Description
Inyector de tipo pluma con una unidad de control
electrónico.
El presente invento se refiere a mejoras en un
dispositivo de inyección, y en particular a mejoras en un
dispositivo de inyección portátil para dispensar cantidades
controladas de un medicamento.
Típicamente, tales dispositivos de inyección son
usados por quienes padecen de diabetes, para administrarse por sí
mismos una dosis de insulina o de una medicina del tipo de la
insulina. Se comprenderá que tales dispositivos de inyección son
adecuados para la inyección de otras medicinas.
Hubo un tiempo en el que tales dosis se
administraban mediante el uso de una jeringa desechable; primero se
llenaba la jeringa desde un vial separado, o desde otro recipiente,
y luego se usaba para inyectar la dosis. Sin embargo, tal
disposición tenía una serie de dificultades. En particular, esa
disposición no era adecuada para personas con achaques. Para otros,
el estigma social asociado a tales medidas hacía su uso público
problemático.
Para vencer esas dificultades se han
desarrollado una serie de inyectores denominados del tipo de pluma.
Estos dispositivos son pequeños, pudiendo ser llevados en el
bolsillo de la chaqueta o similar, y permitir que sean obtenidas
una serie de dosis de un cartucho o ampolla contenido dentro del
inyector. El presente invento es de aplicación en particular a
tales inyectores del tipo de pluma.
Aunque tales inyectores del tipo de pluma
representan una considerable mejora sobre las jeringas hipodérmicas
desechables, los problemas sin embargo subsisten. Es una ventaja del
presente invento que el mismo elimina, o al menos reduce
sustancialmente, tales problemas. El presente invento proporciona
también una mayor facilidad de uso y una mejor interacción con el
usuario.
A continuación se describirá el invento, a modo
de ejemplo únicamente, con referencia a los dibujos que se
acompañan, en los cuales:
En la Figura 1 se ha representado una vista en
planta de un inyector del tipo de pluma de acuerdo con el presente
invento;
En la Figura 2 se ha representado una vista
similar a la de la Figura 1, en la que se ha omitido una tapa
extrema del inyector;
En la Figura 3 se ha representado una vista en
corte transversal del inyector de las Figuras 1 y 2; y
En la Figura 3A se ha representado una vista
similar a la de la Figura 3, con un cartucho lleno en el
inyector.
Con referencia en primer lugar a las Figuras 1 a
3, puede verse en ellas un inyector del tipo de pluma 2 de acuerdo
con el presente invento. El inyector 2 comprende un alojamiento
principal 4 al cual está asegurada de modo liberable una tapa
extrema o cubierta 6.
En un primer extremo del alojamiento principal 4
se ha previsto una región de panel de control 8. Esta región
incluye un panel de presentación 10, típicamente una presentación de
LCD (de cristal líquido), y un primer botón de dosis 12 y un
segundo botón de dosis 14, siendo hechos actuar los botones de dosis
primero y segundo para aumentar o disminuir una dosis de
medicamento a ser entregada. La región del panel de control 10 en la
realización ilustrada incluye también un botón de armar 16.
En el primer extremo del alojamiento principal
se ha previsto también un botón de dispensar 18. Preferiblemente,
cuando no está oprimido, el botón de dispensar 18 está enrasado con
el alojamiento principal 4.
A lo largo de un eje geométrico longitudinal del
inyector 2, a cada lado de la región del panel de control 10, se
han previsto una serie de estrías o rebajos 20. Éstos facilitan el
agarre por el usuario del inyector 2.
En un segundo extremo del alojamiento principal
4 hay una unidad de aguja 22 asegurada de modo liberable al
alojamiento principal. El segundo extremo del alojamiento principal
4 está también provisto de una parte conformada 24.
En uso, se almacena un cartucho 40 o ampolla de
medicamento en el alojamiento 4, detrás de la parte conformada 24.
Se prefiere que la parte conformada sea transparente, para que
permita que el usuario vea el cartucho 40.
También se ha previsto un botón de cebar 26 en
el segundo extremo del alojamiento 4. Se comprenderá que cuando la
tapa extrema 6 esté en posición sobre el segundo extremo del
alojamiento, no será posible oprimir inadvertidamente el botón de
cebar 26, ni que pinche la aguja 22. También puede haber un
interruptor de detección de cubierta 28 incluido en el segundo
extremo del alojamiento principal 4, para detectar si la tapa
extrema o cubierta 6 está, o no, en su posición.
