Description
Profármacos de paclitaxel solubles en agua.
La presente invención se refiere en general al
campo de las composiciones farmacéuticas para usarse en el
tratamiento del cáncer, enfermedades autoinmunes y reestenosis. La
presente invención también se refiere al campo de las preparaciones
farmacéuticas de agentes antineoplásicos tales como paclitaxel
(Taxol) y docetaxel (Taxotere), en particular a hacer paclitaxel
hidrosoluble conjugando el fármaco con restos hidrosolubles.
Paclitaxel, un agente antimicrotubular extraído
de las acículas y de la corteza del tejo del Pacífico, Taxus
brevifolia, ha mostrado un efecto antineoplásico extraordinario
en el cáncer humano en estudios de fase I y en estudios
preliminares de fase II y III (Horwitz et al., 1993). Esto se
ha informado principalmente en cáncer ovárico y de mama avanzados.
Se ha documentado una actividad significativa en cáncer de pulmón
microcítico o no microcítico, cáncer de cabeza y cuello, y en
melanoma metastásico. Sin embargo, una dificultad importante en el
desarrollo de paclitaxel para el uso en estudios clínicos ha sido su
insolubilidad en agua.
Docetaxel se produce de forma semisintética a
partir de 10-desacetil baccatina III, un precursor
no citotóxico extraído de las acículas de Taxus baccata y
esterificado con una cadena lateral sintetizada químicamente (Cortes
y Pazdur, 1995). Se ha demostrado que diversas líneas celulares
cancerosas, que incluyen cáncer de mama, de pulmón, ovárico y
colorrectal y melanomas, responden a docetaxel. Se ha usado
docetaxel en ensayos clínicos para conseguir respuestas completas o
parciales en cáncer de mama, ovárico, y de cabeza y cuello, y en
melanoma maligno.
Paclitaxel se formula típicamente como una
disolución concentrada que contiene 6 mg de paclitaxel por mililitro
de Cremophor EL (aceite de ricino polioxietilado) y alcohol
deshidratado (50% v/v), y se debe diluir adicionalmente antes de la
administración (Goldspiel, 1994). La cantidad de Cremophor EL
necesaria para administrar las dosis necesarias de paclitaxel es
significativamente mayor que la administrada con cualquier otro
fármaco que se formula en Cremophor. Se han atribuido diversos
efectos tóxicos a Cremophor, que incluyen vasodilatación, disnea e
hipotensión. Se ha demostrado también que este vehículo provoca
hipersensibilidad grave en animales de laboratorio y humanos (Weiss
et al., 1990). En realidad, la dosis máxima de paclitaxel que
se puede administrar a ratones mediante inyección rápida i.v. está
impuesta por la toxicidad aguda letal del vehículo de Cremophor
(Eiseman et al., 1994). Además, se sabe que Cremophor EL, un
tensoactivo, lixivia plastificantes de ftalato tales como
di(2-etilhexil)ftalato (DEHP) de las
bolsas y los tubos de administración intravenosa de
poli(cloruro de vinilo). Se sabe que DEHP provoca
hepatotoxicidad en animales y es carcinógeno en roedores. También se
demuestra que esta preparación de paclitaxel forma materia
particulada con el tiempo, y así es necesaria la filtración durante
la administración (Goldspiel, 1994). Por lo tanto, son necesarias
condiciones especiales para la preparación y administración de
disoluciones de paclitaxel para asegurar la administración segura
del fármaco a los pacientes, y estas condiciones conducen de forma
inevitable a costes superiores.
Los intentos anteriores para obtener paclitaxel
hidrosoluble han incluido la preparación de profármacos de
paclitaxel colocando restos solubilizantes, tales como succinato y
aminoácidos, en el grupo 2'-hidroxilo o en la
posición 7-hidroxilo (Deutsch et al., 1989;
Mathew et al., 1992). Sin embargo, no se ha probado que
estos profármacos sean lo suficientemente estables químicamente para
su desarrollo. Por ejemplo, Deutsch et al. (1989) informan
de un derivado 2'-succinato de paclitaxel, pero la
solubilidad en agua de la sal sódica es solamente alrededor del
0,1%, y las sales de trietanolamina y
N-metilglucamina fueron solubles solamente al 1%.
Además, se informó que los ésteres de aminoácidos eran inestables.
Mathew et al. (1992) informaron de resultados similares.
Greenwald et al. informaron la síntesis de ésteres de 2' y
7-polietilenglicol de taxol sumamente hidrosolubles
(Greenwald et al., 1994), sin embargo, no se informó de los
datos concernientes a la actividad antitumoral in vivo de
estos compuestos (Greenwald et al. 1995). Se describen
composiciones de polímero hidrosoluble (PEG) conjugado con taxol en
los documentos WO-A-96/23794,
WO-A-98/07713,
WO-A-93/24476 y
WO-A-95/11924.
Otros intentos para resolver estos problemas han
implicado la microencapsulación de paclitaxel tanto en liposomas
como en nanoesferas (Bartoni y Boitard, 1990). Se informó que la
formulación de liposomas era tan efectiva como paclitaxel libre,
sin embargo solamente las formulaciones de liposomas que contenían
menos del 2% de paclitaxel fueron estables físicamente (Sharma y
Straubinger, 1994). Desgraciadamente, se demostró que la
formulación de nanoesferas era tóxica. Por lo tanto, existe todavía
la necesidad de una formulación de paclitaxel hidrosoluble que
pueda administrar cantidades eficaces de paclitaxel y docetaxel sin
las desventajas provocadas por la insolubilidad del fármaco. El
documento WO-A-94/00156 describe un
polímero basado en unidades de aminoácidos conjugado con taxol para
obtener una solubilidad en agua mejorada.
