones.
Como se utiliza en la presente memoria, el término “humanizado” tiene su significado usual en la técnica. En términos generales, la humanización de un anticuerpo no humano implica la sustitución de las secuencias de CDR de regiones VL y VH de inmunoglobulina no humana en las regiones de marco conservado humanas. Además, como se utiliza en la presente memoria, anticuerpos “humanizados” pueden comprender substituciones y mutaciones adicionales en las regiones CDR y/o de marco conservado introducidas para incrementar la afinidad o para otros propósitos. Por ejemplo, las substituciones de restos de marco conservado no humanos en la secuencia humana pueden incrementar la afinidad. Los dominios variables resultantes tienen secuencias de CDR no humanas y secuencias de marco conservado derivadas de una secuencia o secuencias de marco conservado de anticuerpos humanas o una secuencia de consenso humana. Una diversidad de diferentes regiones de marco conservado humanas pueden utilizarse solas o en combinación como base para un anticuerpo humanizado.
Como se utiliza en la presente memoria, la expresión “agente inmunomodulador” y variaciones de la misma se refieren a un agente que modula un sistema inmunitario hospedador. En ciertas realizaciones, un agente inmunomodulador es un agente inmunosupresor. En algunas otras realizaciones, un agente inmunomodulador es un agente inmunoestimulador. Los agentes inmunomoduladores incluyen, pero sin limitación, moléculas pequeñas, péptidos, polipéptidos, proteínas de fusión, anticuerpos, moléculas inorgánicas, agentes miméticos y moléculas orgánicas.
Como se utiliza en la presente memoria, la expresión “unión inmunoespecífica”, se refiere a la unión específica mostrada entre un anticuerpo y el epítopo que reconoce. Tal unión normalmente mostrará una KD de por lo menos aproximadamente 0,1 mM, con mayor frecuencia por lo menos aproximadamente 1 μM, preferentemente por lo menos aproximadamente 0,1 μM o menos, y con mayor preferencia 0.01 μM o menos. Preferentemente, los anticuerpos de la invención se unen de manera inmunoespecífica con proteínas con alta afinidad (por ejemplo, KD baja).
Un anticuerpo que se une de manera inmunoespecífica con un antígeno puede unirse con otros péptidos o polipéptidos con baja afinidad, según se determina mediante, por ejemplo, inmunoensayos, BIAcore, u otros ensayos conocidos en la técnica. Preferentemente, las moléculas que se unen de manera específica con un antígeno no reaccionan de manera cruzada con otras proteínas. Las moléculas que se unen de manera específica con un antígeno pueden identificarse, por ejemplo, mediante inmunoensayos, BIAcore u otras técnicas conocidas por los expertos en la técnica.

B2. Otras Modificaciones
Las variantes polipeptídicas (en especial las variantes de anticuerpo) de la presente invención incluyen análogos y derivados que se modifican, por ejemplo, mediante la unión covalente de cualquier tipo de molécula, siempre y cuando dicha unión covalente permita que el anticuerpo conserve su inmunoespecificidad de unión con epítopos.
