Description

【発明の詳細な説明】 〔産業上の利用分野１ 本発明は、上部消化管出血の治療を目的とし、室温条件
下でも長期間安定であり、更に、溶解性に優れかつ簡易
に経口投与できる、安定な経口用トロンビン製剤に関す
るものである。
［従来の技術及び解決すべき課題］ トロンビンは、古くから知られている血液ａ固因子のひ
とつであり、血液凝固過程の最終段階、即ちフィブリノ
ーゲンに直接作用してフィブリンに転化する作用を有す
る。従って、血液中にフィブリノーゲンが存在すれば出
血局所の血液を急速に凝固して損傷血管端を閉塞し、血
小板の存在のもとに凝血塊は収縮して血管断端な塞ぎ完
全に止血する。
トロンビンはヒトまたはウシの血液から製造したプロト
ロンビンに、カルシウムイオンの存在下で、トロンボプ
ラスチンを作用させて製造し、従来はこれをバイアルに
充填し凍結乾燥して製剤化している。
従来のトロンビン製剤は、室温保存すると活性の低下を
生ずるので、１０℃以下に保存することとされてきた（
日本薬局方第１１改正）。この製剤の使用時には、通常
、生理食塩液で溶かした溶液（トロンビンとして５０〜
１０００単位／ｍ１）を出血箇所に噴霧するか、あるい
は、バイアルから内容物を取り出し粉末のまま出血箇所
に散布する方法で投与されている。
上部消化管出血の治療に適用する場合は、トロンビンと
して２００〜４００単位／ｍｌになるように適当な緩衝
剤に溶かして経口投与されている。
しかし、前記のように、１−ロンビン製剤を上部消化管
出血の治療に適用する場合は、数時間おきに１日数回投
与するので、患者自身により簡易に経口投与できる製剤
の開発が望まれている。
その為には、バイアルの様な密封容器に充填しなくとも
、室温保存でもトロンビンな安定化させつる組成の製剤
を開発することが必要である。
また、バイアル製剤は注射用として使用されるものがほ
とんどである為、バイアル入りのトロンビン製剤は、医
療機関において誤って注射される危険を避Ｇプる為にバ
イアルに特別な標識を施す等の配慮を施しているが、バ
イアルを使用しな１−ｉればこのような配慮も不要とな
る。
更に、バイアル入りのトロンビン製剤は、患者にとって
携帯に不便であり、また１０℃以下に保存しなければな
らないことも取扱上煩雑であるなどの問題点を有する。
消化管の出血の治療を目的とし、簡易に経口投与を可能
とした先行した技術としては、特開平１−９３５３６号
公報及び特開平１−１２１２２４号公報において開示さ
れている。前者は、腸溶性製剤の形態であることを特徴
とするものであり、後者は、アルカリ性物質を含み胃内
におけるトロンビンの失活を防止することを特徴とする
ものである。しかし、保存安定性については、後者につ
いて冷所２１日間での安定性が示されているのみであり
、室温条件下での安定性についてはなんら示されていな
い。
従って、本発明の目的は、上部消化管出血の治療に適用
しつる、溶解性に優れかつ簡易に経口投与が可能であり
、室温保存下でも安定な経口用トロンビン製剤を提供す
ることにある。
［問題点を解決する手段］ 本発明者らは鋭意検討した結果、止血成分としてのトロ
ンビンに、医薬上許容される特定の安定化剤と賦形剤と
を組み合わせることにより、溶解性に優れ、かつ、簡易
に経口投与ができ、更に室温条件下でも長期間安定な経
口用トロンビン製剤を調製出来ることを見出し、本発明
を完成した。
本発明は、止血成分としてトロンビンを含有し、医薬上
許容される特定の安定化剤と賦形剤との組合せからなる
、溶解性に優れた安定な経口トロンビン製剤である。
本発明に使用されるトロンビンは、生理活性を何するも
のであれば、特に限定されない。
般には、ウシ又はヒトの血漿より抽出して精製したプロ
トロンビンに、カルシウム塩とトロンボプラスチンを作
用させて製造したものが使用出来る。また、トロンビン
は凍結乾燥された粉末、もしくは高単位に濃縮された溶
液でも使用出来る。