Otro obstáculo del uso generalizado de
paclitaxel son los recursos limitados a partir de los que se produce
paclitaxel, lo que provoca que la terapia con paclitaxel sea cara.
Un tratamiento puede costar varios miles de dólares, por ejemplo.
Existe la desventaja añadida de que no todos los tumores responden a
la terapia con paclitaxel, y esto puede ser debido a que paclitaxel
no esté llegando al tumor. Existe una necesidad inmediata, por lo
tanto, de formulaciones eficaces de paclitaxel y fármacos
relacionados que sean hidrosolubles con semividas séricas largas
para el tratamiento de tumores, enfermedades autoinmunes tales como
artritis reumatoide, así como para la prevención de la reestenosis
de los vasos sometidos a traumatismos, tales como angioplastia y
colocación de mallas.
La presente invención persigue superar estas y
otras desventajas inherentes a la técnica anterior proporcionando
composiciones que comprenden un fármaco quimioterápico y
antiangiogénico, tal como paclitaxel o docetaxel, conjugado con un
polímero hidrosoluble, tal como un ácido poliglutámico o un ácido
poliaspártico, por ejemplo. Se demuestra aquí que estas
composiciones son sorprendentemente eficaces como agentes
antitumorales contra modelos de tumores ejemplares, y se espera que
sean al menos tan eficaces como paclitaxel o docetaxel contra
cualquiera de las enfermedades o trastornos para los que se sabe que
los taxanos o taxoides son eficaces. Las composiciones de la
invención proporcionan taxoides hidrosolubles para superar las
desventajas asociadas a la insolubilidad de los fármacos solos, y
también proporcionan la ventaja de liberación controlada, de forma
que se demuestra aquí que los tumores se erradican en los modelos
animales después de una única administración intravenosa.
Los métodos descritos aquí se podrían usar
también para hacer conjugados con polímeros hidrosolubles de otros
agentes terapéuticos y fármacos, que incluyen etopósido, tenipósido,
camptotecina, epotilonas. En particular, los agentes con un grupo
hidroxilo libre se conjugarían con los polímeros mediante reacciones
químicas similares a las descritas aquí para paclitaxel. Tal
conjugación estaría dentro de la experiencia de un practicante
rutinario de la técnica química, y como tal se encontraría dentro
del alcance de la invención reivindicada. Los agentes incluirían
etopósido, tenipósido, camptotecina y las epotilonas. Como se usa
aquí, conjugado con un polímero hidrosoluble significa la unión
mediante enlace covalente del fármaco con el polímero.
También se entiende que los conjugados
hidrosolubles de la presente invención se pueden administrar junto
con otros fármacos, que incluyen otros fármacos antitumorales o
antineoplásicos. Tales combinaciones se conocen en la técnica. El
paclitaxel o docetaxel hidrosoluble de la presente invención se
puede combinar, en ciertos tipos de tratamiento, con un fármaco de
platino, un antibiótico, tal como doxorrubicina o daunorrubicina,
por ejemplo, u otros fármacos que se usan en combinación con
Taxol.
La conjugación de fármacos quimioterápicos con
polímeros es una aproximación atractiva para reducir la toxicidad
sistémica y mejorar el índice terapéutico. Los polímeros con masa
molecular superior de 30 kDa no difunden fácilmente a través del
endotelio normal de los capilares y del glomérulo, evitando así al
tejido normal la toxicidad irrelevante mediada por el fármaco
(Maeda y Matsumura, 1989; Reynolds, 1995). Por otra parte, está
bien establecido que los tumores malignos tienen a menudo un
endotelio capilar alterado y una permeabilidad superior que la
vasculatura tisular normal (Maeda y Matsumura, 1989; Fidler et
al., 1987). Así, un conjugado de
polímero-fármaco que permanecería normalmente en la
vasculatura se puede escapar de forma selectiva de los vasos
sanguíneos hacia los tumores, lo que da como resultado la
acumulación en el tumor del fármaco terapéutico activo. Además, los
conjugados de polímero-fármaco pueden actuar como
fármacos de liberación lenta para liberación sostenida, lo que
produce una exposición prolongada al fármaco de las células
tumorales. Finalmente, los polímeros hidrosolubles se pueden usar
para estabilizar fármacos, así como para solubilizar compuestos de
otra forma insolubles. Actualmente, se ha examinado la capacidad de
potenciar la administración de fármacos específicos de tumores en
una diversidad de polímeros sintéticos y naturales (Kopecek, 1990,
Maeda y Matsumura, 1989). Sin embargo, actualmente se está
sometiendo a evaluación clínica solamente a unos pocos, que incluyen
SMANCS en Japón y HPMA-Dox en el Reino Unido
(Maeda, 1991; Kopecek y Kopeckova, 1993).
En la presente descripción, se entiende que un
taxoide quiere decir aquellos compuestos que incluyen paclitaxel y
docetaxel, y otros productos químicos que tienen el esqueleto de
taxano (Cortes y Pazdur, 1995), y se pueden aislar de fuentes
naturales tales como el tejo, o de cultivo celular, o de moléculas
sintetizadas químicamente, y se prefiere un producto químico de la
fórmula química general C_{47}H_{51}NO_{14}, que incluye éster
6,12b,bis(acetiloxi)-12-(benzoiloxi)-2a,3,4,4a,5,6,9,10,11,12,12a,12b-dodecahidro-4,11-dihidroxi-4a,8,13,13-tetrametil-5-oxo-7,11-metano-1H-ciclodeca[3,4]benz-[1,2-b]oxet-9-ílico
del ácido
[2aR-[2a\alpha,4\beta,4\alpha\beta,6\beta,9\alpha(\alphaR*,\betaS*),11\alpha,12\alpha,12a\alpha,12b\alpha,]]-\beta-(benzoilamino)-\alpha-hidroxibencenpropanoico.