En la Figura 3 puede verse un contacto de cebado
30, un contacto de armado 32, un primer contacto de dosis 34 y un
segundo contacto de dosis 36, correspondientes a los respectivos
botones. También se ha representado un contacto de dispensar 19,
correspondiente al botón de dispensar 18.
Con referencia a la Figura 3, puede verse en
ella que se ha previsto un lugar adecuado para una fuente de
energía 38, tal como una o más pilas. Hay también una región
adecuada para que sea situado en ella un cartucho 40 o ampolla de
medicamento. Se puede tener acceso a esa región por medio de la
parte conformada desmontable 24 del alojamiento principal 4, para
permitir la sustitución del cartucho 40 o ampolla, tal como se
requiera por el usuario.
En una tercera región del alojamiento principal
4 se ha previsto un mecanismo de accionamiento 42 que opera gracias
a la fuente de energía 38 y que actúa sobre el cartucho 40 o ampolla
de medicamento.
El cartucho 40 o ampolla comprende un recipiente
44 o manguito cerrado por un extremo por una cubierta 46 en un
extremo de la cabeza del mismo, y sellado por el otro extremo por un
tapón 48 o taco obturador movible. Cuando está en posición, la
unidad de aguja 22 perfora la cubierta 46 y el movimiento del tapón
48 hacia la cubierta 46 hará que el medicamento contenido dentro
del cartucho 40 o ampolla sea expulsado. El cartucho puede ser un
cartucho de 3 ml, de acuerdo con la Norma ISO/FDIS 11608 Parte 3, o
cualquier otro cartucho adecuado que se adapte al inyector.
El movimiento del tapón 48 o taco obturador es
originado por el movimiento de un émbolo o núcleo móvil 50 que
forma parte del mecanismo de accionamiento 42. El émbolo o núcleo
móvil 50 es movible entre una primera posición de totalmente
retirado (no representado) en la cual permite la sustitución del
cartucho 40 o la ampolla, y una segunda posición de totalmente
extendido, en la cual ha sido expulsado todo el medicamento que es
posible expulsar del cartucho 40 o ampolla. Se puede prever un
interruptor 52 de tope extremo en el alojamiento principal 4, para
detectar cuándo el émbolo 50 está en la posición de totalmente
retirado. El disparo del interruptor de tope final 52 puede liberar
un fiador u otro dispositivo de sujeción, para permitir acceso al
alojamiento principal 4 para la sustitución del cartucho 40.
El mecanismo de accionamiento 42 es hecho
funcionar mediante un motor 54, bajo el control de una unidad de
control electrónico (no representada). El motor 54 deberá ser
reversible, con objeto de permitir que el émbolo 50 sea movido
entre las posiciones primera y segunda. En la Figura 3, puede verse
el motor 54 que acciona al émbolo 50 por medio de un tren de
engranaje 42, de tal modo que la rotación de un tercer rotor 58 hace
que el émbolo 50 sea movido con relación al tercer rotor 58.
Preferiblemente, el usuario puede sentir la
vibración del motor 54 y del mecanismo de accionamiento 42 asociado,
y/o oírlos funcionar. De este modo, se proporciona al usuario un
mayor grado de confianza en el hecho de que el inyector 2 esté
funcionando.
A continuación se describirá la funcionalidad de
un inyector del tipo de pluma de acuerdo con el presente invento,
con referencia en particular a las Figuras 1, 2 y 3.
El inyector 2 está provisto de una unidad de
control electrónico. La unidad de control electrónico está acoplada
tanto al mecanismo de accionamiento como a una interfaz con un
usuario. La interfaz con el usuario incluye el panel de
presentación 10, así como los botones accionables por el usuario (y
los contactos asociados). La unidad de control electrónico está
basada en un microprocesador. Se puede usar una memoria, ya sea
volátil o ya sea no volátil, para el almacenamiento de la
"historia de las dosis" e información específica del
paciente.
La unidad de control electrónico es
preferiblemente alimentada desde la fuente de energía 38 del
inyector.
El inyector 2 incluye también preferiblemente un
puerto para comunicación entre la unidad de control electrónico y
un aparato externo, tal como un ordenador personal.
