Description
A presente invenção refere-se na generalidade a um método aperfeiçoado para iniciar o corte por um agrafador linear cirúrgico. Mais especificamente, a invenção refere-se a uma iniciação aperfeiçoada do corte num agrafador cirúrgico actuando uma energia potencial armazenada num manipulo excêntrico actuado por moías.
ANTECEDENTES DA INVENÇÃO
Recentemente, os agrafadores cirúrgicos tornaram-se a maneira mais popular para coser intervenções cirúrgicas. Alguns dos agrafadores cirúrgicos que têm sido desen volvidos são dispositivos que cortam o tecido entre fiadas de agrafos. O tecido ê cortado enquanto os agrafos são aplicados através do tecido para fechar o corte. Isto ê especialmente verdade em dispositivos gastrointestinais.

Uma das desvantagens dos dispositivos de agrafar e cortar utilizados em cirurgia ê a dificuldade de iniciar o procedimento de corte. Isto é, geralmente, o agrafador será constituído por um par de mandíbulas entre as quais se colo ca o tecido a cortar e a ser agrafado. As mandíbulas são fechadas à volta do tecido, e em seguida ê iniciada a sequência de corte e agrafamento. Geralmente, para iniciar o corte, o utiliza dor necessitará de uma força extra na lâmina de corte. Para este efeito, a lâmina encontra dificuldades em separar inicialmente o tecido. Quando se inicia o corte, torna-se mais fácil cortar o tecido restante. Portanto, apenas no momento de iniciar o corte é necessária força adicional.
SUMÁRIO DA INVENÇÃO
Constitui, assim, um objecto da presen te invenção proporcionar um agrafador que permita um aumento in_i ciai da energia no momento de iniciação do corte. Assim, constitui um objecto da presente invenção armazenar energia potencial perto do actuador de uma lâmina de corte.
Constitui ainda um objecto adicional da presente invenção armazenar energia potencial perto do actuador mas não permitir contudo a utilização desta energia potencial até que o agrafador esteja pronto para utilização.
É ainda outro objecto da presente invenção proporcionar uma força de corte mais suave num agrafador cirúrgico aumentando a força para cortar o tecido após iniciação do movimento de corte.
Este e outros objectos da presente invenção são conseguidos num agrafador cirúrgico com um sistema de agrafar e de corte em que se armazena energia, e um sistema de armazenagem de energia que é susceptível de comunicar energia po tencial ao sistema de iniciação de um agrafador e cortador. Esta comunicação de energia potencial faz com que a energia no sistema de iniciação referido ultrapasse a resistência oferecida pelo tecido contra o sistema de iniciação. Além disso, existe um sistema de restrição para evitar o funcionamento do agrafador até que as mandíbulas do agrafador cirúrgico estejam seguramente en2

DESCRIÇÃO PORMENORIZADA DOS DESENHOS

DESCRIÇÃO PORMENORIZADA DA INVENÇÃO
Referindo às Figs. 1, 2 e 3, apresenta -se um agrafador 10 contendo uma mandíbula inferior ou primeira 22 e uma mandíbula superior ou segunda 24. Estas mandíbulas, 22, 24 formam um corpo agrafador 20. Entre as mandíbulas é mantido o tecido, não apresentado. Quando o manipulo 42 se move ao longo do corpo do agrafador 10, ele actua um sistema motor 12. Estã fi xada uma placa guia 51 com uma barra correspondente 11 para operar o sistema motor 12. Este sistema motor 12 contém saliências • 16 e uma lâmina de corte 18. Estas saliências 16 e lâminas 18 — passam através das fendas 32 no cartucho de agrafos 30. As sali- 3 -
ências 16 actuam motores não representados que fazem com que os agrafos sejam movidos ao longo dos cartuchos de agrafos 100 no cartucho 30 e para uma alavanca na superfície da mandíbula superior ou segunda 24. Enquanto o agrafador 10 faz o movimento dos agrafos, a lâmina de corte 18 corta o tecido. Assim, no final da actuação do manipulo 42, formaram-se duas fiadas de agrafos no tecido que foi cortado entre estas duas fiadas. 0 aperfeiçoamento do presente agrafador 10 ê mais basicamente visto na conversão do manipulo 42 para o sistema de iniciação 40. No sistema de iniciação 40 existe um primeiro sistema de retenção 50 incorpora do no manipulo 42. Isto será melhor visto na Fig. 5. O primeiro sistema de retenção 50 roda ã volta do eixo 52 ao longo do manipulo 42. O sistema de retenção 50 contem um pino ou cavilha 54 que ê susceptível de ser colocado dentro da fenda 68 na mandíbula inferior ou primeira 22 do agrafador 10, como se vê na Fig. 3.
