Abstract
Un dispositivo de inyección para inyectar un medicamento de un cartucho (40) de medicamento, cuyo dispositivo de inyección comprende un alojamiento principal (4), un pistón (50) y un interruptor (52) de tope de extremo, en el que el acceso al cuerpo principal (4) para la sustitución del cartucho (40) de medicamento solamente se puede obtener cuando el pistón (50) dispara el interruptor de tope de extremo.
Description
Dispositivo de inyección con cartucho
reemplazable.
El presente invento se refiere a
perfeccionamientos en un dispositivo de inyección, y en particular a
perfeccionamientos en un dispositivo portátil de inyección para
dispensar cantidades controladas de un medicamento.
Típicamente, dichos dispositivos de inyección lo
usan quienes padecen diabetes para autoadministrarse una dosis de
insulina o de una medicina del tipo de insulina. Se entenderá que
tales dispositivos de inyección son adecuados para la inyección de
otras medicinas.
Anteriormente, dichas dosis se administraban
mediante el uso de una jeringa desechable; la jeringa se llenaba
primero desde un vial separado o desde otro recipiente y luego se
usaba para inyectar la dosis. Sin embargo, en dicho dispositivo se
planteaban una serie de dificultades. En particular, esta clase de
dispositivo no era adecuada para el enfermo. En otros casos, el
estigma social asociado con dichas jeringas hacía que su uso en
público fuese problemático.
Con objeto de superar estas dificultades, se ha
desarrollado una serie de inyectores denominan denominados del tipo
pluma. Estos dispositivos son pequeños, capaces de trasportarse en
el bolsillo de la chaqueta o en un recipiente similar y de permitir
la obtención de una serie de dosis desde un cartucho o ampolla
contenidos dentro del inyector. El presente invento tiene una
aplicación particular a dichos inyectores del tipo pluma.
Aunque dichos inyectores del tipo pluma han
logrado un perfeccionamiento considerable con respecto a las
jeringas hipodérmicas desechables, no obstante los problemas
subsisten. Una ventaja del presente invento es que elimina dichos
problemas, o al menos los reduce sustancialmente. El presente
invento también proporciona una facilidad perfeccionada del uso y de
una interacción mejorada con un usuario.
A continuación se describe el invento, solamente
a título de ejemplo, con referencia a los dibujos adjuntos, en los
que:
La Figura 1 muestra una vista en planta de un
inyector del tipo pluma de acuerdo con el presente invento;
La Figura 2 muestra una vista similar a la de la
Figura 1 en la que se ha omitido un tapón extremo de inyector;
La Figura 3 muestra una vista en corte
transversal del inyector de las Figuras 1 y 2; y
La Figura 3A muestra una vista similar a la de la
Figura 3 con un cartucho lleno en el inyector.
Refiriéndose en primer lugar a las Figuras 1 a
3, se puede ver un inyector 2 del tipo pluma de acuerdo con el
presente invento. El inyector 2 comprende un alojamiento principal
4 al que se ha fijado de forma liberable un tapón extremo o tapa

En un primer extremo del alojamiento principal 4
se ha provisto una región 8 de panel de control. Esta región incluye
un panel 10 de presentación visual, típicamente una pantalla de
cristal líquido (en adelante LCD), y un primer pulsador 12 de
dosis y un segundo pulsador 14 de dosis, accionándose los
pulsadores primero y segundo de dosis para aumentar o disminuir
una dosis de medicamento a descargar. En la realización ilustrada,
la región 10 de panel de control incluye también un pulsador 16 de
brazo.
En el primer extremo del alojamiento principal se
ha provisto también un pulsador 18 de dispensación. Preferiblemente,
cuando no está apretado el pulsador 18 de dispensación, se
encuentra a nivel del alojamiento principal 4.
A lo largo del eje longitudinal del inyector 2,
a cada lado de la región 10 de panel de control se ha provisto una
serie de acanaladuras o rebajos 20. Estas acanaladaduras o rebajos
ayudan al agarre del inyector 2 por un usuario.
