Description
DESCRIZIONE dell'invenzione industriale dal titolo: "Stent per angioplastica con azione antagonista della restenosi, relativo corredo e componenti"
TESTO DELLA DESCRIZIONE
La presente invenzione si riferisce agli stent per angioplastica.
Con tale denominazione si intendono indicare in generale i dispositivi destinati ad un'applicazione endolumlnale (ad esempio all'interno di un vaso sanguigno), attuata di solito tramite cateterismo, con successivo dispiegamento in loco al fine di attuare un'azione di sostegno locale del lume.
Per una generale rassegna sugli stent vascolari si può fare utilmente riferimento all'opera "Textbook of Interventional Cardiology" a cura di Eric J. Topol, W. B. Saunders Company, 1994, ed in particolare alla sezione IV del voi. Il intitolata "Coronary stenting".
All'argomento è stato anche dedicato un numero elevato di documenti brevettuali, così come testimoniato, ad esempio, da EP-A-0 806 190, EP-A-0 850 604, EP-A-0 847 766, EP-A-0 857 470, EP-A-0 875 215, EP-A-0 895 759, EP-A-0895 760.
L'utilizzazione clinica di tali dispositivi, che ha avuto nel corso degli ultimi anni un notevole sviluppo, deve confrontarsi con l'esigenza di assicurare un'effettiva azione antagonista nei confronti del fenomeno correntemente denominato restenosi. Si tratta del fenomeno (legato al manifestarsi di meccanismi fisiologici peraltro non ancora del tutto chiariti) in base al quale il sito stenotico riaperto per effetto dell'impianto dello stent tende gradualmente a richiudersi, il più delle volte per effetto di una graduale crescita tissutale.
Per affrontare questo problema sono state proposte diverse soluzioni che prevedono essenzialmente lo svolgimento, a livello locale, di un'azione antagonista nei confronti dei fenomeni posti alla base della restenosi. In particolare sono state studiate diverse soluzioni che prevedono un rilascio localizzato di farmaci ovvero un uso localizzato di sorgenti radioattive.
Queste soluzioni, e in particolare quelle basate sul rilascio localizzato e controllato di farmaci, si trovano a loro volta a dover affrontare il problema di conseguire in modo efficace la localizzazione in corrispondenza del sito di impianto dello stent. Nel caso degli agenti destinati ad essere rilasciati da parte dello stent è importante evitare che la maggior parte degli agenti, invece di essere rilasciati in modo graduale nella parete del vaso, vengano dilavati in tempi abbastanza brevi dal flusso sanguigno che passa attraverso il lume dello stent. Ciò è particolarmente importante poiché le esperienze cliniche dimostrano che i meccanismi di reazione del vaso sottoposto all'impianto dello stent si manifestano a livello di reazione da parte della parete del vaso stesso in un intervallo di tempo che tipicamente va da 1 a 6 mesi dall'impianto .
La presente invenzione si prefigge lo scopo di fornire una soluzione suscettibile di risultare migliorativa rispetto alle soluzioni sinora sperimentate sotto diversi punti di vista.
In primo luogo si desidera poter conseguire un sicuro collegamento (cosiddetto ancoraggio) degli agenti attivi rispetto allo stent, ed in particolare un collegamento tale da non risultare influenzato né dal movimento di dilatazione dello stent, caratteristico dell'intervento di impianto, né da eventuali trattamenti superficiali (ad esempio deposito di uno strato superficiale di materiale carbonioso biocompatibile) ai quali lo stent è stato eventualmente sottoposto.
In secondo luogo si desidera poter arrivare alla realizzazione di un complesso di elementi suscettibili di fungere da vera e propria "macchina di rilascio" controllato degli agenti antagonisti della restenosi. Questo soprattutto per quanto riguarda la possibilità di controllare con precisione la cinetica di rilascio, con l'ulteriore possibilità di controllare in modo selettivo, anche nel tempo, il rilascio di agenti diversi.