Se entiende que paclitaxel y docetaxel son cada uno más efectivos
que el otro contra ciertos tipos de tumores, y que en la práctica
de la presente invención los tumores que son más susceptibles a un
taxoide particular se tratarían con ese conjugado de taxoide
hidrosoluble.
En ciertas realizaciones de la presente
invención, se puede conjugar paclitaxel o docetaxel con un polímero
hidrosoluble, y preferiblemente el polímero se conjuga con el 2' ó
el 7-hidroxilo, o ambos, de paclitaxel o docetaxel.
Así, cuando se usan grupos funcionales para la conjugación del
fármaco, como anteriormente con el C2'-hidroxilo de
paclitaxel, se usa una unión degradable, en este caso un éster, para
asegurar que el fármaco activo se libera del vehículo polimérico.
Los polímeros preferibles incluyen poli(ácido
L-glutámico), poli(ácido
D-glutámico), poli(ácido
DL-glutámico), poli(ácido
L-aspártico), poli(ácido
D-aspártico), poli(ácido
DL-aspártico), copolímeros de los poliaminoácidos
enumerados anteriormente con policaprolactona, ácido poliglicólico y
ácido poliláctico, así como ácido poliacrílico,
poli(2-hidroxietil
L-glutamina), carboximetildextrano, ácido
hialurónico, albúmina de suero humano y ácido algínico, y se
prefieren particularmente los ácidos poliaspárticos y ácidos
poliglutámicos. Los ácidos poliglutámicos o ácidos poliaspárticos de
la presente invención tienen preferiblemente un peso molecular de
alrededor de 5.000 a alrededor de 100.000, y más preferiblemente
alrededor de 20.000 a alrededor de 80.000, o incluso alrededor de
30.000 a alrededor de 60.000.
Se entiende que las composiciones de la presente
invención se pueden dispersar en una disolución excipiente
farmacéuticamente aceptable como se describe más adelante. Tal
disolución sería estéril o aséptica, y puede incluir agua,
tampones, agentes isotónicos u otros ingredientes conocidos para los
expertos en la técnica que no provocarían reacción alérgica u otra
reacción dañina cuando se administran a un sujeto animal o humano.
Por lo tanto, la presente invención se puede describir también como
una composición farmacéutica que comprende un fármaco
quimioterápico o antineoplásico tal como paclitaxel o docetaxel
conjugado con un polímero hidrosoluble de peso molecular elevado.
La composición farmacéutica puede incluir ácidos poliglutámicos o
ácidos poliaspárticos.
La presente invención se puede describir también
en ciertas realizaciones como un método para tratar el cáncer en un
sujeto. Este método incluye obtener una composición que comprende un
fármaco quimioterápico tal como paclitaxel o docetaxel conjugado
con un polímero hidrosoluble y dispersado en una disolución
farmacéuticamente aceptable, y administrar la disolución al sujeto
en una cantidad eficaz para tratar el tumor. Las composiciones
preferidas comprenden paclitaxel o docetaxel conjugado con un ácido
poliglutámico o un ácido poliaspártico, y más preferiblemente con
poli(ácido L-glutámico) o poli(ácido
L-aspártico). Se entiende que las composiciones de
la invención son eficaces contra cualquier tipo de cáncer para el
que se demuestre que el taxoide sin conjugar es eficaz, e
incluiría, pero no se limitaría a, cáncer de mama, cáncer ovárico,
melanoma maligno, cáncer de pulmón, cáncer gástrico, cáncer de
colon, cáncer de cabeza y cuello o leucemia.
La presente invención se puede describir también
en ciertos aspectos amplios como un método para disminuir al menos
un síntoma de una enfermedad autoinmune sistémica, que comprende
administrar a un sujeto que tiene una enfermedad autoinmune
sistémica una cantidad eficaz de una composición que comprende
paclitaxel o docetaxel conjugado con poli(ácido
L-glutámico) o poli(ácido
L-aspártico). Es de particular interés en el
contexto de la presente descripción el tratamiento de artritis
reumatoide, que se sabe que responde en algunos casos a taxol
cuando se administra en la formulación de Cremophor estándar
(patente de EE.UU. nº 5.583.153). Como en el tratamiento de
tumores, se contempla que la eficacia de los taxoides hidrosolubles
de la presente invención no disminuirá por la conjugación con un
resto hidrosoluble, y que el profármaco hidrosoluble puede actuar
como una formulación de liberación controlada que libera el fármaco
activo a lo largo de un periodo de tiempo. Por lo tanto, se espera
que las composiciones de la presente invención sean tan eficaces
como Taxol contra artritis reumatoide, por ejemplo, pero tendrán la
ventaja de una liberación controlada. También se entiende que las
composiciones de taxoide de la presente invención se pueden usar en
combinación con otros fármacos, tales como un inhibidor de la
angiogénesis (AGM-1470) (Oliver et al., 1994)
o metrotrexato.