En un segundo extremo del alojamiento principal
4 una unidad 22 de aguja está sujeta liberablemente al alojamiento
principal. El segundo extremo del alojamiento principal 4 está
provisto también de la parte perfilada 24.
En uso, un cartucho o ampolla 40 de medicamento
se guardan en el alojamiento 4 detrás de la parte perfilada 24.
Preferiblemente, la parte perfilada es transparente para que un
usuario pueda ver el cartucho 40.
En el segundo extremo del alojamiento 4 se ha
provisto también un pulsador cebador 26. Se entenderá que, cuando
el tapón de extremo 6 está en su sitio sobre el segundo extremo del
alojamiento, no es posible apretar involuntariamente el pulsador
cebador 26 o que sea perforado por la unidad 22 de aguja. Se puede
incluir también un interruptor 28 de detección de tapa en el
segundo extremo del alojamiento principal 4 para detectar si está o
no colocado el tapón de extremo o tapa 6.
En la Figura 3, se puede ver un contacto de
cebado 30, un contacto 32 de brazo, un primer contacto 34 de dosis
y un segundo contacto 36 de dosis correspondientes a los
respectivos pulsadores. También se muestra un contacto 19 de
dispensación correspondiente al botón 18 de dispensación.
Con referencia a la Figura 3, se puede ver que
se ha provisto un lugar adecuado para una fuente de alimentación de
energía eléctrica 38 tal como una batería o baterías. Existe también
una región adecuada en la que se puede colocar un cartucho o ampolla
40 de medicamento. Esta región puede ser accesible por medio de la
parte retirable perfilada 24 del alojamiento principal 4 con el fin
de permitir la sustitución del cartucho o ampolla 40 según sea
requerido por el usuario.
En una tercera región del alojamiento principal 4
se ha provisto un mecanismo de impulsión 42 que funciona con la
energía eléctrica de la fuente de alimentación 38 y actúa sobre
el cartucho o ampolla 40 de medicamento.
El cartucho o ampolla 40 comprende un recipiente
o manguito 44 cerrado en un extremo por una tapa 46 en un extremo
de cabeza del mismo, y cerrado herméticamente en el otro por un
tapón móvil u obturador 48. Cuando están en posición, la unidad 22
de aguja perfora la tapa 46 y el desplazamiento del tapón 48 hacia
la tapa 46 hará que sea expulsado el medicamento contenido en el
interior del cartucho o ampolla 40. Este cartucho puede ser un
cartucho de 3 ml de acuerdo con la norma ISO/FDIS 11608 Parte 3, o
cualquier otro cartucho adecuado para que se adapte al
inyector.
El movimiento del tapón u obturador 48 es
causado por el movimiento de un pistón o émbolo 50 que forma parte
del mecanismo de impulsión 42. El pistón o émbolo 50 es desplazable
entre una primera posición totalmente retirada (no mostrada) que
permite la sustitución del cartucho o ampolla 40, y una segunda
parte totalmente extendida en la que se ha expulsado la mayor
cantidad posible de medicamento desde el cartucho o ampolla 40. Se
ha provisto un interruptor 52 de tope de extremo en el alojamiento
principal 4 para detectar cuándo se encuentra en la porción
totalmente retirada el pistón 50. El disparo del interruptor 52 de
tope de extremo puede liberar un pestillo u otro dispositivo de
sujeción para permitir el acceso al alojamiento principal 4 para la
sustitución del cartucho 40.
El mecanismo de impulsión 42 funciona mediante un
motor 54 controlado por una unidad electrónica de control (no
mostrada). El motor 54 debería ser reversible con el fin de permitir
que el pistón 50 se desplace entre las posiciones primera y segunda.
En la Figura 3, se puede ver que el motor 54 impulsa al pistón 50
por medio de un tren de engranajes 42, de tal manera que la rotación
de un tercer rotor 58 hace que el pistón 50 se mueva con respecto
al tercer rotor 58.