Ancora, si desidera fare in modo che le funzioni sopra specificate possano essere attuate senza riflettersi in modo negativo sugli altri aspetti caratteristici della funzionalità dello stent, ad esempio evitando che, sia al momento dell'impianto, sia in un momento successivo, il lume dello stent possa essere soggetto a fenomeni di ostacolo al libero flusso del sangue.
Secondo la presente invenzione, tale scopo viene raggiunto grazie ad uno stent per angioplastica avente le caratteristiche richiamate in modo specifico nelle rivendicazioni che seguono. L'invenzione riguarda anche il relativo corredo (kit) ed i componenti che veicolano gli agenti antagonisti della restenosi
L'invenzione verrà ora descritta, a puro titolo di esempio non limitativo, con riferimento ai disegni annessi, nei quali:
- la figura 1 è una generale vista in prospettiva, con alcune parti rimosse per chiarezza di illustrazione, di uno stent per angioplastica realizzato secondo l'invenzione,
- la figura 2 è una vista in sezione secondo la linea II-II della figura 1, riprodotta in scala ingrandita, e
- le figure 3 a 7 illustrano in maggior dettaglio le caratteristiche di alcuni degli elementi componenti lo stent rappresentato nella figura 1.
Nella figura 1 il riferimento numerico 1 indica nel suo complesso un cosiddetto stent per angioplastica.
Per una generale identificazione delle modalità di impiego e delle caratteristiche realizzative di un tale dispositivo di impianto si rinvia alla documentazione citata nella parte introduttiva della presente descrizione.
A titolo di sintesi, si ricorderà che lo stent 1 è di solito realizzato - come struttura di base -sotto forma di un corpo dall'inviluppo tubolare con una lunghezza complessiva di solito compresa fra alcuni millimetri ed alcune decine di millimetri ed uno spessore di parete {presentante di solito una struttura aperturata a maglie od anse) dell'ordine, ad esempio, di alcuni centesimi di millimetro. Tutto ciò in vista del possibile inserimento in un lume (quale un vaso sanguigno) in un sito dove si vuole rimediare ad un fenomeno di stenosi. Lo stent viene collocato in sito, ad esempio tramite cateterismo, realizzando quindi un'espansione radiale a partire da un diametro di introduzione dell'ordine, ad esempio, di 1 mm ad un diametro espanso, ad esempio, dell'ordine di 3-5 mm. Nella condizione espansa, lo stent realizza un'azione di sostegno del lume eliminando la stenosi. Il diametro esterno in condizioni di contrazione radiale viene scelto in modo da consentire l'introduzione dello stent nel lume sottoposto al trattamento, mentre il diametro espanso corrisponde in linea di massima al diametro che si vuole stabilire e mantenere nel lume una volta eliminata la stenosi.
Sebbene l'applicazione principale dello stent descritto sia riferita al trattamento di vasi sanguigni, è senz'altro prevedibile (e dunque compresa nell'ambito dell'invenzione) l'applicazione in funzione di elemento di sostegno di un qualunque lume presente nel corpo umano o animale.
Per quanto riguarda le modalità e i criteri che consentono di dispiegare (ossia di espandere in situ) lo stent, la soluzione attualmente più diffusa è quella di ricorrere ad un cosiddetto catetere a palloncino: lo stent viene disposto attorno al palloncino del catetere in condizioni contratte ed il palloncino viene fatto espandere una volta che lo stent è stato portato sul sito di impianto (si veda, a titolo di esempio, la figura 1, dove il profilo del palloncino del catetere, indicato con C, è stato schematicamente rappresentato con linea a tratti). Sono peraltro ipotizzabili anche soluzioni diverse, quale quella di ricorrere a materiali superelastici che, una volta rimossi gli elementi di contenimento destinati a conservare lo stent in condizione contratta sino al raggiungimento del sito di impianto, portano all'espansione dello stent. In aggiunta o in alternativa è stato anche ipotizzato il ricorso, per la realizzazione dello stent, a materiali presentanti una cosiddetta "memoria di forma", così da conseguire l'espansione radiale nella posizione di impianto.