El descubrimiento de que paclitaxel también
inhibe la reestenosis después de angioplastia con globo indica que
los paclitaxeles y docetaxeles hidrosolubles de la presente
invención tendrán una diversidad de aplicaciones aparte de la
administración parenteral directa (documento WO 9625176). Por
ejemplo, se contempla que paclitaxel hidrosoluble será útil como
revestimiento para dispositivos médicos implantados, tales como
tubos, derivaciones, catéteres, implantes artificiales, alambres,
implantes eléctricos tales como marcapasos, y especialmente para
mallas arteriales o venosas, que incluyen mallas expansibles con
globo. En estas realizaciones se contempla que el paclitaxel
hidrosoluble puede estar unido a un dispositivo médico implantable,
o alternativamente, el paclitaxel hidrosoluble puede estar
adsorbido de forma pasiva en la superficie del dispositivo
implantable. Por ejemplo, las mallas se pueden revestir con
conjugados de polímero-fármaco introduciendo la
malla en la disolución de polímero-fármaco o
pulverizando la malla con tal disolución. Los materiales adecuados
para el dispositivo implantable deberían ser biocompatibles y
atóxicos, y se pueden elegir de metales, tales como aleaciones de
níquel-titanio, acero, o polímeros biocompatibles,
hidrogeles, poliuretanos, polietilenos, copolímeros de acetato de
etilenvinilo, etc. En una realización preferida, el paclitaxel
hidrosoluble, especialmente un conjugado de
PG-paclitaxel, se reviste sobre una malla para
inserción en una arteria o vena después de angioplastia con globo.
La invención se puede describir, por lo tanto, en ciertos aspectos
amplios como un método para inhibir la reestenosis arterial o la
oclusión arterial después de traumatismo vascular, que comprende
administrar a un sujeto que lo necesita una composición que
comprende paclitaxel o docetaxel conjugado con poli(ácido
L-glutámico) o poli(ácido
L-aspártico). En la práctica del método, el sujeto
puede ser un paciente de revascularización coronaria, cirugía
vascular, trasplante de órgano o angioplastia coronaria o arterial,
por ejemplo, y la composición se puede administrar directamente, de
forma intravenosa, o incluso revestida sobre una malla, y la malla
se implanta en el lugar de traumatismo vascular.
Una realización de la invención es, por lo
tanto, un dispositivo médico implantable, en el que el dispositivo
está revestido con una composición que comprende paclitaxel o
docetaxel conjugado con ácidos poliglutámicos o ácidos
poliaspárticos en una cantidad efectiva para inhibir la
proliferación de células musculares lisas. Un dispositivo preferido
es una malla revestida con las composiciones de la presente
invención descritas aquí, y en ciertas realizaciones preferidas la
malla se adapta para ser usada después de angioplastia con globo, y
el revestimiento es eficaz para inhibir la reestenosis.
En ciertas realizaciones preferidas, la
invención se puede describir como una composición que comprende
ácidos poliglutámicos conjugados con el 2' ó
7-hidroxilo o ambos de paclitaxel, o incluso una
composición que comprende ácido poliaspártico conjugado con el 2' ó
7-hidroxilo o ambos de paclitaxel. Como se usa aquí,
la expresión "un ácido poliglutámico" o "ácidos
poliglutámicos" incluye poli(ácido L-glutámico),
poli(ácido D-glutámico) y poli(ácido
DL-glutámico), y la expresión "un ácido
poliaspártico" o "ácidos poliaspárticos" incluye poli(ácido
L-aspártico), poli(ácido
D-aspártico) y poli(ácido
DL-aspártico).

La presente invención surge del descubrimiento
de formulaciones nuevas e hidrosolubles de paclitaxel y docetaxel,
y la eficacia sorprendente de estas formulaciones contra células
tumorales in vivo. Paclitaxel conjugado con poli(ácido
L-glutámico) (PG-paclitaxel)
administrado a ratones que albergan carcinoma ovárico
(OCa-I) provocó un retraso significativo en el
crecimiento del tumor comparado con la misma dosis de paclitaxel sin
PG. Los ratones tratados con paclitaxel solamente o con una
combinación de paclitaxel libre y PG mostraron un crecimiento
tumoral retrasado inicialmente, pero los tumores volvieron a crecer
a niveles comparables a los de un grupo de control sin tratar
después de diez días. Además, a la dosis máxima tolerable (DMT) del
conjugado de PG-paclitaxel (160 mg equiv. de
paclitaxel/kg), el crecimiento de los tumores se suprimió
completamente, los tumores encogieron y los ratones observados
durante dos meses después del tratamiento permanecieron sin tumores
(DMT: definida como la dosis máxima que produjo un 15% o menos de
pérdida de peso corporal en dos semanas después de una inyección
i.v. única). En un estudio paralelo, se examinó la actividad
antitumoral de PG-paclitaxel en ratas con
adenocarcinoma mamario de ratas (13762F). Una vez más, se observó la
erradicación tumoral completa a 40-60 mg equiv. de
paclitaxel/kg de PG-paclitaxel. Estos resultados
sorprendentes demuestran que el conjugado de
polímero-fármaco, PG-paclitaxel,
erradica con éxito tumores sólidos bien establecidos tanto en
ratones como en ratas después de una inyección intravenosa única.
Además, con una semivida de 40 días a pH 7,4,
PG-paclitaxel es uno de los derivados de paclitaxel
hidrosolubles más estables conocidos (Deutsch, et al., 1989;
Mathew et al., 1992; Zhao y Kingston,
1991).

Los compuestos y métodos nuevos de la presente
invención proporcionan avances significativos sobre los métodos y
composiciones anteriores, ya que se prevé que los paclitaxeles
hidrosolubles mejoren la eficacia de la terapia antineoplásica
basada en paclitaxel, proporcionando composiciones derivadas de
paclitaxel hidrosolubles y de liberación controlada. Tales
composiciones eliminan la necesidad de disolventes que están
asociados con los efectos secundarios observados con las
composiciones de paclitaxel anteriores.