Além disso, numa realização preferida também se pode ver na Fig. 4 a mola 73, fixada ao longo da mandi. bula inferior 22 numa extremidade. A mola 73 está também ligada a um sistema de armazenagem de energia 70, no pino de ligação 79, que ê fixado no furo 81 do sistema de armazenagem de energia 70. Este sistema de armazenagem de energia 70 ê também actuado por uma mola, por meio de uma mola 72, à mandíbula inferior 22 dentro da câmara de guia 26. As molas 72, 73 armazenarão energia potencial quando o sistema de iniciação 40 estã na sua posição de carregado.
O sistema de armazenagem de energia 70 contém também um excêntrico 74 com a calha 76. O excêntrico 74 roda na câmara de guia 26 â volta do eixo 78, como se pode ver melhor nas Figs. 4 a 9. Será o sistema de armazenagem de energia 70 que permitirá que o sistema de iniciação 40 forneça a energia adicional ao cortador 18 e às cunhas 16 durante a iniciação do corte e agrafamento.
Pode também ver-se na Fig. 4, bem como na Fig. 9, um sistema de contenção ou de detenção secundário 30. Este sistema de retenção 80 compreende uma mola 82 que estã carregada contra a câmara de guia 26. Observa-se também uma barra • em gancho 86, que roda em torno do eixo 84. Esta barra em gancho

tem uma extremidade curva que encaixa no sulco correspondente 88 do sistema motor 12, como se vê melhor nas Figs. 6 e 7. 0 sis tema de retenção pode ser activado pelo pino 90 colocado na alavanca 60 da mandíbula inferior 22. Serã este pino 90 que permite que o sistema de retenção 80 seja desactivado por actuação do primeiro sistema de detenção 50 de forma a permitir que o sistema de armazenagem de energia 70 proporcione energia potencial ao sistema de iniciação 40 no início do serviço.
Assim, a presente invenção será executada da forma seguinte. Em primeiro lugar, coloca-se os tecidos entre as duas mandíbulas 22, 2 4-. Em seguida a alavanca 60 serã fixada utilizando o grampo 62 à volta do pino 64 em tandem com a fenda correspondente 66. Nesta altura, a cavilha 54 do primeiro sistema de detenção 50 foi previamente inserido na fenda 68 da câmara de guia 26 da mandíbula inferior 22. Logo que o tecido se ja seguro, o sistema de iniciação 40 ê movido para a frente (na direcção da extremidade das mandíbulas 22, 24) fazendo com que o sistema motor 12 active o sistema de armazenagem de energia 70.
O sistema de armazenagem de energia 70 ê em seguida rodado quando o bloco de pressão 13 contacta com o sulco 76 para fazer rodar o excêntrico 74. As molas 72, 73 estão assim prontas para Li bertarem a sua energia potencial armazenada. O agrafador 10 estã então pronto para o corte e agrafamento.
O agrafador pode ser apenas utilizado se a cavilha 90 da alavanca 60 tiver desactivado o sistema de re tenção 70. Isto ocorrerá quando a cavilha 90 tiver rodado na sua distância máxima, após a fixação adequada da alavanca 60. Deve notar-se que a barra em gancho 86 do sistema de retenção 80 terá sido colocada dentro do sulco correspondente 88 localizado no sistema motor 12. Isso acontecerá se a cavilha 90 rodar no siste ma de retenção 80 em torno do eixo 84 e então a barra em gancho 86 serã colocada para o exterior do tubo correspondente 88. Apenas nesta altura serã desactivado o segundo sistema de detenção 80. Apenas nesta altura o agrafador 10 permitirá a utilização do primeiro sistema de detenção 50.