Di solito (per più precise indicazioni si rinvia alla documentazione bibliografica e brevettuale citata nell'introduzione della descrizione) lo stent viene realizzato con un materiale metallico in grado di conciliare due esigenze fondamentali per l'applicazione, ossia la capacità di deformarsi plasticamente durante la fase di espansione e la capacità di resistere, conservando la forma espansa, alle sollecitazioni che tenderebbero a fare richiudere lo stent.
Questi aspetti di natura tecnologica non verranno trattati in dettaglio nella presente descrizione in quanto di per sé non rilevanti ai fini della comprensione della realizzazione dell’invenzione. Ciò vale essenzialmente anche per la tecnologia di realizzazione dello stent (realizzazione a partire da uno sbozzo tubolare continuo, realizzazione a partire da un corpo nastriforme con successiva chiusura a tubo, realizzazione a partire da un filo metallico sagomato, ecc.).
Ai fini dell'illustrazione dell'invenzione, la struttura dello stent, indicata nel complesso con 2, può essere semplicemente identificata con un corpo tubolare suscettibile di essere selettivamente espanso in senso radiale secondo i criteri descritti in precedenza. Detto altrimenti, la soluzione secondo la presente invenzione risulta di fatto "trasparente" o comunque non influenzata in modo apprezzabile dalle specifiche caratteristiche geometriche, di trattamento e di tecnologia di realizzazione della struttura di base 2 dello stent.
Prima di passare ad illustrare in maggior dettaglio le caratteristiche dello stent 1 è utile riferirsi alle figure 3 a 7, che rappresentano alcuni dei componenti utilizzati per la realizzazione dello stent in questione.
In particolare le figure 3 a 5 illustrano, in alcune possibili varianti di attuazione, strutture 3 del tipo correntemente denominato "nanoparticelle".
Con tale denominazione si indicano in generale corpuscoli di forma sferica o sostanzialmente sferica aventi dimensioni diametrali tipicamente dell'ordine delle centinaia di nanometri.
Le nanoparticelle 3 comprendono di solito un nucleo 3a circondato da un involucro esterno 3b. La figura 5 fa vedere che il nucleo 3a e/o l'involucro 3b possono presentare una struttura non unitaria, ossia una struttura multipla (ad esempio con la presenza di più nuclei o sottonuclei) e/o stratificata, anche con formulazioni diverse da elemento ad elemento. In più è anche ipotizzabile che il nucleo o i nuclei 3a, invece di trovarsi in posizione sostanzialmente centrale, si trovino in posizione eccentrica rispetto all'involucro 3b. Nel seguito della presente descrizione si farà comunque riferimento - per semplicità - a nuclei 3a (e 4a) e ad involucri 3b con struttura sostanzialmente uniforme ed omogenea, sostanzialmente concentrici fra loro.
Il nucleo 3a è costituito, per quanto interessa ai fini dell'attuazione della presente invenzione, da un qualunque agente in grado di svolgere una funzione antagonista nei confronti della restenosi per effetto di un'azione di rilascio localizzato e/o penetrazione selettiva nella parete del vaso sottoposto all'impianto dello stent.
Tipicamente, il nucleo 3a può essere costituito, ad esempio, da un farmaco o da un complesso di farmaci con azione antinfiammatoria, antimitotica e/o promotrice di processi riparatori della parete del vaso, suscettibili di mitigare o evitare le reazioni alla base del processo di restenosi. In ogni caso, la specifica scelta dell'agente e/o dei relativi complessi non costituisce oggetto specifico della presente invenzione.
Quanto detto sopra vale nella sostanza anche per quanto riguarda la realizzazione dell'involucro 3b. Quest'ultimo è costituito, per quanto interessa ai fini dell'attuazione della presente invenzione, da una qualunque sostanza suscettibile di essere definita come "bioerodibile". Con tale definizione si intende qui indicare qualunque sostanza che -nel contesto della presente invenzione (ossia quello dell'impianto in un lume) - è suscettibile di consumarsi e/o di assumere o dimostrare una morfologia porosa o comunque tale da consentire la diffusione verso l'esterno della sostanza o delle sostanze comprese nel nucleo 3a. Le caratteristiche di bioerodibilità descritte si accompagnano tipicamente con caratteristiche di biocompatibilità e di biodegradabilità, importanti nell’applicazione specifica.