Preferiblemente, el usuario puede sentir la
vibración del motor 54 y el mecanismo de impulsión asociado 42 y/o
escucharlos cuando están funcionando. De esta manera se proporciona
al usuario un grado adicional de confianza por el hecho real del
funcionamiento del inyector 2.
A continuación se describe el funcionamiento de
un inyector del tipo pluma de acuerdo con el invento, en particular
con referencia a las Figuras 1, 2 y 3.
El inyector 2 está provisto de una unidad
electrónica de control. La unidad electrónica de control está
acoplada tanto al mecanismo de impulsión como a una interfaz de
usuario. La interfaz de usuario incluye el panel 10 de presentación
visual, así como los pulsadores accionables por el usuario (y
contactos asociados). La unidad electrónica de control está basada
en un microprocesador. Se pueden usar memorias volátiles o no
volátiles para el almacenamiento del "historial de dosis" o de
la información específica del paciente.
La unidad electrónica de control se alimenta
preferiblemente desde la fuente de alimentación 38 del
inyector.
Preferiblemente, el inyector 2 incluye también
un puerto para la comunicación entre la unidad electrónica de
control y un aparato externo tal como un ordenador personal.
El inyector 2 tiene también una instalación de
detección de cebado, (tal como un interruptor de inclinación o un
acelerómetro), para identificar cuándo está invertido el inyector 2.
Tras la detección de una posición invertida (aguja arriba), el
inyector 2 automáticamente cambiará de estado para estar
preparado para el cebado. Se puede iniciar el cebado apretando el
pulsador 96 de cebado para dar lugar a una acción fija de una
pequeña dispensación. La unidad electrónica de control puede hacer
que suene un altavoz cuando se apriete el pulsador cebador 26.
El pulsador cebador 26 está inactivo en todas las
demás ocasiones. Cuando el pulsador cebador 26 está activo, todos
los demás pulsadores de la región del panel de control están
inactivos, es decir, los pulsadores que se van a usar para
ajustar o dispensar una dosis.
La unidad electrónica de control puede causar que
suene un altavoz cuando el pulsador 16 de brazo se aprieta durante
un período suficiente de tiempo para proporcionar una
realimentación audible para el usuario.
La función del pulsador 16 de brazo es hacer que
se active el pulsador 18 de dispensación. El pulsador de brazo se
mantiene preferiblemente en la posición baja durante un período
determinado tiempo antes de que llegue a armarse el inyector 2. El
estado armado se puede mostrar adicionalmente en el panel 10 de
presentación visual. La funcionalidad del pulsador de brazo
preferiblemente está ligada al interruptor 28 de detección de tapa,
de tal manera que el pulsador 16 de brazo solamente funcione para
armar el inyector 2 cuando no esté colocada la tapa 6.
Adicionalmente, en una realización preferida, un
reloj instalado dentro de la unidad electrónica de control detecta
si el pulsador 18 de dispensación se ha apretado dentro de un
intervalo específico de tiempo posterior al armado del inyector 2.
Si no se ha apretado el pulsador 18 de dispensación dentro del
intervalo especificado de tiempo, la unidad electrónica de control
desarma al inyector 2. Alternativamente, si el usuario aprieta el
pulsador de brazo una segunda vez dentro de un período determinado
de tiempo, el inyector se inhabilita.
En una realización alternativa, el pulsador 18 de
dispensación podría funcionar como un pulsador de cebado y como el
pulsador de dosis. Cuando se activa el detector de cebado, mediante
la orientación del inyector 2 con la aguja hacia arriba, el
pulsador 18 de dispensación cambia de función a la del pulsador de
cebado de la realización anterior.
Los pulsadores del inyector 2 son preferiblemente
de naturaleza táctil, para comunicar una realimentación sensorial
al usuario.