Nesta altura, o primeiro sistema de de tenção 50 pode ser rodado em torno do eixo 52 para permitir a
utilização do manipulo 42. A energia armazenada nas molas 72, 73 fará com que o excêntrico 74 comunique a energia potencial armazenada ao bloco de pressão 13 do sistema motor 12. Esta energia potencial armazenada permite que seja comunicada energia de iniciação ao cortador 18 e ao agrafador 16. Esta energia potencial armazenada iniciará o serviço de corte e agrafamento mais facilmente permitindo um agrafamento e corte melhor, mais fácil e mais preciso.
Assim, a presente invenção pode ser uti lizada em todos os agrafadores que necessitem de uma energia de
6iniciação adicional, em particular agrafadores que utilizam dispositivos de corte. Além disso, embora esta invenção particular tenha sido descrita em ligação com a realização presentemente preferida, deve entender-se que no seu âmbito estão incluídas quaisquer modificações ã invenção que conduzam a funções substan cialmente semelhantes a executar substancialmente da mesma manei, ra. Alem disso, reconhece-se que a invenção é descrita em ligação com as reivindicações anexas e as suas equivalentes.
Dispositivo agrafador cirúrgico possuir, do um sistema de iniciação susceptível de activar sistemas motores para aplicar agrafos em tecidos, caracterizado por compreender um sistema de armazenagem de energia susceptível de comunicar energia potencial ao referido sistema de iniciação, fazendo essa energia comunicada com que o referido sistema de iniciação vença as forças exercidas pelo referido tecido contra o referido sistema de iniciação.
Claims (2)
- Processo de acordo com a reivindicação 1, caracterizado por o referido sistema de armazenagem de energia compreender um sistema de molas ligado a um sistema de excên tricôs possuindo uma ranhura e o corpo do referido agrafador, sendo o referido sistema de molas susceptível de receber energia potencial que lhe é comunicada pelo utilizador, sendo a referida energia potencial mantida em posição no referido sistema de excêntrico pelo primeiro sistema de lingueta ao referido corpo agrafador e susceptível de manter em posição o referido sistema de iniciação no referido agrafador até estar pronto para uso, sendo o referido sistema de iniciação susceptível de deslocar o referido sistema de lingueta e libertar a referida energia poten ciai no referido sistema de iniciação durante a utilização.Processo de acordo com a reivindicação
- 2, caracterizado por o referido sistema de iniciação compreender um sistema de manuseamento ligado ao referido sistema motor, ter minando o referido sistema de manuseamento num pino que se pode inserir no referido sistema de excêntrico do referido sistema de armazenagem de energia, e sendo mantido dentro da referida ranhu ra até se desengatar do referido primeiro sistema de lingueta.Processo de acordo com a reivindicação 3, caracterizado por o referido primeiro sistema de lingueta poder rodar no referido sistema de manuseamento e culminar numa ca vilha que se pode inserir numa fenda do referido corpo agrafador de modo a manter o referido sistema de manuseamento e o referido sistema de armazenagem de energia estável até o referido agrafador estar pronto para uso, após o que se retira o referido primeiro sistema de lingueta para fora da referida fenda e a referi da energia potencial é comunicada ao referido sistema de manusea mento.Processo de acordo com a reivindicação 1, caracterizado por o referido agrafador compreender uma primei ra mandíbula fixável a uma segunda mandíbula para se ajustar â volta do referido tecido, compreendendo ainda o aperfeiçoamento um sistema de contenção susceptível de manter a referida energia potencial em posição até que as referidas mandíbulas sejam adequadamente fixadas uma à outra.