Una possibile scelta per la realizzazione dell'involucro 3b è quella di ricorrere, come materiale costituente, a sostanze quali, ad esempio, polietilenglicole (PEG) o acido polilattico-glicolico (PLGA). La relativa tecnologia di produzione è da ritenersi nota.
La cinetica di rilascio corrispondente può essere controllata (soprattutto per quanto riguarda il tempo necessario per ottenere la diffusione verso l'esterno degli agenti contenuti nel nucleo 3a) agendo, singolarmente o in combinazione, su parametri quali:
- la composizione del nucleo 3a,
- la composizione dell'involucro 3b,
- lo spessore dell'involucro 3b,
- la collocazione relativa del nucleo 3a rispetto all'involucro 3b,
- la struttura (omogenea, plurima e/o stratificata) del nucleo 3a, e
- la struttura (omogenea, plurima, stratificata e/o porosa) dell'involucro 3b.
Le figure 3 e 4 illustrano, a puro titolo di esempio, la possibilità di realizzare due nanoparticelle 3 presentanti nuclei 3a con dimensioni circa corrispondenti fra loro in unione a involucri 3b di spessore diverso.
In ogni caso, i criteri e le modalità, nonché le tecnologie inerenti alla realizzazione di nanoparticelle quali le nanoparticelle 3 rappresentate nelle figure 3 a 5 devono ritenersi del tutto noti nella tecnica.
Quanto detto sopra vale nella sostanza anche per quanto riguarda la possibilità di associare nanoparticelle 3 quali quelle rappresentate nelle figure 3 a 5 (identiche o diverse fra loro) a fibre 4 costituite anch'esse di preferenza da materiali bioerodibili quali i materiali citati in precedenza.
La realizzazione delle fibre 4, nonché l'associazione alle stesse delle nanoparticelle 3 corrispondono a soluzioni note. In particolare, la realizzazione di microfibre sotto forma di membrane a fibra cava è ben nota ed affermata a livello tecnologico nella realizzazione di dispositivi quali ad esempio i dializzatori o gli ossigenatori del sangue .
L'esame delle figure 6 e 7 fa vedere che una microfibra 4 del tipo considerato può essere utilizzata come contenitore per ricevere al suo interno le nanoparticelle 3 (figura 6) e/o come supporto (erodibile) per le nanoparticelle 3 e/o ulteriori nuclei 4a (identici o diversi fra loro) sostanzialmente equivalenti ai nuclei 3a delle nanoparticelle 3 per quanto riguarda la possibilità di convogliare agenti antagonisti della restenosi (figura 7).
Anche se non esplicitamente illustrata nei disegni allegati, è anche ipotizzabile, e dunque ricompresa nell'ambito della presente invenzione, la soluzione di utilizzare, almeno localmente nell'ambito dello stent 1, fibre 4 che non prevedono la disposizione di nanoparticelle 3 nel lume delle fibre 4 stesse. In questo caso, il fatto che le fibre 4 presentino caratteristiche di fibra cava non risulta determinante, per cui è anche ipotizzabile il ricorso a fibre 4 presentanti struttura compatta, dunque prive di una cavità assiale.
In modo simmetrico, con riferimento allo schema della figura 6, è ipotizzabile la soluzione di utilizzare fibre 4 di un materiale non bioerodibile fungenti esclusivamente da contenitori per nanoparticelle 3 disposte nel loro lume e consentendo tuttavia la diffusione degli agenti convogliati dai nuclei 3a delle nanoparticelle 3 realizzando le pareti delle fibre con struttura porosa.
A titolo di riferimento dimensionale, le fibre 4 illustrate nei disegni allegati possono presentare dimensioni diametrali tipicamente comprese fra 30 e 100 pm circa, con spessori di parete tipicamente compresi fra 10 e 20 pm, dunque con corrispondenti dimensioni delle cavità assiali tipicamente comprese fra 10 e 60 pm.