従来のトロンビン製剤は、バイアルに充填されてきたが
、特に経口投与剤としての使用上、前記のような不都合
があった。
かかる問題点を解決するため、本発明の割型としては粉
末、細粒剤、顆粒剤、錠剤等好ましくは粉末、細粒剤、
顆粒剤とするのが望ましい。
本発明の経口用トロンビン製剤は、医薬上許容される特
定の安定化剤と賦形剤との組合せを包含する。本発明で
用いられる安定化剤としては、ゼラチン、アルブミン、
グリシンが挙げられ、好ましくは、ゼラチン、グリシン
である。
本発明に使用されるゼラチンは、動物の骨、皮膚、じん
帯またはけんを酸またはアルカリで処理して得たコラー
ゲンを水で加熱抽出し、抽出液を減圧乾燥してゲル化し
、乾燥して製造されたものであれば、特に限定されない
。安定化剤の含量は、ｌ・ロンピン１万単位あたり５〜
１００ｍｇで、好ましくは１０〜５０ｍｇである。
賦形剤としては糖または糖アルコールが挙げられ、白糖
、乳糖、ブドウ糖、果糖、Ｄ−マンニトール等が好まし
く、特に白糖が好ましい。
賦形剤として糖または糖アルコールが用いられる理由は
、まず第１に、木製剤の製造にあたっては、公知の技術
により製造されるが、乾燥工程において長時間加熱する
と、トロンビンの活性が低下するため、長時間の乾燥工
程を要しない賦形剤が必要とされるからである。本発明
の賦形剤を用いた場合、低水分で造粒が可能であり、乾
燥時間が少なくて良い。
第２に、木製剤にＪ５いて水溶解性に優れかつ沈澱物の
生じない賦形剤が必要とされるからである。即ち、上部
消化管出血の治療に適用するにあたっては、本製剤は適
当な溶媒、例えば、緩衝液、牛乳、生理食塩液または蒸
留水等に溶解して用いられるが、その際、溶解せず沈澱
物があれば、不溶性物質に生理活性成分が吸着して十分
な薬効が望めなかったり、服用時に違和感を残す為であ
る。
ここで用いられる賦形剤としての糖または糖アルコール
の含有量は、トロンビン１万単位あたり　０．１〜２ｇ
で、好ましくは０．５〜１ｇである。
本製剤を上部消化管出血の治療に適用するにあたっては
、上記したように適当な溶媒、例えば、緩衝液、牛乳、
生理食塩液または蒸留水に溶かし、トロンビンとして５
０〜１０００単位／ｍ＋にして投与される。その際、必
要に応じて制酸剤を併用することも出来る。
上記組成からなる本発明の製剤は、室温状態においても
トロンビン活性が長期間にわたり維持されており、安定
性の優れた製剤である。また、本製剤は、適当な溶媒、
例えば緩衝液、牛乳、生理食塩液または蒸留水に溶かし
た場合、１分以内に溶解し沈澱物は見られない。更に、
この溶液中のトロンビン活性は２４時間低下はみられな
かった。
〔発明の効果］ 以下に、本発明を実験例により具体的に説明し、本発明
の効果を明らかにする。
実験例１　安定化剤の効果の実験 後記の実施例１または参考例１の製剤約１ｇを密封した
容器に入れ、６０°Ｃの過酷な条件下で最長２ケ月間放
置し、その間における各製剤のトロンビン活性の低下を
測定することによって、安定化剤の有無による安定化の
比較実験を試みた。
トロンビン活性は日本薬局方第１１改正に準じて測定し
、放置前の製剤のトロンビン活性の値を１００とし、放
置後のもののトロンビン活性の値を求めた。
結果を第１表に示す。
第１表 数値は、放置前のトロンビン活性を　１００とした時の
放置後のトロンビン活性を示す。
第１表より明らかなように、本発明の安定化剤の添加に
より、高い温度に保存した場合でも顕著な安定化効果を
示している。
実験例２　長期安定性の実験 後記の実施例３の製剤約１ｇを、アルミ箔にセロファン
及びプラスチックをラミネートシたシートよりなる袋に
入れヒートシールしたものを、医薬品製造指針の加速試
験基準に準じて、４０°Ｃ（±１°Ｃ）相対湿度７５％
（±５％）及び室温条件下に最長６ケ月間保存し、その
間における製剤のトロンビン活性を測定した。
トロンビン活性の測定は、実験例１と同様に行った。
結果を第２表に示した。