El paclitaxel se puede hacer hidrosoluble
conjugando paclitaxel con polímeros hidrosolubles que sirven como
vehículos para el fármaco. Las estructuras de paclitaxel y del
conjugado de ácido poliglutámico-paclitaxel
(PG-paclitaxel) se muestran en la Fig. 1.
En ciertas realizaciones de la invención, se
puede usar una malla revestida con los conjugados de
polímero-paclitaxel para prevenir la reestenosis,
el cierre de arterias después de angioplastia con globo. Los
resultados recientes en ensayos clínicos usando mallas expansibles
con globo en angioplastia coronaria han demostrado un beneficio
significativo en la permeabilidad y en la reducción de la
reestenosis comparado con la angioplastia con globo estándar
(Serruys et al., 1994). Según la hipótesis de respuesta a
lesión, la formación de neoíntima está asociada a una proliferación
celular incrementada. Actualmente, la opinión general mantiene que
el proceso crítico que conduce a lesiones vasculares tanto en
aterosclerosis espontánea como acelerada es la proliferación de
células musculares lisas (CML) (Phillips-Hughes y
Kandarpa, 1996). Ya que la proliferación fenotípica de CML después
de lesión arterial imita la de las células neoplásicas, es posible
que los fármacos antineoplásicos puedan ser útiles para prevenir la
acumulación de CML neoíntimas. Las mallas revestidas con agentes
antiproliferativos unidos a polímero que son capaces de liberar
estos agentes a lo largo de un periodo de tiempo prolongado con
concentración suficiente prevendrán así el crecimiento de la íntima
y la media hiperplásicas hacia la luz, reduciendo por ello la
reestenosis.
Debido a que se ha demostrado que paclitaxel
suprime la artritis inducida por colágeno en un modelo de ratón
(Oliver et al., 1994), se contempla también que las
formulaciones de la presente invención son útiles en el tratamiento
de enfermedades autoinmunes y/o inflamatorias, tales como artritis
reumatoide. La unión de paclitaxel a tubulina desplaza el
equilibrio hacia polímeros microtubulares estables, y hace de este
fármaco un inhibidor potente de la replicación de células
eucarióticas bloqueando las células en la etapa mitótica G2 tardía.
Diversos mecanismos pueden estar implicados en la supresión de la
artritis por paclitaxel. Por ejemplo, los efectos citotóxicos
específicos de fase de paclitaxel pueden afectar a las células
inflamatorias en proliferación rápida, y además paclitaxel inhibe
la mitosis celular, la migración, la quimiotaxia, el transporte
intracelular y la producción de H_{2}O_{2} en los neutrófilos.
Además, paclitaxel puede tener actividad antiangiogénica bloqueando
la migración coordinada de células endoteliales (Oliver et
al., 1994). Por lo tanto, se contempla que los profármacos
conjugados con polímero de la presente invención son tan útiles como
paclitaxel libre en el tratamiento de artritis reumatoide. La
formulación conjugada con polímero descrita aquí ofrecería también
las ventajas de liberación retrasada o sostenida del fármaco y
solubilidad superior. También es un aspecto del tratamiento de
artritis que las formulaciones se pueden inyectar o implantar
directamente en las zonas de las articulaciones afectadas.
Las preparaciones farmacéuticas de paclitaxel o
docetaxel adecuadas para uso inyectable incluyen disoluciones o
dispersiones acuosas estériles y polvos estériles para la
preparación de disoluciones o dispersiones inyectables estériles.
En todos los casos la forma debe ser estéril y debe ser líquida para
inyección. Debe ser estable en las condiciones de fabricación y de
almacenamiento, y se debe preservar de la acción contaminante de
microorganismos, tales como bacterias y hongos. El vehículo debe
ser un disolvente o un medio de dispersión que contiene, por
ejemplo, agua, etanol, poliol (por ejemplo, glicerol, propilenglicol
y similares), sus mezclas adecuadas, y aceites vegetales. La
prevención de la acción de microorganismos se puede llevar a cabo
mediante diversos agentes antibacterianos y antifúngicos, por
ejemplo, parabenos, clorobutanol, fenol, ácido sórbico, timerosal y
similares. En muchos casos, será preferible incluir agentes
isotónicos, por ejemplo, hidratos de carbono o cloruro sódico.
Las disoluciones inyectables estériles se
preparan incorporando los compuestos activos en la cantidad
necesaria en el disolvente apropiado con diversos ingredientes de
los enumerados anteriormente, según sea necesario, seguido de
esterilización mediante filtración. En general, las dispersiones se
preparan incorporando los diversos ingredientes activos
esterilizados en un vehículo estéril que contiene el medio de
dispersión básico y los otros ingredientes necesarios de los
enumerados anteriormente. En el caso de polvos estériles para la
preparación de disoluciones inyectables estériles, los métodos
preferidos de preparación son las técnicas de secado al vacío y
liofilización, que producen un polvo del ingrediente activo más
cualquier ingrediente adicional deseado de una disolución suya
previamente filtrada en condiciones estériles.
Como se usa aquí, "vehículo farmacéuticamente
aceptable" incluye todos los disolventes, medios de dispersión,
revestimientos, agentes antibacterianos y antifúngicos y agentes
isotónicos y similares. El uso de tales medios y agentes para
sustancias farmacéuticamente activas se conoce bien en la técnica.
Excepto en la medida en que cualquier medio o agente convencional
sea incompatible con el ingrediente activo, se contempla su uso en
las composiciones terapéuticas. Se pueden incorporar también
ingredientes activos complementarios en las composiciones.
La frase "farmacéuticamente aceptable"
también se refiere a entidades moleculares y a composiciones que no
producen una reacción alérgica o adversa similar cuando se
administra a un animal o a un humano.