El panel 10 de presentación visual es típicamente
una pantalla LCD y proporciona información alfanumérica y gráfica
con respecto al funcionamiento del dispositivo Preferiblemente, el
panel 10 de presentación visual indica la cantidad de dosis
seleccionada, la cantidad de dosis anterior y el tiempo transcurrido
desde que se administró la dosis anterior. Típicamente, el tiempo
transcurrido desde que se administró la dosis anterior está limitado
a un período de tiempo dentro de las 48 horas precedentes, aunque
son posibles otros períodos de tiempo.
La información adicional que se puede presentar
visualmente incluye:

- que el inyector está armado y preparado para dispensar (información gráfica)
- -
- que el inyector está dispensando (información gráfica)
- -
- que el inyector ha dispensado la dosis seleccionada y que un usuario debería esperar antes de retirar la aguja de su cuerpo
- -
- que ha transcurrido este periodo de espera
- -
- el historial de la dosis, típicamente durante las últimas 48 horas, en función de la dosificación tomada y el tiempo transcurrido entre dosis.
- -
- la cantidad de medicamento que queda en el cartucho, preferiblemente en función de las unidades de dosificación del medicamento
- -
- que el dispositivo se encuentra en la posición de cebado (bien además de o bien en lugar de la indicación acústica anteriormente indicada)
- -
- ajuste de volumen del altavoz, por ejemplo alto, bajo o silencioso.
- -
- que el inyector está llegando al fin de su vida (por ejemplo, una indicación de nivel bajo de batería (información gráfica) o bien una cuenta atrás en función del número de días o de ciclos completos de funcionamiento hasta una caducidad predeterminada de la vida del producto - información alfanumérica)
- -
- que la aguja probablemente está bloqueada
- -
- que está desarrollándose la sustitución del cartucho 40
- -
- que la dosis seleccionada es la máxima disponible en el cartucho 40
- -
- que la máxima dosis disponible es menor que la dosis esperada.
El panel 10 de presentación visual puede ofrecer
a un usuario una selección de opciones de lenguaje según sea
apropiado para el mercado y/o para el usuario. El texto presentado
visualmente podría incluir el anteriormente citado y/o información
adicional. La opción de lenguaje se puede
pre-programar o bien seleccionar por un usuario.
Preferiblemente, el usuario puede seleccionar la opción de lenguaje
por medio de un menú provisto en el panel 10 de presentación
visual.
El valor de dosis actualmente seleccionado, el
valor de dosis usado anteriormente y el tiempo (en horas)
transcurrido desde que se dispensó la dosis anterior, se pueden
mostrar con claridad al mismo tiempo, en caracteres de gran tamaño
y fácil lectura en la pantalla de presentación visual.
Preferiblemente, la pantalla está provista también de una luz
posterior.
La presentación visual 10 proporciona
preferiblemente una indicación gráfica de que se está dispensando la
dosis seleccionada. Esto se puede lograr, por ejemplo, bien con un
gráfico animado o bien con una cuenta atrás (o con una combinación
de ambas indicaciones).
Los pulsadores de control tienen una serie de
funciones. Los pulsadores 12, 14 de dosis permiten a un usuario
seleccionar una dosificación deseada. El pulsador 16 de brazo de
dosis permite a un usuario confirmar la selección de una
dosificación deseada. El primer pulsador de dosis puede aumentar el
nivel de la dosificación y el segundo pulsador de dosis puede
disminuir el nivel de la dosificación. Los pulsadores 12,14 de
marcado de dosis se pueden apretar hacia abajo (y mantenerse en
esta posición durante un tiempo corto, 1-2
segundos) para volver a ajustar a 0 un valor de dosis. El usuario
puede luego marcar hacia arriba (o hacia abajo) en incrementos de
una unidad (o de la mitad)
Los pulsadores 12, 14 de marcado de dosis están
destinados a apretarse una vez para un solo incremento (o de la
mitad del incremento) en el valor de dosis seleccionado. En una
realización alternativa, al apretar y mantener apretado uno de los
pulsadores se hace que el valor de dosis empiece a desplazarse
(hacia arriba o hacia abajo) con el fin de cambiar con más rapidez
el tamaño de la dosis.