El inyector 2 tiene también una instalación para
la detección del cebado (tal como un interruptor de inclinación o
un acelerómetro), para identificar cuándo el inyector 2 está
invertido. Al detectar una posición invertida (con la aguja hacia
arriba) el inyector 2 cambiará de estado automáticamente, para
quedar preparado para el cebado. El cebado puede iniciarse
oprimiendo el botón de cebar 26 para producir una acción de
dispensación pequeña fija. La unidad de control electrónico puede
hacer que un altavoz emita un sonido cuando se oprima el botón de
cebar 26.
El botón de cebar 26 permanece inactivo en todas
las demás ocasiones. Cuando se activa el botón de cebar 26, todos
los demás botones de la región del panel de control están inactivos,
es decir, que lo están aquellos botones que hayan de ser usados
para establecer o dispensar una dosis.
La unidad de control electrónico puede hacer que
en un altavoz se produzca un sonido cuando el botón de armar 16 sea
oprimido durante un período de tiempo suficiente para proporcionar
una realimentación audible por el usuario.
La función del botón de armar 16 es la de
activar el botón de dispensación 18. El botón de armar se mantiene
preferiblemente bajado durante un período de tiempo predeterminado,
antes de que el inyector 2 quede armado. El estado de armado puede
ser además representado en el panel de presentación 10. La
funcionalidad del botón de armar está preferiblemente enlazada al
interruptor 28 de detección de la cubierta, de tal modo que el botón
de armar 16 solamente funcionará para armar el inyector 2 cuando no
esté presente la tapa 2.
Además, en una realización preferida, un reloj
dentro de la unidad de control electrónico detectará si el botón de
dispensar 18 ha sido oprimido dentro de un intervalo de tiempo
especificado a continuación del armado del inyector 2. Si no ha
sido oprimido el botón de dispensar 18 dentro del intervalo de
tiempo especificado, la unidad de control electrónico desarmará el
inyector 2. Alternativamente, si el usuario oprime el botón de
armar una segunda vez dentro de un período de tiempo predeterminado,
el inyector quedará incapacitado.
En una realización alternativa, el botón de
dosis 18 puede funcionar como botón de cebar y como botón de dosis.
Cuando se hace actuar al detector de cebado, mediante la orientación
del inyector 2 con la aguja hacia arriba, el botón de dispensar 18
cambiará de función a la del botón de cebar de la realización
anterior.
Los botones del inyector 2 son preferiblemente
de naturaleza táctil, para proporcionar realimentación sensorial al
usuario.
El panel de presentación 10 es típicamente una
presentación de LCD y proporcionará información alfanumérica y
gráfica relativa al funcionamiento del dispositivo. El panel de
presentación 10 indica, preferiblemente, la cantidad de dosis
seleccionada, la cantidad de dosis anterior y el tiempo transcurrido
desde que fue administrada la anterior dosis. Típicamente, el
tiempo transcurrido desde la anterior dosis está limitado a un
período de tiempo dentro de las 48 horas anteriores, aunque son
posibles otros períodos de tiempo.
Como información adicional que puede ser
presentada, se incluye:
- -
- que el inyector está armado y listo para dispensar (gráfica)
- -
- que el inyector está dispensando (gráfica)
- -
- que el inyector haya dispensado la dosis seleccionada y que el usuario deba esperar antes de retirar la aguja de su cuerpo
- -
- que ese período de espera haya transcurrido
- -
- la historia de las dosis, típicamente la de las últimas 48 horas, en términos de las dosis tomada y del tiempo transcurrido entre dosis
- -
- la cantidad de medicamento que queda en el cartucho, preferiblemente en términos de unidades de dosis del medicamento
- -
- que el dispositivo está en la posición de cebar (ya sea además o ya sea en vez de la indicación acústica antes indicada)
- -
- el ajuste de volumen del altavoz, por ejemplo, alto, bajo, o silenciado
- -
- que el inyector está llegando al final de su vida (por ejemplo, una indicación del nivel de energía de la pila -gráfica o en términos de cuenta atrás en cuanto al número de días o de ciclos completos de funcionamiento hasta llegar a la expiración predeterminada de la vida del producto- alfanumérica)
- -
- que la aguja está probablemente bloqueada
- -
- que está en curso la sustitución del cartucho 40
- -
- que la dosis seleccionada es la máxima disponible en el cartucho 40
- -
- que la máxima dosis disponible es inferior a la dosis esperada.
El panel de presentación 10 puede ofrecer al
usuario la elección en cuanto a opciones de lenguaje, en la medida
en que sean apropiadas para el mercado y/o el usuario. El texto
presentado puede incluir la información indicada en lo que antecede
y/o información adicional. La opción de lenguaje puede ser
previamente programada o seleccionable por el usuario. El usuario
puede seleccionar, preferiblemente, la opción de lenguaje por medio
de un menú previsto en el panel de presentación 10.