Para administración parenteral en una disolución
acuosa, por ejemplo, la disolución se debería tamponar de forma
adecuada si es necesario, y el disolvente líquido se debería hacer
primero isotónico con suficiente solución salina o glucosa. Estas
disoluciones acuosas particulares son especialmente adecuadas para
la administración intravenosa e intraperitoneal. A este respecto,
los medios acuosos estériles que se pueden emplear serán conocidos
para los expertos en la técnica a la luz de la presente
descripción.
Los siguientes ejemplos se incluyen para
demostrar las realizaciones preferidas de la invención. Los expertos
en la técnica deberían apreciar que las técnicas descritas en los
ejemplos siguientes representan técnicas descubiertas por el
inventor que funcionan bien en la práctica de la invención, y así se
puede considerar que constituyen modos preferidos para su
práctica.
El presente ejemplo demuestra la conjugación de
paclitaxel con un polímero hidrosoluble, poli(ácido
L-glutámico) (PG). El potencial de los polímeros
hidrosolubles usados como vehículos de fármacos está bien
establecido (Kopecek, 1990; Maeda y Matsumura, 1989). Además de su
capacidad para solubilizar fármacos de otra manera insolubles, el
conjugado de fármaco-polímero también actúa como una
preparación de liberación lenta para la liberación controlada del
fármaco.
Se seleccionó PG como vehículo para paclitaxel
porque puede ser degradado fácilmente por las enzimas lisosómicas,
es estable en el plasma y contiene suficientes grupos funcionales
para la unión del fármaco. Se han conjugado a PG diversos fármacos
antineoplásicos, que incluyen adriamicina (Van Heeswijk et
al., 1985; Hoes et al., 1985), ciclofosfamida (Hirano
et al., 1979), y Ara-C (Kato et al.,
1984).
Se disolvió sal sódica de PG (MW 34 K, Sigma,
0,35 g) en agua. El pH de la disolución acuosa se ajustó a 2 usando
HCl 0,2 M. El precipitado se recogió, se dializó con agua destilada
y se liofilizó para producir 0,29 g de PG.
A una disolución de PG (75 mg, unidad repetitiva
FW 170, 0,44 mmoles) en DMF seco (1,5 ml) se le añadieron 20 mg de
paclitaxel (0,023 mmoles, proporción molar PG/paclitaxel = 19), 15
mg de diciclohexilcarbodiimida (DCC) (0,073 mmoles) y una cantidad
traza de dimetilaminopiridina (DMAP). La reacción se dejó
transcurrir a temperatura ambiente durante 4 h. La cromatografía en
capa fina (CCF, sílice) mostró la conversión completa de
paclitaxel
(Rf = 0,55) a conjugado de polímero (Rf = 0, fase móvil, CHCl_{3}/MeOH = 10:1). La mezcla de reacción se vertió en cloroformo. El precipitado resultante se recogió y se secó al vacío para producir 65 mg de conjugado de polímero-fármaco. Cambiando la proporción en peso de paclitaxel respecto de PG de los materiales de partida se pueden sintetizar conjugados poliméricos de diversas concentraciones de paclitaxel.
(Rf = 0,55) a conjugado de polímero (Rf = 0, fase móvil, CHCl_{3}/MeOH = 10:1). La mezcla de reacción se vertió en cloroformo. El precipitado resultante se recogió y se secó al vacío para producir 65 mg de conjugado de polímero-fármaco. Cambiando la proporción en peso de paclitaxel respecto de PG de los materiales de partida se pueden sintetizar conjugados poliméricos de diversas concentraciones de paclitaxel.
Se obtuvo la sal sódica del conjugado de
PG-paclitaxel disolviendo el producto en NaHCO_{3}
0,5 M. La disolución acuosa de PG-paclitaxel se
dializó con agua destilada (MWCO 1.000) para eliminar los
contaminantes de peso molecular bajo y la sal de NaHCO_{3} en
exceso. La liofilización del dializado produjo 88,6 mg de polvo
blanco. El contenido de paclitaxel en este conjugado polimérico se
determinó mediante UV (descrito más adelante) como el 21% (p/p).
Rendimiento (conversión a paclitaxel unido a polímero, UV): 93%. Se
puede sintetizar PG-paclitaxel con contenido de
paclitaxel superior (hasta el 35%) mediante este método,
incrementando simplemente la proporción de paclitaxel respecto de
PG utilizada.

5,63 y 4,78 ppm (C_{2}'-H),
5,55-5,36 ppm (C_{3}'-H y
C_{2}-H, m), 5,10 ppm (C_{5}-H),
4,39 ppm (C_{7}-H), 4,10
(C_{20}-H), 1,97 ppm (OCOCH_{3}), y
1,18-1,20 ppm (C-CH_{3}) se
asignan a los componentes alifáticos de paclitaxel. Otras
resonancias de paclitaxel fueron ocultadas por las resonancias de
PG. Las resonancias de PG a 4,27 ppm (H-\alpha),
2,21 ppm (H-\gamma), y 2,04 ppm
(H-\beta) están de acuerdo con el espectro de PG
puro. Los acoplamientos de paclitaxel conjugado con polímero se
resuelven demasiado pobremente para medirse con suficiente
exactitud. La solubilidad en agua fue >20 mg de
paclitaxel/ml.