- 6- 8-6-Processo de acordo com a reivindicação 5, caracterizado por a referida primeira mandíbula se fixar à re ferida segunda mandíbula por meio de um grampo que actua uma ala vanca na referida primeira mandíbula, compreendendo o referido sistema de contenção um segundo sistema de lingueta com mola de carga e inserível no referido sistema motor, podendo o segundo sistema de lingueta rodar para fora do referido sistema motor por contacto com uma cavilha localizada na referida alavanca após se fecharem as referidas primeira e segunda mandíbulas.Processo de acordo com a reivindicação 6, caracterizado por o referido segundo sistema de lingueta compreender uma barra em gancho susceptível de ser engatada numa ra nhura adequada localizada no referido sistema motor.Processo de acordo com a reivindicaçãc 4, caracterizado por o referido agrafador compreender uma primei ra mandíbula fixãvel a uma segunda mandíbula para se ajustar em volta do referido tecido, compreendendo ainda o aperfeiçoamento um sistema de contenção susceptível de manter em posição o refe rido sistema de armazenagem de energia e o referido sistema de corte até que as referidas mandíbulas sejam adequadamente ajustadas uma à outra.Processo de acordo com a reivindicação 8, caracterizado por a referida primeira mandíbula ser fixada â referida segunda mandíbula por meio de um grampo que actua uma alavanca existente na referida primeira mandíbula, compreendendo o referido sistema de contenção um segundo sistema de lingueta com mola de carga e inserível no referido sistema motor, podendo o segundo sistema de lingueta rodar para fora do referido sistema motor por contacto com uma cavilha localizada na referida ala vanca após se fecharem as referidas primeira e segunda mandíbulas.Processo de acordo com a reivindicação; 9, caracterizado por o segundo sistema de lingueta compreender uma barra em gancho susceptível de ser engatada numa ranhura ade quada localizada no referido sistema motor para manter o referido sistema de extracção em posição até que o referido segundo sistema de lingueta seja rodado para fora do referido sistema mo tor.Dispositivo agrafador cirúrgico caracterizado por compreender:uma primeira mandíbula fixãvel a uma segunda mandíbula por meio de um grampo activado por uma alavanca existente na referida primeira mandíbula, um sistema motor para colocar agrafos existentes numa das referidas mandíbulas em tecido colocado entre as referidas mandíbulas, um manipulo ligado ao referido sistema motor para guiar o referido sistema motor com o referido agrafo, sendo o referido sistema motor inserível em sistemas de excêntricos fixados a uma das referidas mandíbulas por uma mo la carregada, possuindo o referido manipulo uma cavilha que se pode inserir numa fenda dentro duma das referidas mandíbulas para man ter o referido manipulo e a referida mola carregada em posição atê que o referido agrafador esteja pronto para utilização, após o que se faz a rotação da referida cavilha para o exterior da re ferida fenda e se comunica a energia potencial da referida mola pelo referido sistema de excêntricos ao referido sistema motor para vencer as forças exercidas pelo referido tecido contra o re ferido sistema motor e referido manipulo.Processo de acordo com a reivindicação 11, caracterizado por a referida primeira mandíbula compreender ainda uma cavilha localizada na referida alavanca e uma barra com a forma de gancho carregada por uma mola e rodável numa ranhura adequada no referido sistema motor, mantendo a referida barra com a forma de gancho o referido sistema motor dentro do referido agrafador, até que a referida cavilha faça rodar a refe rida barra em gancho para fora da referida ranhura adequada por fixação adequada da referida primeira mandíbula à referida segun da mandíbula do referido agrafador.- 13- Processo de acordo com a reivindicação 12, caracterizado por a rotação da referida barra em forma de gancho permitir que o referido manipulo seja rodado para fora da referida fenda e a comunicação da referida energia potencial pelo referido sistema de excêntricos ao referido sistema motor e manipulo.- 14- Processo para vencer forças exercidas por tecidos contra agrafos num agrafador cirúrgico possuindo um sistema motor e um sistema para iniciar o referido sistema motor caracterizado por:armazenar-se energia potencial no referido sistema de iniciação atê que o referido agrafador esteja pronto para agrafar, e em seguida comunicar a referida energia potencial ao refe rido sistema motor através do referido sistema de iniciação.A requerente reivindica a prioridade do pedido norte-americano apresentado em 26 de Setembro de 1988, sob o número de série 249,537.