El valor de la dosis actualmente seleccionado,
el valor de la dosis anteriormente usado y el tiempo (en horas)
desde que fue dispensada la dosis anterior, pueden ser presentados
claramente al mismo tiempo, en caracteres grandes, de fácil
lectura, en la presentación. Preferiblemente, el panel de
presentación está también provisto de una iluminación
posterior.
La presentación 10 proporciona preferiblemente
una indicación gráfica de que está siendo dispensada la dosis
seleccionada. Esto puede conseguirse, por ejemplo, ya sea como un
gráfico animado, o ya sea como un recuento de cuenta atrás (o bien
como una combinación de ambos).
Los botones de control tienen una serie de
funciones. Los botones de dosis 12, 14 permiten al usuario
seleccionar una dosificación deseada. El botón de armar dosis 16
permite al usuario confirmar la selección de una dosificación
deseada. El primer botón de dosis puede incrementar el nivel de la
dosificación, y el segundo botón de dosis puede decrementar el
nivel de la dosificación. Los botones de marcación de dosis 12, 14
pueden ser oprimidos y mantenerlos oprimidos durante un breve
espacio de tiempo, de 1-2 segundos, para restablecer
a cero el valor de la dosis. El usuario puede entonces marcar
aumento (o disminución) por incrementos simples (o por mitades de
incrementos).
Los botones 12, 14 para marcar dosis están
destinados a ser oprimidos una vez para un incremento simple (o un
incremento mitad) del valor de la dosis seleccionado. En una
realización alternativa, oprimiendo y manteniendo oprimido uno de
los botones se hará que el valor de dosis empiece a desfilar (hacia
arriba o hacia abajo), con objeto de cambiar al tamaño de dosis más
rápidamente.
El botón de dispensar 18 permite a un usuario
iniciar la dispensación de la dosis. El botón de cebar 26 dispensa
una unidad de dosificación desde el cartucho 40. Por consiguiente,
si ha quedado aprisionado algo de aire en el inyector 2, puede ser
expulsado mediante el uso del botón de cebar 26. Se ha previsto un
fiador de suelta de puerta para permitir acceso al cartucho 40.
Claims (5)
1. Un dispositivo de inyección que tiene un
mecanismo de accionamiento (42) para expulsar selectivamente una
dosis de medicamento desde un cartucho de medicamento (40) a través
de una unidad de aguja (22), una unidad de control electrónico para
controlar el mecanismo de accionamiento (42), y un botón de
dispensar (18) para generar una señal de botón de dispensar para la
unidad de control electrónico indicadora de la dosis a ser
expulsada, en el cual un botón de armar (16) hace que sea enviada
una primera señal de botón de armar a la unidad de control
electrónico para permitir que sea expulsada la dosis, en que el
botón de armar (16) está enlazado a un interruptor de detección de
cubierta (28), de tal modo que el botón de armar solamente
funcionará para armar un inyector (2) cuando no esté presente una
cubierta (6).
2. Un dispositivo de inyección de acuerdo con la
reivindicación 1, caracterizado porque cuando la señal de
botón de dispensar no sea generada dentro de un período de tiempo
predeterminado de la señal de armar, la unidad de control
electrónico no hará actuar al mecanismo de accionamiento (42) en
respuesta a la señal de botón de dispensar.
3. Un dispositivo de inyección de acuerdo con la
reivindicación 1 ó la reivindicación 2, caracterizado porque
el botón de armar debe generar la primera señal de botón de armar
para un período de tiempo predeterminado antes de que la unidad de
control electrónico responda a la señal de botón de dispensar.
4. Un dispositivo de inyección de acuerdo con
cualquiera de las reivindicaciones 1 a 3, caracterizado
porque el dispositivo de inyección está además provisto de un panel
de presentación (10), proporcionando el panel de presentación (10)
una indicación de que se permite que sea expulsada la dosis.
5. Un dispositivo de inyección de acuerdo con
cualquiera de las reivindicaciones 1 a 4, caracterizado
porque cuando el botón de armar genera una señal de botón de armar
subsiguiente dentro de un período de tiempo predeterminado de la
primera señal de botón de armar, la unidad de control electrónico no
hará actuar al mecanismo de accionamiento (43) en respuesta a la
señal de botón de dispensar.