第２表 数値は、放置前のトロンビン活性を１００とした時の放
置後のトロンビン活性を示す。
下でも６ケ月間安定であり、上記結果から少なくとも室
温で３年間は安定であると予測することが出来る安定性
に優れた製剤である。
実験例３　製剤の溶解性及び溶液状態での安定性 後記の実施例３の製剤的１ｇを各々　１００ｍ１ビカー
にとり、これに蒸留水、生理食塩水リン酸緩衝液及び牛
乳を各々５０ｍ１加え、緩やかに撹拌した時の、溶解時
間（ビーカー底部に粉体の存在が認められなくなるまで
の時間）及び得られた溶液の性状を観察した。
更に、これらの溶液を室温に放置し、２４時間までの各
溶液のトロンビン活性を測定した。
トロンビン活性の測定は、実験例１と同様に行った。
結果を第３表に示す。
第２表より明らかなように、本発明製剤は、４０°Ｃ（
±１°Ｃ）相対湿度７５％（±５％）の条件１、　１ ２ 第３表 数値は、放置前のトロンビン活性を１００とした時の放
置後のトロンビン活性を示す。
第３表から明らかなように、本発明の製剤の溶解性は極
めて高く、不溶性の沈澱物はなく、服用性に優れている
。更に、製剤を溶解した溶液中のトロンビン活性は長時
間安定であり、有効性に優れている。
〔実施例〕
次に、本発明を実施例及び参考例を挙げて説明するが、
本発明は、これらの実施例に限定されるものではない。
本実施例においては、ウシの血漿より抽出して精製した
プロトロンビンにカルシウム塩とトロンボプラスチンを
作用させて得た、比活性１０００単位／蛋白質ｍｇ、濃
度１万単位／ｍｌのトロンビン溶液を製造し使用した。
実施例１ トロンビン　　　２０００万単位 グリシン　　　　　３０　　ｇ ゼラチン　　　　　２０　　ｇ 白糖　　　　　適　量 計　　　　　　　　　　　１０００ｇ 所定の単位を含有するトロンビン溶液にグリシンを加え
て溶解し、凍結乾燥により粉末を得　４ た。得られた粉末に、白糖を加えて混合撹拌し、混合撹
拌下に蒸留水にて溶解したゼラチン溶液を規定量噴霧し
、造粒物を得た。乾燥機にて加熱乾燥した後、前退をし
細粒剤を得た。
実施例２ トロンビン　　　２０００万単位 乳糖　　　　　　　３０　　ｇ ゼラチン　　　　　２０　　ｇ 白糖　　　　　　適　量 計　　　　　　　　　　　１０００ｇ 所定の単位を含有するｌ・ロンビン溶液に乳糖を加えて
溶解し、凍結乾燥により粉末を得た。
得られた粉末に、白糖を加えて混合撹拌し、混合撹拌下
に蒸留水にて溶解したゼラチン溶液を規定量噴霧し、造
粒物を得た。乾燥機にて加熱乾燥した後、前退をし細粒
剤を得た。
実施例３ １・ロンビン　　　２０００万単位 Ｄ−マンニ］・−ル　３０　　ｇ ゼラチン　　　　　２０　　ｇ 白糖　　　　　　適　量 計　　　　　　　　　　１０００ｇ 所定の単位を含有するトロンビン溶液にＤ−マンニトー
ルを加えて溶解し、凍結乾燥により粉末を得た。得られ
た粉末に、白糖を加えて混合撹拌し、混合撹拌下に蒸留
水にて溶解したゼラチン溶液を噴霧し、造粒物を得た。
乾燥機にて加熱乾燥した後、前退をし細粒剤を得た。
参考例１ トロンビン　　　２０００万単位 白糖　　　　　　適　量 計　　　　　　　　　　１０００ｇ 所定の単位を含有するトロンビン溶液を、凍結乾燥によ
り粉末を得た。得られた粉末に、白糖を加えて混合撹拌
し、混合撹拌下に蒸留水を噴霧し、造粒物を得た。乾燥
機にて加熱乾燥した後、前退をし細粒剤を得た。

Claims (3)

    【特許請求の範囲】
  1. （１）止血成分としてトロンビンを含有し、安定化剤と
    してゼラチン、アルブミン及びグリシンから選ばれた１
    種以上及び賦形剤として糖または糖アルコールを含むこ
    とを特徴とする溶解性に優れた安定な経口用トロンビン
    製剤。
  2. （２）安定化剤がゼラチンで、賦形剤が白糖であること
    を特徴とする請求項第１項記載の製剤。
  3. （３）割型が粉末、細粒剤または顆粒剤であることを特
    徴とする請求項第１項記載の製剤。