La figura 1 e la figura 2 illustrano come le fibre 4 descritte in precedenza possono essere utilizzate per associare alla struttura di base 2.dello stent 1 una o più falde di fibre 4 , eventualmente interallacciate con la struttura 2 sfruttando la configurazione aperturata di quest'ultima.
La disposizione di avvolgimento (o in generale di collocazione) delle fibre 4 sulla struttura 2 deve naturalmente tenere conto del fatto che le fibre 4 vengono normalmente disposte sullo stent 1 quando lo stesso si trova in posizione radialmente contratta e non devono quindi risultare danneggiate o alterate nella loro funzionalità per effetto del movimento di espansione radiale tipico dell'intervento di impianto dello stent. In altre parole, le fibre 4 vengono associate alla struttura 2 in condizioni sostanzialmente non ostative del movimento di dilatazione caratteristico dell'impianto dello stent.
Questo risultato può essere conseguito in modi diversi.
Nell'esempio di attuazione qui illustrato (che - si sottolinea ancora una volta - è tale), le fibre 4 sono state avvolte sulla struttura dello stent ricorrendo a procedimenti e dispositivi noti (si veda, ad esempio, US-A-4952 312) in modo tale da far sì che l'angolo formato dalle fibre stesse rispetto alla famiglia dei piani ortogonali all'asse principale dello stent (in pratica l'angolo di avvolgimento della traiettoria elicoidale definita da ciascuna delle fibre) presenti un valore abbastanza elevato, di preferenza almeno pari a 45° e in modo ancor preferito superiore a 60°.
Sono naturalmente ipotizzabili soluzioni alternative, quali ad esempio la soluzione di utilizzare per la realizzazione delle fibre 4 materiali elastici e/o la soluzione di conferire alle fibre 4 un andamento a serpentina. Tutto ciò in modo da far sì che le fibre presentino caratteristiche di estendibilità longitudinale.
Al fine di disporre le fibre 4 sulla struttura 2 è anche possibile sfruttare la presenza (dettata in via primaria da altre motivazioni, illustrate nel seguito) di una guaina o tunica 5 interposta fra la struttura 2 e la falda o le falde di fibre 4 disposte sulla stessa.
In particolare, è possibile pensare di applicare le fibre 4 sulla struttura 2 dello stent 1 collegandole (ad esempio per effetto di un collegamento adesivo) alla superficie esterna della guaina 5. In questo caso, è possibile collocare le fibre 4 (sia che siano ordinate in un'unica falda, sia che siano presenti due o più falde) in modo tale che le fibre 4,si estendono sostanzialmente lungo le generatrici dell'ideale superficie cilindrica definita dalla struttura 2 dello stent. Una soluzione di questo genere è tale da rendere del tutto ininfluente sulle fibre 4 il movimento di dilatazione radiale della struttura 2.
La guaina 5 ha la funzione principale di evitare indesiderati fenomeni di prolasso delle fibre 4 all'interno del lume dello stent 1. Tali fenomeni sono suscettibili di determinarsi, ad esempio, qualora la struttura 2 presenti - nella sua interezza o anche solo in parte - una configurazione particolarmente aperturata.
Di conseguenza, la guaina 5 ha la funzione di fare in modo che, anche nel momento successivo all'impianto, le fibre 4 conservino di preferenza una collocazione esterna rispetto alla struttura 2 interponendosi di fatto fra lo stent 1 e la parete del vaso.
La presenza della guaina 5 risulta benefica anche nelle fasi successive, vale a dire quando vengono diffusi gli agenti presenti nei nuclei 3a e 4a .
La guaina 5 è infatti in grado di svolgere una funzione, almeno parziale, di barriera, facendo sì che il rilascio dei suddetti agenti attivi avvenga di preferenza ed in via primaria verso la parete del vaso e non già verso il lume interno della struttura 2 con conseguente esposizione al fenomeno di dilavamento da parte del sangue.