El pulsador 18 de dispensación permite a un
usuario iniciar la dispensación de la dosificación. El pulsador
cebador 26 dispensa una unidad de dosificación desde el cartucho 40.
De ese modo, si existe cualquier cantidad de aire atrapado en el
inyector 2, se puede expulsar mediante el uso del pulsador cebador
26. Se ha provisto un pestillo de liberación de puerta para permitir
el acceso al cartucho 40.
Como el cartucho 40 tiene un tamaño estándar,
cada cartucho 40 se vaciará mediante un recorrido idéntico del
émbolo accionado por el mecanismo de impulsión. Una vez que el
émbolo 50 está en la posición totalmente extendida, se sabe que el
cartucho 40 está vacío y el usuario dispone de una indicación de
este estado.
Cuando se acciona el pestillo de liberación de
puerta para que se retire el cartucho vaciado 40, se hace
funcionar el mecanismo de impulsión 42 para invertir el sentido
de un tornillo de avance 60 con el fin de retirar el émbolo 50
hasta que el tornillo de avance 60 choca con un interruptor 52 de
tope de extremo que está instalado en un punto de referencia
conocido.
Cuando se detecta un nuevo cartucho 40, por
ejemplo por medio de un interruptor de contacto (no mostrado), y se
cierra el pestillo de liberación de puerta, la unidad electrónica de
control hace avanzar al tornillo de avance 60 hasta que el émbolo
50 choca con el tapón 48 de cartucho. Esto se hace convenientemente
ajustando un microinterruptor 51, tal como un interruptor de
cúpula hasta un extremo libre del émbolo 50.
Puesto que se puede calcular la posición exacta
del tapón 48 con referencia al tope 52 de extremo posterior, se
puede determinar una serie de unidades de medicamento almacenadas
del cartucho 40. Así, se puede usar un cartucho 40 medio vacío o
llenado incorrectamente con el inyector del presente invento. La
unidad electrónica de control que ha determinado el número de
unidades almacenadas dentro de cartucho preferiblemente no permitirá
que se dispense una dosificación mayor que la cantidad remanente que
se va a marcar para su dispensación.
Claims (4)
1. Un dispositivo de inyección para inyectar un
medicamento de un cartucho (40) de medicamento, cuyo dispositivo de
inyección comprende un alojamiento principal (4), un pistón (50) y
un interruptor (52) de tope de extremo, en el que el acceso al
cuerpo principal (4) para la sustitución del cartucho (40) de
medicamento solamente se puede obtener cuando el pistón (50) dispara
el interruptor de tope de extremo.
2. Un dispositivo de inyección de acuerdo con la
reivindicación 1, caracterizado porque el disparo del
interruptor (52) de tope de extremo libera un pestillo u otro
dispositivo de sujeción para permitir el acceso al cuerpo principal
(4) para la sustitución del cartucho (40) de medicamento.
3. Un dispositivo de inyección de acuerdo con la
reivindicación 1 o con la reivindicación 2, en el que el
dispositivo de inyección comprende además un mecanismo (42) de
impulsión para impulsar al pistón (50), una unidad electrónica de
control y un pestillo de liberación de puerta, caracterizado
porque cuando se acciona el pestillo de liberación de puerta, la
unidad electrónica de control hace funcionar al mecanismo (42) de
impulsión para retirar el pistón (50) hasta que se establece
contacto con el interruptor de tope de extremo.
4. Un dispositivo de inyección de acuerdo con la
reivindicación 3, en el que el dispositivo de inyección comprende
además un interruptor de contacto para detectar la presencia o
ausencia de un cartucho (40) de medicamento, caracterizado
porque tras la detección de un cartucho nuevo y el cierre del
pestillo de liberación de puerta, la unidad electrónica de control
acciona el mecanismo (42) de impulsión con el fin de hacer avanzar
el pistón (50) para establecer contacto con el nuevo cartucho de
medicamento.