Se obtuvieron los espectros ultravioletas (UV)
en un espectrofotómetro Beckman DU-640 (Fullerton,
CA). El contenido de paclitaxel conjugado con PG se estimó mediante
UV basándose en una curva patrón generada con concentraciones
conocidas de paclitaxel en metanol (\lambda = 228 nm), suponiendo
que el conjugado de polímero en agua y el fármaco libre en metanol
tienen los mismos coeficientes de extinción molar y que ambos siguen
la ley de Lambert Beer. Como se demuestra mediante su espectro UV,
PG-paclitaxel tiene la absorción característica de
paclitaxel con desplazamientos de \lambda de 228 a 230 nm. La
concentración de paclitaxel en PG-paclitaxel se
estimó basándose en la curva patrón generada con concentraciones
conocidas de paclitaxel en metanol a partir de la absorción a 228
nm, suponiendo que el conjugado de polímero en agua a 230 nm y el
fármaco libre en metanol a 228 nm tienen la misma extinción molar y
ambos siguen la ley de Lambert Beer.
Se caracterizó el peso molecular relativo de
PG-paclitaxel mediante cromatografía de
penetrabilidad en gel (CPG). El sistema de CPG consistió en dos
bombas LDC modelo III acopladas a un controlador de gradiente LDC,
una columna de CPG con gel PL, y un detector de red de fotodiodos
Waters 990. El eluyente (DMF) se hizo pasar a 1,0 ml/min con la
detección UV fijada a 270 nm. La conjugación de paclitaxel a PG dio
como resultado un incremento en el peso molecular de
PG-paclitaxel, como indica el desplazamiento del
tiempo de retención de 6,4 min de PG a 5,0 min del conjugado de
PG-paclitaxel analizado mediante CPG. El peso
molecular calculado de PG-paclitaxel que contiene
un 15-25% de paclitaxel (p/p) está en el intervalo
de 45-55 kDa. El producto en bruto contenía un
contaminante de peso molecular bajo (tiempo de retención 8,0 a 10,0
min, y 11,3 min), que se puede eliminar de forma efectiva
convirtiendo PG-paclitaxel en su sal de sodio,
seguido de diálisis.
Se disolvió PG-paclitaxel en
disoluciones tamponadas con fosfato (PBS, 0,01 M) a pH 6,0, pH 7,4 y
pH 9,6 a una concentración de paclitaxel equivalente de 0,4 mM. Las
disoluciones se incubaron a 37ºC con agitación suave. A los
intervalos de tiempo seleccionados, se extrajeron alícuotas (100
\mul), se mezclaron con un volumen igual de metanol y se
analizaron mediante cromatografía líquida de alta resolución (HPLC).
El sistema de HPLC consistió en una columna de sílice de fase
inversa (Nova-Pac, Waters, CA), una fase móvil de
metanol-agua (2:1, v/v) administrada a un caudal de
1,0 ml/min, y un detector de fotodiodos. La concentración de
paclitaxel unido a PG, paclitaxel libre y otros productos de
degradación en cada muestra se calculó comparando las áreas de los
picos con una curva patrón obtenida separadamente preparada a partir
de paclitaxel, suponiendo que el coeficiente de extinción molar de
cada pico a 228 nm es el mismo que el de paclitaxel. La semivida del
conjugado, que se estimó que era 132, 40 y 4 días a pH 6,0, 7,4 y
9,6, respectivamente, se determinó mediante un análisis de
regresión lineal de mínimos cuadrados. El análisis de HPLC reveló
que la incubación de PG-paclitaxel en disoluciones
de PBS produjo paclitaxel y diversas especies, que incluyen una que
es más hidrófoba que paclitaxel (metabolito-1). En
realidad, la cantidad de metabolito-1, que fue lo
más probablemente 7-epipaclitaxel, recuperada en
PBS a pH 7,4 superó la de paclitaxel después de 100 horas de
incubación (Fig. 2).
Las alícuotas obtenidas de la disolución en PBS
a pH 7,4 se sometieron a análisis mediante un ensayo de
polimerización de tubulina. La reacción de ensamblaje de tubulina
se realizó a 32ºC en tampón PEM (pH 6,9) a una concentración de
tubulina (cerebro bovino, Cytoskeleton Inc., Boulder, CO) de 1 mg/ml
(10 \muM) en presencia de las muestras de ensayo (1,0 \muM
equiv. de paclitaxel) y 1,0 mM de GTP. La polimerización de tubulina
se siguió midiendo la absorbancia de la disolución a 340 nm a lo
largo del tiempo. Después de 15 min, se añadió cloruro cálcico (125
mM) para medir la despolimerización de microtúbulos inducida por
CaCl_{2}. Mientras PG-paclitaxel recién disuelto
en PBS fue inactivo para producir microtúbulos, las alícuotas de
PG-paclitaxel incubadas durante tres días dieron
como resultado la polimerización de tubulina. Los microtúbulos
formados fueron estables contra la despolimerización inducida por
CaCl_{2}.
El efecto de PG-paclitaxel sobre
el crecimiento celular también se examinó mediante ensayo de sal de
tetrazolio (MTT) (Mosmann, 1983). Se sembraron células
MCF-7 ó células 13762F a 2 x 10^{4} células/ml en
una placa de microtitulación de 96 pocillos tratada 24 h después
con diversas concentraciones de PG-paclitaxel,
paclitaxel o PG, y se incubaron durante 72 h adicionales. Después
se añadió disolución MTT (20 \mul, 5 mg/ml) a cada pocillo y se
incubó durante 4 h. Se aspiró el sobrenadante, y el formazán de MTT
formado por las células metabólicamente viables se midió mediante
un lector de fluorescencia de microplacas a una longitud de onda de
590 nm. A lo largo del periodo de tres días,
PG-paclitaxel inhibió la proliferación de células
tumorales en una medida similar a la de paclitaxel libre. Para la
línea de células de tumor de mama humano MCF-7, los
valores de CI_{50} resultantes fueron 0,59 \muM para paclitaxel
y 0,82 \muM para PG-paclitaxel (medidos en
unidades equivalentes de paclitaxel). Contra la línea celular
13762F, la sensibilidad a PG-paclitaxel (CI_{50} =
1,86 \muM) fue comparable a la de paclitaxel (CI_{50} = 6,79
\muM). Para ambas líneas celulares la CI_{50} de PG solo fue
mayor de 100 \muM.