Per la realizzazione della guaina 5 è possibile ricorrere a materiali diversi, presentanti di preferenza buone doti di biocompatibilità, sia per effetto del materiale costituente, sia per il fatto che la guaina 5 è stata sottoposta ad un trattamento quale il deposito superficiale di uno strato di materiale carbonìoso biocompatibile. Una scelta considerata al momento preferenziale è quella di realizzare la guaina 5 con un materiale siliconico. Sono naturalmente possibili scelte alternative, quale il realizzare la guaina 5 sotto forma di una tunica di rete metallica o polimerica finemente aperturata, in grado di seguire il movimento di espansione dello stent.
La scelta di realizzare la guaina 5 con caratteristiche generali di porosità (o, in generale di permeabilità, eventualmente più marcata nel verso diretto all'esterno dello stent rispetto al verso opposto) è vantaggiosa in quanto favorisce <' >il contatto della parete del vaso con il sangue che fluisce attraverso lo stent evitando nel contempo fenomeni di dilavamento.
Le fibre 4 possono essere collocate sulla struttura 2 in fasi diverse del procedimento di fabbricazione e/o di predisposizione all'impiego dello stent 1.
In particolare {quali che siano i criteri adottati per applicare le fibre 4 sulla struttura 2) le fibre 4 possono essere applicate nelle fasi finali della fabbricazione dello stent, dunque prima che lo stent venga montato sul catetere C utilizzato per la disposizione in situ, oppure in un momento successivo, ad esempio dopo che lo stent 1 è già stato montato sul catetere di impianto.
Il complesso di fibre 4 può quindi costituire un componente del corredo o kit (stent catetere di impianto) che, nella forma di commercializzazione al momento preferita, costituisce il prodotto reso disponibile in vista dell'intervento di impianto.
Come ulteriore alternativa è possibile associare le fibre alla struttura dello stent in una fase ancora successiva, dunque immediatamente prima di procedere all'impianto dello stent (prima o dopo la collocazione dello stesso sul catetere di impianto) .Per questo motivo il complesso di fibre 4 può costituire un prodotto a sé stante, suscettibile di essere commercializzato, ad esempio in versioni diverse comprese in un assortimento e caratterizzate dalla presenza di agenti diversi e/o cinetiche di rilascio diverse. In particolare tale prodotto può presentarsi, ad esempio, sotto forma di un foglietto avvolgibile sullo stent o sotto forma di una calza tubolare suscettibile di essere investita sullo stent.
Da quanto precede risulta evidente che la soluzione descritta consente di realizzare una vera e propria "macchina di rilascio" controllato di agenti antagonisti della restenosi.
In particolare, le dimensioni delle nanoparticelle 3 e dei nuclei 3a, 4a fanno sì che anche nell'ambito di una struttura di piccole dimensioni quale uno stent sia possibile disporre di un numero elevatissimo di nuclei con la conseguente possibilità di differenziare la natura e le caratteristiche degli agenti antagonisti nell'ambito di un'ampia gamma di scelta.
Per quanto riguarda la cinetica di rilascio è importante notare che la soluzione secondo l'invenzione consente di intervenire sia a livello delle nanoparticelle 3 (nei termini richiamati in precendenza), sia a livello delle fibre 4, ad esempio operando sulla composizione, sullo spessore, sulla struttura (omogenea, stratificata e/o porosa) delle fibre 4 stesse, nonché sulla collocazione dei nuclei nelle fibre (collocazione diretta, come nel caso dei nuclei 4a, o indiretta, per effetto dell'incorporazione in una nanoparticella 3 a sua volta incorporata nella parete di una fibra).
Esiste inoltre la possibilità di operare sull'ulteriore parametro rappresentato dalla eventuale porosità della guaina 5.
Particolarmente interessante è la possibilità di operare in modo combinato sui suddetti parametri, ad esempio facendo in modo che gli agenti contenuti in taluni nuclei 3a,4a vengano diffusi n tempi differenziati rispetto agli agenti contenuti in altri nuclei 3a, 4a così da realizzare un intervento antagonista della restenosi esattamente modellato sul manifestarsi dei fenomeni posti alla base della restenosi stessa.
Per fare un esempio, si può pensare di privilegiare, nelle fasi acute, il rilascio di farmaci antinfiammatori e/o promotori di processi riparativi della parete del vaso, dando invece la prevalenza, in una fase successiva, al rilascio di agenti antimitotici .
Naturalmente, fermo restando il principio dell’invenzione, i particolari di realizzazione e le forme di attuazione potranno essere ampiamente variati rispetto a quanto descritto ed illustrato, senza per questo uscire dall'ambito della presente invenzione.
Claims (53)
- RIVENDICAZIONI 1. Stent per angioplastica comprendente una struttura (2) dall'inviluppo complessivamente tubolare suscettibile di essere dilatata a partire da una posizione radialmente contratta verso una posizione radialmente espansa, caratterizzato dal fatto che a detta struttura (2) sono associate fibre (4) costituenti vettori per nuclei {3a, 4a) di agenti antagonisti della restenosi.
- 2. Stent secondo la rivendicazione 1, caratterizzato dal fatto che detti nuclei {3a, 4a) sono almeno in parte contenuti in dette fibre (4), dette fibre essendo costituite da un materiale bioerodibile.
- 3. Stent secondo la rivendicazione 2, caratterizzato dal fatto che detti nuclei (3a, 4a) sono almeno in parte (4a) incorporati nelle pareti di dette fibre (4).
- 4. Stent secondo una qualsiasi delle rivendicazioni 1 a 3, caratterizzato dal fatto che detti nuclei (3a, 4a) sono almeno in parte (3a) incorporati in nanoparticelle (3) associate a dette fibre (4) e provviste di un involucro (3b) di materiale bioerodibile.
- 5. Stent secondo la rivendicazione 4, caratterizzato dal fatto che dette fibre (4) sono fibre cave e dette nanoparticelle (3) sono disposte nella cavità di dette fibre cave (4).

Stent secondo la rivendicazione 4 o la rivendicazione 5, caratterizzato dal fatto che dette nanoparticelle (3) sono presenti in tipi diversi differenziati per almeno un parametro scelto nel gruppo costituito da: - composizione del nucleo (3a), - composizione dell'involucro (3b), - spessore dell'involucro (3b), - collocazione relativa del nucleo (3a) rispetto all'involucro (3b), - struttura del nucleo (3a), e - struttura dell'involucro (3b).

- 27. Componente secondo la rivendicazione 26, caratterizzato dal fatto che detti nuclei (3a, 4a) sono almeno in parte contenuti in dette fibre (4), dette fibre essendo costituite da un materiale bioerodibile .
- 28. Componente secondo la rivendicazione 27, caratterizzato dal fatto che detti nuclei (3a, 4a) sono almeno in parte (4a) incorporati nelle pareti di dette fibre (4).

- 46 . Componente secondo una qualsiasi delle rivendicazioni 42 a 45, caratterizzato dal fatto che detta guaina (5) è di materiale siliconico.
- 47. Componente secondo una qualsiasi delle rivendicazioni 42 a 45, caratterizzato dal fatto che detta guaina (5) è di materiale metallico. o polimerico.
- 48. Componente secondo una qualsiasi delle precedenti rivendicazioni 42 a 47, caratterizzato dal fatto che detta guaina (5) è rivestita da uno strato di materiale carbonioso biocompatibile.
- 49. Componente secondo la rivendicazione 42, caratterizzato dal fatto che dette fibre (4) sono, almeno in parte, ancorate a detta guaina (5).
- 50. Componente secondo una qualsiasi delle rivendicazioni 26 a 49, sotto forma di un foglietto avvolgibile sulla struttura dello stent (1).
- 51. Componente secondo una qualsiasi delle rivendicazioni 26 a 49, sotto forma di una calza tubolare suscettibile di essere investita sulla struttura (2) dello stent (1).