Todo el trabajo con animales se llevó a cabo en
la instalación de animales en el M.D. Anderson Cancer Center según
las directrices institucionales. Se criaron y se mantuvieron ratones
C3H/Kam en una instalación sin patógenos en el Departamento de
Oncología Radioterápica Experimental. Se produjeron tumores únicos
en el músculo del muslo derecho de ratones C3H/Kam hembra
(25-30 g) inyectando 5 x 10^{5} células de
carcinoma ovárico murino (OCa-I), carcinoma mamario
(MCa-4), hepatocarcinoma (HCa-I) o
fibrosarcoma (FSa-II). En un estudio paralelo, se
inyectó a ratas Fischer 344 hembra (125-150 g) 1,0 x
10^{5} células viables de tumor 13762F en 0,1 ml de PBS. Los
tratamientos se iniciaron cuando los tumores en ratones crecieron
hasta 500 mm^{3} (10 mm de diámetro), o cuando los tumores en
ratas crecieron hasta 2400 mm^{3} (diámetro medio 17 mm). Se dio
una dosis única de PG-paclitaxel en solución salina
o paclitaxel en vehículo de Cremophor EL en dosis que variaban de
40 a 160 mg equiv. de paclitaxel/kg de peso corporal. En los
experimentos de control, se usó suero salino, vehículo de Cremophor
(50/50 de Cremophor/etanol diluido con solución salina (1: 4)),
disolución de PG (MW 38K) en solución salina, y una mezcla de
paclitaxel/PG. El crecimiento tumoral se determinó diariamente
(Fig. 3A, 3B, 3C, 3D y 3E) midiendo tres diámetros tumorales
ortogonales. El volumen del tumor se calculó según la fórmula (A x
B x C)/2. El retraso del crecimiento absoluto (RCA) en ratones se
define como el tiempo en días para que los tumores tratados con
diversos fármacos crezcan de 500 a 2.000 mm^{3} en ratones menos
el tiempo en días para los tumores tratados con control de solución
salina para que crezcan de 500 a 2.000 mm^{3}. La Tabla 1 resume
la toxicidad aguda de PG-paclitaxel en ratas en
comparación con paclitaxel/Cremophor. La Tabla 2 resume los datos
que se refieren al efecto de PG-paclitaxel contra
tumores MCa-4, FSa-II I y
HCa-I en ratones. Los datos también se resumen en
las Fig. 3A-Fig. 3E.

1. Una composición que comprende un fármaco
antineoplásico conjugado con un polímero hidrosoluble, en la que
dicho fármaco antineoplásico es paclitaxel, docetaxel, etopósido,
tenipósido, campotecina o epotilona, y en la que dicho polímero
hidrosoluble es poli(ácido L-glutámico), poli(ácido
D-glutámico), poli(ácido
DL-glutámico), poli(ácido
D-aspártico), poli(ácido
L-aspártico), poli(ácido
DL-aspártico), ácido poliacrílico,
poli(2-hidroxietil
L-glutamina), carboximetildextrano, ácido

2. La composición de la reivindicación 1, en la
que dicho fármaco antineoplásico se selecciona de paclitaxel,
docetaxel y camptotecina.
3. La composición de la reivindicación 1, en la
que un polímero de poliamino se define además como un copolímero
con policaprolactona, ácido poliglicólico, ácido poliláctico,
poli(2-hidroxietil
L-glutamina), carboximetildextrano, ácido
hialurónico, albúmina de suero humano, o ácido polialgínico, o una
combinación suya.
4. La composición de cualquiera de las
reivindicaciones precedentes, en la que dicho polímero tiene un
peso molecular de 5.000 a 100.000, preferiblemente de 20.000 a
80.000.
5. La composición de cualquiera de las
reivindicaciones precedentes, en la que dicho polímero hidrosoluble
está conjugado al 2'- ó 7-hidroxilo de paclitaxel o
docetaxel.
6. La composición de cualquiera de las
reivindicaciones precedentes, en la que dicha composición comprende
de 2 a 35% (p/p) de paclitaxel.
7. La composición de la reivindicación 1, en la
que el polímero hidrosoluble es poli(ácido
L-glutámico), poli(ácido
D-glutámico), poli(ácido DL- glutámico), poli(ácido
D-aspártico), poli(ácido
L-aspártico) o poli(ácido
DL-aspártico).
8. El uso de la composición de cualquiera de las
reivindicaciones 1 a 7 para la fabricación de un medicamento para
el tratamiento del cáncer, una enfermedad autoinmune sistémica, o
reestenosis arterial u oclusión arterial después de traumatismo
vascular.
9. El uso según la reivindicación 8, en el que
dicho cáncer es cáncer de mama, cáncer ovárico, melanoma maligno,
cáncer de pulmón, cáncer gástrico, cáncer de próstata, cáncer de
colon, cáncer de cabeza y cuello, leucemia, fibrosarcoma o sarcoma
de tejidos blandos.
10. Un dispositivo médico implantable, en
particular una malla, que está revestido con una composición que
comprende paclitaxel conjugado con un polímero hidrosoluble como se
definió en la reivindicación 1 en una cantidad eficaz para inhibir
la proliferación de células musculares lisas.
